    Avaliação da funcionalidade de idosos com osteoartrite utilizando o Lower Extremity Functional Scale (LEFS)

    Introdução: A osteoartrite é uma doença crônico-degenerativa muito prevalente em idosos, a qual desencadeia limitações funcionais e pior qualidade de vida. Embora existam questionários para avaliação da funcionalidade destes pacientes, a busca por instrumentos específi cos para avaliação de membros inferiores apresenta relevância na prática fi sioterapêutica. Objetivos: Avaliar a correlação do Lower Extremity Functional Scale (LEFS) com a funcionalidade e componentes físicos e mentais da qualidade de vida em idosos com osteoartrite. Método: Estudo transversal, no qual a funcionalidade foi avaliada pelos questionários WOMAC, Lequesne e LEFS e a qualidade de vida foi avaliada pelo questionário SF-36. Resultados: Participaram do estudo 105 idosos (Idade: 68,80± 6,3 anos) com osteoartrite de joelho ou quadril, sendo 31homens e 74 mulheres. Não foi observada associação entre a idade (p>0,05) e o comprometimento funcional avaliado tanto pelo índice WOMAC quanto Lequesne (p>0,05) na população estudada. Foi observada correlação negativa entre o indice WOMAC e o LEFS (rS=-0,81) e o Lequesne e o LEFS (rS=-0,86). Além disso, foi verifi cada associação entre os escores do LEFS e a capacidade funcional (rS=0,80), limitação por aspectos físicos (rS=0,44), dor (rS=0,55) e estado geral de saúde (rS=0,44). Ainda, foi verifi cada correlação entre o escore do LEFS e a vitalidade (rS=0,60), aspectos sociais (rS=0,38), limitação por aspectos emocionais (rS=0,38) e saúde mental (rS=0,41). Conclusão: Concluise que o LEFS apresenta boa correlação com os índices WOMAC e Lequesne e com os componentes físicos e mentais da qualidade de vida, podendo constituir um instrumento clínico para avaliação do impacto funcional da osteoartrite em idosos. Introduction: Osteoarthritis is a chronic degenerative disease very prevalent in the elderly, which triggers functional limitations and poorer quality of life. Although there are already established instruments to evaluate the functional status of these patients, the search for specific instruments for assessment of lower limbs has relevance in physical therapy practice. Objective: This study aimed to evaluate the correlation between LEFS with functional status and physical and mental components of quality of life of older adults with osteoarthritis. Methods: At this cross-sectional study, the functional status was assessed by WOMAC, Lequesne and LEFS indexes and the quality of life was assessed by SF-36. Results: This study included 105 elderly (age: 68.80 ± 6.3 years) with osteoarthritis of the knee or hip, being 31 men and 74 women. It was not observed a correlation between age and functional status (p>0.05) analyzed by WOMAC and Lequesne indexes. However, negative correlations between WOMAC and LEFS(rS=- 0.81) as well as Lequesne and LEFS(rS= -0.86) were observed according to Spearman’s correlation. Moreover, there was a correlation between the LEFS’ score and functional capacity (rS = 0.80), limitation by physical aspects (rS= 0.44), pain (rS= 0.55) and general health (rS = 0.44). Moreover, it was also observed a positive correlation between the LEFS’ score and vitality (rS=0.60), social status (rS= 0.38), limitation for emotional aspects (rS=0.38) and mental health (rS= 0.41). Conclusion: LEFS correlates well with the WOMAC and Lequesne indexes as well as the physical and mental components of quality of life and it may be an alternative clinical tool for assessing the functional impact of osteoarthritis in the elderly

