The ScS precursors for the study of the lowermost mantle
The exploration of the lowermost-mantle structures by means of body waveform modeling allows the small-scale detection of heterogeneity and anomalous layers. In some regions the D00 layer presents a discontinuity at its top that seems to
be a local feature. This anomalous reflector may be recognized by the detection of a small core-reflected phases precursor. These studies may present different order of problems. The main difficulties, are connected to the identification of the precursor and its association to the D00 region. Misunderstandings often result because of phases
produced by heterogeneity and anisotropy along and in the vicinity of the ray paths, in the crust and mantle structures. These complexities are increased when large dataset and recording arrays, which may facilitate the waveform analysis, are not available. In this paper we discuss the body waveform modeling of lower-mantle phases for the study of the D00 with particular focus on the case of sparse data with only few events and stations available
Continuous atom laser with Bose-Einstein condensates involving three-body interactions
We demonstrate, through numerical simulations, the emission of a coherent
continuous matter wave of constant amplitude from a Bose-Einstein Condensate in
a shallow optical dipole trap. The process is achieved by spatial control of
the variations of the scattering length along the trapping axis, including
elastic three body interactions due to dipole interactions. In our approach,
the outcoupling mechanism are atomic interactions and thus, the trap remains
unaltered. We calculate analytically the parameters for the experimental
implementation of this CW atom laser.Comment: 11 pages, 4 figure
Metastability for reversible probabilistic cellular automata with self--interaction
The problem of metastability for a stochastic dynamics with a parallel
updating rule is addressed in the Freidlin--Wentzel regime, namely, finite
volume, small magnetic field, and small temperature. The model is characterized
by the existence of many fixed points and cyclic pairs of the zero temperature
dynamics, in which the system can be trapped in its way to the stable phase.
%The characterization of the metastable behavior %of a system in the context of
parallel dynamics is a very difficult task, %since all the jumps in the
configuration space are allowed. Our strategy is based on recent powerful
approaches, not needing a complete description of the fixed points of the
dynamics, but relying on few model dependent results. We compute the exit time,
in the sense of logarithmic equivalence, and characterize the critical droplet
that is necessarily visited by the system during its excursion from the
metastable to the stable state. We need to supply two model dependent inputs:
(1) the communication energy, that is the minimal energy barrier that the
system must overcome to reach the stable state starting from the metastable
one; (2) a recurrence property stating that for any configuration different
from the metastable state there exists a path, starting from such a
configuration and reaching a lower energy state, such that its maximal energy
is lower than the communication energy
Modelling Complex Bimolecular Reactions in a Condensed Phase: The Case of Phosphodiester Hydrolysis
Background: the theoretical modelling of reactions occurring in liquid phase is a research line of primary importance both in theoretical–computational chemistry and in the context of organic and biological chemistry. Here we present the modelling of the kinetics of the hydroxide-promoted hydrolysis of phosphoric diesters. (2) Method: the theoretical–computational procedure involves a hybrid quantum/classical approach based on the perturbed matrix method (PMM) in conjunction with molecular mechanics. (3) Results: the presented study reproduces the experimental data both in the rate constants and in the mechanistic aspects (C–O bond vs. O–P bond reactivity). The study suggests that the basic hydrolysis of phosphodiesters occurs through a concerted (Formula presented.) mechanism, with no formation of penta-coordinated species as reaction intermediates. (4) Conclusions: the presented approach, despite the approximations, is potentially applicable to a large number of bimolecular transformations in solution and therefore leads the way to a fast and general method to predict the rate constants and reactivities/selectivities in complex environments
Estimates of the effect on hepatic iron of oral deferiprone compared with subcutaneous desferrioxamine for treatment of iron overload in thalassemia major: a systematic review
BACKGROUND: Beta thalassemia major requires regular blood transfusions and iron chelation to alleviate the harmful accumulation of iron. Evidence on the efficacy and safety of the available agents, desferrioxamine and deferiprone, is derived from small, non-comparative, heterogeneous observational studies. This evidence was reviewed to quantitatively compare the ability of these chelators to reduce hepatic iron. METHODS: The literature was searched using Medline and all reports addressing the effect of either chelator on hepatic iron were considered. Data were abstracted independently by two investigators. Analyses were performed using reported individual patient data. Hepatic iron concentrations at study end and changes over time were compared using ANCOVA, controlling for initial iron load. Differences in the proportions of patients improving were tested using χ(2). RESULTS: Eight of 11 reports identified provided patient-level data relating to 30 desferrioxamine- and 68 deferiprone-treated patients. Desferrioxamine was more likely than optimal dose deferiprone to decrease hepatic iron over the average follow-up of 45 months (odds ratio, 19.0, 95% CI, 2.4 to 151.4). The degree of improvement was also larger with desferrioxamine. CONCLUSIONS: This analysis suggests that desferrioxamine is more effective than deferiprone in lowering hepatic iron. This comparative analysis – despite its limitations – should prove beneficial to physicians faced with the challenge of selecting the optimal treatment for their patients
