    Schraubenarthrorise nach De Pellegrin beim kindlichen Knick-Senkfuß – 1-Jahres Follow-Up-Analyse des Gangbildes und der plantaren Druckverteilung – eine Pilotstudie

    Einleitung: Der flexible Knick-Senkfuß ist definiert als ein belastungsabhängiges Absenken des Längsgewölbes kombiniert mit einer Eversionsstellung des Kalkaneus. Die Prävalenz wird bei Kindern im Vorschulalter mit bis zu 90% angegeben. Ein Großteil erfährt jedoch eine spontane Korrektur bis zum 10. Lebensjahr, weshalb lediglich 3-5% an einem persistierenden Knick-Senkfußes leiden. Als Ursache der Pathologie wird dabei vorwiegend eine Laxizität der Sehne des M. tibialis posterior diskutiert. Konservative Ansätze mittels Orthesen konnten in unterschiedlichen Studien keinen signifikanten Nutzen aufweisen, weshalb bei symptomatischen Verläufen eine operative Korrektur indiziert ist. Die hier angewandte Schraubenarthrorise nach de Pellegrin bedient sich einer mechanischen Blockierung des Talus durch eine lateral in den Kalkaneus eingebrachte Schraube. Dadurch wird ein Abkippen des Kalkaneus in eine übermäßige Valgusposition verhindert. Das Outcome dieses Therapieansatzes wurde bisher in Studien nahezu ausschließlich mittels röntgenologischer Parameter beurteilt, klinische funktionelle Betrachtungen fehlen fast vollständig. Ziel dieser Studie ist es, erstmalig die Ergebnisse ein Jahr nach erfolgter Schraubenarthrorise nach de Pellegrin in einer prospektiv klinischen Untersuchung mittels einer funktionellen Ganganalyse zu evaluieren. Methoden: Untersucht wurden 14 Kinder (27 Füße), die im Zeitraum zwischen 2015 und 2018 an einem Knick-Senkfuß am Universitätsklinikum Marburg operiert wurden. Inhalt der Studie war die Untersuchung der Probanden mittels 2D-Ganganalyse und pedobarographischer Druckmessung. Hierbei wurden der Rückfußwinkel, der Fersenbodenwinkel, die Beinachse und die plantare Druckverteilung sowohl statisch, als auch in einer dynamischen Aufnahme vermessen. Zur Beurteilung der Fußfunktion und des subjektiven Verlaufs wurden der AOFAS-Score und die visuelle Analgoskala "Fuß und Sprunggelenk" herangezogen. Daten wurden prä-operativ, drei Tage post-OP, zwei Wochen post-OP, vier Wochen post-OP und ein Jahr nach Operation erhoben. Die statistische Auswertung der Daten erfolgte unabhängig. Da es sich um eine Pilotstudie handelt, wurde auf eine statistische Korrektur des multiplen Testens verzichtet. Das Niveau für statistische Signifikanz wurde auf 5% festgelegt. Ein Einverständnis der lokalen Ethikkommission wurde vor Studienbeginn eingeholt. Ergebnisse: Das Durchschnittsalter bei OP betrug 12,14 Jahre (Spanne: 9,25 – 14,28 Jahre). Es traten keine größeren intraoperativen und postoperativen Komplikationen auf. Die Auswertung der Messwerte ergab sowohl für die Aufnahmen der Ganganalyse als auch der Pedobarografie überwiegend hoch signifikante Verbesserungen der ermittelten Parameter. Dabei zeigte sich in der dynamischen Ganganalyse eine deutliche Reduktion des Fersenbodenwinkel von 7,94° präoperativ (SD: 3,15) auf 1,22° (SD: 2,15°) ein Jahr postoperativ [p<0,001], sowie des Achillessehnenwinkels von präoperativ 12,65° Valgusstellung (SD: 2,56°) auf 6,07° (SD: 2,30°) ein Jahr postoperativ [p<0,001]. In der dynamischen Pedobarografie ergab sich eine Umverteilung der Druckbelastung der lateralen Fußhälfte (in Prozent bezogen auf den Gesamtfuß) von 51,39% (SD: 9,29%) präoperativ zu 56,85% (SD: 5,75%) ein Jahr postoperativ [p=0,006]. Eine signifikante Veränderung der Beinachse konnte nicht nachgewiesen werden. Die klinischen Ergebnisse der Fragebögen besserten sich ebenfalls signifikant. Diskussion: Mit dieser Studie erfolgte erstmals eine prospektive klinische Untersuchung einer Schraubenarthrorise nach de Pellegrin beim kindlichen Knick-Senkfuß mittels funktioneller Ganganalyse. Hierbei zeigten sich eine deutliche Verbesserung der Funktion des unteren Sprunggelenks hin zu einer achsgerechten Rückfußstellung, eine größere Belastung des Außenfußes mit Aufrichten des Längsgewölbes, sowie eine Schmerzreduktion der Probanden. Bei einer weiterführenden Korrelationsanalyse konnten wir zudem zeigen, dass insbesondere die dynamischen Aufnahmen der Ganganalyse in hohem Maße mit den Ergebnissen der Fragebögen korrelieren. Die Ganganalyse erwies sich dabei als einfache und schonende Diagnostikmethode zur genaueren Untersuchung der Fehlstellung.     Isotopic constraints on genetic relationships among group IIIF iron meteorites, Fitzwater Pass, and the Zinder pallasite