    Regulatory T Cells Phenotype in Different Clinical Forms of Chagas' Disease

    CD25High CD4+ regulatory T cells (Treg cells) have been described as key players in immune regulation, preventing infection-induced immune pathology and limiting collateral tissue damage caused by vigorous anti-parasite immune response. In this review, we summarize data obtained by the investigation of Treg cells in different clinical forms of Chagas' disease. Ex vivo immunophenotyping of whole blood, as well as after stimulation with Trypanosoma cruzi antigens, demonstrated that individuals in the indeterminate (IND) clinical form of the disease have a higher frequency of Treg cells, suggesting that an expansion of those cells could be beneficial, possibly by limiting strong cytotoxic activity and tissue damage. Additional analysis demonstrated an activated status of Treg cells based on low expression of CD62L and high expression of CD40L, CD69, and CD54 by cells from all chagasic patients after T. cruzi antigenic stimulation. Moreover, there was an increase in the frequency of the population of Foxp3+ CD25HighCD4+ cells that was also IL-10+ in the IND group, whereas in the cardiac (CARD) group, there was an increase in the percentage of Foxp3+ CD25High CD4+ cells that expressed CTLA-4. These data suggest that IL-10 produced by Treg cells is effective in controlling disease development in IND patients. However, in CARD patients, the same regulatory mechanism, mediated by IL-10 and CTLA-4 expression is unlikely to be sufficient to control the progression of the disease. These data suggest that Treg cells may play an important role in controlling the immune response in Chagas' disease and the balance between regulatory and effector T cells may be important for the progression and development of the disease. Additional detailed analysis of the mechanisms on how these cells are activated and exert their function will certainly give insights for the rational design of procedure to achieve the appropriate balance between protection and pathology during parasite infections

    Iron Behaving Badly: Inappropriate Iron Chelation as a Major Contributor to the Aetiology of Vascular and Other Progressive Inflammatory and Degenerative Diseases

    The production of peroxide and superoxide is an inevitable consequence of aerobic metabolism, and while these particular "reactive oxygen species" (ROSs) can exhibit a number of biological effects, they are not of themselves excessively reactive and thus they are not especially damaging at physiological concentrations. However, their reactions with poorly liganded iron species can lead to the catalytic production of the very reactive and dangerous hydroxyl radical, which is exceptionally damaging, and a major cause of chronic inflammation. We review the considerable and wide-ranging evidence for the involvement of this combination of (su)peroxide and poorly liganded iron in a large number of physiological and indeed pathological processes and inflammatory disorders, especially those involving the progressive degradation of cellular and organismal performance. These diseases share a great many similarities and thus might be considered to have a common cause (i.e. iron-catalysed free radical and especially hydroxyl radical generation). The studies reviewed include those focused on a series of cardiovascular, metabolic and neurological diseases, where iron can be found at the sites of plaques and lesions, as well as studies showing the significance of iron to aging and longevity. The effective chelation of iron by natural or synthetic ligands is thus of major physiological (and potentially therapeutic) importance. As systems properties, we need to recognise that physiological observables have multiple molecular causes, and studying them in isolation leads to inconsistent patterns of apparent causality when it is the simultaneous combination of multiple factors that is responsible.     Catálogo Taxonômico da Fauna do Brasil: setting the baseline knowledge on the animal diversity in Brazil

    The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. The importance of the CTFB is evidenced by the elevated number of citations in the scientific literature in diverse areas of biology, law, anthropology, education, forensic science, and veterinary science, among others

    Condições socioeconômicas estão associadas ao overbite e overjet? Uma avaliação utilizando fotografias digitais

    OBJETIVO: estimar a associação entre fatores socioeconômicos e más oclusões do tipo overbite e overjet em escolares em fase de dentição permanente jovem. MÉTODO: trata-se de um estudo transversal realizado em 251 escolares de sete a 15 anos, de ambos os sexos, regularmente matriculadas no ensino fundamental de escolas públicas e privadas do município de São Luís, Maranhão, Brasil. Todos os sujeitos da amostra realizaram fotografias extrabucais padronizadas de face (frontal em repouso, frontal sorrindo e perfil) e intrabucais (frontal, lateral direita e lateral esquerda em oclusão, oclusal superior e oclusal inferior), para o diagnóstico das alterações. As fotografias foram avaliadas por três examinadores em regime cego e as discordâncias foram discutidas até a obtenção de consenso. Overbite e overjet foram diagnosticados segundo critérios de Angle. RESULTADOS: constatou-se que 35,4% das crianças apresentaram overbite e 18,3%, overjet. Houve associação estatisticamente significante entre a renda familiar com overbite (p=0.003) e com overjet (p=0.029), observando-se que a maioria das crianças que não possuíam overbite (69,4%) era de famílias com baixa renda (até 2 salários mínimos). De modo semelhante, a maioria dos portadores de overjet era de famílias com renda mais elevada, ao passo que a maioria daqueles sem overjet tinha renda familiar baixa. Para as demais variáveis não houve diferenças estatisticamente significante na distribuição de frequência de overjet e overbite. CONCLUSÃO: a prevalência de overbite e overjet nos escolares estudados é alta, especialmente entre aqueles com maior renda familiar

