Action heritage: research, communities, social justice
Societies are unequal and unjust to varying degrees and heritage practitioners unavoidably work with, perpetuate and have the potential to change these inequalities. This article proposes a new framework for undertaking heritage research that can be applied widely and purposefully to achieve social justice, and which we refer to as action heritage. Our primary sources are semi-structured conversations we held with some of the participants in three heritage projects in South Yorkshire, UK: members of a hostel for homeless young people, a primary school, and a local history group. We examine ‘disruptions’ in the projects to understand the repositioning of the participants as researchers. The disruptions include introducing a scrapbook for personal stories in the homeless youth project and giving the school children opportunities to excavate alongside professional archaeologists. These disruptions reveal material and social inequalities through perceptible changes in how the projects were oriented and how the participants thought about the research. We draw on this empirical research and theorisations of social justice to develop a new framework for undertaking co-produced research. Action heritage is ‘undisciplinary’ research that privileges process over outcomes, and which achieves parity of participation between academic and community-based researchers through sustained recognition and redistribution
Magnetic resonance imaging phantoms for quality-control of myocardial T1 and ECV mapping: specific formulation, long-term stability and variation with heart rate and temperature
Background: Magnetic resonance imaging (MRI) phantoms are routinely used for quality assurance in MRI centres; however their long term stability for verification of myocardial T1/ extracellular volume fraction (ECV) mapping has never been investigated. Methods: Nickel-chloride agarose gel phantoms were formulated in a reproducible laboratory procedure to mimic blood and myocardial T1 and T2 values, native and late after Gadolinium administration as used in T1/ECV mapping. The phantoms were imaged weekly with an 11 heart beat MOLLI sequence for T1 and long TR spin-echo sequences for T2, in a carefully controlled reproducible manner for 12 months. Results: There were only small relative changes seen in all the native and post gadolinium T1 values (up to 9.0 % maximal relative change in T1 values) or phantom ECV (up to 8.3 % maximal relative change of ECV, up to 2.2 % maximal absolute change in ECV) during this period. All native and post gadolinium T2 values remained stable over time with <2 % change. Temperature sensitivity testing showed MOLLI T1 values in the long T1 phantoms increasing by 23.9 ms per degree increase and short T1 phantoms increasing by 0.3 ms per degree increase. There was a small absolute increase in ECV of 0.069 % (~0.22 % relative increase in ECV) per degree increase. Variation in heart rate testing showed a 0.13 % absolute increase in ECV (~0.45 % relative increase in ECV) per 10 heart rate increase. Conclusions: These are the first phantoms reported in the literature modeling T1 and T2 values for blood and myocardium specifically for the T1mapping/ECV mapping application, with stability tested rigorously over a 12 month period. This work has significant implications for the utility of such phantoms in improving the accuracy of serial scans for myocardial tissue characterisation by T1 mapping methods and in multicentre work
Australian health care providers' views on opt-out HIV testing
Background: Opt-out HIV testing is a novel concept in Australia. In the opt-out approach, health care providers (HCPs) routinely test patients for HIV unless they explicitly decline or defer. Opt-out HIV testing is only performed with the patients' consent, but pre-test counselling is abbreviated. Australian national testing guidelines do not currently recommend opt-out HIV testing for the general population. Non-traditional approaches to HIV testing (such as opt-out) could identify HIV infections and facilitate earlier treatment, which is particularly important now that HIV is a chronic, manageable disease. Our aim was to explore HCPs' attitudes toward opt-out HIV testing in an Australian context, to further understanding of its acceptability and feasibility. Methods: In this qualitative study, we used purposeful sampling to recruit HCPs who were likely to have experience with HIV testing in Western Australia. We interviewed them using a semi-structured guide and used content analysis as per Graneheim to code the data. Codes were then merged into subcategories and finally themes that unified the underlying concepts. We refined these themes through discussion among the research team. Results: Twenty four HCPs participated. Eleven participants had a questioning attitude toward opt-out HIV testing, while eleven favoured the approach. The remaining two participants had more nuanced perspectives that incorporated some characteristics of the questioning and favouring attitudes. Participants' views about opt-out HIV testing largely fell into two contrasting themes: normalisation and routinisation versus exceptionalism; and a need for proof versus openness to new approaches. Conclusion: Most HCPs in this study had dichotomous attitudes toward opt-out HIV testing, reflecting contrasting analytical styles. While some HCPs viewed it favourably, with the perceived benefits outweighing the perceived costs, others preferred to have evidence of efficacy and cost-effectiveness
