Rapid interhemispheric climate links via the Australasian monsoon during the last deglaciation
Recent studies have proposed that millennial-scale reorganization of the ocean-atmosphere circulation drives increased upwelling in the Southern Ocean, leading to rising atmospheric carbon dioxide levels and ice age terminations. Southward migration of the global monsoon is thought to link the hemispheres during deglaciation, but vital evidence from the southern sector of the vast Australasian monsoon system is yet to emerge. Here we present a 230thorium-dated stalagmite oxygen isotope record of millennial-scale changes in Australian–Indonesian monsoon rainfall over the last 31,000 years. The record shows that abrupt southward shifts of the Australian–Indonesian monsoon were synchronous with North Atlantic cold intervals 17,600–11,500 years ago. The most prominent southward shift occurred in lock-step with Heinrich Stadial 1 (17,600–14,600 years ago), and rising atmospheric carbon dioxide. Our findings show that millennial-scale climate change was transmitted rapidly across Australasia and lend support to the idea that the 3,000-year-long Heinrich 1 interval could have been critical in driving the last deglaciation

The quarter century since the foundation of the Royal College of Ophthalmologists has coincided with immense change in the subspecialty of medical retina, which has moved from being the province of a few dedicated enthusiasts to being an integral, core part of ophthalmology in every eye department. In age-related macular degeneration, there has been a move away from targeted, destructive laser therapy, dependent on fluorescein angiography to intravitreal injection therapy of anti-growth factor agents, largely guided by optical coherence tomography. As a result of these changes, ophthalmologists have witnessed a marked improvement in visual outcomes for their patients with wet age-related macular degeneration (AMD), while at the same time developing and enacting entirely novel ways of delivering care. In the field of diabetic retinopathy, this period also saw advances in laser technology and a move away from highly destructive laser photocoagulation treatment to gentler retinal laser treatments. The introduction of intravitreal therapies, both steroids and anti-growth factor agents, has further advanced the treatment of diabetic macular oedema. This era has also seen in the United Kingdom the introduction of a coordinated national diabetic retinopathy screening programme, which offers an increasing hope that the burden of blindness from diabetic eye disease can be lessened. Exciting future advances in retinal imaging, genetics, and pharmacology will allow us to further improve outcomes for our patients and for ophthalmologists specialising in medical retina, the future looks very exciting but increasingly busy

'Statins in retinal disease'
Statins are known for their blood cholesterol-lowering effect and are widely used in patients with cardiovascular and metabolic diseases. Research over the past three decades shows that statins have diverse effects on different pathophysiological pathways involved in angiogenesis, inflammation, apoptosis, and anti-oxidation, leading to new therapeutic options. Recently, statins have attracted considerable attention for their immunomodulatory effect. Since immune reactivity has been implicated in a number of retinal diseases, such as uveitis, age-related macular degeneration (AMD) and diabetic retinopathy, there is now a growing body of evidence supporting the beneficial effects of statins in these retinopathies. This review evaluates the relationship between statins and the pathophysiological basis of these diseases, focusing on their potential role in treatment. A PubMed database search and literature review was conducted. Among AMD patients, there is inconsistent evidence regarding protection against development of early AMD or delaying disease progression; though they have been found to reduce the risk of developing choroidal neovascular membranes (CNV). In patients with retinal vein occlusion, there was no evidence to support a therapeutic benefit or a protective role with statins. In patients with diabetic retinopathy, statins demonstrate a reduction in disease progression and improved resolution of diabetic macular oedema (DMO). Among patients with uveitis, statins have a protective effect by reducing the likelihood of uveitis development
Indonesian vegetation response to changes in rainfall seasonality over the past 25,000 years
The hydrologic response to climate forcing in the Indo-Pacific warm pool region has varied spatially over the past 25,000 years. For example, drier conditions are inferred on Java and Borneo for the period following the end of the Last Glacial Maximum, whereas wetter conditions are reconstructed for northwest Australia. The response of vegetation to these past rainfall variations is poorly constrained. Using a suite of 30 surface marine sediment samples from throughout the Indo-Pacific warm pool, we demonstrate that today the stable isotopic composition of vascular plant fatty acids (13 Cfa) reflects the regional vegetation composition. This in turn is controlled by the seasonality of rainfall consistent with dry season water stress. Applying this proxy in a sediment core from offshore northeast Borneo, we show broadly similar vegetation cover during the Last Glacial Maximum and the Holocene, suggesting that, despite generally drier glacial conditions, there was no pronounced dry season. In contrast, 13Cfa and pollen data from a core off the coast of Sumba indicate an expansion of C 4 herbs during the most recent glaciation, implying enhanced aridity and water stress during the dry season. Holocene vegetation trends are also consistent with a response to dry season water stress. We therefore conclude that vegetation in tropical monsoon regions is susceptible to increases in water stress arising from an enhanced seasonality of rainfall, as has occurred in past decades. © 2014 Macmillan Publishers Limited
Visual fields in patients with multifocal intraocular lens implants and monovision:an exploratory study
PurposeTo assess the difference in binocular visual fields (VFs) in patients who underwent bilateral cataract surgery with either multifocal (MF; Tecnis ZM900, AMO) intraocular lenses (IOLs) or monofocal IOLs with powers adjusted to give monovision (MV; Akreos AO, Bausch&Lomb).SettingSt George's & Moorfields Eye Hospital, London.MethodsProspective exploratory study. Binocular Esterman VFs (Humphrey Field Analyser II) were compared between 10 participants with MV and 16 participants with MF IOLs. The dominant eye in MV participants had 0 to 0.50DS and the non-dominant eye had between 1.0DS and 1.5DS. Best-corrected Snellen visual acuity for all 52 eyes was six out of nine or better. The main outcome measure was Esterman Efficiency Score. Incidence of suboptimal VF results (≥1 Unseen LocationsULs) and mean testing times in the two groups were compared.ResultsThere was no statistically significant difference in the incidence of suboptimal VF results in these two groups (P=0.662). Test durations in the two groups were not significantly different (P=0.650). However, 310 MV plots (33) had markedly suboptimal right hemi-fields (distance-dominant eye) compared with 015 MF plots. Additionally, the MV group accounted for 79 of total ULs (2029) and all these ULs were distributed in areas outside or bordering the true binocular VFs.ConclusionsAll included VFs met the UK driving standards criteria. The pattern of VF defects encountered in the MV group is of interest because the majority of ULs (1320; 65) corresponded to the monocular VFs of the distance-dominant eye. © 2010 Macmillan Publishers Limited. Testing the annual nature of speleothem banding
Speleothem laminae have been postulated to form annually and this lamina-chronology is widely applied to high-resolution modern and past climate reconstructions. However, this argument has not been directly supported by high resolution dating methods. Here we present contemporary single-lamina 230Th dating techniques with 2σ precision as good as ±0.5 yr on a laminated stalagmite with density couplets from Xianren Cave, China, that covers the last 300 years. We find that the layers do not always deposit annually. Annual bands can be under- or over-counted by several years during different multi-decadal intervals. The irregular formation of missing and false bands in this example indicates that the assumption of annual speleothem laminae in a climate reconstruction should be approached carefully without a robust absolute-dated chronology
