    Optimisation of an acoustic resonator for particle manipulation in air

    An acoustic resonator system has been investigated for the manipulation and entrapment of micron-sized particles in air. Careful consideration of the effect of the thickness and properties of the materials used in the design of the resonator was needed to ensure an optimised resonator. This was achieved using both analytical and finite-element modelling, as well as predictions of acoustic attenuation in air as a function of frequency over the 0.8 to 2.0 MHz frequency range. This resulted in a prediction of the likely operational frequency range to obtain particle manipulation. Experimental results are presented to demonstrate good capture of particles as small as 15 µm in diameter

    Why Tenth Graders Fail to Finish High School: A Dropout Typology Latent Class Analysis

    A large percentage of the students who drop out of K-12 schools in the United States do so at the end of high school, at some point after grade 10. Yet we know little about the differences between different types of students who drop out of the end of high school. The purpose of this study is to examine a typology of high school dropouts from a large nationally representative dataset (ELS:2002) using latent class analysis (LCA). We found three significantly different types of dropouts; Quiet, Jaded, and Involved. Based on this typology of three subgroups, we discuss implications for future dropout intervention research, policy, and practice

    N-1 modal interactions of a three-degree-of-freedom system with cubic elastic nonlinearities

    In this paper the (Formula presented.) nonlinear modal interactions that occur in a nonlinear three-degree-of-freedom lumped mass system, where (Formula presented.), are considered. The nonlinearity comes from springs with weakly nonlinear cubic terms. Here, the case where all the natural frequencies of the underlying linear system are close (i.e. (Formula presented.)) is considered. However, due to the symmetries of the system under consideration, only (Formula presented.) modes interact. Depending on the sign and magnitude of the nonlinear stiffness parameters, the subsequent responses can be classified using backbone curves that represent the resonances of the underlying undamped, unforced system. These backbone curves, which we estimate analytically, are then related to the forced response of the system around resonance in the frequency domain. The forced responses are computed using the continuation software AUTO-07p. A comparison of the results gives insights into the multi-modal interactions and shows how the frequency response of the system is related to those branches of the backbone curves that represent such interactions

    Multi-centre parallel arm randomised controlled trial to assess the effectiveness and cost-effectiveness of a group-based cognitive behavioural approach to managing fatigue in people with multiple sclerosis

    Abstract (provisional) Background Fatigue is one of the most commonly reported and debilitating symptoms of multiple sclerosis (MS); approximately two-thirds of people with MS consider it to be one of their three most troubling symptoms. It may limit or prevent participation in everyday activities, work, leisure, and social pursuits, reduce psychological well-being and is one of the key precipitants of early retirement. Energy effectiveness approaches have been shown to be effective in reducing MS-fatigue, increasing self-efficacy and improving quality of life. Cognitive behavioural approaches have been found to be effective for managing fatigue in other conditions, such as chronic fatigue syndrome, and more recently, in MS. The aim of this pragmatic trial is to evaluate the clinical and cost-effectiveness of a recently developed group-based fatigue management intervention (that blends cognitive behavioural and energy effectiveness approaches) compared with current local practice. Methods This is a multi-centre parallel arm block-randomised controlled trial (RCT) of a six session group-based fatigue management intervention, delivered by health professionals, compared with current local practice. 180 consenting adults with a confirmed diagnosis of MS and significant fatigue levels, recruited via secondary/primary care or newsletters/websites, will be randomised to receive the fatigue management intervention or current local practice. An economic evaluation will be undertaken alongside the trial. Primary outcomes are fatigue severity, self-efficacy and disease-specific quality of life. Secondary outcomes include fatigue impact, general quality of life, mood, activity patterns, and cost-effectiveness. Outcomes in those receiving the fatigue management intervention will be measured 1 week prior to, and 1, 4, and 12 months after the intervention (and at equivalent times in those receiving current local practice). A qualitative component will examine what aspects of the fatigue management intervention participants found helpful/unhelpful and barriers to change. Discussion This trial is the fourth stage of a research programme that has followed the Medical Research Council guidance for developing and evaluating complex interventions. What makes the intervention unique is that it blends cognitive behavioural and energy effectiveness approaches. A potential strength of the intervention is that it could be integrated into existing service delivery models as it has been designed to be delivered by staff already working with people with MS. Service users will be involved throughout this research. Trial registration: Current Controlled Trials ISRCTN7651747

