Detection of distant metastases in patients with oesophageal or gastric cardia cancer: a diagnostic decision analysis
Computed tomography (CT) is presently a standard procedure for the detection of distant metastases in patients with oesophageal or gastric cardia cancer. We aimed to determine the additional diagnostic value of alternative staging investigations. We included 569 oesophageal or gastric cardia cancer patients who had undergone CT neck/thorax/abdomen, ultrasound (US) abdomen, US neck, endoscopic ultrasonography (EUS), and/or chest X-ray for staging. Sensitivity and specificity were first determined at an organ level (results of investigations, i.e., CT, US abdomen, US neck, EUS, and chest X-ray, per organ), and then at a patient level (results for combinations of investigations), considering that the detection of distant metastases is a contraindication to surgery. For this, we compared three strategies for each organ: CT alone, CT plus another investigation if CT was negative for metastases (one-positive scenario), and CT plus another investigation if CT was positive, but requiring that both were positive for a final positive result (two-positive scenario). In addition, costs, life expectancy and quality adjusted life years (QALYs) were compared between different diagnostic strategies. CT showed sensitivities for detecting metastases in celiac lymph nodes, liver and lung of 69, 73, and 90%, respectively, which was higher than the sensitivities of US abdomen (44% for celiac lymph nodes and 65% for liver metastases), EUS (38% for celiac lymph nodes), and chest X-ray (68% for lung metastases). In contrast, US neck showed a higher sensitivity for the detection of malignant supraclavicular lymph nodes than CT (85 vs 28%). At a patient level, sensitivity for detecting distant metastases was 66% and specificity was 95% if only CT was performed. A higher sensitivity (86%) was achieved when US neck was added to CT (one-positive scenario), at the same specificity (95%). This strategy resulted in lower costs compared to CT only, at an almost similar (quality adjusted) life expectancy. Slightly higher specificities (97–99%) were achieved if liver and/or lung metastases found on CT, were confirmed by US abdomen or chest X-ray, respectively (two-positive scenario). These strategies had only slightly higher QALYs, but substantially higher costs. The combination of CT neck/thorax/abdomen and US neck was most cost-effective for the detection of metastases in patients with oesophageal or gastric cardia cancer, whereas the performance of CT only had a lower sensitivity for metastases detection and higher costs. The role of EUS seems limited, which may be due to the low number of M1b celiac lymph nodes detected in this series. It remains to be determined whether the application of positron emission tomography will further increase sensitivities and specificities of metastases detection without jeopardising costs and QALYs
A case of mistaken identity: HSPs are no DAMPs but DAMPERs
Until recently, the immune system was seen solely as a defense system with its primary task being the elimination of unwanted microbial invaders. Currently, however, the functional significance of the immune system has obtained a much wider perspective, to include among others the maintenance and restoration of homeostasis following tissue damage. In this latter aspect, there is a growing interest in the identification of molecules involved, such as the so-called danger or damage-associated molecular patterns (DAMPs), also called alarmins. Since heat shock proteins are archetypical molecules produced under stressful conditions, such as tissue damage or inflammation, they are frequently mentioned as prime examples of DAMPs (Bianchi, J Leukoc Biol 81:1–5, 2007; Kono and Rock, Nat Rev Immunol 8:279–289, 2008; Martin-Murphy et al., Toxicol Lett 192:387–394, 2010). See for instance also a recent review (Chen and Nunez, Science 298:1395–1401, 2010). Contrary to this description, we recently presented some of the arguments against a role of heat shock protein as DAMPs (Broere et al., Nat Rev Immunol 11:565-c1, 2011). With this perspective and reflection article, we hope to elaborate on this debate and provide additional thoughts to further ignite this discussion on this critical and evolving issue
Body taping for contour surgery.
