    European intensive care physicians’ experience of infections due to antibiotic-resistant bacteria

    Background Antimicrobial resistance (AMR) compromises the treatment of patients with serious infections in intensive care units (ICUs), and intensive care physicians are increasingly facing patients with bacterial infections with limited or no adequate therapeutic options. A survey was conducted to assess the intensive care physicians' perception of the AMR situation in the European Union/European Economic Area (EU/EEA). Methods Between May and July 2017, physicians working in European ICUs were invited to complete an online questionnaire hosted by the European Society of Intensive Care Medicine. The survey included 20 questions on hospital and ICU characteristics, frequency of infections with multidrug-resistant (MDR) bacteria and relevance of AMR in the respondent's ICU, management of antimicrobial treatment as well as the use of last-line antibiotics in the six months preceding the survey. For the analysis of regional differences, EU/EEA countries were grouped into the four sub-regions of Eastern, Northern, Southern and Western Europe. Results Overall, 1062 responses from four European sub-regions were analysed. Infections with MDR bacteria in their ICU were rated as a major problem by 257 (24.2%), moderate problem by 360 (33.9%) and minor problem by 391 (36.8%) respondents. Third-generation cephalosporin-resistant Enterobacteriaceae were the most frequently encountered MDR bacteria followed by, in order of decreasing frequency, meticillin-resistant Staphylococcus aureus, carbapenem-resistant Enterobacteriaceae, carbapenem-resistant Pseudomonas aeruginosa and vancomycin-resistant enterococci. Perception of the relevance of the AMR problem and the frequency of specific MDR bacteria varied by European sub-region. Bacteria resistant to all or almost all available antibiotics were encountered by 132 (12.4%) respondents. Many physicians reported not having access to specific last-line antibiotics. Conclusions The percentage of European ICU physicians perceiving AMR as a substantial problem in their ICU is high with variation by sub-region in line with epidemiological studies. The reports of bacteria resistant to almost all available antibiotics and the limited availability of last-line antibiotics in ICUs in the EU/EEA are of concern

    Una Herramienta para la estimación del esfuerzo del estudiante de grado en ingeniería informática

    En el marco de las nuevas titulaciones de Grado en Ingeniería Informática, resulta muy conveniente establecer algún tipo de mecanismo que nos permita recoger información sobre la dedicación real de los estudiantes a nuestras asignaturas. A nivel de asignatura, esta información puede servir al profesorado para determinar el grado de acierto de su planificación temporal inicial. A un nivel más global, los datos recogidos proporcionan a los coordinadores de curso una idea de la carga total a la que está sometido el estudiante, y contribuyen a la identificación de posibles picos (o valles) de trabajo durante el cuatrimestre. En este trabajo se describe la puesta en marcha de un sistema, basado en encuestas, para el conocimiento del esfuerzo de los estudiantes de Grado en Ingeniería Informática de la Escuela Superior de Ingeniería Informática de Albacete. También se presentan y analizan algunos de los resultados obtenidos durante el curso académico 2012/13.SUMMARY -- In the framework of new degrees in Computer Science, it is desirable to establish some mechanism to let us collect some information about our students' commitment to our courses. On the subject level, this information may be useful for the teaching staff to determine the degree of success of this initial temporary planning. In a high level point of view, the data collected provide the course coordination with an idea of the total workload of students and it contributes to identify potential peaks (or valleys) of work throughout the semester. This work presents a surveybased system to obtain and register the efforts of our Computer Engineering students from the School of Computer Engineering of Albacete.     Rapid Molecular Diagnosis of Posttraumatic Keratitis and Endophthalmitis Caused by Alternaria infectoria

    The first case of Alternaria infectoria ocular infection is reported. Keratitis and endophthalmitis developed after eye-perforating trauma from a lemon tree branch. Two months after surgery and empirical steroid and antibiotic treatment, diagnosis by molecular methods was performed. PCR amplification was positive for a fungus after 4 h. Antifungal treatment with amphotericin B and fluconazole was initiated immediately. DNA sequence analysis showed Alternaria infectoria to be the causal agent. After topical and systemic administration of antifungal treatment, ocular inflammation disappeared and visual acuity improved.     Sphaerosporella brunnea (Alb. et Schwein.) Svrcek et Kubicka, a cup-shaped ascomycete related with truffle cultures and afforestation. A casual finding of Sphaerosporella brunnea in some greenhouses from Spain, growing in connection with several ectomycorrhizal plants, is reported. These plants were experimentally inoculated with low level inoculum of Tuber melanosporum.     Con el propósito de analizar la problemática sobre la exigibilidad y justiciabilidad de los derechos económicos, sociales y culturales (DESC), así como superar el abismo entre teoría y praxis de la defensa de los DESC, el artículo se centra en desarrollar una nueva forma de conceptualizar la fundamentación iusfilosófica de estos derechos sociales. Sostenemos que las propiedades incorrectas que se le han atribuido a los DESC y que en mucho han contribuido a formar grandes obstáculos para la exigibilidad y justiciabilidad de los DESC son producto de dos cosas: Una fundamentación filosófica tradicional errónea y una concepción también errónea de derecho y/o ciencia jurídica.     Geologic mapping of the Comet 67P/Churyumov-Gerasimenko's Northern hemisphere

    The Optical, Spectroscopic, and Infrared Remote Imaging System (OSIRIS), the scientific imaging system onboard the Rosetta mission, has been acquiring images of the nucleus of the comet 67P/Churyumov¿Gerasimenko since 2014 August with a resolution which allows a detailed analysis of its surface. Indeed, data reveal a complex surface morphology which is likely the expression of different processes which occurred at different times on the cometary nucleus. In order to characterize these different morphologies and better understand their distribution, we performed a geologic mapping of comet¿s 67P Northern hemisphere in which features have been distinguished based on their morphological, textural and stratigraphic characteristics. For this purpose, we used narrow-angle camera images acquired in 2014 August and September with a spatial scale ranging from 1.2 to 2.4 m pixel¿1. Several different geologic units have been identified on the basis of their different surface textures, granulometry and morphology. Some of these units are distinctive and localized, whereas others are more common and distributed all over the Northern hemisphere. Moreover, different types of linear features have been distinguished on the basis of their morphology. Some of these lineaments have never been observed before on a comet and can offer important clues on the internal structures of the nucleus itself.     Influence of environmental factors on the reproductive cycle of the eared ark Anadara notabilis (Röding, 1798) in Northeastern Venezuela

    5 figuras, 2 tablasThe reproductive cycle of the eared ark Anadara notabilis and its relationship with environmental factors was evaluated every 15 days between March 2004 and February 2005 at the northern part of the Peninsula de Araya, Venezuela. Environmental factors measured included temperature, salinity, chlorophyll a, and total seston, including the particulate organic matter and particulate inorganic matter fractions. Adult specimens were collected using a net drag and then randomly selected to estimate wet live biomass and dry biomass of soft body tissues. Gonad samples were processed with histological analysis to determine sex ratio, developmental stages, and variations of follicle size. Reproduction in A. notabilis was continuous throughout the year, with spawning peaks occurring in June and October, coinciding with the lowest water temperatures. In March, September, and November 2004, specimens attained the highest dry biomass values, whereas lowest dry biomass occurred in June and October 2004. Stepwise regression analyses demonstrated that temperature and particulate organic matter values are the main modulators of reproductive events.
