    Thrombus in the Non-aneurysmal, Non-atherosclerotic Descending Thoracic Aorta – An Unusual Source of Arterial Embolism

    AbstractIntroductionMural thrombus of the thoracic aorta is a rare clinical finding in the absence of aneurysm or atherosclerosis.MethodsThe medical records of all patients diagnosed with a thrombus of a non-aneurysmatic and non-atherosclerotic descending thoracic aorta (NAADTA) and treated by the senior author between 04/1997 and 04/2010 were reviewed.ResultsEight patients with mural thrombus of the NAADTA were identified. Arterial embolism was the main clinical finding in all cases and involved the lower extremities (n = 6), mesenteric (n = 3) or renal arteries (n = 2). Hypercoagulable disorders were present in 3 cases and a concurrent malignancy in another 3. Two patients underwent open surgery while 4 patients were treated conservatively with anticoagulation. Of the remaining 2 patients, one was treated with a thoracic stent-graft and aorto-biiliac bypass and the other one with transfemoral thrombectomy. Technical success was achieved in all surgical cases and thrombus resolution or stable disease in the conservative management group. No thrombus recurrence was observed during a mean follow-up of 49 months.ConclusionThe management of mural thrombus in NAADTA represents a challenge, especially in case of malignant disease or hypercoagulable disorder as a potential underlying pathology and should be individualized. Although no consensus exists in the literature, therapeutic anticoagulation is proposed as first-line therapy. The indication for surgical intervention results from contraindication to anticoagulation, mobile thrombus or recurrent embolism. Whenever possible, endovascular therapy should be preferred

    Sea ice concentration satellite retrievals influenced by surface changes due to warm air intrusions: A case study from the MOSAiC expedition

    Warm air intrusions over Arctic sea ice can change the snow and ice surface conditions rapidly and can alter sea ice concentration (SIC) estimates derived from satellite-based microwave radiometry without altering the true SIC. Here we focus on two warm moist air intrusions during the Multidisciplinary drifting Observatory for the Study of Arctic Climate (MOSAiC) expedition that reached the research vessel Polarstern in mid-April 2020. After the events, SIC deviations between different satellite products, including climate data records, were observed to increase. Especially, an underestimation of SIC for algorithms based on polarization difference was found. To examine the causes of this underestimation, we used the extensive MOSAiC snow and ice measurements to model computationally the brightness temperatures of the surface on a local scale. We further investigated the brightness temperatures observed by ground-based radiometers at frequencies 6.9 GHz, 19 GHz, and 89 GHz. We show that the drop in the retrieved SIC of some satellite products can be attributed to large-scale surface glazing, that is, the formation of a thin ice crust at the top of the snowpack, caused by the warming events. Another mechanism affecting satellite products, which are mainly based on gradient ratios of brightness temperatures, is the interplay of the changed temperature gradient in the snow with snow metamorphism. From the two analyzed climate data record products, we found that one was less affected by the warming events. The low frequency channels at 6.9 GHz were less sensitive to these snow surface changes, which could be exploited in future to obtain more accurate retrievals of sea ice concentration. Strong warm air intrusions are expected to become more frequent in future and thus their influence on SIC algorithms will increase. In order to provide consistent SIC datasets, their sensitivity to warm air intrusions needs to be addressed

    A Theory of Cheap Control in Embodied Systems

    We present a framework for designing cheap control architectures for embodied agents. Our derivation is guided by the classical problem of universal approximation, whereby we explore the possibility of exploiting the agent's embodiment for a new and more efficient universal approximation of behaviors generated by sensorimotor control. This embodied universal approximation is compared with the classical non-embodied universal approximation. To exemplify our approach, we present a detailed quantitative case study for policy models defined in terms of conditional restricted Boltzmann machines. In contrast to non-embodied universal approximation, which requires an exponential number of parameters, in the embodied setting we are able to generate all possible behaviors with a drastically smaller model, thus obtaining cheap universal approximation. We test and corroborate the theory experimentally with a six-legged walking machine. The experiments show that the sufficient controller complexity predicted by our theory is tight, which means that the theory has direct practical implications.     Finding a short and accurate decision rule in disjunctive normal form by exhaustive search

