Egy új citotoxinnak, cytholethalis distending toxinnak (CDT) magyarországi előfordulása és molekulárbiológiai jellemzése = The occurrence and molecular biological characterization of a new cytotoxin -cytolethal distending toxin (CDT)- in Hungary
Jelen pályázatunkban egy új és nagy jelentÅ‘séggel rendelkezÅ‘ bakteriális citotoxin családnak a cytolethalis duzzasztó (distending) toxinnak (CDT) a magyarországi elterjedését és molekuláris biológiai jellemzését végezve megállapÃtottuk, hogy a CDT potenciális virulencia faktora lehet az emberi húgyúti és egyéb extraintesztinális infekciókat okozó, valamint a sertések választási hasmenésének és baromfi szepszisének eseteibÅ‘l izolált E. coli törzseknek. A CDT-termelÅ‘ baromfi eredetű törzsek között új virulens klónokat azonosÃtottunk. A CDT ismert tÃpusainak a kimutatására és tipizálására alkalmas diagnosztikai PCR eljárást dolgoztunk ki. A CDT-nek egy új tÃpusát, a CDT-IV toxint fedeztük fel. MegállapÃtottuk, hogy a CDT-IV termelÅ‘ törzsek változatos szerocsoportú emberi és állati eredetű patogén E. coli törzsekben is elÅ‘fordul. A cdtABC-IV operont klónoztuk, és antibiotikum-rezisztenciát kódoló kazettával cdtB mutáns törzset alakÃtottunk ki egy baromfi eredetű O115 szerocsoportú törzsben. MegállapÃtottuk, hogy a cdt-IV operon fág eredetű génekkel határolt. | This study focused on monitoring of a significant and new toxin family, cytolethal distending toxin (CDT) in Hungary, on molecular characterisation of CDT-producing strains and on developing new diagnostic tools. Results indicate that CDT could be a novel virulence factor of E. coli strains isolated from human urinary tract infections (UTI) and other (non-urinary) extraintestinal infections and stains isolated from porcine post-weaning diarrhoeal and poultry septicaemic cases. A new PCR system that specifically detects all known types of CDT lead to identification of a new member of a the CDT family (CDT-IV) was developed. It has been established that the CDT-IV producing animal and human pathogenic E. coli strains belonged to a wide variety of serogroupes. The cdtABC-IV operon from a poultry O115 strain was cloned and a the cdtB gene was mutated by side directed mutagenesis. PCR investigations revealed that cdtABC-IV was flanked by phage genes

Quaternary pulse position modulation electronics for free-space laser communications
The development of a high data-rate communications electronic subsystem for future application in free-space, direct-detection laser communications is described. The dual channel subsystem uses quaternary pulse position modulation (QPPM) and operates at a throughput of 650 megabits per second. Transmitting functions described include source data multiplexing, channel data multiplexing, and QPPM symbol encoding. Implementation of a prototype version in discrete gallium arsenide logic, radiofrequency components, and microstrip circuitry is presented
QPPM receiver for free-space laser communications
A prototype receiver developed at NASA Lewis Research Center for direct detection and demodulation of quaternary pulse position modulated (QPPM) optical carriers is described. The receiver enables dual-channel communications at 325-Megabits per second (Mbps) per channel. The optical components of the prototype receiver are briefly described. The electronic components, comprising the analog signal conditioning, slot clock recovery, matched filter and maximum likelihood data recovery circuits are described in more detail. A novel digital symbol clock recovery technique is presented as an alternative to conventional analog methods. Simulated link degradations including noise and pointing-error induced amplitude variations are applied. The bit-error-rate performance of the electronic portion of the prototype receiver under varying optical signal-to-noise power ratios is found to be within 1.5-dB of theory. Implementation of the receiver as a hybrid of analog and digital application specific integrated circuits is planned
An Observational Study on the Pharmacokinetics of Oseltamivir in Lactating Influenza Patients
Influenza infection may lead to serious complications in the postpartum period, therefore, oseltamivir treatment in these patients and their breastfed infants is of great importance. However, the pharmacokinetics of oseltamivir in postpartum lactating women with acute influenza infection, and the consequent infant exposure to oseltamivir are still unknown, and these data would help in assessing risk and the need for dose adjustment in breastfed infants
Management of chronic myeloid leukaemia: current treatment options, challenges, and future strategies.
Small molecule therapy is a critical component of targeted anticancer treatment, with tyrosine kinase inhibitors (TKIs) being the first compounds to treat the clonal Chronic Myelogenous Leukaemia (CML) translocation t (9;22) (q34; q11) effectively since 2001. TKIs, such as imatinib, have improved the 10-year survival rate of CML patients to 80%. They bind the kinase and inhibit downstream signaling pathways. However, therapy failure may be seen in 20-25% of CML patients due to intolerance or inadequacy related to dependent or independent mechanisms. This review aimed to summarize current treatment options involving TKIs, resistance mechanisms and the prospective approaches to overcome TKI resistance. We highlight -dependent mechanisms of TKI resistance by reviewing clinically-documented mutations and their consequences for TKI binding. In addition, we summarize independent pathways, including the relevance of drug efflux, dysregulation of microRNA, and the involvement of alternative signaling pathways. We also discuss future approaches, such as gene-editing techniques in the context of CML, as potential therapeutic strategies

A Fabry-kór a lizoszomális tárolási betegségek csoportjába tartozó, X-kromoszómához kötötten, recesszÃv módon öröklÅ‘dÅ‘ betegség, amely a globotriaozilceramid felhalmozódásához vezet a szervezet legkülönbözÅ‘bb szöveteiben. A betegség elsÅ‘ tünetei többnyire gyermekkorban jelentkeznek, a progresszió során a betegek súlyos szervi károsodásokkal és korai halálozással számolhatnak. ElsÅ‘sorban férfiak érintettek, azonban a betegség tüneteit heterozigóta nÅ‘k esetében is megfigyelhetjük, de náluk a kórkép súlyossága változó, általában enyhébb lefolyású. Az enzimpótló kezelések megjelenése szükségessé tette, hogy részletes diagnosztikus és terápiás protokollt dolgozzunk ki. A jelen dolgozatban megjelenÅ‘ ajánlásokat egy, a magyarországi Fabry-kóros betegek kezelésében részt vevÅ‘ orvosokból, a diagnosztika területén dolgozó biológosukból és egyéb szakemberekbÅ‘l álló multidiszciplináris munkacsoport foglalta össze. A munkacsoport áttekintette a korábbi klinikai tanulmányokat, a publikált vizsgálatokat és a közelmúltban megjelent nemzetközi és nemzeti útmutatókat.

Fabry disease is a rare, X-linked lysosomal storage disorder that leads to accumulation of globotriaosylceramide in different tissues of the body. The disease is progressive and the first symptoms usually present in childhood. Consequences of the disease are disability and premature death. The disease in females could be as severe as in males although women may be asymptomatic. The possibility of enzyme replacement therapy has made it necessary to elaborate a comprehensive guideline for the diagnosis and treatment follow-up. The guideline has been summarized by a Hungarian multi-disciplinary working group consisting of physicians who are involved in diagnosis and care of Fabry patients. Previous clinical studies, published articles, and recently established international treatment guidelines were reviewed by the group
