    This study, drawing upon contemporary theories in the field of migration, postcolonialism, and translation, offers an analysis of literary works by Monica Ali and Jhumpa Lahiri. Ali and Lahiri epitomize second-generation immigrant literature, play with the linguistic concept of translating and interpreting as forms of hybrid connections, and are significant examples of how a text may become a space where multi-faceted identities co-habit in a process of deconstructing and reconstructing their own sense of emplacement in non-native places. Each immigrant text becomes a hybrid site, where second- and third generations of immigrant subjects move as mobile, fluctuating and impermanent identities, caught up in the act of transmitting their bicultural and bilingual experience through the use of the English language as their instrument of communication in a universe which tends to marginalize them.     A randomized controlled trial comparing behavioral, educational, and pharmacological treatments in youths with chronic tic disorder or Tourette syndrome

    Context: The existing literature on the treatment of pediatric chronic tic disorder (CTD) and Tourette syndrome (TS) indicates that both behavioral therapy (BT) and pharmacotherapy (PT) are effective for reducing symptoms. Objective: To evaluate the efficacy of BT compared to psychoeducation (PE) or PT for reducing tics and co-occurring symptoms and for improving quality of life (QoL) in a sample of youths with CTD and TS. Design: A 10 weeks, 2 sites (Catania, Rome) randomized controlled trial. Participants were randomized to receive one of the following treatments: BT, PE, or PT. Participants: 110 outpatients aged between 8 and 17 years affected by CTD or TS. Results: Patients in the BT and PT groups showed a significant reduction in the severity of tic symptoms, while the PE group did not show any improvement. PT was more effective for reducing obsessive compulsive symptoms than BT, while PE group did not show any improvement. Both BT and PT groups showed an improvement in most QoL domains, whereas no differences were found in the PE group. Conclusions: BT is as effective as pharmacological therapy in the treatment of tic disorders in children and adolescents, thus offering an alternative to medications for CTD and TS

    Universality in Accessibility for All

    Accessibility has been facing several challenges within Audiovisual Translation Studies and has also gained great opportunities for its establishment as a methodologically and theoretically well-founded discipline. Audiovisual translation modes have achieved a crucial role in the transmission of what scholarly studies have discussed in relation to media accessibility as a set of services and practices providing access to audiovisual media content for persons with sensory impairment. Today accessibility has become a concept involving more and more universality, since it is extensively contributing to the dissemination of audiovisual and visual products about issues on minorities, and also addressing all human beings, regardless of cultural and social differences. Against this theoretical backdrop, accessibility is scrutinised within the context of aesthetics of marginalisation, migration, and minorities as modalities which encourage the diffusion of 'niche' knowledge, and as universal processes of translation and interpretation that provide access to all knowledge as counter discourse. Within this framework, the ways in which language is used can be considered the beginning of a type of local grammar for interlingual translation and subtitling applied to museum contexts of marginalisation, migration and minorities. Drawing upon well-established research in the field of audiovisual translation and media accessibility, and by adopting systemic-functional and lexical-semantic methodological approaches for translation quality assessment of museum text types, this study aims to put emphasis on accessibility as a societal instrument that contributes to giving voice to minorities through knowledge dissemination in English as a lingua franca by means of aesthetic narrative types within the field of the visual arts (i.e. museum settings). In this sense, accessibility is viewed as the embodiment of universality that ensures universal access to knowledge for all citizens as a human rights principle, while acting as an agent for the democratisation and transparency of information against media discourse distortions and oversimplifications

    A density functional study of open-shell cyclopentadienyl-molybdenum(II) complexes. A comparison of stabilizing factors: Spin-pairing, Mo-X π bonding, and release of steric pressure

    The dissociation of PH3 from the 18-electron system CpMoX(PH3)3 to afford the corresponding 16-electron CpMoX(PH3)2 fragment has been investigated theoretically by density functional theory for X = H, CH3, F, Cl, Br, I, OH, and PH2. The product is found to prefer a triplet spin state for all X ligands except PH2, the singlet-triplet gap varying between 1.7 kcal/mol for OH to 8.7 kcal/mol for F. The Mo-PH3 bond dissociation energy to the 16-electron ground state varies dramatically across the series, from 4.5 kcal/mol for OH to 23.5 kcal/mol for H, and correlates with experimental observations on trisubstituted phosphine derivatives. Geometry-optimized spin doublet CpMo(PH3)3, on the other hand, has a Mo-PH3 bond dissociation energy of 24.3 kcal/mol. The modulation of the Mo-PH3 bond dissociation energy by the introduction of X is analyzed in terms of three effects that stabilize the 16-electron product relative to the 18-electron starting complex: (i) adoption of the higher (triplet) spin state by release of pairing energy; (ii) Mo-X π interactions; (iii) release of steric pressure. A computational model for the approximate separation and evaluation of these three stabilizing effects is presented. According to the results of these calculations, the relative importance of the three effects depends on various factors related to the nature of X. For double-sided π-donor X ligands, the larger triplet-singlet gap is provided by the more electronegative atoms (F \u3e CL \u3e Br \u3e I), whereas single-sided π donors favor the singlet state. The π-stabilization ability goes in the order PH2 \u3e OH \u3e F \u3e other halogens \u3e H. Finally, the major steric interaction appears to be associated with the presence of inactive lone pairs and by their orientation/proximity to the PH3 ligands (Cl, Br \u3e I, OH \u3e F, PH2, H, CH3). The 16-electron methyl system establishes a marked α-agostic interaction in the singlet state, which nevertheless remains unfavored relative to an undistorted triplet configuration

