    Central and mixed venous oxygen saturation in septic shock: is there a clinically relevant difference?

    INTRODUCTION: Central venous oxygen saturation (SvcO2) has been proposed as an alternative for mixed venous oxygen saturation (SvO2), with a variable level of acceptance according to available data. This study aimed to evaluate possible differences between SvO2 and SvcO2 or atrial venous saturation (SvaO2), with emphasis on the role of cardiac output and their impact on clinical management of the septic patient. METHODS: This is an observational, prospective study of patients with septic shock monitored by pulmonary artery catheter. Blood was obtained simultaneously for SvcO2, SvO2 and SvaO2 determination. Linear correlation (significant if p<0.05) and agreement analysis (Bland-Altman) were performed with samples and subgroups according to cardiac output. Moreover, agreement about clinical management based on these samples was evaluated. RESULTS: Sixty one measurements from 23 patients were obtained, median age of 65.0 (49.0-75.0) years and mean APACHE II of 27.7±6.3. Mean values of SvO2, SvcO2 and SvaO2 were 72.20±8.26%, 74.61±7.60% and 74.64±8.47%. Linear correlation test showed a weak correlation between SvO2 and SvcO2 (r=0.61, p<0.0001) and also between SvO2 and SvaO2 (r=0.70, p<0.0001). Agreements between SvcO2/SvO2 and SvaO2/SvO2 were -2.40±1.96 (-16.20 and 11.40) and -2.40±1.96 (-15.10 and 10.20), respectively, with no difference in the cardiac output subgroups. No agreement was found in clinical management for 27.8% of the cases, both for SvcO2/SvO2 and for SvaO2/SvO2. CONCLUSION: This study showed that the correlation and agreement between SvO2 and SvcO2 is weak and may lead to different clinical management.INTRODUÇÃO: A medida da saturação venosa central de oxigênio (SvcO2) tem sido proposta como alternativa a saturação venosa mista (SvO2), com grau de concordância variável nos dados atualmente disponíveis. Esse estudo objetivou avaliar as possíveis diferenças entre a SvO2 e a SvcO2 ou saturação venosa atrial de oxigênio (SvaO2), com ênfase na interferência do débito cardíaco, e o impacto delas no manejo clínico do paciente séptico. MÉTODOS: Estudo prospectivo observacional em pacientes com choque séptico monitorizados com cateter de artéria pulmonar. Foi obtido sangue simultaneamente para determinação da SvcO2, SvO2 e SvaO2. Realizado testes de correlação linear (significativos se p<0,05) e análise de concordância (Bland-Altman) entre as amostras e nos subgupos de débito cardíaco. Além disso, foi avaliada a concordância entre condutas clínicas baseadas nessas medidas. RESULTADOS: Foram obtidas 61 medidas de 23 pacientes, mediana de idade de 65,0 (49,0-75,0) anos, APACHE II médio de 27,7±6,3. Os valores médios encontrados foram 72,20±8,26%, 74,61±7,60% e 74,64±8,47% para SvO2, SvcO2 e SvaO2. O teste de correlação linear mostrou baixa correlação tanto entre a SvO2 e a SvcO2 (r=0,61, p<0,0001) quanto entre a SvO2 e a SvaO2 (r=0,70, p<0,0001). As concordâncias entre SvcO2/SvO2 e SvaO2/SvO2 foram, respectivamente, de -2,40±1,96 (-16,20 e 11,40) e -2,40±1,96 (-15,10 e 10,20), sem diferença nos subgrupos de débito cardíaco. Não houve concordância na conduta clínica em 27,8% dos casos, tanto entre SvcO2/SvO2 como de SvaO2/SvO2.     Catálogo Taxonômico da Fauna do Brasil: setting the baseline knowledge on the animal diversity in Brazil

    The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. The importance of the CTFB is evidenced by the elevated number of citations in the scientific literature in diverse areas of biology, law, anthropology, education, forensic science, and veterinary science, among others

    Digital Communication and a Concern with the Community: A Case Study in a Cooperative Credit Araguaina, Brazil

    This article seeks to address the attributes of Digital Communication used by a cooperative of Credit to meet the seventh cooperative principle of Concern for Community. The same occurred in a credit union in the city of Araguaina Tocantins, with the aim of identifying the digital communication developed by the cooperative. The article begins with a literature review on the emergence of cooperative, then the conceptualization of the credit union and its history in Brazil, then the cooperative principles and their evolution, conceptualizations of concern for the community, digital communication, social marketing and the presentation and analysis of results. The methodology was based on the case study, which according to Yin (2001) it is important because it is the reality studied. The research showed that the performance of the cooperative is geared entirely to their members and do not have a lot of focus related to social actions, but it promotes actions to attract members and to promote community development through digital media. Another important result of the research was that the principle addressed is one of the goals of the cooperative that aims to meet the interests, promote the welfare of members and the entire community where the cooperative operates

