    Urban heritage conservation and rapid urbanization : insights from Surat, India

    Currently, heritage is challenged in the Indian city of Surat due to diverse pressures,  including rapid urbanization, increasing housing demand, and socio‐cultural and climate changes.  Where rapid demographic growth of urban areas is happening, heritage is disappearing at an  alarming rate. Despite some efforts from the local government, urban cultural heritage is being  neglected and historic buildings keep being replaced by ordinary concrete buildings at a worryingly  rapid pace. Discussions of challenges and issues of Surat’s urban area is supported by a qualitative  dataset, including in‐depth semi‐structured interviews and focus groups with local policy makers,  planners, and heritage experts, triangulated by observation and a photo‐survey of two historic  areas. Findings from this study reveal a myriad of challenges such as: inadequacy of urban  conservation management policies and processes focused on heritage, absence of skills, training,  and resources amongst decision makers and persistent conflict and competition between heritage  conservation needs and developers’ interests. Furthermore, the values and significance of Surat’s  tangible and intangible heritage is not fully recognized by its citizens and heritage stakeholders. A  crucial opportunity exists for Surat to maximize the potential of heritage and reinforce urban  identity for its present and future generations. Surat’s context is representative of general trends  and conservation challenges and therefore recommendations developed in this study hold the  potential to offer interesting insights to the wider planners and conservationists’ international  community.  This  paper  recommends  thoughtful  integration  of  sustainable  heritage  urban  conservation into local urban development frameworks and the establishment of approaches that  recognize the plurality of heritage values

    In attempt to find new pharmacologically active molecules, we report here the synthesis and in vitro antimicrobial activity of various 3-(1,3,4-oxadiazol-2-yl)-quinazolin-4(3H)-ones. The antimicrobial activity of title compounds were examined against two gram positive bacteria (S. aureus, S. pyogenes), two gram negative bacteria (E. coli, P. aeruginosa) and three fungi (C. albicans, A. niger, A. clavatus) using the broth microdilution method. Some derivatives bearing a bromo or iodo group exhibited very good antimicrobial activity

    A Study to Compare Major Adverse Cardiac Event in Patient Undergoing PCI with Drug Eluting Stents Vs Bare Metal Stents

    This study compared Major Adverse Cardiac Event in patient with Acute Coronary Syndromes undergoing PCI with Drug Eluting Stents Vs Bare Metal Stents. A retrospective, observational study was carried out in an inpatient setting of the private tertiary care hospital. Patients with >18 years, diagnosed for Acute Coronary Syndromes (ACS), required intervention in coronary artery with implantation of Drug Eluting Stents (DES) or Bare Metal Stents (BMS) were recruited in the study. The data had been collected from file or database of the hospital. All subjects were followed for major adverse cardiac event. Result. A total of 202 patients who underwent percutaneous coronary intervention (PCI) were enrolled into DES group (N=101) and BMS group (N=101). All patients were followed up at 1 month, 3 months, 6 months & 12 months for Major Adverse Cardiac Events (MACE). Clinical outcomes during 12 months were compared between DES group & BMS group. There was no significant difference in baseline parameters including demographic, risk factors of ACS, diagnosis, angiographic parameters between both groups. Overall MACE rates were reported non-significantly high in BMS group patients (14.85%) compare to DES group patients (8.91%) (P=0.458). However, DES group had lower rates of death (0.99% vs 1.98%, P=0.57), rate of MI (3.96% vs 4.95% P=0.73), rate of revascularization (1.98% vs 3.96% p=0.42) & rate of sub acute thrombosis (1.98% vs 3.96% P=0.42) and higher rate of bleeding (1.98% vs 0.99% p=0.57) compare to cohort-II. Conclusion. The use of DES in the setting of Acute Coronary Syndrome is associated with lower Major Adverse Cardiac Event (MACE) rate compared to BMS without compromising the overall safety over the course of one-year follow-up. The long-term safety of drug-eluting stents needs to be ascertained in large, randomized trials

    HESI: A Quality Improvement Study Dr. Jessica Naber Jalpaben Patel and Sarah Locke Murray State University School of Nursing Abstract Health Education Systems Incorporated (HESI) is an Elsevier company that provides educational material and exams that aim to improve nursing education and pass rates on the nurse-licensing exam (NCLEX). Numerous studies have examined the relationship between HESI exams and NCLEX pass rates in various nursing programs throughout the country. However, improving the implementation of HESI in nursing programs has not been the focus of much research. The purpose of this study was to identify possible improvements that could be made in the use of HESI in order for it to better serve the educational needs of students in the Murray State University School of Nursing. A mixed methods approach was used in this study, and a one-time survey consisting of eight items was administered to nursing students at Murray State in each semester of the program during the fall of 2016. The data collected in these surveys was analyzed, and the results were used to improve the use of HESI in Murray State’s School of Nursing and better the education of current and future nursing students

