    Association between C-reactive protein with all-cause mortality in ELSA-Brasil cohort

    Background: High-sensitive C-reactive protein (hsCRP) has been proposed as a marker of incident cardiovascular disease and vascular mortality, and it may also be a marker of non-vascular mortality. However, most evidence comes from either North American or European cohorts. The present proposal aims to investigate the association of high-sensitive C-reactive protein with the risk of all-cause mortality in a multi-ethnic Brazilian population Methods: Cohort data from baseline (2008–2010) of 14 792 subjects participating in the Brazilian Longitudinal Study of Adult Health were used. HsCRP was assayed with Immunochemistry. The association of baseline covariates with all-cause mortality was calculated by Cox regression for univariate model and adjusted for different confounders after mean follow-up of 8.0 ± 1.1 years. The final model was adjusted for age, sex, self-rated race/ethnicity, schooling, health behaviours and prevalent chronic disease. Results: The risk of death increased steadily by quartiles of hsCRP from 1.45 (95% Confidence Interval: 1.05, 2.01) in Quartile 2 to 1.95 (1.42, 2.69) in Quartile 4 compared to Quartile 1. Furthermore, the persistence of a significant graded association after the exclusion of deaths in the first year of follow-up suggests that these results are unlikely to be due to reverse causality. Finally, the hazard ratios were unaffected by the exclusion of participants that had self-reported past medical history for diabetes, cancer and chronic obstructive pulmonary disease. Conclusions: Our study shows that hsCRP levels is associated with mortality in a highly admixed population, independently of a large set of lifestyle and clinical variables

    A importância do clima organizacional vem sendo retratado por inúmeros estudiosos da área de gestão como um dos fatores de maior importância para a compreensão da organização, desde a concepção na formatação da sua cultura organizacional até as formulações de estratégias de competividade. Isto significa dizer, segundo os estudiosos desta área que o clima organizacional passa a ser uma determinante extremamente importante para a criação e geração de valores de cunho pessoal, profissional e organizacional. Daí a importância de diagnosticá-lo, com o objetivo de ter-se em mãos, uma “fotografia” real e situacional que possibilite a implantação de medidas preventivas ou mesmo corretivas dos possíveis desajustes encontrados. O presente estudo tem como objetivo identificar a eficácia da pesquisa do clima organizacional como ferramenta de auxilio para o gestor público em uma unidade administrativa da Universidade Federal do Espírito Santo – UFES. Este estudo busca conhecer os servidores do Departamento de Administração e seu ambiente de trabalho, identificar suas percepções e/ou aspirações, mapear e retratar os pontos fracos e descobrir quais dos fatores avaliados influenciam positivamente em seu ambiente de trabalho

    Genomic African and Native American Ancestry and Chagas Disease: The Bambui (Brazil) Epigen Cohort Study of Aging.

    BackgroundThe influence of genetic ancestry on Trypanosoma cruzi infection and Chagas disease outcomes is unknown.Methodology/principal findingsWe used 370,539 Single Nucleotide Polymorphisms (SNPs) to examine the association between individual proportions of African, European and Native American genomic ancestry with T. cruzi infection and related outcomes in 1,341 participants (aged ≥ 60 years) of the Bambui (Brazil) population-based cohort study of aging. Potential confounding variables included sociodemographic characteristics and an array of health measures. The prevalence of T. cruzi infection was 37.5% and 56.3% of those infected had a major ECG abnormality. Baseline T. cruzi infection was correlated with higher levels of African and Native American ancestry, which in turn were strongly associated with poor socioeconomic circumstances. Cardiomyopathy in infected persons was not significantly associated with African or Native American ancestry levels. Infected persons with a major ECG abnormality were at increased risk of 15-year mortality relative to their counterparts with no such abnormalities (adjusted hazard ratio = 1.80; 95% 1.41, 2.32). African and Native American ancestry levels had no significant effect modifying this association.Conclusions/significanceOur findings indicate that African and Native American ancestry have no influence on the presence of major ECG abnormalities and had no influence on the ability of an ECG abnormality to predict mortality in older people infected with T. cruzi. In contrast, our results revealed a strong and independent association between prevalent T. cruzi infection and higher levels of African and Native American ancestry. Whether this association is a consequence of genetic background or differential exposure to infection remains to be determined

    Effect of State Feedback Coupling and System Delays on the Transient Performance of Stand-Alone VSI with LC Output Filter

    The influence of state feedback coupling in the dynamics performance of power converters for stand-alone microgrids is investigated. Computation and PWM delays are the main factors that limit the achievable bandwidth of current regulators in digital implementations. In particular, the performance of state feedback decoupling is degraded because of these delays. Two decoupling techniques to improve the transient response of the system are investigated, named non-ideal and ideal capacitor voltage decoupling respectively. In particular, the latter solution consists in leading the capacitor voltage on the state feedback decoupling path in order to compensate for system delays. Practical implementation issues are discussed with reference to both the decoupling techniques. A design methodology for the voltage loop, that considers the closed loop transfer functions developed for the inner loop, is also provided. A proportional resonant voltage controller is designed according to Nyquist criterion taking into account application requirements. For this purpose, a mathematical expression based on root locus analysis is proposed to find the minimum value of the fundamental resonant gain.     IgG4-Related Hashimoto's Thyroiditis - A New Variant of a Well Known Disease

