    Emergence of thin shell structure during collapse in isotropic coordinates

    Numerical studies of gravitational collapse in isotropic coordinates have recently shown an interesting connection between the gravitational Lagrangian and black hole thermodynamics. A study of the actual spacetime was not the main focus of this work and in particular, the rich and interesting structure of the interior has not been investigated in much detail and remains largely unknown. We elucidate its features by performing a numerical study of the spacetime in isotropic coordinates during gravitational collapse of a massless scalar field. The most salient feature to emerge is the formation of a thin shell of matter just inside the apparent horizon. The energy density and Ricci scalar peak at the shell and there is a jump discontinuity in the extrinsic curvature across the apparent horizon, the hallmark that a thin shell is present in its vicinity. At late stages of the collapse, the spacetime consists of two vacuum regions separated by the thin shell. The interior is described by an interesting collapsing isotropic universe. It tends towards a vacuum (never reaches a perfect vacuum) and there is a slight inhomogeneity in the interior that plays a crucial role in the collapse process as the areal radius tends to zero. The spacetime evolves towards a curvature (physical) singularity in the interior, both a Weyl and Ricci singularity. In the exterior, our numerical results match closely the analytical form of the Schwarzschild metric in isotropic coordinates, providing a strong test of our numerical code.Comment: 24 pages, 10 figures. version to appear in Phys.     Dietary Quality is Associated with Better Self-Efficacy and Depression in Patients with Fibromyalgia from a Comparative Effectiveness Trial: A Small Pilot Study

    Background: Diet and exercise have been suggested as potentially beneficial for symptom management of fibromyalgia; however, data on the relationship between diet quality and fibromyalgia symptoms are limited. Objective: To investigate diet quality and its relationship with 1) fibromyalgia impact, and 2) psychological health in patients with fibromyalgia who participated in a randomized controlled trial (RCT) of Tai Chi versus aerobic exercise for fibromyalgia. Methods: A cross-sectional study enrolled participants from a RCT. Diet quality and fibromyalgia symptoms were assessed using the Healthy Eating Index 2010 (HEI-2010) and Revised Fibromyalgia Impact Questionnaire (FIQR), respectively. Retrospective analyses were performed using Spearman’s coefficient (r) to examine the association of diet quality with pre-intervention FIQR, psychological variables, and quality of life. Results: Twenty-six female participants (mean age = 56 y; mean pre-intervention BMI = 29.6) of 223 trial participants (11.7%) were included in the analyses. Higher diet quality was associated with higher pre-intervention Chronic Pain Self-Efficacy scores (r=0.62, p=0.01), and lower Hospital Depression scores (r=-0.47, p=0.02). There were no significant associations between diet quality and pre-intervention severity of depressive symptoms, mental or physical health quality of life, sleep quality, or FIQR scores. Conclusion: Preliminary results suggest a positive association between diet quality and self-efficacy and psychological health in women with fibromyalgia. Future prospective studies are needed

    The clinical features of the piriformis syndrome: a systematic review

    Piriformis syndrome, sciatica caused by compression of the sciatic nerve by the piriformis muscle, has been described for over 70 years; yet, it remains controversial. The literature consists mainly of case series and narrative reviews. The objectives of the study were: first, to make the best use of existing evidence to estimate the frequencies of clinical features in patients reported to have PS; second, to identify future research questions. A systematic review was conducted of any study type that reported extractable data relevant to diagnosis. The search included all studies up to 1 March 2008 in four databases: AMED, CINAHL, Embase and Medline. Screening, data extraction and analysis were all performed independently by two reviewers. A total of 55 studies were included: 51 individual and 3 aggregated data studies, and 1 combined study. The most common features found were: buttock pain, external tenderness over the greater sciatic notch, aggravation of the pain through sitting and augmentation of the pain with manoeuvres that increase piriformis muscle tension. Future research could start with comparing the frequencies of these features in sciatica patients with and without disc herniation or spinal stenosis

    Irradiation of the potential cancer stem cell niches in the adult brain improves progression-free survival of patients with malignant glioma

