    Is the combination therapy of IKr-channel blocker and left stellate ganglion block effective for intractable ventricular arrhythmia in a cardiopulmonary arrest patient?

    Background: We have previously reported that the defibrillation success rate of intravenous nifekalant hydrochloride (NIF), a pure IKr-channel (IKr: the rapid components of the delayed rectifier potassium current) blocker, was more than 75% for lidocaine-resistant ventricular tachycardia and fibrillation (VT/VF) in patients with out-of-hospital cardiopulmonary arrest (CPA). However, there was no effective treatment for the remaining 25% of patients in whom defibrillation was unsuccessful. We hypothesised that the combination therapy of NIF and left stellate ganglion block (LSGB) was useful for defibrillation in NIF-resistant VT/VF and investigated its efficacy in a retrospective study. Methods and results: We investigated sequentially 272 out-of-hospital CPA patients treated at Tokai University between April and December 2006. VT/VF occurred in 55 patients on arrival or during cardiopulmonary resuscitation (CPR). On the basis of our CPR algorithm, NIF was administered (0.15-0.3 mg/kg, i.v.) after the first direct-current cardioversion. NIF-resistant VT/VFs were observed in 15 out of 55 patients and LSGB was performed on 11 of these with administration of NIF. Sinus rhythm was restored in 7 patients following LSGB (64%) and complete recovery was achieved in 2 patients. In the non-LSGB group, however, all the patients died. Conclusions: The combination therapy of intravenous NIF and LSGB was useful for defibrillation in intractable VT/VF.     Curative two-stage resection for synchronous triple cancers of the esophagus, colon, and liver: Report of a case

    AbstractIntroductionCases of synchronous triple cancers of the esophagus and other organs curatively resected are rare.Presentation of caseA 73-year-old man was admitted to our hospital with bloody feces. He was diagnosed with synchronous triple cancers of the esophagus, colon, and liver. We selected a two-stage operation to safely achieve curative resection for all three cancers. The first stage of the operation comprised a laparoscopy-assisted sigmoidectomy and partial liver resection via open surgery. The patient was discharged without complications. Thirty days later, he was readmitted and thoracoscopic esophagectomy was performed. Although pneumonia-induced pulmonary aspiration occurred as a postoperative complication, it was treated conservatively. The patient was discharged on postoperative day 24.DiscussionEsophagectomy is a highly invasive procedure; thus, simultaneous surgery for plural organs, including the esophagus, may induce life-threatening, severe complications. Two-stage surgery is useful in reducing surgical stress in high-risk patients. For synchronous multiple cancers, the planning of two-stage surgery should be considered for each cancer to maintain organ function and reduce the stress and difficulty of each stage.ConclusionWe successfully treated synchronous triple cancers, including esophageal cancer, by a two-stage operation

    Mechanical Strength of the Mayo Clinic Congruent Elbow Plate System for Distal Humerus Fractures: Cadaveric and Model Bone Model

    The purpose of this study was to compare the Mayo Clinic Congruent Elbow Plate System with a plate system having the same biomechanical features as a conventional non-locking system. For testing, we used a biaxial adjustable material testing machine (Mini-Bionix®, MTS, Minnetonka, MN). A compressive load of 500 N/min was applied in the axial direction, and a torque of 30 deg/min was applied in the rotational direction to simulate internal rotation of the elbow. The relationships between force (N) and displacement (mm), and between the internal rotational angle (deg) and torque (N/m), were measured. The Mayo Clinic Congruent Elbow Plate and the two types of screws used in this study may provide sufficient fixation strength. The results obtained here warrant further studies that are more dynamic (e.g., cyclic loading and repetitive torsion tests), with a larger number of specimens, and that facilitate appropriate nomal elbow movement in daily living, to assess the utility of plate fixation with locking and non-locking systems for distal humerus fractures

    Effectiveness of Initial Fixation of a Grasping Pin for Proximal Femoral Fractures

