    Iron Deficiency Anemia: An Unexpected Cause of an Acute Occipital Lobe Stroke in an Otherwise Healthy Young Woman

    A 29-year-old caucasian woman who presented to the hospital with an acute onset of right eye visual disturbance and headache was found to have an acute left occipital lobe infarction. Past medical history was significant for iron deficiency anemia (IDA) secondary to menorrhagia. Her initial hemoglobin level was 7.8 G/DL, and her symptoms improved after iron and blood transfusions. Hypercoagulable studies were completed in the outpatient setting, and the results were unremarkable. Her acute stroke was most likely related to IDA as she had low cardiovascular risk factors along with a negative complete stroke workup

    Background: It has previously been suggested that the use of illustrations in MCQs may have variable effects on individual items. This study examines the effect of illustrated questions, as opposed to pure text, to discern if any overall bias between the two formats is detectable. Summary of work: We reviewed 6 Histology MCQ papers from our Medical Junior Cycle. Classical test theory analysis was performed on all MCQs, which were then divided into two groups, those with associated images and those without. Summary of results: We analysed 195 single best answer MCQs; 100 with associated illustrations, 95 without. The number of students per examination ranged from 277 to 347, with a total of 60,850 student-question interactions. There was no difference in question difficulty between the two groups (0.800 vs. 0.770; p = 0.862, Mann-Whitney-U). The discriminating power of the questions, as measured by point biserial correlation, was also identical (0.315 vs. 0.300; p = 0.939; Independent t-test). Conclusions: We found no overall bias or effect on either item difficulty or discrimination resulting from the addition of illustrations. We suggest that illustrated questions, as with textual vignettes, may test a range of cognitive levels depending on how they are employed

    Evolution of JAK-STAT pathway components : mechanisms and role in immune system development

    BackgroundLying downstream of a myriad of cytokine receptors, the Janus kinase (JAK) &ndash; Signal transducer and activator of transcription (STAT) pathway is pivotal for the development and function of the immune system, with additional important roles in other biological systems. To gain further insight into immune system evolution, we have performed a comprehensive bioinformatic analysis of the JAK-STAT pathway components, including the key negative regulators of this pathway, the SH2-domain containing tyrosine phosphatase (SHP), Protein inhibitors against Stats (PIAS), and Suppressor of cytokine signaling (SOCS) proteins across a diverse range of organisms.ResultsOur analysis has demonstrated significant expansion of JAK-STAT pathway components co-incident with the emergence of adaptive immunity, with whole genome duplication being the principal mechanism for generating this additional diversity. In contrast, expansion of upstream cytokine receptors appears to be a pivotal driver for the differential diversification of specific pathway components.ConclusionDiversification of JAK-STAT pathway components during early vertebrate development occurred concurrently with a major expansion of upstream cytokine receptors and two rounds of whole genome duplications. This produced an intricate cell-cell communication system that has made a significant contribution to the evolution of the immune system, particularly the emergence of adaptive immunity.<br /

    A biologging database of juvenile white sharks from the northeast Pacific

    Species occurrence records are vital data streams in marine conservation with a wide range of important applications. From 2001–2020, the Monterey Bay Aquarium led an international research collaboration to understand the life cycle, ecology, and behavior of white sharks (Carcharodon carcharias) in the southern California Current. The collaboration was devoted to tagging juveniles with animal-borne sensors, also known as biologging. Here we report the full data records from 59 pop-up archival (PAT) and 20 smart position and temperature transmitting (SPOT) tags that variously recorded pressure, temperature, and light-level data, and computed depth and geolocations for 63 individuals. Whether transmitted or from recovered devices, raw data files from successful deployments (n = 70) were auto-ingested from the manufacturer into the United States (US) Animal Telemetry Network’s (ATN) Data Assembly Center (DAC). There they have attributed a full suite of metadata, visualized within their public-facing data portal, compiled for permanent archive under the DataONE Research Workspace member node, and are accessible for download from the ATN data portal

    Method for the Collection and HPLC Analysis of Hydrogen Peroxide and C\u3csub\u3el\u3c/sub\u3e and C\u3csub\u3e2\u3c/sub\u3e Hydroperoxides in the Atmosphere

