    El objetivo de este artículo es estudiar las diferentes posibilidades de manipulación por parte del usuario que presenta el vídeo musical interactivo, como nuevo formato surgido al amparo de Internet como vehículo de distribución de productos audiovisuales y entorno capaz de albergar prácticas artísticas. Para ello se parte del vídeo musical lineal como soporte artístico y su posible adaptación al nuevo medio interactivo. La intención es justificar una unión de décadas entre arte y cultura popular que podría tener continuidad en el nuevo formato que utiliza como base las nuevas tecnologías en materia de comunicación. Por último se hace un estudio de cincuenta piezas representativas del vídeo musical interactivo que nos ayude a encontrar patrones comunes a nivel de interacción y posibilidades plásticas vinculando imagen y sonido

    Tardive Dyskinesia After Aripiprazole Treatment That Improved With Tetrabenazine, Clozapine, and Botulinum Toxin

    We report on a patient with tardive dyskinesia (TDK) treated with aripiprazole, a third-generation antipsychotic with partial D2 agonist-antagonist activity at both the dopamine and serotonin receptors. The patient’s condition improved with administration of a combination of tetrabenazine, botulinum toxin, and clozapine, which has previously not been used. We suggest that this treatment combination may have potential benefits for patients with TDK. After aripiprazole discontinuation, the patient was treated with clozapine (150 mg/day) and biperiden (8 mg/day). Due to a lack of improvement, we administered 300 units (intramuscularly; IM) of botulinum toxin into the paravertebral muscles every 3 months and 1,000 units IM every 4 months in addition to tetrabenazine (75 mg/day) and biperiden (8 mg/day). The patient stopped this treatment, at which point TDK reappeared. After starting a treatment regimen of clozapine (100 mg/day), tetrabenazine (75 mg/day), and botulinum toxin (300 units IM), the patient’s symptoms remitted

    Resumen: Durante los últimos años parece evidente que el estrés psicológico y los trastornos afectivos ejercen influencia sobre el curso de la enfermedad inflamatoria intestinal. En una muestra de 35 sujetos se analizó la eficacia de un tratamiento psicológico cognitivo-conductual en formato grupal. Para ello, se analizaron los cambios en las estrategias de afrontamiento del estrés y manejo de la enfermedad, en la sintomatología depresiva y en la calidad de vida tras el tratamiento. Se observaron, diferencias significativas en la calidad de vida, en todas las escalas de afrontamiento del estrés, excepto en autofocalización negativa, mejorando la sintomatología depresiva. La terapia cognitivo-conductual es la intervención psicológica más prometedora según los datos empíricos obtenidos hasta la fecha y podría empezar a ser considerada como parte del tratamiento integral para estos pacientes.Abstract: Results of a group psychological treatment protocol for inflammatory bowel disease. During the last years, it has become clear that stress and affective disorders have influence on the course of inflammatory bowel disease. In a sample of 35 subjects, the effectiveness of a cognitive-behavioral treatment was analyzed in a group format. For this, changes in coping strategies and stress and disease management, in depressive symptoms and quality of life after treatment were analyzed. Significant differences were observed in quality of life and in all stress coping scales except negative self-targeting, improving depressive symptoms. Cognitive-behavioral therapy is the most promising psychological intervention according to the empirical data obtained so far and could begin to be considered part of a comprehensive treatment for these patients

    Introduction: The social, economic and cultural environment is undergoing a transformation caused by computer technologies, becoming almost indispensable in our lives. However, the elderly can present problems of adaptation, triggering a lack of technological literacy that in the health field could have an impact on their quality of life. The objective is to observe how new technologies affect the elderly, and propose nursing interventions that favor their adaptation to this new era. Material and Methods: Articles published since 2009, from anywhere in the world, have been consulted free of charge, in Spanish and English. The databases consulted have been PubMed, Google Scholar, Mesh, ScienceDirect, Trip Database, IBECS and DeCS. Results: Currently, technologies are adapting and evolving differently around the world, assuming progress in all places. A positive effect has been observed regarding the relationship and communication between the elderly and their environment, being advisable to provide greater cognitive stimulation with respect to these. Discussion: Most of the evidence found points to the benefit that technology brings regarding the connection of the elderly with their social environment. In healthcare, telecare has proven to improve the quality of life of the elderly. Just as the electronic prescription helps with pharmacological adherence and provides comfort for chronic and / or multipathological patients, the telematic appointment reduces long waiting times. Conclusion: The technologies, associated with social isolation, telecare and electronic prescriptions, have a positive effect on the quality of life of the elderly population. Providing them with an exercise of their brain activity, greater selfesteem and comfort, solving their health problems from home without having to approach a health center.Introducción: El entorno social, económico y cultural está sufriendo una transformación causada por las tecnologías informáticas, convirtiéndose casi indispensables en nuestras vidas. Sin embargo, la edad anciana puede presentar problemas de adaptación, desencadenando una falta de alfabetización tecnológica que en el ámbito sanitario podría repercutir en su calidad de vida. El objetivo es observar cómo afectan las nuevas tecnologías a los ancianos, y proponer intervenciones enfermeras que favorezcan su adaptación a esta nueva era. Material y Métodos: Se han consultado artículos publicados a partir de 2009, de cualquier parte del mundo, de extensión completa gratuita, en español e inglés. Las bases de datos consultadas han sido PubMed, Google Académico, Mesh, ScienceDirect, Trip Database, IBECS y DeCS. Resultados: Actualmente las tecnologías están adaptándose y evolucionando de forma diferente por el mundo, suponiendo un avance en todos los lugares. Se ha observado un efecto positivo en cuanto a la relación y comunicación entre los ancianos y su entorno,siendo recomendable proporcionar una mayor estimulación cognitiva con respecto a estas. Discusión: La mayor parte de la evidencia encontrada apunta al beneficio que la tecnología aporta respecto a la conexión de los ancianos con su entorno social. En sanidad, la teleasistencia ha demostrado mejorar la calidad de vida de los ancianos. Así como la receta electrónica ayuda con la adherencia farmacológica y supone comodidad para pacientes crónicos y/o pluripatológicos, la cita telemática disminuye las largas esperas de consulta. Conclusión: Las tecnologías, asociadas al aislamiento social, teleasistencia y receta electrónica, tienen efecto positivo en la calidad de vida de la población anciana. Proporcionándoles una ejercitación de su actividad cerebral, una mayor autoestima y comodidad, solucionando sus problemas de salud desde casa sin tener que acercarse a un centro sanitario

