Temporal Trends in Incidence, Sepsis-Related Mortality, and Hospital-Based Acute Care After Sepsis.
OBJECTIVES: A growing number of patients survive sepsis hospitalizations each year and are at high risk for readmission. However, little is known about temporal trends in hospital-based acute care (emergency department treat-and-release visits and hospital readmission) after sepsis. Our primary objective was to measure temporal trends in sepsis survivorship and hospital-based acute care use in sepsis survivors. In addition, because readmissions after pneumonia are subject to penalty under the national readmission reduction program, we examined whether readmission rates declined after sepsis hospitalizations related to pneumonia.
DESIGN AND SETTING: Retrospective, observational cohort study conducted within an academic healthcare system from 2010 to 2015.
PATIENTS: We used three validated, claims-based approaches to identify 17,256 sepsis or severe sepsis hospitalizations to examine trends in hospital-based acute care after sepsis.
INTERVENTIONS: None.
MEASUREMENTS AND MAIN RESULTS: From 2010 to 2015, sepsis as a proportion of medical and surgical admissions increased from 3.9% to 9.4%, whereas in-hospital mortality rate for sepsis hospitalizations declined from 24.1% to 14.8%. As a result, the proportion of medical and surgical discharges at-risk for hospital readmission after sepsis increased from 2.7% to 7.8%. Over 6 years, 30-day hospital readmission rates declined modestly, from 26.4% in 2010 to 23.1% in 2015, driven largely by a decline in readmission rates among survivors of nonsevere sepsis, and nonpneumonia sepsis specifically, as the readmission rate of severe sepsis survivors was stable. The modest decline in 30-day readmission rates was offset by an increase in emergency department treat-and-release visits, from 2.8% in 2010 to a peak of 5.4% in 2014.
CONCLUSIONS: Owing to increasing incidence and declining mortality, the number of sepsis survivors at risk for hospital readmission rose significantly between 2010 and 2015. The 30-day hospital readmission rates for sepsis declined modestly but were offset by a rise in emergency department treat-and-release visits
Transcription and processing of the rodent ID repeat family in germline and somatic cells
ID elements comprise a rodent SINE (short Interspersed DNA repetitive element) family that has amplified by retroposition of a few master genes. In order to understand the Important factors of SINE amplification, we investigated the transcription of rat ID elements. Three different size classes of ID transcripts, BC1, BC2 and T3, have been detected In various rat tissues, including brain and testes. We have analysed the nucleotlde sequences of testes- and brain-derived ID transcripts isolated by size-fractlonation, C-talling and RACE. Nucleotide sequence variation of testes ID transcripts demonstrated derivation from different loci. However, the transcripts represent a preferred set of ID elements that closely match the subfamily consensus sequences. The small ID transcripts, T3, are not comprised of primary transcripts, but are instead processed polyA- transcripts generated from many different loci. These truncated transcripts would be expected to be retroposition-incompetent forms. Therefore, the amplification of ID elements is likely to be regulated at multiple steps of retroposition, which Include transcription and processing. Although brain ID transcripts showed a similar pattern, with the addition of very high levels of transcription from the BC1 locus, we also found evidence that a single locus dominated the production of brain BC2 RNA species. BC1 RNA is highly stable In both germ line and brain cells, based on the low level of detection of the processing product, T3. This stability of BC1 RNA might have been a contributing factor in its role as a master gene for ID amplification. © 1995 Oxford University Press
The Stripe 82 1-2 GHz Very Large Array Snapshot Survey: Multiwavelength Counterparts
We have combined spectrosopic and photometric data from the Sloan Digital Sky
Survey (SDSS) with GHz radio observations, conducted as part of the
Stripe 82 GHz Snapshot Survey using the Karl G. Jansky Very Large Array
(VLA), which covers sq degrees, to a flux limit of 88 Jy rms.
Cross-matching the radio source components with optical data via
visual inspection results in a final sample of cross-matched objects,
of which have spectroscopic redshifts and objects have
photometric redshifts. Three previously undiscovered Giant Radio Galaxies
(GRGs) were found during the cross-matching process, which would have been
missed using automated techniques. For the objects with spectroscopy we
separate radio-loud Active Galactic Nuclei (AGN) and star-forming galaxies
(SFGs) using three diagnostics and then further divide our radio-loud AGN into
the HERG and LERG populations. A control matched sample of HERGs and LERGs,
matched on stellar mass, redshift and radio luminosity, reveals that the host
galaxies of LERGs are redder and more concentrated than HERGs. By combining
with near-infrared data, we demonstrate that LERGs also follow a tight
relationship. These results imply the LERG population are hosted by population
of massive, passively evolving early-type galaxies. We go on to show that
HERGs, LERGs, QSOs and star-forming galaxies in our sample all reside in
different regions of a WISE colour-colour diagram. This cross-matched sample
bridges the gap between previous `wide but shallow' and `deep but narrow'