    Complex interelement trends among magmatic IIIF iron meteorites are difficult to explain by fractional crystallization and have raised uncertainty about their genetic relationships. Nucleosynthetic Mo isotope anomalies provide a powerful tool to assess if individual IIIF irons are related to each other. However, while trace-element data are available for all nine IIIF irons, Mo isotopic data are limited to three samples. We present Mo isotopic data for all but one IIIF irons that help assess the genetic relationships among these irons, together with new Mo and W isotopic data for Fitzwater Pass (classified IIIF), and the Zinder pallasite (for which a cogenetic link with IIIF irons has been proposed). After correction for cosmic-ray exposure, the Mo isotopic compositions of the IIIF irons are identical within uncertainty and confirm their belonging to carbonaceous chondrite-type (CC) meteorites. The mean Mo isotopic composition of Group IIIF overlaps those Groups IIF and IID, but a common parent body for these groups is ruled out based on distinct trace element systematics. The new Mo isotopic data do not argue against a single parent body for the IIIF irons, and suggest a close genetic link among these samples. By contrast, Fitzwater Pass has distinct Mo and W isotopic compositions, identical to those of some non-magmatic IAB irons. The Mo and W isotope data for Zinder indicate that this meteorite is not related to IIIF irons, but belongs to the non-carbonaceous (NC) type and has the same Mo and W isotopic composition as main-group pallasites.Comment: Accepted for publication in Meteoritics & Planetary Scienc

    Orthopedic trauma patient satisfaction with telemedicine during the Covid-19 lockdown: an interview study among 25 patients

    BACKGROUND: Telemedicine can take many forms, from telephone-only consultations to video consultations via smartphone or personal computer, depending on the goals of the treatment. One of the advantages of videoconferencing is the direct visual contact between patients and therapists even over long distances. While some telemedicine models require specially designed add-on devices, others get by with off-the-shelf equipment and software and achieve similarly successful results. This depends, among other things, on the nature of the injury, the desired outcome of therapy, and the medical consultation. In the last decade, the science and practice of telemedicine have grown exponentially and even more so during the Covid-19 pandemic. Depending on the traumatic lesion, post-traumatic and post-operative treatment and care of trauma patients may require medical or physical therapy consultation in a clinic or office. However, due to the Covid-19 lockdown, direct physical follow-up was more difficult and telemedicine solutions were sought and implemented. OBJECTIVE: The aim of this study was to assess orthopedic trauma patients` satisfaction with telemedical aftercare. METHODS: Between March and July 2020, a standardized interview using a standardized questionnaire (Freiburg Index of Patient Satisfaction; FIPS) amongst orthopedic trauma patients who received telemedical post-surgical or physiotherapeutic care was conducted. The FIPS is composed of five questions regarding treatment and one question on the overall treatment satisfaction. Furthermore, we assessed patients` demographics and their telemedical use. Sub-group analysis was performed for age groups (< 65 years vs. ≥ 65 years), the used device and gender. RESULTS: In total, we assessed 25 patients with a mean age of 43 years (14 female). The majority of patients used their smartphone for the consultations (76%). The assessed mean overall FIPS score was 2.14. The mean FIPS score of younger patients was 2.23 (versus 1.91 for elderly patients). The vast majority of the surveyed patients (80%) were absolutely confident with their smartphone or tablet use. CONCLUSIONS: The majority of patients surveyed stated a high satisfaction with the telemedical follow-up. Older patients showed a higher satisfaction rate than their counterparts. It seems that telemedical post-surgical or physiotherapeutic care is a viable option, especially in times of reduced contact, like the current Covid-19 pandemic. Thus, telemedicine offers the opportunity to ensure access to effective patient care even over long distances while maintaining patient satisfaction

    A Privacy-aware Data Access System for Automotive Applications

    The introduction of Information technology (IT) in modern vehicles enables a plethora of new applications ranging from value added services up to autonomous driving vehicles. However, this also introduces new threats with regard to IT security and privacy. In this paper, we discuss the new privacy issues and propose a privacy-aware data access system for automotive applications. Our system informs the user over all privacy aspects and enables him to control third-party access to his personal data. We developed an easily usable human machine interface (HMI) and an underlying policy system to control data flows which is compliant to the European General Data Protection Regulation (GDPR). Our system can be easily integrated in future automotive architectures

    Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis

    BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration.     Predicting DNA-Binding Specificities of Eukaryotic Transcription Factors

