    Myogenic stem cells.

    Both skeletal muscle and bone marrow tissue contain myogenic stem cells. The population residing in muscles is heterogenic. Predominant in number are "typical" satellite cells - muscle progenitors migrating from somites during embryonic life. Another population is group of multipotent muscle stem cells which, at least in part, are derived from bone marrow. These cells are tracked by gradient of growth factors releasing from muscle during injury or exercise. Recruited bone marrow-derived cells gradually change their phenotype becoming muscle stem cells and eventually can attain satellite cell position and express Pax7 protein. Mesenchymal stem cells (MSC) isolated directly from bone marrow also display myogenic potential, although methods of induction of myogenic differentiation in vitro have not been optimized yet. Concerning efforts of exploiting myogenic stem cells in cell-mediated therapies it is important to understand the cause of impaired regenerative potential of aged muscle. Up to now, most of research data suggest that majority of age related changes in skeletal muscles are reversible, thus depending on extrinsic factors.     Śródmiąższowe zapalenie płuc u chorego leczonego amiodaronem - opis przypadku

    Amiodarone, antiarrhythmic drug of III class is used in patients with supraventricular and ventricular arrhythmias, often with coexisting congestive heart failure. Side effects of amiodarone treatment are observed in approximately 75% of patients. Most dangerous are the symptoms of amiodarone pulmonary toxicity occuring in 2-17% of patients. We present a patient with COPD, in whom interstitial pneumonitis with radiologic features of organizing pneumonia developed after one year of amiodaron treatment due to supraventricular and ventricular arrhythmias. The drug was stopped and steroids were introduced due to marked respiratory insufficiency. Regression of pulmonary symptoms and improvement of ventilatory parameters were observed after 3 months of treatment. Pathogenesis, diagnostic procedures and current methods of treatment of this jatrogenic disease are discussed.Amiodaron, lek antyarytmiczny III klasy, jest stosowany w zaburzeniach rytmu pochodzenia komorowego i nadkomorowego, często ze współistniejącą dysfunkcją lewej komory. U około 75% chorych w czasie leczenia amiodaronem rozwijają się działania niepożądane, które zmuszają niekiedy do odstawienia leku. W 2-17% przypadków dochodzi do powikłań płucnych. W pracy przedstawiono chorego na przewlekłą obturacyjną chorobę płuc, u którego po roku leczenia amiodaronem z powodu nadkomorowych i komorowych zaburzeń rytmu serca stwierdzono zmiany śródmiąższowe w płucach, najbardziej odpowiadające organizującemu się zapaleniu płuc, z objawami ciężkiej hipoksemicznej niewydolności oddychania. Po odstawieniu leku i zastosowaniu kortykosteroidów uzyskano znaczącą poprawę kliniczną.     Methemoglobinemia jest rzadko rozpoznawaną, groźną dla życia patologią polegającą na obecności we krwi ponad 1% utlenionej hemoglobiny, która jest niezdolna do przenoszenia tlenu. Przypadek dotyczy 49-letniego mężczyzny z gwałtownie narastającymi objawami ostrej, hipoksemicznej niewydolności oddechowej, u którego wykluczono ostre i przewlekłe choroby układów krążenia i oddechowego. Istotna rozbieżność pomiędzy wartością saturacji tlenem, odczytanej z pulsoksymetru, a saturacją tlenem oraz ciśnieniem parcjalnym tlenu, obliczonymi w badaniu gazometrycznym krwi włośniczkowej, a także wyraźny brak reakcji na tlenoterapię były ważnymi wskazówkami do poszukiwania hemoglobinopatii i rozpoznania methemoglobinemii. Stężenie methemoglobiny wynosiło 16%. Objawy ustąpiły samoistnie. Czynnik wywołujący methemoglobinemię nie został zidentyfikowany. Pneumonol. Alergol. The authors present a case of 49-year-old man who was admitted to the department of chest medicine with dyspnea, weakness and cyanosis in whom differential diagnosis excluded acute and chronic pulmonary and cardiovascular disease. Clinical cyanosis and low measured oxygen saturation in the presence of normal arterial oxygen tension was highly suggestive of methemoglobinemia ("saturation gap"). Methemoglobin level, measured at the acute phase of disease was elevated at 16%. Episode resolved spontaneously. Causes of methemoglobinemia was not established. Pneumonol. Alergol.     The improvement in plasma cell myeloma (PCM) treatment and much longer overall survival of the patients led to the observation of long-term complications of the therapy. These include secondary malignancies, such as myelodysplastic syndromes (MDS) and acute myeloid leukemia. Definitely less common is the co-occurrence of MDS at the point of diagnosis of PCM or MDS evolving during the observation of untreated patients with monoclonal gammapathy of undetermined significance. In the article we describe 5 patients: one with a therapy-related MDS, the co-occurrence of both diseases from the diagnosis and MDS preceding PCM for several years.Wprowadzenie do praktyki klinicznej nowych terapii wpłynęło na istotne wydłużenie przeżycia chorych na szpiczaka plazmocytowego (PCM). Dłuższa obserwacja pacjentów spowodowała ujawnienie odległych powikłań po leczeniu. Jednym z nich są wtórne nowotwory, między innymi zespoły mielodysplastyczne (MDS) i ostre białaczki szpikowe. Znacznie rzadziej MDS może współistnieć z PCM od momentu diagnozy albo się pojawić w przebiegu obserwacji u nieleczonych chorych. W artykule opisano 5 chorych: z wtórnym do terapii szpiczaka plazmocytowego MDS, ze współistnieniem obu jednostek chorobowych od początku diagnozy, z MDS poprzedzającym o kilka lat rozpoznanie PCM oraz z gammapatią monoklonalną o nieustalonym znaczeniu z towarzyszącym MDS