Distant galaxy clusters in the COSMOS field found by HIROCS
We present the first high-redshift galaxy cluster candidate sample from the
HIROCS survey found in the COSMOS field. It results from a combination of
public COSMOS with proprietary H-band data on a 0.66 square degree part of the
COSMOS field and comprises 12 candidates in the redshift range 1.23 < z < 1.55.
We find an increasing fraction of blue cluster members with increasing
redshift. Many of the blue and even some of the reddest member galaxies exhibit

Parity violating pion electroproduction off the nucleon
Parity violating (PV) contributions due to interference between and
exchange are calculated for pion electroproduction off the nucleon. A
phenomenological model with effective Lagrangians is used to determine the
resulting asymmetry for the energy region between threshold and
resonance. The resonance is treated as a Rarita-Schwinger field with
phenomenological transition currents. The background contributions
are given by the usual Born terms using the pseudovector Lagrangian.

Trial and Settlement: A Study of High-Low Agreements
This paper presents the first systematic theoretical and empirical study of high-low agreements in civil litigation. A high-low agreement is a private contract that, if signed by litigants before the conclusion of a trial, constrains the future damages payment to lie between a minimum and a maximum amount. Whereas the existing literature describes litigation as a choice between trial and settlement, our examination of high-low agreements—a relatively new phenomenon in civil litigation—introduces partial or incomplete settlements. In our theoretical model, trial is both costly and risky. When litigants have divergent subjective beliefs and are mutually optimistic about their trial prospects, cases may fail to settle. In these cases, high-low agreements can be in the litigants’ mutual interest because they limit the risk of outlier damages awards while still allowing for an optimal degree of speculation. Using unique insurance claims data from a national insurance company, we describe the features of these agreements and empirically investigate the factors that may influence whether litigants discuss or enter into high-low agreements. Our empirical findings are consistent with the predictions of the theoretical model. We also explore extensions and alternative explanations for high-low agreements, including their use to mitigate excessive, offsetting trial expenditures
The Diversity of Diffuse Ly Nebulae around Star-Forming Galaxies at High Redshift
We report the detection of diffuse Ly emission, or Ly halos
(LAHs), around star-forming galaxies at and in the NOAO
Deep Wide-Field Survey Bo\"otes field. Our samples consist of a total of
1400 galaxies, within two separate regions containing spectroscopically
confirmed galaxy overdensities. They provide a unique opportunity to
investigate how the LAH characteristics vary with host galaxy large-scale
environment and physical properties. We stack Ly images of different
samples defined by these properties and measure their median LAH sizes by
decomposing the stacked Ly radial profile into a compact galaxy-like
and an extended halo-like component. We find that the exponential scale-length
of LAHs depends on UV continuum and Ly luminosities, but not on
Ly equivalent widths or galaxy overdensity parameters. The full
samples, which are dominated by low UV-continuum luminosity Ly emitters
(), exhibit LAH sizes of 5kpc. However, the
most UV- or Ly-luminous galaxies have more extended halos with
scale-lengths of 7kpc. The stacked Ly radial profiles decline
more steeply than recent theoretical predictions that include the contributions
from gravitational cooling of infalling gas and from low-level star formation
in satellites. On the other hand, the LAH extent matches what one would expect
for photons produced in the galaxy and then resonantly scattered by gas in an
outflowing envelope. The observed trends of LAH sizes with host galaxy
properties suggest that the physical conditions of the circumgalactic medium
(covering fraction, HI column density, and outflow velocity) change with halo
