Análise do perfil de medicamentos prescritos em mulheres com problemas de fertilidade do Serviço de Reprodução Humana dos CHUC : caracterização do seu potencial impacto na fertilidade
Monografia realizada no âmbito da unidade de Estágio Curricular do Mestrado Integrado em Ciências Farmacêuticas, apresentada à Faculdade de Farmácia da Universidade de CoimbraA infertilidade é um problema de saúde pública que está associada a diversas causas,
como endometriose, obstrução tubar, lesões do endométrio, anomalias na produção de
espermatozóides, ou outras causas como as do foro psicológico, a obesidade, carência de
vitaminas ou consumo excessivo de drogas.
Tratando-se de um problema do nosso quotidiano, existem um conjunto de técnicas
que têm sido aplicadas no tratamento destas situações, todas elas convergindo e tendo
como objetivo final a promoção da gravidez.
No entanto, em muitas situações convém fazer o levantamento da medicação crónica
que os doentes associados a esta patologia estão a fazer, na medida em que algumas das
causas idiopáticas que estão conectadas à infertilidade, podem ter resposta verificando os
fármacos que estes doentes estão a tomar.
Nos dados recolhidos das consultas do Serviço de Reprodução Humana do CHUC foi
possível verificar a presença de situações que podem ter na sua causa de infertilidade, uma
direta implicação dos fármacos que estão a ser tomados pelas doentes para outras
patologias, e se extrapolarmos os impactos de outros fármacos na fertilidade, encontramos
relatos onde a influência de fármacos antineoplásicos, anti-inflamatórios não esteróides ou
antirretrovirais pode ser prejudicial, na medida em que podem provocar alterações na
ovulação, chegando mesmo a provocar toxicidade nos ovários.
Cabe por isso aos profissionais de saúde fazer um levantamento de todos os dados
clínicos associados aos doentes com esta patologia, na medida em que todas as situações
possam ser avaliadas e verificadas, para fazer um despiste com maior eficácia e segurança da
eventual origem da infertilidade, para que o tratamento aplicado tenha a possibilidade de
atingir elevadas taxas de sucesso.Infertility is a public health problem that is associated with several causes, such as
endometriosis, tubal obstruction, endometrial lesions, abnormalities in sperm production, or
other causes like psychological conditions, obesity, lack of vitamins or excessive
consumption of drugs.
Since this is a problem of our daily lives, there are a set of techniques that have been
applied in the treatment of these conditions, all converging and with the ultimate objective of
promoting pregnancy.
However, in many situations it is appropriate to analyze chronic medications that
patients associated with this condition are taking to the extent that some of idiopathic
causes that are connected to infertility, can be answered by checking medications that these
patients are taking.
From the data collected on the Human Reproduction Service of CHUC was possible to
verify the presence of situations that may have in their cause of infertility, a direct implication
of the medications that are being taken by the patient for other conditions. When we
extrapolate the impact of other medications on fertility, there are reports where the
influence of anticancer drugs, nonsteroidal anti-inflammatory or antiretroviral can be
detrimental, to the extent that may cause changes in ovulation, and even induce toxicity in
the ovaries.
It is therefore up to the healthcare professionals to monitor all clinical data associated
with patients with this condition, since all situations can be evaluated and verified, to
enhance screening with a higher efficacy and safety of possible origin of infertility, so that the
applied treatment is able to achieve high success rates
Ultra-high resolution structure determination of transition metal substituted human carbonic anhydrase 2 - inhibitor complexes
Paramagnetic Nuclear Magnetic Resonance (NMR) is developing to aid the characterization of
paramagnetic molecules, whose paramagnetic centers changes the spectroscopic proprieties of said
molecules. These paramagnetic centers can be exploited to overcome some troublesome aspects of
NMR, such as sensitivity, by increasing the number of experiments. To aid this development, we used
human Carbonic Anhydrase II (hCAII), which is a model protein that contains zinc(II) in its active center,
which is diamagnetic. hCAII is an enzyme capable of interconverting carbon dioxide to bicarbonate,
making it one of the most important proteins in life. Several comprehensive studies have structurally
and functionally characterized hCAII making it an excellent model protein. Furthermore, the metal ion in
the active center can be substituted by other transition metals ions (see chapter 1), such as cobalt(II)
(see chapter 3), nickel(II) (see chapter 4) and copper(II) (see chapter 5), which are paramagnetic that
will help answering different problems described in this thesis.