    We developed a grasping pin with a hook for osteosynthesis of proximal femoral fractures and compared its performance with that of a lag screw. Cyclic compressive tests were performed to simulate cut-outs, and quasi-static torsion, tests were conducted to simulate rotational displacement in polyurethane model bones and femoral heads collected after hip replacement surgery, and cadaveric femoral heads. In the polyurethane model bones and femoral head collected after hip replacement surgery, implant displacement was increased in the cut-out simulation test in both the grasping pin group and lag screw group, deformation was less in the grasping pin group than in the lag screw group. In polyurethane bones and cadaveric bones, the grasping pins showed higher rotational resistance compared with the lag screws in the quasi-static torsion test because of the high compression force generated during implantation. In contrast, in the collected femoral head after hip replacement surgery model, the lag screws destroyed bone tissue, the lag screw group exhibited a higher rotational resistance and a lower risk of rotational displacement than the grasping pin model. The depth of cadaveric femoral heads was 60mm compared with 30mm for femoral heads obtained after surgery; therefore, the pins could be completely inserted up to the octagonal portion in the cadaveric bones, resulting in higher rotational resistance

    Redox oscillations in 18650-type lithium-ion cell revealed by in operando Compton scattering imaging

    Compton scattering imaging using high-energy synchrotron x rays allows the visualization of the spatiotemporal lithiation state in lithium-ion batteries probed in operando. Here, we apply this imaging technique to the commercial 18650-type cylindrical lithium-ion battery. Our analysis of the line shapes of the Compton scattering spectra taken from different electrode layers reveals the emergence of inhomogeneous lithiation patterns during the charge-discharge cycles. Moreover, these patterns exhibit oscillations in time where the dominant period corresponds to the timescale of the charging curve.Peer reviewe

    Impact of the day of the week on the discontinuation of broad-spectrum antibiotic prescriptions; a multi-centered observational study

    To encourage and guide antimicrobial stewardship team (AST) activity and promote appropriate antibiotic use, we studied the impact of day of the week on the initiation and discontinuation of antibiotic administration. This was a multicenter observational study conducted at 8 Japanese hospitals from April 1 to September 30, 2019, targeting patients who underwent treatment with broad-spectrum antibiotics, such as anti-methicillin-resistant Staphylococcus aureus agents and anti-pseudomonal agents. We compared the weekly numbers of initiations and discontinuations of antibiotic prescription on each day of the week or on the days after a holiday. There was no statistical difference in the number of antibiotic initiations on both weekdays and the day after a holiday. However, antibiotic discontinuation was significantly higher from Tuesday onward than Monday and from the second day than the first day after a holiday. Similar trends were observed regardless of the categories of antibiotics, hospital and admission ward, and AST activity. This study suggests that broad-spectrum antibiotics tend to be continued during weekends and holidays and are most likely to be discontinued on Tuesday or the second day after a holiday. This was probably due to behavioral factors beyond medical indications, requiring further antimicrobial stewardship efforts in the future

    LDL-C/HDL-C Ratio Predicts Carotid Intima-Media Thickness Progression Better Than HDL-C or LDL-C Alone

    High-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol (LDL-C) are strong predictors of atherosclerosis. Statin-induced changes in the ratio of LDL-C to HDL-C (LDL-C/HDL-C) predicted atherosclerosis progression better than LDL-C or HDL-C alone. However, the best predictor of subclinical atherosclerosis remains unknown. Our objective was to investigate this issue by measuring changes in carotid intima-media thickness (IMT). A total of 1,920 subjects received health examinations in 1999, and were followed up in 2007. Changes in IMT (follow-up IMT/baseline IMT × 100) were measured by ultrasonography. Our results showed that changes in IMT after eight years were significantly related to HDL-C (inversely, P < 0.05) and to LDL-C/HDL-C ratio (P < 0.05). When the LDL-C/HDL-C ratios were divided into quartiles, analysis of covariance showed that increases in the ratio were related to IMT progression (P < 0.05). This prospective study demonstrated the LDL-C/HDL-C ratio is a better predictor of IMT progression than HDL-C or LDL-C alone
