    When words matter: a cross-cultural perspective on lyrics and their relationship to musical emotions

    Several studies have investigated emotional reactions to instrumental music. However, studies on the effect of lyrics on emotions are limited. Previous studies suggest that the importance of lyrics may vary cross-culturally. The aim of this study was twofold: to investigate the effects of lyrics on aroused emotions and psychological mechanisms with music and to explore whether these differ cross-culturally. Fifty participants from Portugal and Sweden listened to six musical stimuli based on two songs, one representing each culture. These were presented in three versions each: the original, an instrumental, and the instrumental version with lyrics on the screen. The Portuguese and Swedish participants differed notably: the presence of lyrics did not affect listeners’ happiness in neither group as predicted, but did increase sadness, albeit only in the Portuguese group. Lyrics also increased nostalgia for the Portuguese listeners as predicted and surprise-astonishment for the Swedish listeners. Regarding the mechanisms, lyrics increased the activation of episodic memory in both groups, and the activation of evaluative conditioning, contagion, and visual imagery in the Portuguese group.     Effects of antiplatelet therapy on stroke risk by brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases: subgroup analyses of the RESTART randomised, open-label trial

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage.     New insights into the genetic etiology of Alzheimer's disease and related dementias

    Characterization of the genetic landscape of Alzheimer's disease (AD) and related dementias (ADD) provides a unique opportunity for a better understanding of the associated pathophysiological processes. We performed a two-stage genome-wide association study totaling 111,326 clinically diagnosed/'proxy' AD cases and 677,663 controls. We found 75 risk loci, of which 42 were new at the time of analysis. Pathway enrichment analyses confirmed the involvement of amyloid/tau pathways and highlighted microglia implication. Gene prioritization in the new loci identified 31 genes that were suggestive of new genetically associated processes, including the tumor necrosis factor alpha pathway through the linear ubiquitin chain assembly complex. We also built a new genetic risk score associated with the risk of future AD/dementia or progression from mild cognitive impairment to AD/dementia. The improvement in prediction led to a 1.6- to 1.9-fold increase in AD risk from the lowest to the highest decile, in addition to effects of age and the APOE Δ4 allele

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    Multiancestry analysis of the HLA locus in Alzheimer’s and Parkinson’s diseases uncovers a shared adaptive immune response mediated by HLA-DRB1*04 subtypes

    Across multiancestry groups, we analyzed Human Leukocyte Antigen (HLA) associations in over 176,000 individuals with Parkinson’s disease (PD) and Alzheimer’s disease (AD) versus controls. We demonstrate that the two diseases share the same protective association at the HLA locus. HLA-specific fine-mapping showed that hierarchical protective effects of HLA-DRB1*04 subtypes best accounted for the association, strongest with HLA-DRB1*04:04 and HLA-DRB1*04:07, and intermediary with HLA-DRB1*04:01 and HLA-DRB1*04:03. The same signal was associated with decreased neurofibrillary tangles in postmortem brains and was associated with reduced tau levels in cerebrospinal fluid and to a lower extent with increased AÎČ42. Protective HLA-DRB1*04 subtypes strongly bound the aggregation-prone tau PHF6 sequence, however only when acetylated at a lysine (K311), a common posttranslational modification central to tau aggregation. An HLA-DRB1*04-mediated adaptive immune response decreases PD and AD risks, potentially by acting against tau, offering the possibility of therapeutic avenues

    Spontaneous Music-Evoked Autobiographical Memories in Individuals Experiencing Depression

    Listening to music often triggers strong memories of events from our past, which influence how we affectively experience music listening and can therefore contribute to music’s therapeutic capacity. The aim of this study was to examine the valence and content of spontaneous music-evoked autobiographical memories (MEAMs) in listeners with self-reported depression, who typically demonstrate negatively biased autobiographical memory. Eighteen depressed and 21 controls participated in a music-listening experiment where they listened to a personalized music stimulus, described their memories, and thereafter rated the valence of these memories and of their induced affect. Participants’ ratings were statistically analysed, while the memory content was analysed with the use of a computerized text-analysis method and with a qualitative thematic analysis. Quantitative ratings of valence revealed a significant difference between groups: half of the depressed, compared to none of the controls, recalled a negative memory, and these were experienced with negative induced affect. The qualitative thematic analysis of the memory descriptions revealed that both depressed and control participants’ memories could be categorized into three first-level themes: (1) personal, (2) relationships, and (3) activities. Depressed participants’ negative memories were mainly located in the ‘relationships’ theme and included memories about loss and dysfunctional relationships, such as bullying, and in the ‘personal’ theme, including memories of mental health struggles and coping with music. Approximately a third of depressed participants recalled positive memories, and these were either related to loving family relationships or to activities. Limitations concerning the small sample size and implications regarding the function of music listening for depressed individuals are discussed.peerReviewe