    <p>Abstract</p> <p>Background</p> <p>Glioblastoma is the most common brain tumor in adults. The mechanisms leading to glioblastoma are not well understood but animal studies support that inactivation of tumor suppressor genes in neural stem cells (NSC) is required and sufficient to induce glial cancers. This suggests that the NSC niches in the brain may harbor cancer stem cells (CSCs), Thus providing novel therapy targets. We hypothesize that higher radiation doses to these NSC niches improve patient survival by eradicating CSCs.</p> <p>Methods</p> <p>55 adult patients with Grade 3 or Grade 4 glial cancer treated with radiotherapy at UCLA between February of 2003 and May of 2009 were included in this retrospective study. Using radiation planning software and patient radiological records, the SVZ and SGL were reconstructed for each of these patients and dosimetry data for these structures was calculated.</p> <p>Results</p> <p>Using Kaplan-Meier analysis we show that patients whose bilateral subventricular zone (SVZ) received greater than the median SVZ dose (= 43 Gy) had a significant improvement in progression-free survival if compared to patients who received less than the median dose (15.0 vs 7.2 months PFS; P = 0.028). Furthermore, a mean dose >43 Gy to the bilateral SVZ yielded a hazard ratio of 0.73 (P = 0.019). Importantly, similarly analyzing total prescription dose failed to illustrate a statistically significant impact.</p> <p>Conclusions</p> <p>Our study leads us to hypothesize that in glioma targeted radiotherapy of the stem cell niches in the adult brain could yield significant benefits over radiotherapy of the primary tumor mass alone and that damage caused by smaller fractions of radiation maybe less efficiently detected by the DNA repair mechanisms in CSCs.</p

    Aflatoxin-Exposure of Vibrio gazogenes as a Novel System for the Generation of Aflatoxin Synthesis Inhibitors

    Aflatoxin is a mycotoxin and a secondary metabolite, and the most potent known liver carcinogen that contaminates several important crops, and represents a significant threat to public health and the economy. Available approaches reported thus far have been insufficient to eliminate this threat, and therefore provide the rational to explore novel methods for preventing aflatoxin accumulation in the environment. Many terrestrial plants and microbes that share ecological niches and encounter the aflatoxin producers have the ability to synthesize compounds that inhibit aflatoxin synthesis. However, reports of natural aflatoxin inhibitors from marine ecosystem components that do not share ecological niches with the aflatoxin producers are rare. Here, we show that a non-pathogenic marine bacterium, Vibrio gazogenes, when exposed to low non-toxic doses of aflatoxin B1, demonstrates a shift in its metabolic output and synthesizes a metabolite fraction that inhibits aflatoxin synthesis without affecting hyphal growth in the model aflatoxin producer, Aspergillus parasiticus. The molecular mass of the predominant metabolite in this fraction was also different from the known prodigiosins, which are the known antifungal secondary metabolites synthesized by this Vibrio. Gene expression analyses using RT-PCR demonstrate that this metabolite fraction inhibits aflatoxin synthesis by down-regulating the expression of early-, middle-, and late- growth stage aflatoxin genes, the aflatoxin pathway regulator, aflR and one global regulator of secondary metabolism, laeA. Our study establishes a novel system for generation of aflatoxin synthesis inhibitors, and emphasizes the potential of the under-explored Vibrio’s silent genome for generating new modulators of fungal secondary metabolism

    A polycystic variant of a primary intracranial leptomeningeal astrocytoma: case report and literature review

    <p>Abstract</p> <p>Background</p> <p>Primary leptomeningeal astrocytomas are rare intracranial tumors. These tumors are believed to originate from cellular nests which migrate by means of aberration, ultimately settling in the leptomeningeal structure. They may occur in both solitary and diffuse forms. The literature reports only fifteen cases of solitary primary intracranial leptomeningeal astrocytomas.</p> <p>Case presentation</p> <p>The authors report the case of a seventy-eight year-old woman with a polycystic variant of a solitary primary intracranial leptomeningeal astrocytoma. The first neurological signs were seizures and aphasia. CT and MRI scans demonstrated a fronto-parietal polycystic tumor adherent to the sub arachnoid space. A left fronto-temporo-parietal craniotomy revealed a tight coalescence between the tumor and the arachnoid layer which appeared to wrap the mass entirely. Removal of the deeper solid part of the tumor resulted difficult due to the presence of both a high vascularity and a tight adherence between the tumor and the ventricular wall.</p> <p>Conclusion</p> <p>A new case of a solitary primitive intracranial leptomeningeal astrocytoma of a rare polycystic variant is reported. Clinical, surgical, pathologic and therapeutic aspects of this tumor are discussed.</p