    Today, annotated amino acid sequences of more and more transcription factors (TFs) are readily available. Quantitative information about their DNA-binding specificities, however, are hard to obtain. Position frequency matrices (PFMs), the most widely used models to represent binding specificities, are experimentally characterized only for a small fraction of all TFs. Even for some of the most intensively studied eukaryotic organisms (i.e., human, rat and mouse), roughly one-sixth of all proteins with annotated DNA-binding domain have been characterized experimentally. Here, we present a new method based on support vector regression for predicting quantitative DNA-binding specificities of TFs in different eukaryotic species. This approach estimates a quantitative measure for the PFM similarity of two proteins, based on various features derived from their protein sequences. The method is trained and tested on a dataset containing 1 239 TFs with known DNA-binding specificity, and used to predict specific DNA target motifs for 645 TFs with high accuracy

    Context-Dependent Encoding of Fear and Extinction Memories in a Large-Scale Network Model of the Basal Amygdala

    The basal nucleus of the amygdala (BA) is involved in the formation of context-dependent conditioned fear and extinction memories. To understand the underlying neural mechanisms we developed a large-scale neuron network model of the BA, composed of excitatory and inhibitory leaky-integrate-and-fire neurons. Excitatory BA neurons received conditioned stimulus (CS)-related input from the adjacent lateral nucleus (LA) and contextual input from the hippocampus or medial prefrontal cortex (mPFC). We implemented a plasticity mechanism according to which CS and contextual synapses were potentiated if CS and contextual inputs temporally coincided on the afferents of the excitatory neurons. Our simulations revealed a differential recruitment of two distinct subpopulations of BA neurons during conditioning and extinction, mimicking the activation of experimentally observed cell populations. We propose that these two subgroups encode contextual specificity of fear and extinction memories, respectively. Mutual competition between them, mediated by feedback inhibition and driven by contextual inputs, regulates the activity in the central amygdala (CEA) thereby controlling amygdala output and fear behavior. The model makes multiple testable predictions that may advance our understanding of fear and extinction memories

    Surgical site infection after gastrointestinal surgery in high-income, middle-income, and low-income countries: a prospective, international, multicentre cohort study

    Background: Surgical site infection (SSI) is one of the most common infections associated with health care, but its importance as a global health priority is not fully understood. We quantified the burden of SSI after gastrointestinal surgery in countries in all parts of the world. Methods: This international, prospective, multicentre cohort study included consecutive patients undergoing elective or emergency gastrointestinal resection within 2-week time periods at any health-care facility in any country. Countries with participating centres were stratified into high-income, middle-income, and low-income groups according to the UN's Human Development Index (HDI). Data variables from the GlobalSurg 1 study and other studies that have been found to affect the likelihood of SSI were entered into risk adjustment models. The primary outcome measure was the 30-day SSI incidence (defined by US Centers for Disease Control and Prevention criteria for superficial and deep incisional SSI). Relationships with explanatory variables were examined using Bayesian multilevel logistic regression models. This trial is registered with ClinicalTrials.gov, number NCT02662231. Findings: Between Jan 4, 2016, and July 31, 2016, 13 265 records were submitted for analysis. 12 539 patients from 343 hospitals in 66 countries were included. 7339 (58·5%) patient were from high-HDI countries (193 hospitals in 30 countries), 3918 (31·2%) patients were from middle-HDI countries (82 hospitals in 18 countries), and 1282 (10·2%) patients were from low-HDI countries (68 hospitals in 18 countries). In total, 1538 (12·3%) patients had SSI within 30 days of surgery. The incidence of SSI varied between countries with high (691 [9·4%] of 7339 patients), middle (549 [14·0%] of 3918 patients), and low (298 [23·2%] of 1282) HDI (p < 0·001). The highest SSI incidence in each HDI group was after dirty surgery (102 [17·8%] of 574 patients in high-HDI countries; 74 [31·4%] of 236 patients in middle-HDI countries; 72 [39·8%] of 181 patients in low-HDI countries). Following risk factor adjustment, patients in low-HDI countries were at greatest risk of SSI (adjusted odds ratio 1·60, 95% credible interval 1·05–2·37; p=0·030). 132 (21·6%) of 610 patients with an SSI and a microbiology culture result had an infection that was resistant to the prophylactic antibiotic used. Resistant infections were detected in 49 (16·6%) of 295 patients in high-HDI countries, in 37 (19·8%) of 187 patients in middle-HDI countries, and in 46 (35·9%) of 128 patients in low-HDI countries (p < 0·001). Interpretation: Countries with a low HDI carry a disproportionately greater burden of SSI than countries with a middle or high HDI and might have higher rates of antibiotic resistance. In view of WHO recommendations on SSI prevention that highlight the absence of high-quality interventional research, urgent, pragmatic, randomised trials based in LMICs are needed to assess measures aiming to reduce this preventable complication

    Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial

    Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.
