Experimental Investigation of Helicopter Noise While Approaching an Elevated Helipad
The present paper describes a test campaign performed to investigate the noise footprint emitted by a helicopter in an idealised urban context, reproducing the approach to an elevated helipad. The test campaign was performed in Politecnico di Milano’s anechoic chamber and was finalised to investigate the effects produced only by helicopter noise. The set up consisted of a two-blade main rotor helicopter model and an aluminium rectangular prism model reproducing the landing building. Ground observer perceptions were recorded by means of a surface microphone and a realistic landing trajectory was approximated as a succession of fixed point measurements. Collected data were analysed through acoustic spectra and sound maps. Spectra were used to comprehend physical phenomena, such as reflection, diffraction and shielding, and to analyse the different contributions of helicopter noise. A sound map analysis enabled us to obtain a global perspective of the involved phenomena and to understand th extent to which people close to a building are stressed by a helicopter approaching an elevated urban helipad. Moreover, the experimental database, obtained over a free geometry, can be considered a useful tool for the validation of aeroacoustic solvers with different levels of fidelity
Surgical management of a life-threatening retro-pharyngeal haematoma following trans-oesophageal echocardiography = Trattamento chirurgico di un pericoloso ematoma retrofaringeo, sviluppatosi a seguito di un\u2019ecocardiografia transesofagea
Nonostante siano state descritte complicanze potenzialmente letali, l\u2019ecocardiografia transesofagea viene considerata una procedura diagnostica sicura. Scopo del presente lavoro \ue8 sottolineare come, nei casi di voluminosi ematomi secondari a tale procedura, possa diventare indispensabile il ricorso alla chirurgia per tutelare la vita del paziente. Nel caso qui descritto, un paziente affetto da spondilosi cervicale e in terapia anticoagulante ha sviluppato, a seguito di un\u2019ecocardiografia transesofagea, un ematoma retrofaringeo tanto voluminoso da determinare un\u2019ostruzione delle vie aeree superiori. Per assicurarne la perviet\ue0 \ue8 stato necessario ricorrere a una tracheotomia, l\u2019ematoma, invece, \ue8 stato drenato per via transcervicale. Il paziente \ue8 guarito completamente in 10 giorni. Il chirurgo otorinolaringoiatrico dovrebbe essere a conoscenza, non solo di questa rara complicanza, ma anche della necessit\ue0 di un suo trattamento chirurgico.Trans-oesophageal echocardiography is generally considered a safe procedure, but occasional life-threatening complications have been reported. The aim of this clinical investigation is to outline the need of surgical management in cases of large retro-pharyngeal haematoma following trans-oesophageal echocardiography. In the case reported here, a patient with cervical spondylosis on anti-coagulant therapy was referred to the Head and Neck Department because of a retro-pharyngeal haematoma with severe upper airway obstruction following transoesophageal echocardiography. Tracheotomy was required to guarantee respiratory function, while trans-cervical surgery was performed to evacuate the haematoma. Total recovery was achieved within 10 days. In conclusion, the head and neck surgeon should consider the need of surgical management in cases of retro-pharyngeal haematoma following trans-oesophageal echocardiography
Consistent Grasping Interactions with Virtual Actors Based on the Multi-sensor Hand Model
Proposes a general framework to enhance grasping interactions of an operator wearing a digital glove. The authors focus on a consistent interpretation of the posture information acquired with the glove in order to reflect the grasp of virtual artifacts. This allows higher skill manipulations in virtual environments and also improves interactions with virtual human models. A handshake case-study highlights the application range of this methodology
Optimal d-statcom placement tool for low voltage grids
In low-voltage grids with a wide spread of domestic and/or small commercial consumers, mostly single-phase, problems can appear due to unbalanced power consumption between the different phases. These problems are mainly caused due to voltage unbalances between phases and the increase in distribution losses. This phenomenon occurs more frequently at the end of highly radial grids and can be stressed by the installation of renewable generators next to the consumers. Amongst the various techniques that have been proposed to solve this problem, this article explores the use of a D-STATCOM, presenting and testing a new method for the optimal location of this type of D-FACT. The developed method starts from a detailed analysis of the existing voltage unbalances in a distribution network and identifies the optimal location of the D-STATCOM (i.e., the one that reduces these unbalances while reducing energy losses). The developed method has been successfully tested for one year at four real European locations with different characteristics and different kinds of users. © 2021 by the authors. Perceived Hospital Preparedness Is Negatively Associated With Pandemic-Induced Psychological Vulnerability in Primary Care Employees: A Multicentre Cross-Sectional Observational Study.
The COVID-19 pandemic had a profound negative impact on the psychological wellbeing of healthcare providers (HPs), but little is known about the factors that positively predict mental health of primary care staff during these dire situations.