    A 24 year-old man with 3-months medical history of recurrent respiratory infections and pericardial effusion, despite treatment with nonsteroid anti-inflammatory drugs, was admitted to the hospital with dyspnea on exertion. On admission he presented the symptoms of right heart insufficiency. Computed tomography of the chest demonstrated a thickened pericardium. Echocardiographic examination and right heart catheterisation established the diagnosis of constrictive pericarditis. Serologic tests suggested viral aetiology.     Conclusively, for the first time, the large set of miRNAs was identified in mature cabbage leaves. Potential targets designation for these miRNAs may suggest their essential role in many plants primary biological processes. Presented study not only supplements the knowledge about B. oleracea miRNAs, but additionally it may be used in other research concerning the improvement of the cabbage cultivation

    The ability of MSCs to modulate the inflammatory environment is well recognized, but understanding the molecular mechanisms responsible for these properties is still far from complete. Prostaglandin E2 (PGE2), a product of the cyclooxygenase 2 (COX-2) pathway, is indicated as one of the key mediators in the immunomodulatory effect of MSCs. Due to the pleiotropic effect of this molecule, determining its role in particular intercellular interactions and aspects of cell functioning is very difficult. In this article, the authors attempt to summarize the previous observations regarding the role of PGE2 and COX-2 in the immunomodulatory properties and other vital functions of MSCs. So far, the most consistent results relate to the inhibitory effect of MSC-derived PGE2 on the early maturation of dendritic cells, suppressive effect on the proliferation of activated lymphocytes, and stimulatory effect on the differentiation of macrophages into M2 phenotype. Additionally, COX-2/PGE2 plays an important role in maintaining the basic life functions of MSCs, such as the ability to proliferate, migrate and differentiate, and it also positively affects the formation of niches that are conducive to both hematopoiesis and carcinogenesis

    Vadadustat, a HIF Prolyl Hydroxylase Inhibitor, Improves Immunomodulatory Properties of Human Mesenchymal Stromal Cells

    The therapeutic potential of mesenchymal stromal cells (MSCs) is largely attributed to their immunomodulatory properties, which can be further improved by hypoxia priming. In this study, we investigated the immunomodulatory properties of MSCs preconditioned with hypoxia-mimetic Vadadustat (AKB-6548, Akebia). Gene expression analysis of immunomodulatory factors was performed by real-time polymerase chain reaction (real-time PCR) on RNA isolated from six human bone-marrow derived MSCs populations preconditioned for 6 h with 40 μM Vadadustat compared to control MSCs. The effect of Vadadustat preconditioning on MSCs secretome was determined using Proteome Profiler and Luminex, while their immunomodulatory activity was assessed by mixed lymphocyte reaction (MLR) and Culturex transwell migration assays. Real-time PCR revealed that Vadadustat downregulated genes related to immune system: IL24, IL1B, CXCL8, PDCD1LG1, PDCD1LG2, HIF1A, CCL2 and IL6, and upregulated IL17RD, CCL28 and LEP. Vadadustat caused a marked decrease in the secretion of IL6 (by 51%), HGF (by 47%), CCL7 (MCP3) (by 42%) and CXCL8 (by 40%). Vadadustat potentiated the inhibitory effect of MSCs on the proliferation of alloactivated human peripheral blood mononuclear cells (PBMCs), and reduced monocytes-enriched PBMCs chemotaxis towards the MSCs secretome. Preconditioning with Vadadustat may constitute a valuable approach to improve the therapeutic properties of MSCs