    Influence of Storage Temperature on Radiochemical Purity of 99mTc-Radiopharmaceuticals

    The influence of effective room temperature on the radiochemical purity of 99mTc-radiopharmaceuticals was reported. This study was born from the observation that in the isolators used for the preparation of the 99mTc-radiopharmaceuticals the temperatures can be higher than those reported in the commercial illustrative leaflets of the kits. This is due, in particular, to the small size of the work area, the presence of instruments for heating, the continuous activation of air filtration, in addition to the fact that the environment of the isolator used for the 99mTc-radiopharmaceuticals preparation and storage is completely isolated and not conditioned. A total of 244 99mTc-radiopharmaceutical preparations (seven different types) have been tested and the radiochemical purity was checked at the end of preparation and until the expiry time. Moreover, we found that the mean temperature into the isolator was significantly higher than 25 C, the temperature, in general, required for the preparation and storage of 99mTc-radiopharmaceuticals. Results confirmed the radiochemical stability of radiopharmaceutical products. However, as required in the field of quality assurance, the impact that different conditions than those required by the manufacturer on the radiopharmaceuticals quality have to be verified before human administration

    Standard treatment–refractory cytomegalovirus encephalitis unmasked by immune reconstitution inflammatory syndrome and successfully treated with virus‐specific hyperimmune globulin

    Objectives Cytomegalovirus (CMV)‐related encephalitis is a rare but potentially life‐threatening complication of CMV infection in immunocompromised patients. The high mortality rate is associated with deficient immune system reconstitution after hematopoietic stem cell transplant (HSCT) and poor bioavailability of antiviral drugs in cerebrospinal fluid (CSF). CMV‐related central nervous system (CNS) infection may occur with aspecific symptoms, without evidence of either blood viral load or magnetic resonance imaging (MRI) signs of encephalitis. Methods Here, we describe a 10‐year‐old girl who underwent an allogeneic HSCT and subsequently developed CMV encephalitis. Because of the absence of CMV antigen in the blood, the diagnosis of encephalitis was proposed only after a delay, following the onset of immune reconstitution inflammatory syndrome (IRIS). Two months of combined dual antiviral therapy with ganciclovir and foscarnet proved ineffective against CMV and caused significant bone marrow and renal toxicity. To avoid further toxicity, the girl was given daily treatment with CMV‐hyperimmune globulins alone. Results After three weeks, the CSF viral load dropped significantly and was undetectable within three more weeks. In the meantime, the renal impairment resolved, and there was a complete bone marrow recovery. Conclusion We suggest that this patient succeeded in achieving CMV CSF clearance with high dose of CMV‐hyperimmune globulin, given alone, because of the ability of immunoglobulins to penetrate the blood–brain barrier (BBB)

    The imbalance between Treg and Th17 cells caused by FTY720 treatment in skin allograft rejection

    OBJECTIVES: FTY720 modulates CD4+T cells by the augmentation of regulatory T cell activity, secretion of suppressive cytokines and suppression of IL-17 secretion by Th17 cells. To further understand the process of graft rejection/acceptance, we evaluated skin allograft survival and associated events after FTY720 treatment. METHODS: F1 mice (C57BL/6xBALB/c) and C57BL/6 mice were used as donors for and recipients of skin transplantation, respectively. The recipients were transplanted and either not treated or treated with FTY720 by gavage for 21 days to evaluate the allograft survival. In another set of experiments, the immunological evaluation was performed five days post-transplantation. The spleens, axillary lymph nodes and skin allografts of the recipient mice were harvested for phenotyping (flow cytometry), gene expression (real-time PCR) and cytokine (Bio-Plex) analysis. RESULTS: The FTY720 treatment significantly increased skin allograft survival, reduced the number of cells in the lymph nodes and decreased the percentage of Tregs at this site in the C57BL/6 recipients. Moreover, the treatment reduced the number of graft-infiltrating cells and the percentage of CD4+ graft-infiltrating cells. The cytokine analysis (splenocytes) showed decreased levels of IL-10, IL-6 and IL-17 in the FTY720-treated mice. We also observed a decrease in the IL-10, IL-6 and IL-23 mRNA levels, as well as an increase in the IL-27 mRNA levels, in the splenocytes of the treated group. The FTY720-treated mice exhibited increased mRNA levels of IL-10, IL-27 and IL-23 in the skin graft. CONCLUSIONS: Our results demonstrated prolonged but not indefinite skin allograft survival by FTY720 treatment. This finding indicates that the drug did not prevent the imbalance between Tr1 and Th17 cells in the graft that led to rejection
