    Telemedicina e sua relação com comunicação, tecnologia e convergência

    Este artigo discorre sobre o papel do profissional jornalista em promover a informação em saúde, principalmente em serviço de telemedicina. Questiona-se, via cultura da convergência, como a comunicação pode contribuir no serviço de telemedicina? Como a inteligência coletiva por meio de experiências reais de vida pode colaborar na convergência da medicina tradicional para a telemedicina? Em paralelo a visão defendida por Henry Jenkins (2009) quando define meios de comunicação, observa-se uma mudança de protocolos das pessoas que buscam esclarecimentos sobre saúde. Supõe-se que essas mudanças de protocolo contribuem para desdobramentos de tratamentos e da saúde geral dos pacientes. Este trabalho resume por meio de pesquisa bibliográfica algumas ideias de autores que abordam sobre a telemedicina. Espera-se, pela cultura da convergência, a autonomia do paciente, como por exemplo, em escolher qual forma deseja ser consultado, qual método que lhe oferece menor risco e maior benefício de saúde

    Visceral leishmaniasis (VL) is a zoonosis with high morbidity and mortality. Endemic in tropical and subtropical regions. Autoimmune Hemolytic Anemia (AIHA) is a clinical condition in which autoantibodies bind to antigens located on the erythrocyte surface. The coexistence of the hematological picture of AIHA is rarely observed in patients with VL. The symptomatic manifestation of the case can become a dilemma, as the evolution to a more severe condition, due to intrinsic factors of the patient. In this report, we present the case of a patient with VL infection associated with a hematological picture of AIHA, with clinical manifestations of asthenia, weight loss, pyrexia, night sweats, jaundice, choluria, faecal acholia, nausea, vomiting, pancytopenia and hepatosplenomegaly. This article aims to present a case report, as a form of alert to health authorities, about the clinical-epidemiological impact of the VL-AIHA association in the area of circulation of the VL transmitting vector