    FUNDAMENTO: A hipertensão pulmonar (HP) é fator de mau prognóstico no pós-operatório de transplante cardíaco (TC) e, desta forma, o estudo do grau de reversibilidade a vasodilatadores é obrigatório durante avaliação pré-operatória. OBJETIVO: Avaliar os efeitos hemodinâmicos pulmonares e sistêmicos do sildenafil como droga vasodilatadora durante o teste de reversibilidade da HP em candidatos a transplante cardíaco. MÉTODOS: Pacientes em fila para TC foram submetidos à medida de variáveis hemodinâmicas sistêmicas e pulmonares antes e após a administração de 100mg dose única e sublingual de sildenafil, durante cateterização cardíaca direita. RESULTADOS: Quatorze pacientes (idade: 47±12 anos, 71,4% homens) com insuficiência cardíaca avançada, Fração de Ejeção (FE) 25 ± 7%, Classe Funcional (CF- NYHA) CF III - 6 e CF IV - 8, foram avaliados neste estudo. A administração aguda de sildenafil mostrou ser eficaz na redução das pressões sistólica (62,4 ± 12,1 vs. 51,5 ± 9,6 mmHg, IC=95%, p<0,05) e média (40,7 ± 7,3 vs. 33,8 ± 7,6 mmHg, IC=95%, p <0,05) da artéria pulmonar. Houve também uma redução significativa da resistência vascular pulmonar (4,2 ± 3 vs. 2,0 ± 0,9 uWood, IC=95%, p<0,05) e sistêmica (22,9 ± 6,8 vs. 18,6 ± 4,1 Wood, IC=95%, p<0,05), associada a uma elevação do débito cardíaco (3,28 ± 0,79 vs. 4,12 ±1,12 uWood, IC=95%, p<0,05) sem, no entanto, interferir de maneira significativa na pressão arterial sistêmica (87,8 ± 8,2 vs. 83,6 ± 9,1 mmHg, IC=95%, p=0,3). CONCLUSÃO: O sildenafil sublingual é uma alternativa eficaz e segura como droga vasodilatadora durante o teste de reversibilidade da HP em portadores de insuficiência cardíaca e em fila para transplante cardíaco.BACKGROUND: Pulmonary hypertension (PH) is a factor of poor prognosis in the postoperative period of heart transplant (HT) and thus, the study of the degree of reversibility to vasodilators is mandatory during the preoperative assessment. OBJECTIVE: To evaluate the pulmonary and systemic hemodynamic effects of sildenafil as a vasodilator during the PH reversibility test in patients that are candidates to HT. METHODS: Patients awaiting HT were submitted to the measurement of systemic and pulmonary hemodynamic variables before and after the administration of a single sublingual dose of 100 mg of sildenafil during right heart catheterization. RESULTS: Fourteen patients (age: 47±12 years, 71.4% men) with advanced heart failure Ejection Fraction (EF) 25 ± 7%, Functional Class (FC - NYHA) FC III - 6 and FC IV - 8, were evaluated in this study. The acute administration of sildenafil showed to be effective in decreasing the systolic (62.4 ± 12.1 vs 51.5 ± 9.6 mmHg, CI=95%, p<0.05) and mean (40.7 ± 7.3 vs 33.8 ± 7.6 mmHg, CI=95%, p <0.05) pressures of the pulmonary artery. There was also a significant decrease in the pulmonary (4.2 ± 3 vs 2.0 ± 0.9 uWood, CI=95%, p<0.05) and systemic vascular resistance (22.9 ± 6.8 vs 18.6 ± 4.1 Wood, CI=95%, p<0.05), associated to an increase in the cardiac output (3.28 ± 0.79 vs 4.12 ±1.12 uWood, CI=95%, p<0.05) without, however, significantly interfering in the systemic arterial pressure (87.8 ± 8.2 vs 83.6 ± 9.1 mmHg, CI=95%, p=0.3). CONCLUSION:The sublingual administration of sildenafil is an effective and safe alternative as a vasodilator during the PH reversibility test in patients with heart failure and awaiting a HT.FUNDAMENTO: La hipertensión pulmonar (HP) se muestra factor de mal pronóstico en el postoperatorio de transplante cardiaco (TC) y, de esta forma, el estudio del grado de reversibilidad a vasodilatadores se vuelve obligatorio durante evaluación preoperatoria. OBJETIVO: Evaluar los efectos hemodinámicos pulmonares y sistémicos del Sildenafil como droga vasodilatadora durante la prueba de reversibilidad de la HP en candidatos a transplante cardiaco. MÉTODOS: Pacientes en fila para TC fueron sometidos a la medición de variables hemodinámicas sistémicas y pulmonares, antes y luego de la administración de 100mg en dosificación única y sublingual de Sildenafil, durante cateterización cardiaca derecha. RESULTADOS: Se evaluaron en este estudio a 14 pacientes (edad: 47±12 años, el 71,4% varones) con insuficiencia cardiaca avanzada, fracción de eyección (FE) 25 ± 7%, clase funcional (CF-NYHA) CF III - 6 y CF IV - 8. La administración aguda de Sildenafil se mostró eficaz en la reducción de las presiones sistólica (62,4 ± 12,1 vs 51,5 ± 9,6 mmHg, IC=95%, p<0,05) y media (40,7 ± 7,3 vs 33,8 ± 7,6 mmHg, IC=95%, p <0,05) de la arteria pulmonar.     Laparoscopic Adrenalectomy in Children for Neuroblastoma: Report of Case Series

