Aiming toWards Evidence baSed inTerpretation of Cardiac biOmarkers in patients pResenting with chest pain using Point of Care Testing (WESTCOR-POC): study design.
OBJECTIVES: Patients presenting with symptoms suggestive of acute coronary syndrome (ACS) contribute to a high workload and overcrowding in the Emergency Department (ED). Accelerated diagnostic protocols for non-ST-elevation myocardial infarction have proved challenging to implement. One obstacle is the turnaround time for analyzing high-sensitivity cardiac troponin (hs-cTn). In the WESTCOR-POC study (Clinical Trials number NCT05354804) we aim to evaluate safety and efficiency of a 0/1 h hs-cTn algorithm utilizing a hs-cTnI point of care (POC) instrument in comparison to central laboratory hs-cTnT measurements. DESIGN: This is a prospective single-center randomized clinical trial aiming to include 1500 patients admitted to the ED with symptoms suggestive of ACS. Patients will receive standard investigations following the European Society of Cardiology 0/1h protocols for centralized hs-cTnT measurements or the intervention using a 0/1h POC hs-cTnI algorithm. Primary end-points are 1) Safety; death, myocardial infarction or acute revascularization within 30 days 2) Efficiency; length of stay in the ED, 3) Cost- effectiveness; total episode cost, 4) Patient satisfaction, 5) Patient symptom burden and 6) Patients quality of life. Secondary outcomes are 12-months death, myocardial infarction or acute revascularization, percentage discharged after 3 and 6 h, total length of hospital stay and all costs related to hospital contact within 12 months. CONCLUSION: Results from this study may facilitate implementation of POC hs-cTn testing assays and accelerated diagnostic protocols in EDs, and may serve as a valuable resource for guiding future investigations for the use of POC high sensitivity troponin assays in outpatient clinics and prehospital settings
A protective personal factor against disability and dependence in the elderly: an ordinal regression analysis with nine geographically-defined samples from Spain
Background
Sense of Coherence (SOC) is defined as a tendency to perceive life experiences as comprehensible, manageable and meaningful. The construct is split in three major domains: Comprehensibility, Manageability, and Meaningfulness. SOC has been associated with successful coping strategies in the face of illness and traumatic events and is a predictor of self-reported and objective health in a variety of contexts. In the present study we aim to evaluate the association of SOC with disability and dependence in Spanish elders.
Methods
A total of 377 participants aged 75 years or over from nine locations across Spain participated in the study (Mean age: 80.9 years; 65.3% women). SOC levels were considered independent variables in two ordinal logistic models on disability and dependence, respectively. Disability was established with the World health Organization-Disability Assessment Schedule 2.0 (36-item version), while dependence was measured with the Extended Katz Index on personal and instrumental activities of daily living. The models included personal (sex, age, social contacts, availability of an intimate confidant), environmental (municipality size, access to social resources) and health-related covariates (morbidity).
Results
High Meaningfulness was a strong protective factor against both disability (Odds Ratio [OR] = 0.50; 95% Confidence Interval [CI] = 0.29–0.87) and dependence (OR = 0.33; 95% CI = 0.19–0.58) while moderate and high Comprehensibility was protective for disability (OR = 0.40; 95% CI = 0.22–0.70 and OR = 0.39; 95%CI = 0.21–0.74), but not for dependence. Easy access to social and health resources was also highly protective against both disability and dependence.
Conclusions
Our results are consistent with the view that high levels of SOC are protective against disability and dependence in the elderly. Elderly individuals with limited access to social and health resources and with low SOC may be a group at risk for dependence and disability in Spain.This project was partially funded by a research contract in support of the project “Epidemiological Study of Dementia in Spain” signed by the Pfizer Foundation and Carlos III Institute of HealthS

В статье рассматриваются особенности бытования фольклора белорусов в социокультурных условиях информационного общества. Авторы дают определение понятию «информационное общество», приводят аргументы в пользу социокультурных факторов, вызывающих резкое изменение условий бытования фольклора на современном этапе, среди которых доминируют глобализация и явления, ее сопровождающие. Показано, что в Беларуси в фольклорный процесс вовлечены такие разнокачественные явления, как аутентичный фольклор, носителем которого является взрослое, как правило, деревенское население, и постфольклор, стирающий грани как между деревенской и городской сферами бытования, так и между возрастными и церемониальными ограничениями. В выводах утверждается, что нарастающий процесс формирования глобального информационного общества неизбежно приводит к тому, что стираются элементы культурной дифференциации. Однако, с другой стороны, оно усиливаетспрос на развитие разнообразия культур, запускается процесс «этнического возрождения» отдельных народов.The article deals with the peculiarities of the existence of Belarusian folklore in the socio-cultural conditions of the information society. The authors define the concept of “information society”, argues in favor of socio-cultural factors that cause a sharp change in the conditions of existence of folklore at the present stage, among which globalization and the phenomena accompanying it dominate. It is shown that in Belarus the folk involved are
of different quality phenomena as authentic folklore, which the carrier is an adult, as a rule, rural population, and PostalCode, blur the line between rural and urban areas of existence, and between age and ceremonial restrictions. The conclusions state that the growing process of formation of the global information society inevitably leads to the fact that the elements of cultural differentiation are erased. However, on the other hand, it increases the demand for the development of cultural diversity, the process of "ethnic revival" of individual peoples
