Correlation between the concentration of air pollutants (CO, SO2 and NO2) and pulmonary function
زمینه و هدف: آلاینده های استنشاقی نقش مهمی در بیماری های ریوی دارند. این مطالعه با هدف بررسی سطح آلاینده های استنشاقی دی اکسید نیتروژن، دی اکسید گوگرد، منواکسید کربن در اراک (شهر صنعتی و آلوده) و خمین (غیر صنعتی و غیر آلوده) و تعیین چگونگی اثر آن بر عملکرد ریه در مقطع زمان اسپیرومتری انجام شد. روش بررسی: دو شهر اراک و خمین به ترتیب به ده و پنج منطقه تقسیم و آلاینده های استنشاقی دی اکسید نیتروژن، دی اکسید گوگرد و منواکسید کربن اندازه گیری شدند. همزمان به طور تصادفی در فصول مختلف سال 522 نفر از بالغ غیر سیگاری خمین و 547 نفر از اراک انتخاب و از آنان اسپیرومتری به عمل آمد و از شاخص کیفیت هوا (PSI=Poliution standard index) برای ارزیـــابی کیفیت هوا استفاده شد. میـانگین غلظت آلاینده ها و شاخص پراکندگی ظرفیت های ریوی با آزمون های آماری آنالیز رگرسیون مورد تجزیه و تحلیل قرار گرفتند. یافته ها: میانگینPSI اراک 54/14±83/101 و خمین 58/18±17/89 بود (05/0>P). حداکثر جریان بازدمی (PEF) و ظرفیت حیاتی فعال (FVC) در اراک به طور معنی داری کمتر از خمین بود (05/0

The V600 BRAF mutation has been described as a key mutation in the pathogenesis of classical hairy cell leukemia (c-HCL) cases without expression of a mutant immunoglobulin heavy chain (IgHV). Here we present a rare case of c-HCL with neither V600 BRAF mutation nor the aforementioned IgHV variant successfully treated with cladribine and review the current literature on its use in women of childbearing age/pregnancy

Context Dependent Role of Type 2 Innate Lymphoid Cells in Allergic Skin Inflammation.
The discovery of innate lymphoid cells (ILC) has profoundly influenced the understanding of innate and adaptive immune crosstalk in health and disease. ILC and T cells share developmental and functional characteristics such as the lineage-specifying transcription factors and effector cytokines, but importantly ILC do not display rearranged antigen-specific receptors. Similar to T cells ILC are subdivided into 3 different helper-like subtypes, namely ILC1-3, and a killer-like subtype comprising natural killer (NK) cells. Increasing evidence supports the physiological relevance of ILC, e.g., in wound healing and defense against parasites, as well as their pathogenic role in allergy, inflammatory bowel diseases or psoriasis. Group 2 ILC have been attributed to the pathogenesis of allergic diseases like asthma and atopic dermatitis. Other inflammatory skin diseases such as allergic contact dermatitis are profoundly shaped by inflammatory NK cells. This article reviews the role of ILC in allergic skin diseases with a major focus on ILC2. While group 2 ILC are suggested to contribute to the pathogenesis of type 2 dominated inflammation as seen in atopic dermatitis, we have shown that lack of ILC2 in type 1 dominated contact hypersensitivity results in enhanced inflammation, suggesting a regulatory role of ILC2 in this context. We provide a concept of how ILC2 may influence context dependent the mutual counterbalance between type I and type II immune responses in allergic skin diseases
Optimal network reconfiguration in active distribution networks with soft open points and distributed generation