    Emotion regulation with music in depressed and non-depressed individuals

    Music is often used for regulating emotions in everyday life and could have both beneficial and harmful effects on emotional health. Depression is associated with impaired emotion-regulation skills, but few studies have examined whether such impairments apply to a musical context also. The aim of the present study was thus to compare depressed and non-depressed individuals with regard to their use of music for emotion regulation in everyday life. Seventy-seven listeners (19–65 years old) filled out a brief questionnaire, which measured emotion regulation in terms of the prevalence of various regulation goals, strategies, and mechanisms. The overall results indicated that the most frequent goal was to enhance positive emotions. Discharge was the most frequently used strategy and suppression was the least frequently used. A range of causal mechanisms were reported to occur. Contrary to our hypotheses, there were few differences between depressed and non-depressed listeners. We argue that knowledge gained from studies of emotion regulation may be used in field interventions that aim to teach depressed individuals how to use music to regulate emotions effectively

    Emotions, mechanisms, and individual differences in music listening: A stratified random sampling approach

    Emotions have been found to play a paramount role in both everyday music experiences and health applications of music, but the applicability of musical emotions depends on: 1) which emotions music can induce, 2) how it induces them, and 3) how indi- vidual differences may be explained. These questions were addressed in a listening test, where 44 participants (aged 19–66 years) reported both felt emotions and subjective impressions of emotion mechanisms (Mec Scale), while listening to 72 pieces of music from 12 genres, selected using a stratified random sampling pro- cedure. The results showed that: 1) positive emotions (e.g., happiness) were more prevalent than negative emotions (e.g., anger); 2) Rhythmic entrainment was the most and Brain stem reflex the least frequent of the mechanisms featured in the BRECVEMA theory; 3) felt emotions could be accurately predicted based on self- reported mechanisms in multiple regression analyses; 4) self-reported mechanisms predicted felt emotions better than did acoustic features; and 5) individual listeners showed partly different emotion-mechanism links across stimuli, which may help to explain individual differences in emotional responses. Implications for future research and applications of musical emotions are discussed

    Transmission of HIV drug resistance and the predicted effect on current first-line regimens in Europe

    Numerous studies have shown that baseline drug resistance patterns may influence the outcome of antiretroviral therapy. Therefore, guidelines recommend drug resistance testing to guide the choice of initial regimen. In addition to optimizing individual patient management, these baseline resistance data enable transmitted drug resistance (TDR) to be surveyed for public health purposes. The SPREAD program systematically collects data to gain insight into TDR occurring in Europe since 2001. Demographic, clinical, and virological data from 4140 antiretroviral-naive human immunodeficiency virus (HIV)-infected individuals from 26 countries who were newly diagnosed between 2008 and 2010 were analyzed. Evidence of TDR was defined using the WHO list for surveillance of drug resistance mutations. Prevalence of TDR was assessed over time by comparing the results to SPREAD data from 2002 to 2007. Baseline susceptibility to antiretroviral drugs was predicted using the Stanford HIVdb program version 7.0. The overall prevalence of TDR did not change significantly over time and was 8.3% (95% confidence interval, 7.2%-9.5%) in 2008-2010. The most frequent indicators of TDR were nucleoside reverse transcriptase inhibitor (NRTI) mutations (4.5%), followed by nonnucleoside reverse transcriptase inhibitor (NNRTI) mutations (2.9%) and protease inhibitor mutations (2.0%). Baseline mutations were most predictive of reduced susceptibility to initial NNRTI-based regimens: 4.5% and 6.5% of patient isolates were predicted to have resistance to regimens containing efavirenz or rilpivirine, respectively, independent of current NRTI backbones. Although TDR was highest for NRTIs, the impact of baseline drug resistance patterns on susceptibility was largest for NNRTIs. The prevalence of TDR assessed by epidemiological surveys does not clearly indicate to what degree susceptibility to different drug classes is affected