    An HPLC (high-performance liquid chromatography) method was developed to quantify hydrogen peroxide, methyl hydroperoxide. Hydroxymethyl hydroperoxide, ethyl hydroperoxide, and peroxyaectic acid in the atmosphere. Gas-phase hydroperoxides are collected in aqueous solution using a continuous-flow glass scrubbing coil and then analyzed by an HPLC postcolumn derivatization system. The detection system is based on fluorescence, produced by the product of the reaction of hydroperoxides with peroxidase and p-hydroxyphenylacetic acid. Reproducibilities are better than 3% for all hydroperoxides in aqueous concentrations of 1 × 10−7–6 × 10−7 M. Detection limits in aqueous concentration are 1.2 × 10−9 M for hydrogen peroxide, 1.5 × 10−9 M for hydroxymethyl hydroperoxide, 2.9 × 10−9 M for methyl hydroperoxide, 16 × 10−9 M for peroxyaectic acid, and 19 × 10−9 M for ethyl hydroperoxide. Corresponding gas-phase detection limits are 5 PPtv for hydrogen peroxide, 7 pptv for hydroxymethyl hydroperoxide, 13 pptv for methyl hydroperoxide, 72 pptv for peroxyacetic acid, and 84 pptv for ethyl hydroperoxide for an air sample flow rate of two standard liters per minute and collection solution flow rate of 4 × 10−4 L min−1. The gas-phase detection limits for the latter three hydroperoxides vary depending on temperature, pressure, air sample flow rate, and collection solution flow rate.     Feedback from activity trackers improves daily step count after knee and hip arthroplasty: A randomized controlled trial

    Background: Commercial wrist-worn activity monitors have the potential to accurately assess activity levels and are being increasingly adopted in the general population. The aim of this study was to determine if feedback from a commercial activity monitor improves activity levels over the first 6 weeks after total hip arthroplasty (THA) or total knee arthroplasty (TKA). Methods: One hundred sixty-three consecutive subjects undergoing primary TKA or THAwere randomized into 2 groups. Subjects received an activity tracker with the step display obscured 2 weeks before surgery and completed patient-reported outcome measures (PROMs). On day 1 after surgery, participants were randomized to either the “feedback (FB) group” or the “no feedback (NFB) group.” The FB group was able to view their daily step count and was given a daily step goal. Participants in the NFB group wore the device with the display obscured for 2 weeks after surgery, after which time they were also able to see their daily step count but did not receive a formal step goal. The mean daily steps at 1, 2, 6 weeks, and 6 months were monitored. At 6 months after surgery, subjects repeated PROMs and daily step count collection. Results: Of the 163 subjects, 95 underwent THA and 68 underwent TKA. FB subjects had a significantly higher (P \u3c .03) mean daily step count by 43% in week 1, 33% in week 2, 21% in week 6, and 17% at 6 months, compared with NFB. The FB subjects were 1.7 times more likely to achieve a mean 7000 steps per day than the NFB subjects at 6 weeks after surgery (P ¼ .02). There was no significant difference between the groups in PROMs at 6 months. Ninety percent of FB and 83% of NFB participants reported that they were satisfied with the results of the surgery (P ¼ .08). At 6 months after surgery, 70% of subjects had a greater mean daily step count compared with their preoperative level. Conclusion: Subjects who received feedback from a commercial activity tracker with a daily step goal had significantly higher activity levels after hip and knee arthroplasty over 6 weeks and 6 months, compared with subjects who did not receive feedback in a randomized controlled trial. Commercial activity trackers may be a useful and effective adjunct after arthroplasty

    Pixel Entanglement: Experimental Realization of Optically Entangled D=3 and D=6 Qudits

    We demonstrate a simple experimental method for creating entangled qudits. Using transverse-momentum and position entanglement of photons emitted in spontaneous parametric down-conversion, we show entanglement between discrete regions of space, i.e., pixels. We map each photon onto as many as six pixels, where each pixel represents one level of our qudit state. The method is easily generalizable to create even higher dimensional, entangled states. Thus, the realization of quantum information processing in arbitrarily high dimensions is possible, allowing for greatly increased information capacity