    Producción CientíficaBackground: Stratification of the severity of infection is currently based on the Sequential Organ Failure Assessment (SOFA) score, which is difficult to calculate outside the ICU. Biomarkers could help to stratify the severity of infection in surgical patients. Methods: Levels of ten biomarkers indicating endothelial dysfunction, 22 indicating emergency granulopoiesis, and six denoting neutrophil degranulation were compared in three groups of patients in the first 12 h after diagnosis at three Spanish hospitals. Results: There were 100 patients with infection, 95 with sepsis and 57 with septic shock. Seven biomarkers indicating endothelial dysfunction (mid-regional proadrenomedullin (MR-ProADM), syndecan 1, thrombomodulin, angiopoietin 2, endothelial cell-specific molecule 1, vascular cell adhesion molecule 1 and E-selectin) had stronger associations with sepsis than infection alone. MR-ProADM had the highest odds ratio (OR) in multivariable analysis (OR 11·53, 95 per cent c.i. 4·15 to 32·08; P = 0·006) and the best area under the curve (AUC) for detecting sepsis (0·86, 95 per cent c.i. 0·80 to 0·91; P < 0·001). In a comparison of sepsis with septic shock, two biomarkers of neutrophil degranulation, proteinase 3 (OR 8·09, 1·34 to 48·91; P = 0·028) and lipocalin 2 (OR 6·62, 2·47 to 17·77; P = 0·002), had the strongest association with septic shock, but lipocalin 2 exhibited the highest AUC (0·81, 0·73 to 0·90; P < 0·001). Conclusion: MR-ProADM and lipocalin 2 could be alternatives to the SOFA score in the detection of sepsis and septic shock respectively in surgical patients with infection.Instituto de Salud Carlos III (grants PI15/01959, PI15/01451 and PI16/01156

La globalización ha contribuido al incremento de parejas mixtas, en el que cada sujeto en ocasiones tiene un origen diferente, algo que complica la situación de sus hijos cuando se producen separaciones o divorcios matrimoniales. Principalmente por ese elemento sentimental, que no se aprecia en otros sectores del Derecho, es importante una especial fijación y sensibilidad por el legislador para que se vele siempre por el interés y el bienestar del menor que en algunas ocasiones es el que sale más perjudicado de las situaciones descritas

    Incidence, Clinical Characteristics and Management of Inflammatory Bowel Disease in Spain : Large-Scale Epidemiological Study

    (1) Aims: To assess the incidence of inflammatory bowel disease (IBD) in Spain, to describe the main epidemiological and clinical characteristics at diagnosis and the evolution of the disease, and to explore the use of drug treatments. (2) Methods: Prospective, population-based nationwide registry. Adult patients diagnosed with IBD-Crohn's disease (CD), ulcerative colitis (UC) or IBD unclassified (IBD-U)-during 2017 in Spain were included and were followed-up for 1 year. (3) Results: We identified 3611 incident cases of IBD diagnosed during 2017 in 108 hospitals covering over 22 million inhabitants. The overall incidence (cases/100,000 person-years) was 16 for IBD, 7.5 for CD, 8 for UC, and 0.5 for IBD-U; 53% of patients were male and median age was 43 years (interquartile range = 31-56 years). During a median 12-month follow-up, 34% of patients were treated with systemic steroids, 25% with immunomodulators, 15% with biologics and 5.6% underwent surgery. The percentage of patients under these treatments was significantly higher in CD than UC and IBD-U. Use of systemic steroids and biologics was significantly higher in hospitals with high resources. In total, 28% of patients were hospitalized (35% CD and 22% UC patients, p < 0.01). (4) Conclusion: The incidence of IBD in Spain is rather high and similar to that reported in Northern Europe. IBD patients require substantial therapeutic resources, which are greater in CD and in hospitals with high resources, and much higher than previously reported. One third of patients are hospitalized in the first year after diagnosis and a relevant proportion undergo surgery