    Acute kidney injury and its outcome in obstetrics

    Background: Acute kidney injury occurring during pregnancy, labour, delivery, and/or postpartum period. Proper management of AKI (acute kidney injury) is challenging because (i) both maternal and fetal health must be considered and (ii) the cardiovascular and renal adaptations of pregnancy add to the complexity for management.Methods: The objective of this study was to study association and contributing factors in AKI, a retrospective study of 20 cases of AKI complicating pregnancies carried out in department of obstetrics and gynecology, SVPIMSR over a period of 12 months and results were studied and analysed. Etiological factors, associated liver pathology, coagulation abnormality, thrombocytopenia, sepsis, recovery status and fetomaternal outcome were studied and tabulated. AKI was analysed in terms of maximal stage of renal injury attained as per risk, injury, failure, loss of function, and end-stage renal disease (RIFLE) criteria.Results: The incidence of ARF (acute renal failure) in pregnancy was about 0.3%. Hypertensive disorders were the major causative factor. Amongst the 20 cases, 8 cases were referred from outside and two of them died. Total 5 of 20 cases required hemodialysis and two of them had partial recovery.Conclusions: AKI complicating pregnancies are not uncommon. If recognized and treated promptly, recovery is assured in majority of cases. Early identification and prompt management of pre-eclampsia and sepsis can prevent majority of cases. Ultrasonography revelation of placenta previa or abruption helps in early management eventually decreases the chances of bleeding which is one of the causes of AKI (pre-renal cause)

    The Influence of Georgia’s Quality Rated System on School Readiness in Preschool Children

    Background: Initially launched in 2012, the Quality Rated (QR) program in Georgia is a policy tool for assessing, improving, and communicating the quality of early childhood education and care. Star level designations – zero-star, one-star, two-star, and three-star – are assigned based on a portfolio of QR standards and an onsite Environment Rating Scales observation. In the present study, we used QR data to determine if childcare quality measures (QR ratings) are linked with the readiness of children to enter kindergarten. Methods: QR data were collected from one-star, two-star, and three-star childcare learning centers (n = 16) located within Child Care Resource and Referral Regions 3 and 5 in Georgia. The Bracken School Readiness Assessment – 3rd Edition (BSRA-3) was administered to each child (n =318) eligible for inclusion. Height and weight information (n =335) was collected before assessment. Multilevel analyses were used to establish whether children in programs that score higher on QR (0 to 3 stars) show better school readiness (i.e., by BSRA-3 assessment or body mass index) than peers in programs with lower scores. Results: For the children, there were no significant differences in scores of school readiness based on the QR of childcare and learning centers. Conclusions: The results reinforce the need for continued research of the childcare QR system in Georgia to inform stakeholders in early childcare and education about ways to enhance the quality of early child care and to improve the long-term educational and health outcomes of children across the state. Future studies should examine a larger sample size, which would allow for more precise, multilevel analyses

    Temporary device malfunction of an MR conditional cardiac resynchronization defibrillator when undergoing MRI without appropriate re-programming: a case report

    Background Magnetic resonance (MR) imaging (MRI) for patients with implantable cardiac devices is becoming more routine, with the development of MR conditional devices allowing more patients access to the imaging they need. However, for this to be performed safely, strict protocols must be followed necessitating close collaboration between cardiology and radiology departments. We present a case where mandatory device re-programming of a cardiac resynchronization therapy defibrillator device into MRI mode was not performed pre-scan leading to temporary device dysfunction with no clinical consequences. Case summary A 72-year-old man presented to a device clinic for a routine device interrogation. An atrial tachycardia response episode was recorded at the same time as the patient reported having undergone an MRI scan at a local centre. The electrogram demonstrated temporary right ventricular loss of capture with standard output programming, and a short episode of oversensing on the atrial and ventricular channel which was not sustained for long enough to meet tachycardia detection. Discussion We demonstrate two potential electrophysiological effects of MRI on pacemakers, where the device had not been appropriately re-programmed pre-procedure. This illustrates that whilst MRI in patients with implantable cardiac devices is safe, strict protocols must be followed requiring robust multidisciplinary communication

There is scope for the dairy industry to introduce newer products as healthy, convenience and ready to eat foods for capacity utilization and value addition, but because of complex biochemical composition and high water content, milk and milk products act as an excellent culture medium for growth and multiplication of varieties of microorganisms. Vacuum packaging reduces product shrinkage, trim losses by eliminating oxidation and freezer burn resulting it can enhance product quality. Now a day metabolized polyethylene terephthalate (MET PET) with vacuum packaging have a promising role in storage of various value added milk product. The developed value added Kalakand product (Indian cookie) could be stored successfully for 5 days in MET PET packaging material at 4±1°C and when the product was packaged under vacuum the shelf life increased up to 10 days at 4±1°C
