Digital literacy linked to engagement and psychological benefits among breast cancer survivors in Internet-based peer support groups
© 2019 John Wiley & Sons Ltd Objective: Internet-based peer support groups (ISGs) represent an innovative, scalable approach to addressing information and support needs of cancer survivors. However, this innovation may not benefit survivors equally due to population variance in digital literacy. This study examined how digital literacy influences level of engagement in and psychological benefits from participating in ISGs for breast cancer (N = 183). Methods: Secondary analysis of data from a randomised trial of ISGs that included behavioural measures of engagement, subjective ratings and psychological distress symptoms. Results: Digital literacy was positively related to education level (p =.005). Relative to women with high digital literacy, those with lower digital literacy were more likely to report difficulties using the ISG and to value the user's guide and facilitator assistance (all p's <.05). Digital literacy was negatively correlated with computer anxiety pre-intervention, distress before and after online chat during the intervention and post-intervention depressive symptoms (all p's <.05). Conclusion: Low digital literacy is associated with computer anxiety and barriers to ISG use, as well as distress during and after ISG use. Digital literacy must be taken into account when designing or delivering innovative digital interventions for cancer survivors
Palaeoenvironmental reconstruction of Holocene calcareous tufas distributed over a manganese deposit in Mexico
The Holocene calcareous tufa deposits are located in the north-central sector of the state of Hidalgo, Mexico. These deposits outcrop on the Upper Jurassic Chipoco Formation in the Molango Manganese District. Calcareous tufa deposits correspond to carbonate formations of inactive tufas disconnected from the current hydrological network. To better understand the textural, geochemical, and biological characteristics and environmental conditions during their formation, a detailed study involving field analysis, X-ray diffraction analysis, X-ray fluorescence analysis, and morphometric analysis with light microscopy, and scanning electron microscopy was carried out. Similarly, 14C analysis was undertaken to determine the age of the deposit. On this basis, two biofacies were defined, one with stem structures and the other with Phyto clasts, bryophytes (Plagiomnium cuspidatum), and copepods, limited both at the base and the top by lenticular-shaped erosive surfaces and sigmoidal clinoforms. These bio facies formed in lacustrine and paludal environments, and barrier waterfalls. From a geomorphological point of view, the accumulations of these calcareous tufas originated on slopes and/or at the foot of karstic springs, where the accumulation of material originated from the height of the spring with respect to the bottom of the valley, giving rise to a set of stepped tuffaceous plains with a wedge-shaped profile, colonized by important masses of bryophyte mounds, copepods, and vascular plants. This suggests a swampy environment where sedimentation took place in shallow water accumulations, where there is clear evidence of exposure and pedogenetic modification, indicating wet and dry episodes that translate into seasonal fluctuations in the water table. Six mineral phases were observed including silicates, carbonates and arsenates. The 14C analysis indicates that the plant found in the charcoal sample within the calcareous tufa deposit is of Holocene age

Assessment of Chemical Inhibitor Addition to Improve the Gas Production from Biowaste
The coexistence of sulphate-reducing bacteria and methanogenic archaea in the reactors during the anaerobic digestion from sulphate-containing waste could favor the accumulation of sulfide on the biogas, and therefore reduce its quality. In this study, the effect of sulphate-reducing bacteria inhibitor (MoO−2
4 ) addition in a two phase system from sulphate-containing municipal solid waste to improve the quality of the biogas has been investigated. The results showed that although SRB and sulphide production decreased, the use of inhibitor was not effective to improve the anaerobic digestion in a two phase
system from sulphate-containing waste, since a significant decrease on biogas and organic matter removal were observed. Before MoO−2 4 addition the average values of volatile solid were around 12 g/kg, after 5 days of inhibitor use, those values did exceed to 28 g/kg. Molybdate caused acidification in the reactor and it was according to decrease in the pH values. In relation to microbial consortia, the effect of inhibitor was a decrease in Bacteria (44%; 60% in sulphate-reducing bacteria) and Archaea (38%) population