Perturbative analysis of disordered Ising models close to criticality
We consider a two-dimensional Ising model with random i.i.d. nearest-neighbor
ferromagnetic couplings and no external magnetic field. We show that, if the
probability of supercritical couplings is small enough, the system admits a
convergent cluster expansion with probability one. The associated polymers are
defined on a sequence of increasing scales; in particular the convergence of
the above expansion implies the infinite differentiability of the free energy
but not its analyticity. The basic tools in the proof are a general theory of
graded cluster expansions and a stochastic domination of the disorder
Metastability in Interacting Nonlinear Stochastic Differential Equations II: Large-N Behaviour
We consider the dynamics of a periodic chain of N coupled overdamped
particles under the influence of noise, in the limit of large N. Each particle
is subjected to a bistable local potential, to a linear coupling with its
nearest neighbours, and to an independent source of white noise. For strong
coupling (of the order N^2), the system synchronises, in the sense that all
oscillators assume almost the same position in their respective local potential
most of the time. In a previous paper, we showed that the transition from
strong to weak coupling involves a sequence of symmetry-breaking bifurcations
of the system's stationary configurations, and analysed in particular the
behaviour for coupling intensities slightly below the synchronisation
threshold, for arbitrary N. Here we describe the behaviour for any positive
coupling intensity \gamma of order N^2, provided the particle number N is
sufficiently large (as a function of \gamma/N^2). In particular, we determine
the transition time between synchronised states, as well as the shape of the
"critical droplet", to leading order in 1/N. Our techniques involve the control
of the exact number of periodic orbits of a near-integrable twist map, allowing
us to give a detailed description of the system's potential landscape, in which
the metastable behaviour is encoded
Step-down from high dose fixed combination therapy in asthma patients: a randomized controlled trial.
Asthma guidelines suggest that therapy can be reduced once asthma is controlled. Despite these recommendations, asthmatic patients are seldom stepped down in clinical practice, and questions remain about when and how to reduce asthma therapy. The purpose of the present study was to evaluate lung function and asthma control in patients who were stepped down from the highest recommended dose of inhaled corticosteroid/long acting \u3b22 agonist combination therapy.
METHODS:
This was a prospective, randomised, controlled, two-arm parallel group study. Asthmatic patients who were fully controlled with a high daily dose (1000/100\u2009\u3bcg) of fluticasone/salmeterol were randomly assigned to 6\u2009months of open-label treatment with either 500/100\u2009\u3bcg fluticasone/salmeterol Diskus daily or 400/24\u2009\u3bcg extrafine beclomethasone/formoterol pMDI daily. The primary outcome was the change in morning peak expiratory flow (PEF) values between baseline and the end of treatment. The secondary outcomes included asthma control and exacerbation frequency.
RESULTS:
Four hundred twenty-two patients were included in the analysis. The PEF values remained above 95% of the predicted values throughout the study. The end-study morning PEF rates showed equivalence between the groups (difference between means, 2.49\u2009L/min; 95% CI, -13.43 to 18.42). No changes from baseline were detected in PEF and forced expiratory volume in 1 second measured at the clinics, in the symptom scores or in the use of rescue medication. Asthma control was maintained in 95.2% of the patients at 6\u2009months. No significant differences between the groups were detected in any other parameter, including exacerbation frequency and adverse events.
CONCLUSIONS:
Stepping down patients whose asthma is controlled with the highest recommended dose of fluticasone/salmeterol to either 500/100\u2009\u3bcg fluticasone/salmeterol daily or 400/24\u2009\u3bcg extra-fine beclomethasone/formoterol daily provides comparable maintenance of lung function and asthma control.

Experimental assessment of a new form of scaling law for near-wall turbulence
Scaling laws and intermittency in the wall region of a turbulent flow are
addressed by analyzing moderate Reynolds number data obtained by single
component hot wire anemometry in the boundary layer of a flat plate. The paper
aims in particular at the experimental validation of a new form of refined
similarity recently proposed for the shear dominated range of turbulence, where
the classical Kolmogorov-Oboukhov inertial range theory is inappropriate. An
approach inspired to the extended self-similarity allows for the extraction of
the different power laws for the longitudinal structure functions at several
wall normal distances. A double scaling regime is found in the logarithmic
region, confirming previous experimental results. Approaching the wall, the
scaling range corresponding to the classical cascade-dominated range tends to
disappear and, in the buffer layer, a single power law is found to describe the
available range of scales. The double scaling is shown to be associated with
two different forms of refined similarity. The classical form holds below the
shear scale L s . The other, originally introduced on the basis of DNS data for
a turbulent channel, is experimentally confirmed to set up above L s . Given
the experimental diffulties in the evaluation of the instantaneous dissipation
rate, some care is devoted to check that its one-dimensional surrogate does not
bias the results. The increased intermittency as the wall is approached is
experimentally found entirely consistent with the failure of the refined
Kolmogorov-Oboukhov similarity and the establishment of its new form near the
