Sources and processes of social influence on health-related choices:A systematic review based on a social-interdependent choice paradigm
Background: Most choices in healthcare are not made in social isolation. However, current econometric models treat patients' preferences as the sole determinants of their choices. Through the lens of sociology and medical sociology theories, this paper presents a systematic literature review of identifiable social influences on patients’ choices, serving as a first step in developing a social-interdependent choice paradigm. Methods: Following the PRISMA guideline and using nine databases, we identified the individual agents or groups involved in health-related choices, the functional content through which social relationships influence patients, and the choice constructs affected by these processes. From 9036 screened articles, we selected 208 to develop an analytical framework connecting social relationships with choice constructs. Results: Social influences predominantly come from family, friends, specialized physicians, and general practitioners. We decomposed the functional content of social relationships into functions and contents. Dyadic interactions and expert knowledge were prominent functions, followed by social control. Prescriptive and informational contents were prevalent, followed by instrumental and emotional ones. Expert knowledge and social norms aligned with prescriptive and informational signals, while dyadic interactions provide emotional and instrumental signals. Reference points for social norms included friends, coworkers, and patients. Social relationships primarily impact which alternatives are evaluated, followed by alternative evaluation strategies and goal selection. Distinctions between medical domains and dimensions emerged, highlighting how the medical area conditions the social influence process. Conclusion: This systematic review presents a comprehensive framework that elucidates the social influence process in healthcare patient decision-making. By detailing the functional content of social relationships into functions and contents and linking these components to the elements of the choice process, we created a structured approach to understanding how social relationships impact patient choices. This will facilitate the systematic integration of social relationships into econometric models of patient choice.</p
Ancient human mitochondrial genomes from Bronze Age Bulgaria: new insights into the genetic history of Thracians
Abstract One of the best documented Indo-European civilizations that inhabited Bulgaria is the Thracians, who lasted for more than five millennia and whose origin and relationships with other past and present-day populations are debated among researchers. Here we report 25 new complete mitochondrial genomes of ancient individuals coming from three necropolises located in different regions of Bulgaria – Shekerdja mogila, Gabrova mogila and Bereketska mogila – dated to II-III millennium BC. The identified mtDNA haplogroup composition reflects the mitochondrial variability of Western Eurasia. In particular, within the ancient Eurasian genetic landscape, Thracians locate in an intermediate position between Early Neolithic farmers and Late Neolithic-Bronze Age steppe pastoralists, supporting the scenario that the Balkan region has been a link between Eastern Europe and the Mediterranean since the prehistoric time. Spatial Principal Component Analysis (sPCA) performed on Thracian and modern mtDNA sequences, confirms the pattern highlighted on ancient populations, overall indicating that the maternal gene pool of Thracians reflects their central geographical position at the gateway of Europe
Clinical course, therapeutic responses and outcomes in relapsing MOG antibody-associated demyelination.
Abstract
OBJECTIVE:
We characterised the clinical course, treatment and outcomes in 59 patients with relapsing myelin oligodendrocyte glycoprotein (MOG) antibody-associated demyelination.
METHODS:
We evaluated clinical phenotypes, annualised relapse rates (ARR) prior and on immunotherapy and Expanded Disability Status Scale (EDSS), in 218 demyelinating episodes from 33 paediatric and 26 adult patients.
RESULTS:
The most common initial presentation in the cohort was optic neuritis (ON) in 54% (bilateral (BON) 32%, unilateral (UON) 22%), followed by acute disseminated encephalomyelitis (ADEM) (20%), which occurred exclusively in children. ON was the dominant phenotype (UON 35%, BON 19%) of all clinical episodes. 109/226 (48%) MRIs had no brain lesions. Patients were steroid responsive, but 70% of episodes treated with oral prednisone relapsed, particularly at doses <10\u2009mg daily or within 2 months of cessation. Immunotherapy, including maintenance prednisone (P=0.0004), intravenous immunoglobulin, rituximab and mycophenolate, all reduced median ARRs on-treatment. Treatment failure rates were lower in patients on maintenance steroids (5%) compared with non-steroidal maintenance immunotherapy (38%) (P=0.016). 58% of patients experienced residual disability (average follow-up 61 months, visual loss in 24%). Patients with ON were less likely to have sustained disability defined by a final EDSS of 652 (OR 0.15, P=0.032), while those who had any myelitis were more likely to have sustained residual deficits (OR 3.56, P=0.077).
