Field Trials of Health Interventions: A Toolbox
Before new interventions can be used in disease control programmes, it is essential that they are carefully evaluated in âfield trialsâ, which may be complex and expensive undertakings. Descriptions of the detailed procedures and methods used in trials that have been conducted in the past have generally not been published. As a consequence, those planning such trials have few guidelines available and little access to previously accumulated knowledge. In this book the practical issues of trial design and conduct are discussed fully and in sufficient detail for the text to be used as a âtoolboxâ by field investigators. The toolbox has now been extensively tested through use of the first two editions and this third edition is a comprehensive revision, incorporating the many developments that have taken place with respect to trials since 1996 and involving more than 30 contributors. Most of the chapters have been extensively revised and 7 new chapters have been added
Right Hemisphere Partial Complex Seizures: Mania, Hallucinations, and Speech Disturbances During Ictal Events
A patient with right hemisphere complex partial seizures exhibited extreme emotional lability resembling mania, neologisms resembling those found in fluent aphasia, and hallucinations during ictal periods. The electroencephalographic and clinical findings in this case suggest that cortical and subcortical structures of the right hemisphere may play a role in mediating the expression of language content. RĂSUMĂ Chez un patient prĂsentant des crises partielles complexes de l'hĂmisphĂre droit, nous avons observĂ pendant les crises une labilitĂĂmotionnelle Ăvoquant le diagnostic de manie. des nĂologismes ressemblant Ă ceux de l'aphasie de Wernicke et des hallucinations. Nos constatation EEG et cliniques suggĂrent que les structures corticales et sous-corticales de l'hĂmisphĂre droit peuvent jouer un rĂle dans la mĂdiation de l'expression du contenu linguistique. Underwater Acoustic Signatures of Recreational Swimmers, Divers, Surfers and Kayakers
© 2016 Australian Acoustical Society. Non-motorised, recreational water activities were recorded underwater in the controlled setting of a public swimming pool during the off-season. Individuals, one at a time, swam freestyle and breaststroke, snorkelled, scuba-dived, kicked a boogie board and a surfboard, kayaked, and simply jumped into the water. Underwater video and still images were recorded at the same time to interpret the sounds recorded. Most of the sound was due to bubbles generated underwater. Activities involving fins (flippers) were the loudest (boogie boarding and snorkelling), followed by freestyle swimming, surfboard paddling, and kayaking. Breaststroke generated the fewest bubbles and was the quietest. All activities produced bubbles, hence noise, at a characteristic temporal pattern. Scuba-diving exhibited two distinct noise spectra related to inhalation and exhalation. Received levels ranged from 110 to 131 dB re 1 ” Pa (10â16,000 Hz) for all of the activities at the closest point of approach (1 m). The results might have applicability to the monitoring of pools for security reasons, to performance assessments of swimmers, and to studies of the distances at which humans may be detectible by marine animals in the sea
Total humerus replacement for osteosarcoma with proximal part of humerus: a case report
Incisional biopsy and intramedullary pinning were performed for pathological fracture associated with a malignant bone tumor of the proximal humerus. Osteosarcoma, for which preoperative chemotherapy had been performed, was confirmed by postoperative pathological examination. To achieve wide resection and acquire a safe resected margin, total humerus replacement was performed, and the whole humerus was reconstructed using the Howmedica Modular Reconstruction system. The patient resumed normal activities, although mild contracture of the elbow joint remains 8 years after surgery
Extinction times in the subcritical stochastic SIS logistic epidemic
Many real epidemics of an infectious disease are not straightforwardly super-
or sub-critical, and the understanding of epidemic models that exhibit such
complexity has been identified as a priority for theoretical work. We provide
insights into the near-critical regime by considering the stochastic SIS
logistic epidemic, a well-known birth-and-death chain used to model the spread
of an epidemic within a population of a given size . We study the behaviour
of the process as the population size tends to infinity. Our results cover
the entire subcritical regime, including the "barely subcritical" regime, where
the recovery rate exceeds the infection rate by an amount that tends to 0 as but more slowly than . We derive precise asymptotics for
the distribution of the extinction time and the total number of cases
throughout the subcritical regime, give a detailed description of the course of
the epidemic, and compare to numerical results for a range of parameter values.
We hypothesise that features of the course of the epidemic will be seen in a
wide class of other epidemic models, and we use real data to provide some
tentative and preliminary support for this theory.Comment: Revised; 34 pages; 6 figure
Current Status of a Model System: The Gene Gp-9 and Its Association with Social Organization in Fire Ants
The Gp-9 gene in fire ants represents an important model system for studying the evolution of social organization in insects as well as a rich source of information relevant to other major evolutionary topics. An important feature of this system is that polymorphism in social organization is completely associated with allelic variation at Gp-9, such that single-queen colonies (monogyne form) include only inhabitants bearing B-like alleles while multiple-queen colonies (polygyne form) additionally include inhabitants bearing b-like alleles. A recent study of this system by Leal and Ishida (2008) made two major claims, the validity and significance of which we examine here. After reviewing existing literature, analyzing the methods and results of Leal and Ishida (2008), and generating new data from one of their study sites, we conclude that their claim that polygyny can occur in Solenopsis invicta in the U.S.A. in the absence of expression of the b-like allele Gp-9b is unfounded. Moreover, we argue that available information on insect OBPs (the family of proteins to which GP-9 belongs), on the evolutionary/population genetics of Gp-9, and on pheromonal/behavioral control of fire ant colony queen number fails to support their view that GP-9 plays no role in the chemosensory-mediated communication that underpins regulation of social organization. Our analyses lead us to conclude that there are no new reasons to question the existing consensus view of the Gp-9 system outlined in Gotzek and Ross (2007)
Degenerate Stars and Gravitational Collapse in AdS/CFT
We construct composite CFT operators from a large number of fermionic primary
fields corresponding to states that are holographically dual to a zero
temperature Fermi gas in AdS space. We identify a large N regime in which the
fermions behave as free particles. In the hydrodynamic limit the Fermi gas
forms a degenerate star with a radius determined by the Fermi level, and a mass
and angular momentum that exactly matches the boundary calculations. Next we
consider an interacting regime, and calculate the effect of the gravitational
back-reaction on the radius and the mass of the star using the
Tolman-Oppenheimer-Volkoff equations. Ignoring other interactions, we determine
the "Chandrasekhar limit" beyond which the degenerate star (presumably)
undergoes gravitational collapse towards a black hole. This is interpreted on
the boundary as a high density phase transition from a cold baryonic phase to a
hot deconfined phase.Comment: 75 page
