Differential rotation of nonlinear r-modes
Differential rotation of r-modes is investigated within the nonlinear theory
up to second order in the mode amplitude in the case of a slowly-rotating,
Newtonian, barotropic, perfect-fluid star. We find a nonlinear extension of the
linear r-mode, which represents differential rotation that produces large scale
drifts of fluid elements along stellar latitudes. This solution includes a
piece induced by first-order quantities and another one which is a pure
second-order effect. Since the latter is stratified on cylinders, it cannot
cancel differential rotation induced by first-order quantities, which is not
stratified on cylinders. It is shown that, unlikely the situation in the
linearized theory, r-modes do not preserve vorticity of fluid elements at
second-order. It is also shown that the physical angular momentum and energy of
the perturbation are, in general, different from the corresponding canonical
quantities.Comment: 9 pages, revtex4; section III revised, comments added in Introduction

Hartree-Fock-Bogoliubov theory versus local-density approximation for superfluid trapped fermionic atoms
We investigate a gas of superfluid fermionic atoms trapped in two hyperfine
states by a spherical harmonic potential. We propose a new regularization
method to remove the ultraviolet divergence in the Hartree-Fock-Bogoliubov
equations caused by the use of a zero-range atom-atom interaction. Compared
with a method used in the literature, our method is simpler and has improved
convergence properties. Then we compare Hartree-Fock-Bogoliubov calculations
with the semiclassical local-density approximation. We observe that for systems
containing a small number of atoms shell effects, which cannot be reproduced by
the semiclassical calculation, are very important. For systems with a large
number of atoms at zero temperature the two calculations are in quite good
agreement, which, however, is deteriorated at non-zero temperature, especially
near the critical temperature. In this case the different behavior can be
explained within the Ginzburg-Landau theory.Comment: 12 pages, 8 figures, revtex; v2: references and clarifying remarks

Nariai, Bertotti-Robinson and anti-Nariai solutions in higher dimensions
We find all the higher dimensional solutions of the Einstein-Maxwell theory
that are the topological product of two manifolds of constant curvature. These
solutions include the higher dimensional Nariai, Bertotti-Robinson and
anti-Nariai solutions, and the anti-de Sitter Bertotti-Robinson solutions with
toroidal and hyperbolic topology (Plebanski-Hacyan solutions). We give explicit
results for any dimension D>3. These solutions are generated from the
appropriate extremal limits of the higher dimensional near-extreme black holes
in a de Sitter, and anti-de Sitter backgrounds. Thus, we also find the mass and
the charge parameters of the higher dimensional extreme black holes as a
function of the radius of the degenerate horizon.Comment: 10 pages, 11 figures, RevTeX4. References added. Published versio
Impact and limitations of 3D computational modelling in transcatheter mitral valve replacement—a two-centre Dutch experience
Background: Transcatheter mitral valve replacement (TMVR) has emerged as a minimally invasive alternative to mitral valve surgery for patients at high or prohibitive operative risk. Prospective studies reported favourable outcomes in patients with annulus calcification (valve-in-mitral annulus calcification; ViMAC), failed annuloplasty ring (mitral valve-in-ring; MViR), and bioprosthetic mitral valve dysfunction (mitral valve-in-valve; MViV). Multi-slice computed tomography (MSCT)-derived 3D-modelling and simulations may provide complementary anatomical perspectives for TMVR planning. Aims: We aimed to illustrate the implementation of MSCT-derived modelling and simulations in the workup of TMVR for ViMAC, MViR, and MViV. Methods: For this retrospective study, we included all consecutive patients screened for TMVR and compared MSCT data, echocardiographic outcomes and clinical outcomes. Results: Sixteen out of 41 patients were treated with TMVR (ViMAC n = 9, MViR n = 3, MViV n = 4). Eleven patients were excluded for inappropriate sizing, 4 for anchoring issues and 10 for an unacceptable risk of left ventricular outflow tract obstruction (LVOTO) based on 3D modelling. There were 3 procedure-related deaths and 1 non-procedure-related cardiovascular death during 30 days of follow-up. LVOTO occurred in 3 ViMAC patients and 1 MViR patient, due to deeper valve implantation than planned in 3 patients, and anterior mitral leaflet displacement with recurrent basal septum thickening in 1 patient. TMVR significantly reduced mitral mean gradients as compared with baseline measurements (median mean gradient 9.5 (9.0–11.5) mm Hg before TMVR versus 5.0 (4.5–6.0) mm Hg after TMVR, p = 0.03). There was no residual mitral regurgitation at 30 days. Conclusion: MSCT-derived 3D modelling and simulation provide valuable anatomical insights for TMVR with transcatheter balloon expandable valves in ViMAC, MViR and MViV. Further planning iterations should target the persistent risk for neo-LVOTO.</p
Spectral analysis-based risk score enables early prediction of mortality and cerebral performance in patients undergoing therapeutic hypothermia for ventricular fibrillation and comatose status
Background: Early prognosis in comatose survivors after cardiac arrest due to ventricular fibrillation (VF) is unreliable, especially in patients undergoing mild hypothermia. We aimed at developing a reliable risk-score to enable early prediction of cerebral performance and survival.
Methods: Sixty-one out of 239 consecutive patients undergoing mild hypothermia after cardiac arrest, with eventual return of spontaneous circulation (ROSC), and comatose status on admission fulfilled the inclusion criteria. Background clinical variables, VF time and frequency domain fundamental variables were considered. The primary and secondary outcomes were a favorable neurological performance (FNP) during hospitalization and survival to hospital discharge, respectively. The predictive model was developed in a retrospective cohort (n = 32; September 2006 September 2011, 48.5 ± 10.5 months of follow-up) and further validated in a prospective cohort (n = 29; October 2011 July 2013, 5 ± 1.8 months of follow-up).
Results: FNP was present in 16 (50.0%) and 21 patients (72.4%) in the retrospective and prospective cohorts, respectively. Seventeen (53.1%) and 21 patients (72.4%), respectively, survived to hospital discharge. Both outcomes were significantly associated (p < 0.001). Retrospective multivariate analysis provided a prediction model (sensitivity = 0.94, specificity = 1) that included spectral dominant frequency, derived power density and peak ratios between high and low frequency bands, and the number of shocks delivered before ROSC. Validation on the prospective cohort showed sensitivity = 0.88 and specificity = 0.91. A model-derived risk-score properly predicted 93% of FNP. Testing the model on follow-up showed a c-statistic ≥ 0.89.
Conclusions: A spectral analysis-based model reliably correlates time-dependent VF spectral changes with acute cerebral injury in comatose survivors undergoing mild hypothermia after cardiac arrest.the CNIC is supported by the Spanish Ministry of Economy and Competitiveness and the Pro-CNIC Foundation.