    Background: Neuroblastoma is one of the most common solid tumors in the pediatric population and the adrenal gland is the main abdominal site of this tumor. The laparoscopic approach has become the standard of care for most benign adrenal tumors in adults, but the role of laparoscopic adrenalectomy in children for malignant tumor is still a point of controversy. However, there is a growing experience with laparoscopic neuroblastoma resection of small lesions and the use of minimally invasive techniques for the initial management of infiltrative neuroblastoma in the last years. The aim of this study is to describe our initial experience with laparoscopic adrenalectomy for neuroblastoma in children, based on surgical outcomes. Methods: A retrospective review of 7 laparoscopic adrenalectomies performed in a single institution between October 2008 and October 2009. We focused our analysis on early surgical outcomes. Results: The mean tumoral size was 2.8 +/- 0.9 cm, the average surgical time was 38.6 +/- 65.5 minutes, and the mean hospital stay was 2.9 +/- 1.6 days. One stage IV patient was submitted to conversion due to bleeding and needed blood transfusion. There were no late complications or deaths and the mean follow-up time was 18.8 +/- 6.1 months. Conclusions: The laparoscopic approach for adrenal neuroblastoma resection is feasible in children with good outcomes, but should be reserved to patients with small, well-circumscribed adrenal lesions, without invasive or infiltrative disease

    O objetivo deste estudo é contribuir para o debate sobre a queda da desigualdade na distribuição domiciliar de renda no Brasil no início do século XXI. Assim, nessa diminuição, analisamos a participação relativa de três fontes de renda (trabalho, aposentadorias e pensões e transferências públicas) nas cinco macrorregiões brasileiras - Norte (urbana), Nordeste, Centro-Oeste, Sudeste e Sul, entre os anos de 2001 e 2006. Para atingir os propósitos, estruturamos o texto em três seções. A primeira seção apresenta aspectos selecionados da literatura especializada brasileira sobre as causas da variação da desigualdade de renda. A segunda apresenta a técnica matemática de decomposição utilizada. Finalmente, na terceira seção, analisam-se os resultados empíricos para Brasil e cinco macrorregiões selecionadas. À guisa de conclusão, destacamos, dentre os resultados, a importância das transferências públicas de renda e, principalmente, as mudanças nos mercados de trabalho regionais para a diminuição do grau de desigualdade.<br>The aim of this paper is to contribute to the debate on the decrease of the concentration degree of the family's income distribution in Brazil in the first five years of the XXI Century. On this purpose, we approach the phenomenon analyzing the importance of five types of income ( labor, property rents, donation, retirement and public cash transference) in the five macro regions in Brazil - North, Northeast, Middle-West, Southeast, South, during the period 2001-2006.To reach the purposes, the text was structured in three parts. The first one highlights stylized facts on this decreasing from the Brazilian specialized literature. The second section presents the mathematical technique used to build the decomposition. Finally, in the third section we analyze the results obtained for the five macro regions. We detached, specially, the magnitude of the employment's creation and, secondarily, the public cash transference to poor families as the main causes to explain the decline on the concentration degree of the income distribution
