Direct and indirect orthotic management of medial compartment osteoarthritis of the knee
Osteoarthritis (OA) is a painful condition and affects approximately 80% of individuals by the age of 55 [1], with knee OA occurring two times more frequently than OA of the hand or hip [2].The condition is more prevalent in the medial compartment and restricts the daily lives of individuals due to pain and a lack of functional independence. Patients with medial compartment osteoarthritis often have a varus alignment, with the mechanical axis and load bearing passing through this compartment with a greater adduction moment leading to greater pain and progression of osteoarthritis [3]. Surgery for the condition is possible although in some cases, particularly younger patients or those not yet requiring surgery, clinical management remains a challenge. Before surgery is considered, however, conservative management is advocated, though no one treatment has been shown to be most effective, and there are few quality biomechanical or clinical studies. Of the conservative approaches the principal orthotic treatments are valgus knee braces and laterally wedged foot inlays. Studies of knee valgus bracing have consistently demonstrated an associated decreased pain and improved function [4], and greater confidence [5]. A laterally wedged foot inlay has a thicker lateral border and applies a valgus moment to the heel. It is theorised that by changing the position of the ankle and subtalar joints during weight-bearing [6] the lateral wedges may apply a valgus moment across the knee as well as the rearfoot, with the assumed reduction on load in the medial knee compartment [7]. However, there has been no study to directly compare these orthotic treatments in the same study. Water Quality, nutrients and the European Union’s Water Framework Directive in a lowland agricultural region: Suffolk, south-east England.
The water quality of 13 rivers in the lowland, agricultural county of Suffolk is investigated using routine monitoring data for the period 1981 to 2006 collected by the Environment Agency of England and Wales (EA), and its predecessors, with particular emphasis on phosphorus (as total reactive phosphorus, TRP) and total (dissolved and particulate) oxidised nitrogen (TOxN – predominantly nitrate NO3). Major ion and flow data are used to outline fundamental hydrochemical characteristics related to the groundwater provenance of base-flow waters. Relative load contributions from point and diffuse sources are approximated using Load Apportionment Modelling for both TRP and TOxN where concurrent flow and concentration data are available. Analyses indicate a mixture of point and diffuse sources of TRP, with the former being dominant during low flow periods, while for TOxN diffuse sources dominate.
Out of 59 sites considered, 53 (90%) were found to have annual average TRP concentrations greater than 0.05 mg P l-1, and 36 (61%) had average concentrations over 0.120 mg P l-1, the upper thresholds for ‘High’ and ‘Good’ ecological status, respectively. Correspondingly, for TOxN, most of the rivers are already within 70% of the 11.3 mg N l-1 threshold, with two rivers (Wang and Ore) being consistently greater than this.
It is suggested that the major challenge is to characterise and control point-source TRP inputs which, being predominant during the late spring and summer low-flow period, coincide with the peak of primary biological production, thus presenting the major challenge to achieving ‘good’ ecological status under the Water Framework Directive. Results show that considerable effort is still required to ensure appropriate management and develop tools for decision-support
The Landshoff-Nachtmann Pomeron on the Lattice
We investigate the Landshoff-Nachtmann two-gluon-exchange model of the
Pomeron using gluon propagators computed in the Landau gauge within quenched
lattice QCD calculations. We first determine an effective gluon-quark coupling
by constraining the Pomeron-quark coupling to its phenomenological value
\beta_0 = 2\, \gev^{-1}. We then provide predictions for a variety of
diffractive processes. As the propagators have been evaluated entirely from QCD
first principles (although in the quenched approximation), our results provide
a consistency check of the Landshoff-Nachtmann model. We address the issue of
the possible gauge-dependence of our results, which will be the object of a

Lattice Study of the Decay B^0-bar -> rho^+ l^- nu_l-bar: Model-Independent Determination of |V_{ub}|
We present results of a lattice computation of the vector and axial-vector
current matrix elements relevant for the semileptonic decay B^0-bar -> rho^+
l^- nu_l-bar. The computations are performed in the quenched approximation of
lattice QCD on a 24^3 x 48 lattice at beta = 6.2, using an O(a) improved
fermionic action. Our principal result is for the differential decay rate,
dGamma/dq^2, for the decay B^0-bar -> rho^+ l^- nu_l-bar in a region beyond the
charm threshold, allowing a model-independent extraction of |V_{ub}| from
experimental measurements. Heavy quark symmetry relations between radiative and
semileptonic decays of B-bar mesons into light vector mesons are also