CONCLUSION:
Relapsing MOG antibody-associated demyelination is strongly associated with ON across all age groups and ADEM in children. Patients are highly responsive to steroids, but vulnerable to relapse on steroid reduction and cessation
Structural Integrity of Single Shell Tanks at Hanford -9491
ABSTRACT The 149 Single Shell Tanks at the Hanford Site were constructed between the 1940's and the 1960's. Many of the tanks are either known or suspected to have leaked in the past. While the free liquids have been removed from the tanks, they still contain significant waste volumes. Recently, the tank farm operations contractor established a Single Shell Tank Integrity Program. Structural integrity is one aspect of the program. The structural analysis of the Single Shell Tanks has several challenging factors. There are several tank sizes and configurations that need to be analyzed. Tank capacities range from fifty-five thousand gallons to one million gallons. The smallest tank type is approximately twenty feet in diameter, and the three other tank types are all seventy-five feet in diameter. Within each tank type there are varying concrete strengths, types of steel, tank floor arrangements, in-tank hardware, riser sizes and locations, and other appurtenances that need to be addressed. Furthermore, soil properties vary throughout the tank farms. The Pacific Northwest National Laboratory has been conducting preliminary structural analyses of the various single shell tank types to address these parameters. The preliminary analyses will assess which aspects of the tanks will require further detailed analysis. Evaluation criteria to which the tanks will be analyzed are also being developed for the Single Shell Tank Integrity Program. This information will be reviewed by the Single Shell Tank Integrity Expert Panel that has been formed to issue recommendations to the DOE and to the tank farm operations contractor regarding Single Shell Tank Integrity. This paper provides a summary of the preliminary analysis of the single shell tanks, a summary of the recommendations for the detailed analyses, and the proposed evaluation criteria by which the tanks will be judged
Biometrically linking document leakage to the individuals responsible
Insider threats are a significant security issue. The last decade has witnessed countless instances of data loss and exposure in which data has become publicly available and easily accessible. Losing or disclosing sensitive data or confidential information may cause substantial financial and reputational damage to a company. Whilst more recent research has specifically focused on the insider misuse problem, it has tended to focus on the information itself – either through its protection or approaches to detect leakage. In contrast, this paper presents a proactive approach to the attribution of misuse via information leakage using biometrics and a locality-sensitive hashing scheme. The hash digest of the object (e.g. a document) is mapped with the given biometric information of the person who interacted with it and generates a digital imprint file that represents the correlation between the two parties. The proposed approach does not directly store or preserve any explicit biometric information nor document copy in a repository. It is only the established correlation (imprint) is kept for the purpose of reconstructing the mapped information once an incident occurred. Comprehensive experiments for the proposed approach have shown that it is highly possible to establish this correlation even when the original version has undergone significant file modification. In many scenarios, such as changing the file format r removing parts of the document, including words and sentences, it was possible to extract and reconstruct the correlated biometric information out of a modified document (e.g. 100 words were deleted) with an average success rate of 89.31%
SARS-CoV-2 neutralizing antibodies: Longevity, breadth, and evasion by emerging viral variants.
The Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) antibody neutralization response and its evasion by emerging viral variants and variant of concern (VOC) are unknown, but critical to understand reinfection risk and breakthrough infection following vaccination. Antibody immunoreactivity against SARS-CoV-2 antigens and Spike variants, inhibition of Spike-driven virus-cell fusion, and infectious SARS-CoV-2 neutralization were characterized in 807 serial samples from 233 reverse transcription polymerase chain reaction (RT-PCR)-confirmed Coronavirus Disease 2019 (COVID-19) individuals with detailed demographics and followed up to 7 months. A broad and sustained polyantigenic immunoreactivity against SARS-CoV-2 Spike, Membrane, and Nucleocapsid proteins, along with high viral neutralization, was associated with COVID-19 severity. A subgroup of "high responders" maintained high neutralizing responses over time, representing ideal convalescent plasma donors. Antibodies generated against SARS-CoV-2 during the first COVID-19 wave had reduced immunoreactivity and neutralization potency to emerging Spike variants and VOC. Accurate monitoring of SARS-CoV-2 antibody responses would be essential for selection of optimal responders and vaccine monitoring and design