Evolutionary Characterization of Lung Adenocarcinoma Morphology in TRACERx
Lung adenocarcinomas (LUADs) display a broad histological spectrum from low-grade lepidic tumors through to mid-grade acinar and papillary and high-grade solid, cribriform and micropapillary tumors. How morphology reflects tumor evolution and disease progression is poorly understood. Whole-exome sequencing data generated from 805 primary tumor regions and 121 paired metastatic samples across 248 LUADs from the TRACERx 421 cohort, together with RNA-sequencing data from 463 primary tumor regions, were integrated with detailed whole-tumor and regional histopathological analysis. Tumors with predominantly high-grade patterns showed increased chromosomal complexity, with higher burden of loss of heterozygosity and subclonal somatic copy number alterations. Individual regions in predominantly high-grade pattern tumors exhibited higher proliferation and lower clonal diversity, potentially reflecting large recent subclonal expansions. Co-occurrence of truncal loss of chromosomes 3p and 3q was enriched in predominantly low-/mid-grade tumors, while purely undifferentiated solid-pattern tumors had a higher frequency of truncal arm or focal 3q gains and SMARCA4 gene alterations compared with mixed-pattern tumors with a solid component, suggesting distinct evolutionary trajectories. Clonal evolution analysis revealed that tumors tend to evolve toward higher-grade patterns. The presence of micropapillary pattern and \u27tumor spread through air spaces\u27 were associated with intrathoracic recurrence, in contrast to the presence of solid/cribriform patterns, necrosis and preoperative circulating tumor DNA detection, which were associated with extra-thoracic recurrence. These data provide insights into the relationship between LUAD morphology, the underlying evolutionary genomic landscape, and clinical and anatomical relapse risk
Can English perceivers match cantonese auditory and visual prosody?
The prosody of an utterance can be varied by changing F0, duration and amplitude. Such changes are typically accompanied by variation in the talker’s face/head motion (visual prosody). For native language utterances, people can match auditory and visual prosody accurately. We tested whether English perceivers can do this with an unfamiliar language, Cantonese, which differs from English specifically with regard to suprasegmental properties (e.g., different rhythm type; use of lexical tone). These differences may make extraction of prosody difficult, because they distract English perceivers and/or because they affect the way prosody is realized. However, AV cues for prosody may be similar across languages and sufficiently salient to overcome the suprasegmental differences. We tested native Australian- English participants (N=27) with 50 Cantonese sentences spoken as questions, narrowly focused or broad focused utterances by two native Cantonese talkers. Participants completed a same-different matching task for auditoryauditory (AA); visual-visual (VV) and auditory-visual (AV) pairs. Each pair type consisted of the same sentence and talker, but different tokens. Matching performance was above chance for all conditions: AA > AV = VV. Results are discussed in terms of how auditory and visual prosody is conveyed and how this may be affected by language properties
Investigating the role of familiar face and voice cues in speech processing in noise
The speech of a familiar talker is better recognized in noise than an unfamiliar one, suggesting that listeners access talkerspecific models to assist with degraded input. This study investigated whether a talker model could be accessed by presenting the face of a talker. In the experiment, participants were trained in recognizing three talkers’ faces and voices to ceiling-level. Participants were then given a speech in noise recognition task consisting of four talker conditions: familiar face then familiar voice; unfamiliar face then familiar voice, familiar face then unfamiliar voice; and unfamiliar face then unfamiliar voice. A talker familiarity effect was found, i.e., speech perception was more accurate in the familiar face and familiar voice condition than all other ones. A familiar voice did not produce a talker familiarity effect when paired with an unfamiliar face. The familiar face and unfamiliar voice condition had the poorest performance, indicating that pairing a familiar face and unfamiliar voice had a disruptive effect. The results suggest that listeners develop a talker model that includes details of both the voice and the face; and that accessing this model can in some circumstances be wholly determined by face cues