    Numerical continuation in nonlinear experiments using local Gaussian process regression

    Control-based continuation (CBC) is a general and systematic method to probe the dynamics of nonlinear experiments. In this paper, CBC is combined with a novel continuation algorithm that is robust to experimental noise and enables the tracking of geometric features of the response surface such as folds. The method uses Gaussian process regression to create a local model of the response surface on which standard numerical continuation algorithms can be applied. The local model evolves as continuation explores the experimental parameter space, exploiting previously captured data to actively select the next data points to collect such that they maximise the potential information gain about the feature of interest. The method is demonstrated experimentally on a nonlinear structure featuring harmonically coupled modes. Fold points present in the response surface of the system are followed and reveal the presence of an isola, i.e. a branch of periodic responses detached from the main resonance peak

    Urinary tract infections in children after renal transplantation

    Urinary tract infections (UTI) after pediatric kidney transplantation (KTX) are an important clinical problem and occur in 15–33% of patients. Febrile UTI, whether occurring in the transplanted kidney or the native kidney, should be differentiated from afebrile UTI. The latter may cause significant morbidity and is usually associated with acute graft dysfunction. Risk factors for (febrile) UTI include anatomical, functional, and demographic factors as well as baseline immunosuppression and foreign material, such as catheters and stents. Meticulous surveillance, diagnosis, and treatment of UTI is important to minimize acute morbidity and compromise of long-term graft function. In febrile UTI, parenteral antibiotics are usually indicated, although controlled data are not available. As most data concerning UTI have been accumulated retrospectively, future prospective studies have to be performed to clarify pathogenetic mechanisms and risk factors, improve prophylaxis and treatment, and ultimately optimize long-term renal graft survival

    Renal malformations associated with mutations of developmental genes: messages from the clinic

    Renal tract malformations (RTMs) account for about 40% of children with end-stage renal failure. RTMs can be caused by mutations of genes normally active in the developing kidney and lower renal tract. Moreover, some RTMs occur in the context of multi-organ malformation syndromes. For these reasons, and because genetic testing is becoming more widely available, pediatric nephrologists should work closely with clinical geneticists to make genetic diagnoses in children with RTMs, followed by appropriate family counseling. Here we highlight families with renal cysts and diabetes, renal coloboma and Fraser syndromes, and a child with microdeletion of chromosome 19q who had a rare combination of malformations. Such diagnoses provide families with often long-sought answers to the question “why was our child born with kidney disease”. Precise genetic diagnoses will also help to define cohorts of children with RTMs for long-term clinical outcome studies

    The consequences of traumatic brain injury from the classroom to the courtroom: understanding pathways through structural equation modelling

    Purpose: Paediatric traumatic brain injury (TBI) can have resultant ongoing significant impairments which can impact life outcomes. The primary aim of this research was to explore whether TBI contributes to the relationship between poor educational outcomes and offending trajectories. Materials and methods: Through analysis of a dataset consisting of self-reported health, educational, and offending histories of 70 incarcerated young males, structural equation modelling was used to explore the mediation of educational outcomes and patterns in offending behaviour by chronic symptoms following TBI. Results: Symptoms related to TBI significantly mediated the relationship between decreased educational attainment and more frequent convictions. It did not mediate any relationships involving age at first conviction. Conclusions: Traumatic brain injury appears to have more influence over frequency of offending patterns than age at first conviction. However, TBI remains a pervasive factor in both higher rates of offending and poorer educational attainment. In order to tackle this effect on adverse social outcomes, greater attention to the impact of TBI is required in education and criminal justice systems. IMPLICATIONS FOR REHABILITATION Highlights traumatic brain injury as a contributory factor in some education to offending pathways, suggesting that greater focus on rehabilitation within the education and criminal justice systems is required. Reinforces that greater understanding of educational pathways post-injury is needed to better facilitate rehabilitation within the school system