The ion cobalt(II), explored in chapter 3, can induce considerable changes on the NMR
observables and is useful to understand the interactions of ligands with proteins. For this we used
cobalt(II)-hCAII and used NMR, Electron Paramagnetic Resonance (EPR) and X-ray crystallography to
characterize the interaction of thiocyanate under high concentrations with the hCAII. The addition of 500
mM of sodium thiocyanate changes the dynamics of the protein without changing the protein structure.
Solid-state NMR (SSNMR) is another field of NMR where the methodological and practical
aspects are currently under development to reach better sensitivity and resolution. We proposed the
usage of nickel(II)-hCAII as a paramagnetic molecule to increase the amount of tools in SSNMR, which
is explored in chapter 4. The nickel(II) ion is capable of breaking the dipolar bath by changing the
frequency of the nuclei close to the paramagnetic center, thus increasing the resolution and sensitivity
of the SSNMR experiments. Furthermore, in parallel we discovered that hCAII is capable of binding two
nickel(II) ions, one in the active center, as expected and described in literature, and one in the N-terminal
site of the protein, a novel discovery.
The description of the paramagnetic effects, such as the Pseudocontact Shifts (PCS), in the NMR
observables have been subjected to debate, where different treatments of theoretical equations were
clashing. The experimental proof to determine which equation holds true is fully described in chapter 5.
For this, we developed copper(II)-hCAII to acquire NMR and EPR data under the same conditions, and
determine which equations describe better the PCS. The data interpretation from different techniques
(both NMR and EPR) led us to conclude that the original treatment from Kurland and McGarvey equation
is the correct one.A Ressonância Magnética Nuclear Paramagnética (RMN) tornou-se uma importante ferramenta
para complementar as técnicas clássicas de RMN em estudos estruturais de metaloproteínas. Os seus
centros paramagnéticos intrínsecos ou extrínsecos perturbam suas propriedades espectroscópicas de
RMN, incluindo desvios químicos, acoplamentos dipolares residuais (RDCs) e tempos de relaxação
nuclear. Os desvios de pseudocontato (PCS), as mudanças de RDC e as contribuições paramagnéticas
nas taxas de relaxamento nuclear, induzidas por esses centros, podem ser exploradas para melhorar
a qualidade das estruturas de proteínas derivadas de RMN, aumentando o número e o tipo de restrições
estruturais que podem ser usadas.
Contribuindo para este desenvolvimento, foi utilizada a Anidrase Carbónica II (hCAII) humana,
como uma proteína modelo que contém um ião zinco(II) diamagnético no centro ativo. hCAII catalisa a
interconversão de dióxido de carbono em bicarbonato, tornando-se uma das proteínas mais
importantes da vida. Vários estudos caracterizaram estruturalmente e funcionalmente a hCAII,
tornando-a uma excelente proteína modelo. Além disso, o ião metálico no centro ativo pode ser
substituído por outros iões de metais de transição paramagnéticos, conforme apresentado no capítulo
1, como cobalto(II) (ver capítulo 3), níquel(II) (ver capítulo 4) e cobre(II) (ver capítulo 5), cujas
contribuições paramagnéticas para os parâmetros de RMN (apresentadas no capítulo 2) podem ajudar
a responder aos diferentes problemas descritos nesta tese.
No capítulo 3, cobalto(II)-hCAII foi usada para caracterizar as interações de hCAII com altas
concentrações de tiocianato. Ao combinar as alterações induzidas por paramagnéticos nas observáveis
de RMN, dados de Ressonância Paramagnética Eletrônica (EPR) e cristalografia de raios-X, concluiu-
se que a adição de 500 mM de tiocianato de sódio altera a dinâmica da proteína sem alterar a estrutura
da proteína.
A RMN de estado sólido (SSNMR) é outro campo da RMN onde os aspetos metodológicos e
práticos estão atualmente em desenvolvimento para alcançar melhor sensibilidade e resolução. No
capítulo 4, níquel(II)-hCAII é explorada como uma molécula paramagnética para aumentar a quantidade
de ferramentas disponíveis em SSNMR. O ião níquel(II) é capaz de quebrar o banho dipolar alterando
a frequência dos núcleos próximos ao centro paramagnético, aumentando assim a resolução e a
sensibilidade dos experimentos SSNMR. Além disso, em paralelo, descobriu-se que hCAII é capaz de
se ligar a dois iões níquel(II), um no centro ativo, conforme esperado e descrito na literatura, e um novo
na sequência N-terminal da proteína.