We conducted an online questionnaire survey among 702 emergency department workers across 10 hospitals in Switzerland and Belgium following the first COVID-19 wave in 2020, to explore their psychological vulnerability, perceived concerns, self-reported impact and level of pandemic workplace preparedness. Participants included physicians, nurses, psychologists and nondirect care employees (administrative staff). We tested for predictors of psychological vulnerability through both an exploratory cross-correlation with rigorous correction for multiple comparisons and model-based path modelling.
Findings showed that the self-reported impact of COVID-19 at work, concerns about contracting COVID-19 at work, and a lack of personal protective equipment were strong positive predictors of Depression, Anxiety, and Stress, and low Resilience. Instead, knowledge of the degree of preparedness of the hospital/department, especially in the presence of a predetermined contingency plan for an epidemic and training sessions about protective measures, showed the opposite effect, and were associated with lower psychological vulnerability. All effects were confirmed after accounting for confounding factors related to gender, age, geographical location and the role played by HPs in the hospital/department.
Difficult working conditions during the pandemic had a major impact on the psychological wellbeing of emergency department HPs, but this effect might have been lessened if they had been informed about adequate measures for minimizing the risk of exposure
Gut microbiota analysis reveals a marked shift to bifidobacteria by a starter infant formula containing a synbiotic of bovine milk-derived oligosaccharides and Bifidobacterium animalis subsp. lactis CNCM I-3446.
Non-digestible milk oligosaccharides were proposed as receptor decoys for pathogens and as nutrients for beneficial gut commensals like bifidobacteria. Bovine milk contains oligosaccharides, some of which are structurally identical or similar to those found in human milk. In a controlled, randomized double-blinded clinical trial we tested the effect of feeding a formula supplemented with a mixture of bovine milk-derived oligosaccharides (BMOS) generated from whey permeate, containing galacto-oligosaccharides and 3'- and 6'-sialyllactose, and the probiotic Bifidobacterium animalis subsp. lactis (B. lactis) strain CNCM I-3446. Breastfed infants served as reference group. Compared with a non-supplemented control formula, the test formula showed a similar tolerability and supported a similar growth in healthy newborns followed for 12 weeks. The control, but not the test group, differed from the breast-fed reference group by a higher faecal pH and a significantly higher diversity of the faecal microbiota. In the test group the probiotic B. lactis increased by 100-fold in the stool and was detected in all supplemented infants. BMOS stimulated a marked shift to a bifidobacterium-dominated faecal microbiota via increases in endogenous bifidobacteria (B. longum, B. breve, B. bifidum, B. pseudocatenulatum)
Metabolic Versatility and Antibacterial Metabolite Biosynthesis Are Distinguishing Genomic Features of the Fire Blight Antagonist Pantoea vagans C9-1

PLoS ONE. 2011;6(7): e22247.Background: Pantoea vagans is a commercialized biological control agent used against the pome fruit bacterial disease fire blight, caused by Erwinia amylovora. Compared to other biocontrol agents, relatively little is currently known regarding Pantoea genetics. Better understanding of antagonist mechanisms of action and ecological fitness is critical to improving efficacy. Principal Findings: Genome analysis indicated two major factors contribute to biocontrol activity: competition for limiting substrates and antibacterial metabolite production. Pathways for utilization of a broad diversity of sugars and acquisition of iron were identified. Metabolism of sorbitol by P. vagans C9-1 may be a major metabolic feature in biocontrol of fire blight. Biosynthetic genes for the antibacterial peptide pantocin A were found on a chromosomal 28-kb genomic island, and for dapdiamide E on the plasmid pPag2. There was no evidence of potential virulence factors that could enable an animal or phytopathogenic lifestyle and no indication of any genetic-based biosafety risk in the antagonist. Conclusions: Identifying key determinants contributing to disease suppression allows the development of procedures to follow their expression in planta and the genome sequence contributes to rationale risk assessment regarding the use of the biocontrol strain in agricultural systems

The influence of the accessory genome on bacterial pathogen evolution
Bacterial pathogens exhibit significant variation in their genomic content of virulence factors. This reflects the abundance of strategies pathogens evolved to infect host organisms by suppressing host immunity. Molecular arms-races have been a strong driving force for the evolution of pathogenicity, with pathogens often encoding overlapping or redundant functions, such as type III protein secretion effectors and hosts encoding ever more sophisticated immune systems. The pathogens’ frequent exposure to other microbes, either in their host or in the environment, provides opportunities for the acquisition or interchange of mobile genetic elements. These DNA elements accessorise the core genome and can play major roles in shaping genome structure and altering the complement of virulence factors. Here, we review the different mobile genetic elements focusing on the more recent discoveries and highlighting their role in shaping bacterial pathogen evolution