In this study, we allocated soft open points (SOPs) and distributed generation (DG) units simultaneously with and without network reconfiguration (NR), and investigate the contribution of SOP losses to the total active losses, as well as the effect of increasing the number of SOPs connected to distribution systems under different loading conditions. A recent meta-heuristic optimization algorithm called the discrete-continuous hyper-spherical search algorithm is used to solve the mixed-integer nonlinear problem of SOPs and DGs allocation, along with new NR methodology to obtain radial configurations in an efficient manner without the possibility of getting trapped in local minima. Further, multi-scenario studies are conducted on an IEEE 33-node balanced benchmark distribution system and an 83-node balanced distribution system from a power company in Taiwan. The contributions of SOP losses to the total active losses, as well as the effect of increasing the number of SOPs connected to the system, are investigated to determine the real benefits gained from their allocation. It was clear from the results obtained that simultaneous NR, SOP, and DG allocation into a distribution system creates a hybrid configuration that merges the benefits offered by radial distribution systems and mitigates drawbacks related to losses, power quality, and voltage violations, while offering a far more efficient and optimal network operation. Also, it was found that the contribution of the internal loss of SOPs to the total loss for different numbers of installed SOPs is not dependent on the number of SOPs and that loss minimization is not always guaranteed by installing more SOPs or DGs along with NR. One of the findings of the paper demonstrates that NR with optimizing tie-lines could reduce active losses considerably. The results obtained also validate, with proper justifications, that SOPs installed for the management of constraints in LV feeders could further reduce losses and efficiently address issues related to voltage violations and network losses
Prevalence of high blood pressure in Iranian adults based on the 2017 ACC/AHA guideline
Background:: In 2017, American College of Cardiology (ACC) and the American Heart Association (AHA) presented a new guideline for assessing blood pressure in adults. This study aimed to assess the prevalence of hypertension in Iranian adults based on ACC/AHA 2017 guideline. Methods: Data from 9801 Iranian adults (59.2 women) aged between 20-69 years were obtained from the sixth round of National Surveillance of Risk Factors of Non-Communicable Diseases (SuRFNCD) performed in 2011. Blood pressure was classified as normal, elevated blood pressure, and stage 1 and 2 hypertension using a weighted analysis and 2017 ACC/AHA guidelines. Data were presented as prevalence and 95 confidence interval (95 CI). All analyses were performed in Stata/SE 14.0. Results: Overall prevalence of hypertension in Iranian men was 52.0. Also, 32.9 (95 CI: 29.9-36.0) and 19.1 (95 CI: 16.921.6) of men had stage 1 and 2 hypertension, respectively. In addition, 44.3 of women had hypertension, of whom 26.3 (95 CI: 24.5 - 28.2) had stage 1 and 18.0 (95 CI: 16.1-20.1) stage 2 hypertension. Furthermore, 16.5 (95 CI: 14.4-18.9) and 9.6 (95 CI: 7.86-11.7) of men and women had elevated blood pressure, respectively. Conclusion: The findings of this study indicated that adopting the 2017 ACC/AHA guidelines showed a higher prevalence of adult hypertension (48.2) in Iran. In this study, the prevalence of hypertension in men was higher than in women, which was steadily increased by age in older adults in both sexes. © Iran University of Medical Sciences
A global survey on changes in the supply, price, and use of illicit drugs and alcohol, and related complications during the 2020 COVID-19 pandemic
Background and Aims : COVID-19 has infected more than 77 million people worldwide and impacted the lives of many more, with a particularly devastating impact on vulnerable populations, including people with substance use disorders (SUDs). Quarantines, travel bans, regulatory changes, social distancing, and "lockdown" measures have affected drug and alcohol supply chains and subsequently their availability, price, and use patterns, with possible downstream effects on presentations of SUDs and demand for treatment. Given the lack of multicentric epidemiologic studies, we conducted a rapid global survey within the International Society of Addiction Medicine (ISAM) network in order to understand the status of substance-use patterns during the current pandemic. Design : Cross-sectional survey. Setting : Worldwide. Participants : Starting on April 4, 2020 during a 5-week period, the survey received 185 responses from 77 countries. Measurements : To assess addiction medicine professionals' perceived changes in drug and alcohol supply, price, use pattern, and related complications during the COVID-19 pandemic. Findings : Participants reported (among who answered "decreased" or "increased") a decrease in drug supply (69.0%) and at the same time an increase in price (95.3%) globally. With respect to changes in use patterns, an increase in alcohol (71.7%), cannabis (63.0%), prescription opioids (70.9%), and sedative/hypnotics (84.6%) use was reported, while the use of amphetamines (59.7%), cocaine (67.5%), and opiates (58.2%) was reported to decrease overall. Conclusions : The global report on changes in the availability, use patterns, and complications of alcohol and drugs during the COVID-19 pandemic should be considered in making new policies and in developing mitigating measures and guidelines during the current pandemic (and probable future ones) in order to minimize risks to people with SUD.Publisher PDFPeer reviewe