A descrição teórica dos efeitos paramagnéticos, como os PCS, nas observáveis de RMN tem
sido objeto de debate, onde diferentes tratamentos foram conflitantes, fornecendo diferentes equações
teóricas. A prova experimental para determinar qual equação é correta está descrita de forma completa
no capítulo 5. Para isso, a cobre(II)-hCAII foi preparada e usada para adquirir dados de RMN e EPR
nas mesmas condições, e determinou-se qual equação descreve melhor os PCS experimentais. A
interpretação dos dados de diferentes técnicas (tanto RMN quanto EPR) levou-nos a concluir que o tratamento clássico da equação de Kurland e McGarvey é o correto e não aquele derivado da mecânica
quântica
Detection and Classification of Anomalies in Railway Tracks
Em Portugal, existe uma grande afluência dos transportes ferroviários. Acontece que as
empresas que providenciam esses serviços por vezes necessitam de efetuar manutenção às
vias-férreas/infraestruturas, o que leva à indisponibilização e/ou atraso dos serviços e máquinas,
e consequentemente perdas monetárias. Assim sendo, torna-se necessário preparar um plano
de manutenção e prever quando será fundamental efetuar manutenções, de forma a minimizar
perdas.
Através de um sistema de manutenção preditivo, é possível efetuar a manutenção apenas
quando esta é necessária. Este tipo de sistema monitoriza continuamente máquinas e/ou
processos, permitindo determinar quando a manutenção deverá existir. Uma das formas de
fazer esta análise é treinar algoritmos de machine learning com uma grande quantidade de
dados provenientes das máquinas e/ou processos.
Nesta dissertação, o objetivo é contribuir para o desenvolvimento de um sistema de
manutenção preditivo nas vias-férreas. O contributo específico será detetar e classificar
anomalias. Para tal, recorrem-se a técnicas de Machine Learning e Deep Learning, mais
concretamente algoritmos não supervisionados e semi-supervisionados, pois o conjunto de
dados fornecido possui um número reduzido de anomalias.
A escolha dos algoritmos é feita com base naquilo que atualmente é mais utilizado e apresenta
melhores resultados. Assim sendo, o primeiro passo da dissertação consistiu em investigar
quais as implementações mais comuns para detetar e classificar anomalias em sistemas de
manutenção preditivos.
Após a investigação, foram treinados os algoritmos que à primeira vista seriam capazes de se
adaptar ao cenário apresentado, procurando encontrar os melhores hiperparâmetros para os
mesmos. Chegou-se à conclusão, através da comparação da performance, que o mais
enquadrado para abordar o problema da identificação das anomalias seria uma rede neuronal
artifical Autoencoder. Através dos resultados deste modelo, foi possível definir thresholds para
efetuar posteriormente a classificação da anomalia.In Portugal, the railway tracks commonly require maintenance, which leads to a stop/delay of
the services, and consequently monetary losses and the non-full use of the equipment. With
the use of a Predictive Maintenance System, these problems can be minimized, since these
systems continuously monitor the machines and/or processes and determine when
maintenance is required.
Predictive Maintenance systems can be put together with machine and/or deep learning
algorithms since they can be trained with high volumes of historical data and provide diagnosis,
detect and classify anomalies, and estimate the lifetime of a machine/process.
This dissertation contributes to developing a predictive maintenance system for railway
tracks/infrastructure. The main objectives are to detect and classify anomalies in the railway
track. To achieve this, unsupervised and semi-supervised algorithms are tested and tuned to
determine the one that best adapts to the presented scenario. The algorithms need to be
unsupervised and semi-supervised given the few anomalous labels in the dataset
Development of a localized drug delivery system with chitosan and ferromagnetic nanoparticles produced by wetspinning for cancer treatment
Dissertação de mestrado integrado em Biomedical Engineering Biomaterials, Rehabilition and BiomechanicsO cancro é uma das principais causas de mortalidade no mundo, depois das doenças cardiovasculares. Assim, há uma necessidade urgente de desenvolver novas estratégias para melhorar a eficácia da prevenção, diagnóstico e tratamento desta patologia.
Uma das terapias estudadas para tal na atualidade é a hipertermia. Este procedimento baseia- -se no aumento da temperatura para 40-43 °C do tecido corporal para danificar e destruir as células cancerígenas ou torná-las mais suscetíveis aos efeitos de outras terapias como a radio ou a quimioterapia. Recentemente, e devido aos avanços científicos na área da nanotecnologia, surge o conceito de hipertermia magnética que se baseia na utilização de nanopartículas magnéticas. Apesar das diversas vantagens de utilizar nanopartículas, estas podem apresentar alguma dificuldade em alcançar os tecidos específicos a tratar e, devido ao comum processo de formação de agregados, a sua aplicabilidade pode tornar-se difícil. No sentido de ultrapassar estas limitações, as nanopartículas podem ser incorporadas em sistemas de entrega localizada, nomeadamente compostos por fibras, capazes de atuar como agentes terapêuticos localizados.