The illness has a significant impact on the quality of life of the affected person. Since a disease causes intense and prolonged symptoms of polyarthralgia and myalgia, it requires health care, during a recovery, more than other arboviruses. The objective of this study was to study clinicians and clinicians suggestive of FCHIK, residing in the States of Amapá and Goiás, aiming to correlate the results of laboratory tests with the presented symptomatology. Materials and methods: The study was carried out at the Center for Immunological Studies and Research of the Pontifical Catholic University of Goiás, Goiânia, and in Emergency Care Units in the cities of Macapá, Oiapoque and Santana-AP. The study population consisted of 80 individuals with suspected FCHIK and for investigators of inflammatory markers, the control group consisted of 20 blood samples from healthy donors from Goiana Central de Serologia e Imunohematologia. Viral RNA extraction was performed, followed by RNA detection by Real-Time Polymerase Chain Reaction. In addition to ELISA for detection of IgM and IgG against Chikungunya virus. Participants symptoms were correlated with serology and Creactive protein (CRP), which was evaluated in healthy subjects and in people with FCHIK. Results: No data presented for detection of viral RNA by RT-qPCR for CHIKV, but three samples were positive in this technique for zika virus and one for dengue subtype 1 (DENV1). In an enzyme-linked immunosorbent assay, 26 samples were positive for IgG and 3 for IgM. Regarding the stage of the disease, 10 were in the acute phase, 04 in the subacute phase and 12 in the chronic phase. Correlated the results of the serology with a symptomatology it was observed that the acute phase, all have fever, 90% headache, 70% arthralgia and 60% edema. (100%), myalgia and edema (75%). (100%), arthralgia (92%) and myalgia (75%). When comparing participants with negative serology, n = 54, the most prevalent symptoms were rash, headache, fever, and arthralgia. The CRP levels in individuals infected with more than four symptoms were higher when compared with healthy individuals. Conclusion: The study focused on people with a clinical picture characteristic of FCHIK. The most common symptom in the three phases presented for arthralgia, followed by edema and myalgia, a fever was frequent only in the acute phase. All participants were negative in the evaluation of viral RNA by RT-qPCR for CHIKV, for the virus has a short duration in the body, and this methodology is limited to the time of symptom onset and sample collection, DENV and ZIKV. CRP levels have been shown to be high relative to healthy subjects.Introdução: A Febre Chikungunya (FCHIK) é uma doença de início abrupto, transmitida por mosquitos artrópodes hospedeiros intermediários do vírus Chikungunya (CHIKV). A enfermidade representa um significativo impacto na qualidade de vida da pessoa afetada. Uma vez que a doença causa sintomas intensos e prolongados de poliartralgia e mialgia, requerendo atenção de saúde, durante a recuperação, mais do que outras arboviroses. Objetivou-se estudar aspectos clínicos e sorológicos de indivíduos apresentando quadro clínico sugestivo de FCHIK, residentes nos Estados de Amapá e Goiás, visando correlacionar os resultados de testes laboratoriais com a sintomatologia apresentada. Materiais e métodos: O estudo foi realizado no Núcleo de Estudos e Pesquisa Imunológica da Pontifícia Universidade Católica de Goiás, em Goiânia, e em Unidades de Pronto Atendimento de Saúde das cidades de Macapá, Oiapoque e Santana-AP. A população de estudo foi constituída de 80 indivíduos com suspeita de FCHIK e para comparar os marcadores inflamatórios, o grupo controle foi constituído de 20 amostras de sangue de doadores saudáveis da Central Goiana de Sorologia e Imunohematologia. Foi realizada a extração do RNA viral, seguido de detecção do RNA por meio de Reação em Cadeia de Polimerase em Tempo Real. Além de ELISA para detecção de IgM e IgG específicos para o CHIKV. Os sintomas dos participantes foram correlacionados com o resultado da sorologia e da proteína C reativa (PCR), que foi avaliada em indivíduos saudáveis e em pessoas com FCHIK. Resultados: Nenhuma amostra apresentou limiar de detecção do RNA viral por RT-qPCR para CHIKV, porém três amostras foram positivas nessa técnica para vírus zika (ZIKV) e uma para dengue subtipo 1 (DENV1). Em ensaio imunoenzimático, 26 amostras foram positivas para IgG e 3 dessas para IgM. Em relação ao estágio da doença, 10 encontravam-se em fase aguda, 04 em fase subaguda e 12 em fase crônica. Correlacionados os resultados da sorologia com a sintomatologia observou-se que os de fase aguda, todos tiveram febre, 90% cefaleia, 70% artralgia e 60% edema. Enquanto que, os de fase subaguda tiveram: artralgia e cefaleia (100%), mialgia e edema (75%). Os de fase crônica tiveram edema (100%), artralgia (92%) e mialgia (75%). Quando comparados os participantes com sorologia negativa, n=54, os sintomas mais apresentados foram exantema, cefaleia, febre e artralgia. Os níveis de PCR nos indivíduos infectados e que apresentavam mais de quatro sintomas foram maiores quando comparados com indivíduos saudáveis. Conclusão: O estudo focou em pessoas com quadro clínico característico para FCHIK. O sintoma mais comum nas três fases apresentadas foi a artralgia, seguido de edema e mialgia, a febre foi frequente somente na fase aguda. Todos os participantes foram negativos na avaliação do RNA viral por RT-qPCR para CHIKV, pois o vírus tem uma curta duração no organismo, e esta metodologia é limitada ao tempo de início dos sintomas e coleta de amostra, ainda assim foi encontrado RNA viral do DENV e ZIKV. Alguns participantes foram positivos para sorologia IgG. Aqueles com sorologia negativa para CHIKV, apesar de terem dor nas articulações, tinham sintomas comuns a outras arboviroses.     Secondary Metabolites Found among the Species <i>Trattinnickia rhoifolia</i> Willd

    Plant-derived products may represent promising strategies in the treatment of Neglected Tropical Diseases (NTDs). From this perspective, it is observed that the Amazon phytogeographic region contains the tribe Canarieae of the Burseraceae family, composed of trees and shrubs supplied with resin channels. Its uses in folk medicine are related to aromatic properties, which have numerous medicinal applications and are present in reports from traditional peoples, sometimes as the only therapeutic resource. Despite its economic and pharmacological importance in the region, and although the family is distributed in all tropical and subtropical regions of the world, most of the scientific information available is limited to Asian and African species. Therefore, the present work aimed to review the secondary metabolites with possible pharmacological potential of the species Trattinnickia rhoifolia Willd, popularly known as “Breu sucuruba”. To this end, an identification key was created for chemical compounds with greater occurrence in the literature of the genus Trattinnickia. The most evident therapeutic activities in the consulted studies were antimicrobial, antioxidant, anti-inflammatory, antiviral, antifungal, anesthetic and antiparasitic. An expressive chemical and pharmacological relevance of the species was identified, although its potential is insufficiently explored, mainly in the face of the NTDs present in the Brazilian Amazon

