    Cárie dentária e fatores relacionados em crianças Brasileiras de região com água fluoretada e não fluoretada

    PURPOSE: To compare the caries prevalence, saliva buffering capacity (SBC), oral hygiene (OH), dietary habits, family income (FI) and frequency of visits to a dental office (Do) between Brazilian children living in areas with and without fluoridated public water supply. METHODS: Forty-six 5-7-year-old preschoolers were selected in Itatiba, SP, Brazil; 19 were from a fluoridated area, and 27 were from a non-fluoridated area. The caries index was determined according to the World Health Organization criteria, and the SBC was assessed by titration with hydrochloric acid. The FI, frequency of OH and visits to Do were estimated by questionnaire. The dietary habits were assessed with a diet chart. The differences between the groups were analyzed with Mann-Whitney-U tests (α=0.05). RESULTS: Children from the non-fluoridated area showed significantly higher dmft/DMFT than those from the fluoridated area, but they showed significantly lower SBC, OH frequency and FI. No significant differences were observed between the areas for dietary habits and visits to Do. CONCLUSION: Children from fluoridated areas showed higher salivary buffering capacity, family income and oral hygiene frequency as well as lower caries prevalence, supporting the beneficial effect of fluoride in the tap water for caries prevention.OBJETIVO: Comparar prevalência de cárie, capacidade tamponante da saliva (CTS), higiene bucal (HB), hábitos dietéticos, renda familiar (RF) e frequência de visita a consultórios odontológicos (Co) entre crianças brasileiras residentes em áreas de água de abastecimento público fluoretadas e não fluoretadas. METODOLOGIA: Quarenta e seis crianças entre 5-7 anos foram selecionadas em Itatiba-SP-Brasil, sendo 19 pertencentes à área fluoretada e 27 à área não fluoretada. O índice de cárie foi determinado de acordo com o critério da Organização Mundial de Saúde e a CTS foi medida por titulação com ácido clorídrico. A RF, frequência de HB e visita ao Co foram estimadas por questionário. Os hábitos dietéticos foram avaliados com diário de dieta. As diferenças entre as variáveis foram analisadas pelo teste de Mann Whitney (α=0,05). RESULTADOS: As crianças da área não fluoretada apresentaram significativamente maior ceod/CPOD que àquelas da área fluoretada, porém significativamente menor CTS, HB e RF. Com relação a hábitos dietéticos e visitas ao Co, não foi observada diferença significativa entre as áreas. CONCLUSÃO: As crianças da área fluoretada apresentaram maior capacidade tamponante da saliva, renda familiar e frequência de higiene bucal, assim como menor prevalência de cárie, reforçando o efeito benéfico do flúor nas águas de abastecimento para prevenir a cárie

    Determinants of HIV late presentation among men who have sex with men in Portugal (2014–2019): who’s being left behind?

    Introduction: HIV late presentation (LP) remains excessive in Europe. We aimed to analyze the factors associated with late presentation in the MSM population newly diagnosed with HIV in Portugal between 2014 and 2019. Methods: We included 391 newly HIV-1 diagnosed Men who have Sex with Men (MSM), from the BESTHOPE project, in 17 countrywide Portuguese hospitals. The data included clinical and socio-behavioral questionnaires and the viral genomic sequence obtained in the drug resistance test before starting antiretrovirals (ARVs). HIV-1 subtypes and epidemiological surveillance mutations were determined using different bioinformatics tools. Logistic regression was used to estimate the association between predictor variables and late presentation (LP). Results: The median age was 31 years, 51% had a current income between 501-1,000 euros, 28% were migrants. 21% had never been tested for HIV before diagnosis, with 42.3% of MSM presenting LP. 60% were infected with subtype B strains. In the multivariate regression, increased age at diagnosis, higher income, lower frequency of screening, STI ever diagnosed and higher viral load were associated with LP. Conclusion: Our study suggests that specific subgroups of the MSM population, such older MSM, with higher income and lower HIV testing frequency, are not being targeted by community and clinical screening services. Overall, targeted public health measures should be strengthened toward these subgroups, through strengthened primary care testing, expanded access to PrEP, information and promotion of HIV self-testing and more inclusive and accessible health services.info:eu-repo/semantics/publishedVersio

    SARS-CoV-2 introductions and early dynamics of the epidemic in Portugal

    Genomic surveillance of SARS-CoV-2 in Portugal was rapidly implemented by the National Institute of Health in the early stages of the COVID-19 epidemic, in collaboration with more than 50 laboratories distributed nationwide. Methods By applying recent phylodynamic models that allow integration of individual-based travel history, we reconstructed and characterized the spatio-temporal dynamics of SARSCoV-2 introductions and early dissemination in Portugal. Results We detected at least 277 independent SARS-CoV-2 introductions, mostly from European countries (namely the United Kingdom, Spain, France, Italy, and Switzerland), which were consistent with the countries with the highest connectivity with Portugal. Although most introductions were estimated to have occurred during early March 2020, it is likely that SARS-CoV-2 was silently circulating in Portugal throughout February, before the first cases were confirmed. Conclusions Here we conclude that the earlier implementation of measures could have minimized the number of introductions and subsequent virus expansion in Portugal.     Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.     Callus cell culture of Pothomorphe umbellata (L.) under stress condition leads to high content of peroxidase enzyme