    Greedy approaches suffer from a restricted search space which could lead to suboptimal classifiers in terms of performance and classifier size. This study discusses exhaustive search as an alternative to greedy search for learning short and accurate decision rules. The Exhaustive Procedure for LOgic-Rule Extraction (EXPLORE) algorithm is presented, to induce decision rules in disjunctive normal form (DNF) in a systematic and efficient manner. We propose a method based on subsumption to reduce the number of values considered for instantiation in the literals, by taking into account the relational operator without loss of performance. Furthermore, we describe a branch-and-bound approach that makes optimal use of user-defined performance constraints. To improve the generalizability we use a validation set to determine the optimal length of the DNF rule. The performance and size of the DNF rules induced by EXPLORE are compared to those of eight well-known rule learners. Our results show that an exhaustive approach to rule learning in DNF results in significantly smaller classifiers than those of the other rule learners, while securing comparable or even better performance. Clearly, exhaustive search is computer-intensive and may not always be feasible. Nevertheless, based on this study, we believe that exhaustive search should be considered an alternative for greedy search in many problems

    Lattice Blind Signatures with Forward Security

    Blind signatures play an important role in both electronic cash and electronic voting systems. Blind signatures should be secure against various attacks (such as signature forgeries). The work puts a special attention to secret key exposure attacks, which totally break digital signatures. Signatures that resist secret key exposure attacks are called forward secure in the sense that disclosure of a current secret key does not compromise past secret keys. This means that forward-secure signatures must include a mechanism for secret-key evolution over time periods. This paper gives a construction of the first blind signature that is forward secure. The construction is based on the SIS assumption in the lattice setting. The core techniques applied are the binary tree data structure for the time periods and the trapdoor delegation for the key-evolution mechanism.Comment: ACISP 202

    Constant-Size Structure-Preserving Signatures: Generic Constructions and Simple Assumptions

    This paper presents efficient structure-preserving signature schemes based on assumptions as simple as Decision-Linear. We first give two general frameworks for constructing fully secure signature schemes from weaker building blocks such as variations of one-time signatures and random-message secure signatures. They can be seen as refinements of the Even-Goldreich-Micali framework, and preserve many desirable properties of the underlying schemes such as constant signature size and structure preservation. We then instantiate them based on simple (i.e., not q-type) assumptions over symmetric and asymmetric bilinear groups.     Genome-wide association and HLA fine-mapping studies identify risk loci and genetic pathways underlying allergic rhinitis

    Allergic rhinitis is the most common clinical presentation of allergy, affecting 400 million people worldwide, with increasing incidence in westernized countries1,2. To elucidate the genetic architecture and understand the underlying disease mechanisms, we carried out a meta-analysis of allergic rhinitis in 59,762 cases and 152,358 controls of European ancestry and identified a total of 41 risk loci for allergic rhinitis, including 20 loci not previously associated with allergic rhinitis, which were confirmed in a replication phase of 60,720 cases and 618,527 controls. Functional annotation implicated genes involved in various immune pathways, and fine mapping of the HLA region suggested amino acid variants important for antigen binding. We further performed genome-wide association study (GWAS) analyses of allergic sensitization against inhalant allergens and nonallergic rhinitis, which suggested shared genetic mechanisms across rhinitis-related traits. Future studies of the identified loci and genes might identify novel targets for treatment and prevention of allergic rhinitis

    Dengue type 1 viruses circulating in humans are highly infectious and poorly neutralized by human antibodies

    The four dengue virus (DENV) serotypes are mosquito-borne flaviviruses of humans. The interactions between DENVs and the human host that lead to asymptomatic, mild, or severe disease are poorly understood, in part, because laboratory models are poor surrogates for human DENV disease. Virologists are interested in how the properties of DENVs replicating in people compare with virions propagated on laboratory cell lines, which are widely used for research and vaccine development. Using clinical samples from a DENV type 1 epidemic in Sri Lanka and new ultrasensitive assays, we compared the properties of DENVs in human plasma and after one passage on laboratory cell lines. DENVs in plasma were 50- to 700-fold more infectious than cell culture-grown viruses. DENVs produced by laboratory cell lines were structurally immature and hypersensitive to neutralization by human antibodies compared with DENVs circulating in people. Human plasma and cell culture-derived virions had identical genome sequences, indicating that these phenotypic differences were due to the mature state of plasma virions. Several dengue vaccines are under development. Recent studies indicate that vaccine-induced antibodies that neutralized DENVs in cell culture assays were not sufficient for protecting people from DENV infections. Our results about structural differences between DENVs produced in humans versus cell lines may be key to understanding vaccine failure and developing better models for vaccine evaluation