Magnetic Reversal on Vicinal Surfaces
We present a theoretical study of in-plane magnetization reversal for vicinal
ultrathin films using a one-dimensional micromagnetic model with
nearest-neighbor exchange, four-fold anisotropy at all sites, and two-fold
anisotropy at step edges. A detailed "phase diagram" is presented that catalogs
the possible shapes of hysteresis loops and reversal mechanisms as a function
of step anisotropy strength and vicinal terrace length. The steps generically
nucleate magnetization reversal and pin the motion of domain walls. No sharp
transition separates the cases of reversal by coherent rotation and reversal by
depinning of a ninety degree domain wall from the steps. Comparison to

Implementing NICE guidelines for the psychological treatment of depression and anxiety disorders: The IAPT experience
The Improving Access to Psychological Therapies (IAPT) programme is a large-scale initiative that aims to greatly increase the availability of NICE recommended psychological treatment for depression and anxiety disorders within the National Health Service in England. This article describes the background to the programme, the arguments on which it is based, the therapist training scheme, the clinical service model, and a summary of progress to date. At mid-point in a national roll-out of the programme progress is generally in line with expectation, and a large number of people who would not otherwise have had the opportunity to receive evidence-based psychological treatment have accessed, and benefited from, the new IAPT services. Planned future developments and challenges for the programme are briefly described
Classical approach in quantum physics
The application of a classical approach to various quantum problems - the
secular perturbation approach to quantization of a hydrogen atom in external
fields and a helium atom, the adiabatic switching method for calculation of a
semiclassical spectrum of hydrogen atom in crossed electric and magnetic
fields, a spontaneous decay of excited states of a hydrogen atom, Gutzwiller's
approach to Stark problem, long-lived excited states of a helium atom recently
discovered with the help of Poincar section, inelastic
transitions in slow and fast electron-atom and ion-atom collisions - is
reviewed. Further, a classical representation in quantum theory is discussed.
In this representation the quantum states are treating as an ensemble of
classical states. This approach opens the way to an accurate description of the
initial and final states in classical trajectory Monte Carlo (CTMC) method and
a purely classical explanation of tunneling phenomenon. The general aspects of
the structure of the semiclassical series such as renormgroup symmetry,
criterion of accuracy and so on are reviewed as well. In conclusion, the
relation between quantum theory, classical physics and measurement is

Measurements of long-range near-side angular correlations in TeV proton-lead collisions in the forward region
Two-particle angular correlations are studied in proton-lead collisions at a
nucleon-nucleon centre-of-mass energy of TeV, collected
with the LHCb detector at the LHC. The analysis is based on data recorded in
two beam configurations, in which either the direction of the proton or that of
the lead ion is analysed. The correlations are measured in the laboratory
system as a function of relative pseudorapidity, , and relative
azimuthal angle, , for events in different classes of event
activity and for different bins of particle transverse momentum. In
high-activity events a long-range correlation on the near side, , is observed in the pseudorapidity range . This
measurement of long-range correlations on the near side in proton-lead
collisions extends previous observations into the forward region up to
. The correlation increases with growing event activity and is found
to be more pronounced in the direction of the lead beam. However, the
correlation in the direction of the lead and proton beams are found to be
compatible when comparing events with similar absolute activity in the

Evidence for the strangeness-changing weak decay
Using a collision data sample corresponding to an integrated luminosity
of 3.0~fb, collected by the LHCb detector, we present the first search
for the strangeness-changing weak decay . No
hadron decay of this type has been seen before. A signal for this decay,
corresponding to a significance of 3.2 standard deviations, is reported. The

Study of the production of and hadrons in collisions and first measurement of the branching fraction
The product of the () differential production
cross-section and the branching fraction of the decay () is
measured as a function of the beauty hadron transverse momentum, ,
and rapidity, . The kinematic region of the measurements is and . The measurements use a data sample
corresponding to an integrated luminosity of collected by the
LHCb detector in collisions at centre-of-mass energies in 2011 and in 2012. Based on previous LHCb
results of the fragmentation fraction ratio, , the