BACKGROUND: Preoperative marking is of primary importance in body contouring and when precise simulation of skin excisions is difficult. Because the "cut as you go" principle can be delicate, especially in patients after massive weight loss, a simple and quick method is needed for preoperative planning. We suggest an approach that helps visualize the optimal skin incision lines and simulates the postoperative result by body taping. METHODS: Twelve patients who underwent abdominal contouring, including classic and vertical abdominoplasties as well as dog ear and scar revision, were prospectively analyzed. The skin to be excised was preoperatively folded, taped, and then marked. The area marked was measured and compared with the actual intraoperatively resected area and the postoperative result was evaluated after 1 year by the patients and three surgeons. RESULTS: With body taping, an 83% congruence between the preoperative planning and the surgery was obtained and only two patients had additional skin resected. No wound dehiscence and flap necrosis occurred and patients as well as surgeons scored the final body contour positively. CONCLUSION: Body taping is a simple, quick, and economic method for planning contour surgery with high accuracy as demonstrated by the low rate of intraoperative changes of the planned resection and low complication rate
Ressecção de cordoma sacral com abaixamento de cólon: relato de caso Sacral chordoma resection with colo anal anastomosis: case report
O cordoma é uma lesão maligna da medula espinhal, que se origina nos remanescentes ectópicos de tecido notocordial. Trata-se de neoplasia rara e sua localização preferencial é o sacro. Tem crescimento lento, mas comportamento localmente agressivo. Relatamos um caso de ressecção de cordoma sacral com abaixamento de cólon. Para extirpação oncológica da lesão foi necessária a abordagem colorretal e ortopédica. Novas técnicas preconizando cirurgias mais agressivas melhoraram significativamente a expectativa de vida dos pacientes portadores de cordoma, bem como o tempo de vida livre de doença. Na busca dessa cirurgia oncológica ideal, a abordagem multidisciplinar é essencial.<br>Chordoma is a rare malignant lesion of spinal cord originated from ectopic remnants of notocordial tissue. Its presentation is characterized by slow growth in spite of locally aggressive behaviour. We report a case of multidisciplinary oncologic resection with coloanal anastomosis. Extended and aggressive resections improved significantly the disease free survival in patients with chordoma. The multidisciplinary oncologic approach is essential in order to reach this goal
Impacto do projeto acerto na morbi-mortalidade pós-operatória em um hospital universitário
OBJETIVO: Avaliar resultados pós-operatórios de pacientes do Serviço de Cirurgia Geral do Hospital Universitário Julio Muller antes e após a implantação do protocolo multimodal ACERTO. MÉTODO: Estudo retrospectivo em fichas preenchidas prospectivamente referentes a 5974 pacientes. Foram comparadas duas fases: de Janeiro de 2002 a Dezembro de 2004 (antes da implantação do protocolo ACERTO: período AA, n=1987); e de Janeiro de 2005 a Dezembro de 2008 (após a implantação do mesmo: período DA, n=3987). Comparou-se tempo de internação hospitalar, hemotransfusões realizadas, infecções de sítio cirúrgico (ISC), complicações operatórias e óbitos. RESULTADOS: Houve uma diminuição em um dia no tempo de internação entre o período AA e DA (mediana [variação]: 4 [0-137] vs. 3 [0-126] dias e moda: 3 vs. 2 dias; p<0,001). No período AA houve uma relação de 2,53 bolsas de hemoderivados transfundidas por paciente contra 0,77 no período DA (p<0,001). Notou-se tendência decrescente no número de casos de ISC ao longo dos anos estudados (A=-153,08; AA:7,51% vs. DA: 3,36% (p<0,001; RR=2,23; IC95%:1,73-2,89). Houve ainda tendência decrescente em relação a complicações operatórias (A=-51,41; AA:7,9% vs. DA: 6,14%; p=0,02; RR=1,29; IC95%:1,03-1,60), reoperações (A=-57,10; AA:/2,65% vs. DA:1,19%; p<0,001; RR=2,22; IC95% 1,43-3,44) e óbitos (A=-62,07;2,81% vs.1,73%; p<<0,01; RR=1,63; IC95%:1,15-2,31). CONCLUSÃO: A introdução do protocolo ACERTO proporcionou melhora dos resultados cirúrgicos, expressos por menor tempo de permanência hospitalar, uso de hemoderivados, e diminuição dos casos de infecção do sítio cirúrgico, complicações operatórias e óbitos
Patient outcome after surgical management of the spinal accessory nerve injury: A long-term follow-up study
Objectives: A lesion in the spinal accessory nerve is typically iatrogenic: related to lymph node biopsy or excision. This injury may cause paralysis of the trapezius muscle and thus result in a characteristic group of symptoms and signs, including depression and winging of the scapula, drooped shoulder, reduced shoulder abduction, and pain. The elements evaluated in this long-term follow-up study include range of shoulder motion, pain, patients’ satisfaction, delay of surgery, surgical procedure, occupational status, functional outcome, and other clinical findings. Methods: We reviewed the medical records of a consecutive 37 patients (11 men and 26 women) having surgery to correct spinal accessory nerve injury. Neurolysis was the procedure in 24 cases, direct nerve repair for 9 patients, and nerve grafting for 4. Time elapsed between the injury and the surgical operation ranged from 2 to 120 months. The patients were interviewed and clinically examined after an average of 10.2 years postoperatively. Results: The mean active range of movement of the shoulder improved at abduction 44° (43%) in neurolysis, 59° (71%) in direct nerve repair, and 30° (22%) in nerve-grafting patients. No or only slight atrophy of the trapezius muscle was observable in 75%, 44%, and 50%, and no or controllable pain was observable in 63%, 56%, and 50%. Restriction of shoulder abduction preceded deterioration of shoulder flexion. Patients’ overall dissatisfaction with the state of their upper extremity was associated with pain, lower strength in shoulder movements, and occupational problems. Conclusion: We recommend avoiding unnecessary delay in the exploration of the spinal accessory nerve, if a neural lesion is suspected