Sendo assim, este trabalho teve como objetivo desenvolver novos filamentos por wetspinning capazes de serem utilizados na construção e design de estruturas fibrosas de entrega localizada de agentes bioativos.
Vários parâmetros da técnica de wetspinning foram otimizados para obter os melhores filamentos, sendo que os melhores foram obtidos utilizando uma taxa de fluxo de 1 mL/min, uma agulha de 0.41 mm de diâmetro e um banho de coagulação de 1M de hidróxido de sódio. As fibras obtidas com melhores resultados mecânicos foram produzidas com uma formulação otimizada de 3 %(m/v) de óxido de polietileno com 3 %(m/v) de quitosano em 12 %(v/v) de ácido acético. O banho de crosslinking também foi otimizado, sendo que se utilizou um banho de trifosfato de sódio a 1 %(v/v) durante 4 horas. As nanopartículas ferromagnéticas foram então sintetizadas usando o método de co-precipitação e foram introduzidas nas fibras em diferentes percentagens. Os filamentos funcionalizados com 2 %(m/v) de nanopartículas mostraram o melhor desempenho mecânico, melhorando também a estabilidade estrutural do sistema ao longo do tempo.Cancer is one of the leading causes of mortality in the world, after cardiovascular diseases. Thus, there is an urgent need to develop new strategies to improve the effectiveness of prevention, diagnosis and treatment of this disease.
One of the therapies currently being studied for this is hyperthermia. This procedure is based on increasing the temperature to 40-43 °C of the body tissue to damage and destroy cancer cells or make them more susceptible to the effects of other therapies such as radio or chemotherapy. Recently, and due to scientific advances in nanotechnology, the concept of magnetic hyperthermia has emerged, which is based on the use of magnetic nanoparticles. Despite the several advantages of using nanoparticles, these may present some difficulty in reaching the specific tissues to be treated and, due to the common process of aggregate formation, their applicability may become difficult. In order to overcome these limitations, nanoparticles can be incorporated into localized delivery systems, namely composed of fibres, capable of acting as localized therapeutic agents.
Therefore, this work aimed to develop new filaments by wetspinning that can be used in the construction and design of fibrous structures for localized delivery of bioactive agents.
Several parameters of the wetspinning technique were optimized to obtain the best filaments, and the best ones were obtained using a flow rate of 1 mL/min, a 0.41 mm diameter needle, and a 1M sodium hydroxide coagulation bath. The fibres obtained with the best mechanical results were produced with an optimized formulation of 3 %(w/v) polyethylene oxide with 3 %(w/v) chitosan in a 12 %(v/v) acetic acid aqueous solution. The crosslinking bath was also optimized, and a 1 %(v/v) sodium triphosphate bath was used for 4 hours. Ferromagnetic nanoparticles were then synthesized using the co-precipitation method and were introduced into the fibres in different percentages. The filaments functionalized with 2 %(w/v) nanoparticles showed the best mechanical performance, also improving the structural stability of the system over time
Biophysical characterization of a novel membrane mimicking system
NADH: ubiquinone oxidoredutase is one of the most intricate enzymes. It is part of the respiratory chains and is present in several organisms. This enzyme couples the transfer of two electron, from NADH to ubiquinone, and translocate ions across the membrane establishing a transmembrane difference of electrochemical potential. In this work, we present the initial experimental steps to express and purify a subunit from this intricate enzyme, which is directly related with the translocation of ions (i.e. NuoL). The objective was to characterize the subunit reconstituted in membrane-mimicking nanoparticles called nanodiscs using solution Nuclear Magnetic Resonance spectroscopy

Web-based Interface for Tailored Time Series Analysis and Visualization
In the field of movement disorders there is a clear need for more accurate and insightful
metrics to support clinicians in their decision-making process. These metrics should be
easy to understand and get presented to the clinician in an intuitive way, to avoid creating
a wall between clinicians and their patients. This could happen if the information
system that presents these metrics requires too much interaction, if it doesn’t show the
relevant data for each situation or even by delaying the information collection and its
processing. Clinical Decision Support Information Systems have been growing in usage
and usefulness, but still, there are cases of miss-implementations, that don’t put the user
in the first place, or that don’t take into consideration their workflows. This leads to less
effective health care and poorly spent resources.
With this project we will produce a customizable web interface, to integrate such a system,
that must have clinicians and their needs as the first priority. This interface has to be
fast and intuitive, while, at the same time, present useful and clinically relevant metrics.
Additional information will be presented as optional and non-intrusive, not requiring
any input from the user unless he wants to interact with it