    Quantifying habitat selection and variability in habitat suitability for juvenile white sharks

    While adult white sharks (Carcharodon carcharias) are apex predators with a circumglobal distribution, juvenile white sharks (JWS) feed primarily on bottom dwelling fishes and tend to be coastally associated. Despite the assumedly easier access to juveniles compared to large, migratory adults, limited information is available on the movements, environments, and distributions of individuals during this life stage. To quantify movement and understand their distribution in the southern California Bight, JWS were captured and fitted with dorsal fin-mounted satellite transmitters (SPOT tags; n = 18). Nine individuals crossed the U.S. border into Baja California, Mexico. Individuals used shallow habitats (134.96 +/- 191.1 m) close to shore (7.16 +/- 5.65 km). A generalized linear model with a binomial distribution was used to predict the presence of individuals based on several environmental predictors from these areas. Juveniles were found to select shallow habitats (\u3c 1000 m deep) close to land (\u3c 30 km of the shoreline) in waters ranging from 14 to 24 degrees C. Southern California was found to be suitable eight months of the year, while coastal habitats in Baja California were suitable year-round. The model predicted seasonal movement with sharks moving from southern California to Baja California during winter. Additionally, habitat distribution changed inter annually with sharks having a more northerly distribution during years with a higher Pacific Decadal Oscillation index, suggesting sharks may forego their annual fall migrations to Baja California, Mexico, during El Nino years. Model predictions aligned with fishery-dependent catch data, with a greater number of sharks being captured during periods and/or areas of increased habitat suitability. Thus, habitat models could be useful for predicting the presence of JWS in other areas, and can be used as a tool for potentially reducing fishery interactions during seasons and locations where there is increased susceptibility of incidental catch

    Measurement of Formic Acid, Acetic Acid and Hydroxyacetaldehyde, Hydrogen Peroxide, and Methyl Peroxide in Air by Chemical Ionization Mass Spectrometry: Airborne Method

    A chemical ionization mass spectrometry (CIMS) method utilizing a reagent gas mixture of O2, CO2, and CH3I in N2 is described and optimized for quantitative gas-phase measurements of hydrogen peroxide (H2O2), methyl peroxide (CH3OOH), formic acid (HCOOH), and the sum of acetic acid (CH3COOH) and hydroxyacetaldehyde (HOCH2CHO; also known as glycolaldehyde). The instrumentation and methodology were designed for airborne in situ field measurements. The CIMS quantification of formic acid, acetic acid, and hydroxyacetaldehyde used I− cluster formation to produce and detect the ion clusters I−(HCOOH), I−(CH3COOH), and I−(HOCH2CHO), respectively. The CIMS also produced and detected I− clusters with hydrogen peroxide and methyl peroxide, I−(H2O2) and I−(CH3OOH), though the sensitivity was lower than with the O2− (CO2) and O2− ion clusters, respectively. For that reason, while the I− peroxide clusters are presented, the focus is on the organic acids. Acetic acid and hydroxyacetaldehyde were found to yield equivalent CIMS responses. They are exact isobaric compounds and indistinguishable in the CIMS used. Consequently, their combined signal is referred to as the acetic acid equivalent sum. Within the resolution of the quadrupole used in the CIMS (1m∕z), ethanol and 1- and 2-propanol were potential isobaric interferences to the measurement of formic acid and the acetic acid equivalent sum, respectively. The CIMS response to ethanol was 3.3% that of formic acid and the response to either 1- or 2-propanol was 1% of the acetic acid response; therefore, the alcohols were not considered to be significant interferences to formic acid or the acetic acid equivalent sum. The multi-reagent ion system was successfully deployed during the Front Range Air Pollution and Photochemistry Éxperiment (FRAPPÉ) in 2014. The combination of FRAPPÉ and laboratory calibrations allowed for the post-mission quantification of formic acid and the acetic acid equivalent sum observed during the Deep Convective Clouds and Chemistry Experiment in 2012