    Hashimoto's thyroiditis (HT) has been characterized for many years as a well-defined clinicopathologic entity, but is now considered a heterogeneous disease. IgG4-related HT is a new subtype characterized by thyroid inflammation rich in IgG4-positive plasma cells and marked fibrosis. It may be part of the systemic IgG4-related disease. We report a case of a 56-year-old Portuguese man who presented with a one-month history of progressive neck swelling and dysphagia. Laboratory testing revealed increased inflammatory parameters, subclinical hypothyroidism and very high levels of thyroid autoantibodies. Cervical ultrasound (US) demonstrated an enlarged and heterogeneous thyroid gland and two hypoechoic nodules. US-guided fine needle aspiration cytology was consistent with lymphocytic thyroiditis. The patient was submitted to total thyroidectomy and microscopic examination identified typical findings of HT, marked fibrosis limited within the thyroid capsule and lymphoplasmacytic infiltration, with >50 IgG4-positive plasma cells per high-power field and an IgG4/IgG ratio of >40%. After surgery, serum IgG4 concentration was high-normal. Symptoms relief and reduction in laboratory inflammatory parameters were noticed. Thyroid function is controlled with levothyroxine. To our knowledge we report the first case of IgG4-related HT in a non-Asian patient. We also perform a review of the literature regarding IgG4-related disease and IgG4-related HT. Our case highlights this new variant of the well known HT, and helps physicians in recognizing its main clinical features, allowing for proper diagnosis and treatment

    A Mach-Zehnder Interferometer for a Two-Photon Wave Packet

    We propose an experiment that permits observation of the de Broglie two-photon wave packet behavior for a pair of photons, using a Mach-Zehnder interferometer. It is based on the use of pulsed lasers to generate pairs of photons via spontaneous parametric down-conversion and the post-selection of events. It differs from previous realizations by the use of a third time-correlated photon to engineer the state of the photons. The same technique can give us which-path information via an ``interaction-free'' experiment and can be used in other experiments on the foundations of quantum mechanics related to wave-particle duality and to nonlocality.Comment: Submmited for publication in Physical Review

    Desempenho do consórcio entre repolho e rabanete com pré-cultivo de crotalária, sob manejo orgânico.

    Foram conduzidos dois experimentos no Sistema Integrado de Produção Agroecológica, em Seropédica, (RJ), durante dois anos consecutivos. O objetivo foi avaliar o desempenho do consórcio entre as culturas de repolho e rabanete com pré-cultivo de crotalária, sob manejo orgânico. Usou-se delineamento experimental de blocos ao acaso com sete repetições, em parcelas subdivididas, representando um fatorial 2 x 3, sendo o primeiro fator o pré-cultivo, com Crotalaria juncea e pousio (vegetação espontânea); e o segundo fator o sistema de cultivo (consórcio entre repolho e rabanete e os respectivos monocultivos). Não houve diferença significativa no tocante à produtividade do repolho ou rabanete, entre C. juncea e pousio, independentemente do tipo de manejo (monocultivos ou consórcio). O desenvolvimento do repolho não foi influenciado pela presença do rabanete, no consórcio entre essas espécies sob cultivo orgânico. O rabanete sob consórcio apresentou redução no diâmetro médio, massa média e produtividade de raízes, sem, contudo,desqualificar o padrão comercial das raízes colhidas. Considerando a média dos dois anos experimentais, o IEA atingiu 1,59, o que indicou a viabilidade do consórcio, otimizando práticas culturais, incluindo adubação,capina e irrigação

    CXCR1 and SLC11A1 polymorphisms affect susceptibility to cutaneous leishmaniasis in Brazil: a case-control and family-based study.

    BACKGROUND: L. braziliensis causes cutaneous (CL) and mucosal (ML) leishmaniasis. Wound healing neutrophil (PMN) and macrophage responses made following the bite of the vector sand fly contribute to disease progression in mice. To look at the interplay between PMN and macrophages in disease progression in humans we asked whether polymorphisms at genes that regulate their infiltration or function are associated with different clinical phenotypes. Specifically, CXCR1 (IL8RA) and CXCR2 (IL8RB) are receptors for chemokines that attract PMN to inflammatory sites. They lie 30-260 kb upstream of SLC11A1, a gene known primarily for its role in regulating macrophage activation, resistance to leishmaniasis, and wound healing responses in mice, but also known to be expressed in PMN, macrophages and dendritic cells. METHODS: Polymorphic variants at CXCR1, CXCR2 and SLC11A1 were analysed using Taqman or ABI fragment separation technologies in cases (60 CL; 60 ML), unrelated controls (n = 120), and multicase families (104 nuclear families; 88 ML, 250 CL cases) from Brazil. Logistic regression analysis, family-based association testing (FBAT) and haplotype analysis (TRANSMIT) were performed. RESULTS: Case-control analysis showed association between the common C allele (OR 2.38; 95% CI 1.23-4.57; P = 0.009) of CXCR1_rs2854386 and CL, supported by family-based (FBAT; Z score 2.002; P = 0.045) analysis (104 nuclear families; 88 ML, 250 CL cases). ML associated with the rarer G allele (Z score 1.999; P = 0.046). CL associated with a 3' insertion/deletion polymorphism at SLC11A1 (Z score 2.549; P = 0.011). CONCLUSIONS: The study supports roles for CXCR1 and SLC11A1 in the outcome of L. braziliensis infection in humans. Slc11a1 does not influence cutaneous lesion development following needle injection of Leishmania in mice, suggesting that its role here might relate to the action of PMN, macrophage and/or dendritic cells in the wound healing response to the sand fly bite. Together with the CXCR1 association, the data are consistent with hypotheses relating to the possible role of PMN in initiation of a lesion following the delivery of parasites via the sand fly bite.