Microvascular function is preserved in newly diagnosed rheumatoid arthritis and low systemic inflammatory activity
Rheumatoid arthritis (RA) is associated with increased cardiovascular morbidity and mortality. Microvascular function has been linked to several risk factors for cardiovascular disease and may be affected in RA. It is, however, presently unknown at what point in the disease course the abnormalities in microvascular function occur. We determined whether microvascular function is already disturbed in early disease-modifying antirheumatic drugs (DMARD)-naive RA patients with low systemic inflammation. Fifteen consecutive RA patients with a median symptom duration of 5 months, a C-reactive protein level of ≤20 mg/l and without a history of cardiovascular disease, and age 15 and sex-matched healthy controls were recruited. Endothelium-dependent and endothelium-independent vasodilatation in skin was evaluated with laser Doppler fluxmetry after iontophoresis of acetylcholine and sodium nitroprusside, respectively. Videomicroscopy was used to measure recruitment of skin capillaries after arterial occlusion. CRP and ESR levels were mildly, but significantly elevated in patients compared to controls. No differences in both endothelium-dependent vasodilatation and capillary recruitment were observed between groups [709% (95% CI, 457–961%) vs 797% (95% CI, 556–1,037%), P = 0.59 and 37% (95% CI, 26–47%) vs 41% (95% CI, 31–50%), P = 0.59, respectively]. Skin microvascular function is preserved in early, DMARD-naive RA patients with moderately active RA but low systemic inflammatory activity. Both the extent of the systemic inflammation and disease duration, therefore, may be important determinants of microvascular dysfunction and subsequent increased risk for cardiovascular disease

Study of a nonlinear Kirchhoff equation with non-homogeneous material
In this paper we study a non-homogeneous elliptic Kirchhoff equation with nonlinear reaction term. We analyze the existence and uniqueness of positive solution. The main novelty is the inclusion of non-homogeneous term making the problem without a variational structure. Carotid artery plaque in women with rheumatoid arthritis and low estimated cardiovascular disease risk: a cross-sectional study
INTRODUCTION:
We previously reported that most patients with rheumatoid arthritis (RA) and moderate cardiovascular disease (CVD) risk according to the Systematic COronary Evaluation score (SCORE) experience carotid artery plaque. In this study, we aimed to identify patient characteristics that can potentially predict carotid plaque presence in women with RA and a concurrent low CVD risk according to the SCORE.
METHODS:
A cohort of 144 women with an evaluated low risk of CVD (SCORE value of zero) was assembled amongst 550 consecutive patients with RA that underwent CVD risk factor recording and carotid artery ultrasound. Participants had no established CVD, moderate or severe chronic kidney disease, or diabetes. We assessed carotid plaque(s) presence and its associated patient characteristics.
RESULTS:
Carotid artery plaque was present in 35 (24.3%) of women with RA. Age, the number of synthetic disease-modifying agents (DMARDs) and total cholesterol concentrations were independently associated with plaque in multivariable stepwise backward regression analysis (odds ratio (95% confidence interval)=1.15 (1.07 to 1.24), P49.5 years or/and total cholesterol concentration of >5.4 mmol/l, respectively, compared to only 7.8% in those (n=64; 44.4%) with age≤49.5 years or/and total cholesterol concentration of ≤5.4 mmol/l, respectively.
CONCLUSIONS:
Approximately one-third of women with RA who experience a low SCORE value and are aged >49.5 years or/and have a total cholesterol concentration of >5.4 mmol/l, experience high-risk atherosclerosis, which requires intensive CVD risk management
Modulating sensitivity to drug-induced apoptosis: the future for chemotherapy?
Drug resistance is a fundamental problem in the treatment of most common human cancers. Our understanding of the cellular mechanisms underlying death and survival has allowed the development of rational approaches to overcoming drug resistance. The mitogen activated protein kinase family of protein serine/threonine kinases has been implicated in this complex web of signalling, with some members acting to enhance death and other members to prevent it. A recent publication by MacKeigan et al is the first to demonstrate an enhancement of drug-induced cell death by simultaneous blockade of MEK-mediated survival signalling, and offers the potential for targeted adjuvant therapy as a means of overcoming drug resistance