O artigo inicia-se com uma revis&atilde;o liter&aacute;ria sobre o surgimento do cooperativismo, depois a conceitua&ccedil;&atilde;o do cooperativismo de cr&eacute;dito e a sua hist&oacute;ria no Brasil, em seguida os princ&iacute;pios cooperativistas e sua evolu&ccedil;&atilde;o, conceitua&ccedil;&otilde;es sobre a preocupa&ccedil;&atilde;o com a comunidade, a comunica&ccedil;&atilde;o digital, o marketing social e a apresenta&ccedil;&atilde;o e an&aacute;lise dos resultados. A metodologia adotada foi embasada no estudo de caso, que segundo Yin (2001) o mesmo &eacute; importante por se tratar da realidade pesquisada.     CONTEXT AND OBJECTIVE: Intrahospital transportation of mechanically ventilated patients is a high-risk situation. We aimed to determine whether transfers could be safely performed by using a transportation routine. DESIGN AND SETTING: Prospective cohort study with before and after evaluation. METHODS: Mechanically ventilated patients who needed transportation were included. Hemodynamic and respiratory parameters were measured before and after transportation. Statistical analysis consisted of variance analysis and paired Student's t test. Results were considered significant if P 5, FiO2 > 0.4 and vasoactive drug use comprised 42.4%, 24.3%, 21.6% and 33.0% of cases, respectively. Mean duration of transportation was 43.4 ± 18.9 minutes. Complications occurred in 32.4%. There was a significant increase in CO2 (before transportation, 29.6 ± 7.3 and after transportation, 34.9 ± 7.0; P = 0.000); a trend towards improved PO2/FiO2 ratio (before transportation, 318.0 ± 137.0 and after transportation, 356.8 ± 119.9; P = 0.053); increased heart rate (before transportation, 80.9 ± 18.7 and after transportation, 85.5 ± 17.6; P = 0.08); and no significant change in mean arterial blood pressure (P = 0.93). CONCLUSION: These results suggest that intrahospital transportation can be safely performed. Our low incidence of complications was possibly related to both the presence of a multidisciplinary transportation team and proper equipment.CONTEXTO E OBJETIVO: O transporte intra-hospitalar de pacientes sob ventilação mecânica (VM) é uma situação sabidamente de alto risco. Nosso objetivo foi determinar se o transporte poderia ser realizado com segurança, seguindo uma rotina de transporte. TIPO DE ESTUDO E LOCAL: Estudo prospectivo, de coorte único com análise antes e depois. MÉTODOS: Foram incluídos pacientes sob VM que necessitaram de transporte. Os parâmetros hemodinâmicos e respiratórios foram medidos antes (AT) e após (DT) o retorno à Unidade de Terapia Intensiva. Utilizou-se análise de variância e o teste t de Student. Os resultados foram considerados significativos se P 5 cmH2O, FiO2 > 0,4 ou em uso de drogas vasoativas compreenderam 42,4%, 24,3%, 21,6% e 33,0% dos casos, respectivamente. A duração média do transporte foi de 43,4 ± 18,9 min. Complicações ocorreram em 32,4% dos casos. Houve aumento significativo no CO2 (AT - 29,59 ± 7,27 e DT - 34,95 ± 7,01, P = 0,000), tendência na melhora da relação PO2/FiO2 após o transporte (AT- 318,0 ± 137,0 e DT- 356,8 ± 119,9, P = 0,053) e aumento na freqüência cardíaca (AT- 80,9 ± 18,7 e DT- 85,5 ± 17.6, P = 0,08), sem alterações significativas na pressão arterial média (P = 0,93). CONCLUSÃO: Estes resultados sugerem que o transporte intra-hospitalar pode ser executado com segurança.     Effects of Transcranial Direct Current Stimulation Combined with Treadmill Training on Kinematics and Spatiotemporal Gait Variables in Stroke Survivors: A Randomized, Triple-Blind, Sham-Controlled Study

    The present study assessed the effects of anodal transcranial direct current stimulation (tDCS) combined with treadmill training on spatiotemporal and kinematic variables in stroke survivors using gait speed as the primary outcome. A randomized, sham-controlled, triple-blind, study was conducted involving 28 patients with hemiparesis allocated to two groups. The experimental group was submitted to treadmill training combined with anodal tDCS over the primary motor cortex (M1) of the damaged hemisphere. The control group was submitted to treadmill training combined with sham tDCS. Stimulation was administered (2 mA, 20 min) five times a week for two weeks during treadmill training. No significant differences (p > 0.05) in spatiotemporal variables were found in the intra-group and inter-group analyses. However, the experimental group demonstrated improvements in kinematic variables of the knee and ankle (p p < 0.05) with regard to the pelvis, hip and knee. Anodal tDCS over M1 of the damaged hemisphere combined with treadmill training did not affect spatiotemporal variables, but promoted improvements in kinematic variables of the pelvis, hip, knee and ankle and results were maintained one month after treatment

    A violência contra a criança e o adolescente na perspectiva de Agentes Comunitários da Saúde

    A violência intrafamiliar é aquela praticada por familiares, constituindo-se em grave violação dos direitos da criança e do adolescente. O objetivo desse estudo foi investigar se Agentes Comunitários de Saúde (ACS) reconhecem sinais de violência intrafamiliar, se fazem notificação, se conhecem fatores de risco e proteção e se na sua formação receberam informações sobre essas temáticas. Participaram do estudo 119 ACS que responderam três questionários. Constatou-se que os conhecimentos sobre sinais de violência, em especial, de violência sexual e psicológica, sobre notificação, sobre fatores de risco e proteção e sobre práticas parentais e habilidades sociais infantis deveriam fazer parte de cursos de capacitação dos ACS, bem como de toda a Equipe de Saúde, pelo fato de atuarem cotidianamente com as famílias no território. Conclui-se que os ACS não tiveram acesso aos conhecimentos produzidos pela psicologia do desenvolvimento infantil que poderiam orientar sua ação protetiva e preventiva junto às famílias