    Acute Toxicity and Anti-Inflammatory Activity of Trattinnickia rhoifolia Willd (Sucuruba) Using the Zebrafish Model

    The species Trattinnickia rhoifolia Willd, (T.&nbsp;rhoifolia), which belongs to the Burseraceae family, is widely used in ethnopharmacological cultural practices by traditional Amazonian people for anti-inflammatory purposes, sometimes as their only therapeutic resource. Although it is used in teas, infusions, macerations and in food, the species is still unexplored in regard to its pharmacophoric potential and chemical profile. Therefore, the aim of this study was to conduct a phytochemical characterization of the hydroethanolic extract of T. rhoifolia leaves (HELTr) and to evaluate the acute toxicity and anti-inflammatory activity of this species using zebrafish (Danio rerio). The extract was analyzed by gas chromatography&ndash;mass spectrometry (GC-MS). The evaluation of the acute toxicity of the HELTr in adult zebrafish was determined using the limit test (2000 mg/kg), with behavioral and histopathological evaluations, in addition to the analysis of the anti-inflammatory potential of HELTr in carrageenan-induced abdominal edema, followed by the use of the computational method of molecular docking. The phytochemical profile of the species is chemically diverse, suggesting the presence of the fatty acids, ester, alcohol and benzoic acid classes, including propanoic acid, ethyl ester and hexadecanoic acid. In the studies of zebrafish performed according to the index of histopathological changes (IHC), the HELTr did not demonstrate toxicity in the behavioral and histopathological assessments, since the vital organs remained unchanged. Carrageenan-induced abdominal edema was significantly reduced at all HELTr doses (100, 200 and 500 mg/kg) in relation to the negative control, dimethyl sulfoxide (DMSO), while the 200 mg/kg dose showed significant anti-inflammatory activity in relation to the positive control (indomethacin). With these activities being confirmed by molecular docking studies, they showed a good profile for the inhibition of the enzyme Cyclooxygenase-2 (COX-2), as the interactions established at the sites of the receptors used in the docking study were similar to the controls (RCX, IMN and CEL). Therefore, the HELTr has an acceptable degree of safety for acute toxicity, defined in the analysis of behavioral changes, mortality and histopathology, with a significant anti-inflammatory action in zebrafish at all doses, which demonstrates the high pharmacophoric potential of the species. These results may direct future applications and drug development but still require further elucidation

    Acute Zika Virus Infection in an Endemic Area Shows Modest Proinflammatory Systemic Immunoactivation and Cytokine-Symptom Associations

    An early immune response to Zika virus (ZIKV) infection may determine its clinical manifestation and outcome, including neurological effects. However, low-grade and transient viremia limits the prompt diagnosis of acute ZIKV infection. We have investigated the plasma cytokine, chemokine, and growth factor profiles of 36 individuals from an endemic area displaying different symptoms such as exanthema, headache, myalgia, arthralgia, fever, hyperemia, swelling, itching, and nausea during early-phase infection. These profiles were then associated with symptoms, revealing important aspects of the immunopathophysiology of ZIKV infection. The levels of some cytokines/chemokines were significantly higher in acute ZIKV-infected individuals compared to healthy donors, including interferon (IFN) gamma-induced protein 10 (IP-10), regulated on activation, normal T cell expressed and secreted (RANTES), IFN-γ, interleukin (IL)-9, IL-7, IL-5, and IL-1ra, including some with predominantly immunoregulatory activity. Of note, we found that higher levels of IP-10 and IL-5 in ZIKV-infected individuals were strongly associated with exanthema and headache, respectively. Also, higher levels of IL-1ra were associated with subjects with arthralgia, whereas those with fever showed lower levels of granulocyte-colony stimulating factor (G-CSF). No correlation was observed between the number of symptoms and ZIKV viral load. Interestingly, only IP-10 showed significantly decreased levels in the recovery phase. In conclusion, our results indicate that acute ZIKV infection in a larger cohort resident to an endemic area displays a modest systemic immune activation profile, involving both proinflammatory and immunoregulatory cytokines and chemokines that could participate of virus control. In addition, we showed that differential cytokine/chemokine levels are related to specific clinical symptoms, suggesting their participation in underlying mechanisms