    Pothomorphe umbellata (L.), conhecida no Brasil como caapeba, possui diversos usos na medicina popular, sendo estudada especialmente em relação a sua atividade farmacológica. A atividade especifica da peroxidase (unidade/ mg proteina) foi determinada em amostras de callus (cultura de células) de P. umbellata, crescidos em dois tipos de meio MS (meio 1 e meio 2), sob condição de fotoperiodo de 16 horas ou mantidas no escuro (condições de estresse). Pela curva de velocidade de crescimento da cultura de células amostras do meio 2, sob fotoperiodo, apresentaram o melhor índice de crescimento, seguido de amostras do mesmo meio mantidas em condições de estresse. Os resultados também mostraram que a cultura de células crescidas sob condições de estresse levou a um aumento expressivo da quantidade da enzima peroxidase (aumento de 700%). Os valores das constantes cinéticas determinadas foram 3,3mmol.L-1 e 2,8 sec-1, respectivamente, para kM e v max, usando guaiacol como substrato.Pothomorphe umbellata (L.) known on Brazil as Caapeba has a number of popular medicinal use, and it has been studied in relation to its pharmacological activity. Peroxidase specific activity (units/mg protein) was evaluated in callus cell culture samples of the P.umbellata, grown in two different MS medium (media 1 and media 2), submitted to 16 hours photoperiod or kept in darkness. Cell growth rate curve showed that the best growth indices were observed when media 2 submitted to the photoperiod regime was used, followed by the same media kept in darkness (stress condition). The results obtained also showed that the cell culture grown under stress conditions (darkness) lead to high content of peroxidase enzyme (an increase of 700% was observed).     Nicotine, an oxidizing agent, is certainly one of the most widely used alkaloids in the world. It is, together with its main metabolite, cotinine, responsible for tobacco-dependence. The use of tobacco is closely associated with lung disease, morphological leukocyte modification and generation of oxidant species. The aim of this study was to look for a possible relationship between cotinine, oxidant species generation and oxidative processes. After studying the action of cotinine in some chemical oxidation models and on the enzymatic kinetics of peroxidases (myeloperoxidase and horseradish peroxidase), we concluded that cotinine does not act directly upon H 2O 2, HOCl, taurine chloramines, horseradish peroxidase or myeloperoxidase

    Cogumelos apresentam propriedades nutricionais e medicinais bastante empregadas na manutenção da saúde humana e têm sido considerado por pesquisadores como possíveis fontes de seqÜestradores de radicais livres. Neste trabalho, as propriedades antioxidantes de extratos aquosos de Agaricus blazei Murill, produzidos por maceração e decocção, são demonstrados in vitro. A resistência à oxidação é demonstrada através de três mecanismos: i) inibição de processos oxidativos enzimáticos, with 100% de inibição da HRP (horseradish peroxidase) e MPO (myeloperoxidase); ii) inibição do estresse oxidativo celular, com 80% de inibição do oxidative burst de neutrófilos polimorfonucleares (PMN) e iii) ação direta sobre espécies reativas, com 62% e 87% de supressão do HOCl e ânion superóxido (O2• –), respectivamente. A partir destes dados, concluise que o extrato aquoso deste cogumelo tem significativa capacidade antioxidante, indicando sua possível aplicação para propósitos nutracêuticos e medicinais.Mushrooms possess nutritional and medicinal properties that have long been used for human health preservation and that have been considered by researchers as possible sources of free radical scavengers. In this work, the antioxidant properties of water extracts from Agaricus blazei Murill, produced by maceration and decoction, are demonstrated in vitro.     Characterization of a large cluster of HIV-1 A1 infections detected in Portugal and connected to several Western European countries

    HIV-1 subtypes associate with differences in transmission and disease progression. Thus, the existence of geographic hotspots of subtype diversity deepens the complexity of HIV-1/AIDS control. The already high subtype diversity in Portugal seems to be increasing due to infections with sub-subtype A1 virus. We performed phylogenetic analysis of 65 A1 sequences newly obtained from 14 Portuguese hospitals and 425 closely related database sequences. 80% of the A1 Portuguese isolates gathered in a main phylogenetic clade (MA1). Six transmission clusters were identified in MA1, encompassing isolates from Portugal, Spain, France, and United Kingdom. The most common transmission route identified was men who have sex with men. The origin of the MA1 was linked to Greece, with the first introduction to Portugal dating back to 1996 (95% HPD: 1993.6-1999.2). Individuals infected with MA1 virus revealed lower viral loads and higher CD4+ T-cell counts in comparison with those infected by subtype B. The expanding A1 clusters in Portugal are connected to other European countries and share a recent common ancestor with the Greek A1 outbreak.
