    Get PDF
Background: Multiple sclerosis is a major cause of disability in young adults. Vitamin D is a potent immunomodulator and the protective effect of Vitamin D on multiple sclerosis is supported by the reduced risk associated with sun exposure and use of Vitamin D supplements. Moreover, high circulating levels of Vitamin D have been associated with lower risk of multiple sclerosis. Objectives: The aim of this study is to clarify the relationship between vitamin D metabolite level and disability and relapse rate in multiple sclerosis. Methods: Fifty four Multiple sclerosis patients fulfilling McDonald's criteria and 70 controls matched in age, sex and same geographical areas,35 (64.8 %) patients were with relapsing remitting type,9 (16.6 %) patients were primary progressive type and 10 (18.6 %) patients were secondary progressive type. For the control group, 28 of them were with non-MS demyelinated neurological diseases including 18 with chronic inflammatory demyelinating polyneuropathy, 6 were Gillian Barrie syndrome and 4 were acute demyelinating encephalomyelitis, and the remaining forty two were apparently healthy controls. All multiple sclerosis patients and controls are subjected to assessment of 25(OH) D serum level by RIA. Results: very high statistically significant difference between patients and controls, also very high statistically significant negative correlation in all patients, and disability and relapse rate                                               .                                                                                                                Conclusion: There is an important role of vitamin D in outcome of multiple sclerosis and the relapsing rate of the relapsing remitting type Keywords: multiple sclerosis, vitamin D,relapse, disabilit

11 Principles Every Educational Organization Should Be Implementing

    Get PDF
Many articles and reports issued by educational institutions and research centers discussed the basic requirements that should be implemented by universities, schools, vocational schools, training centers and other categories of educational institutions to assure they can provide quality of education. Although these references have been widely used to achieve quality in education, still differences in requirements may exist. Hence the importance of shedding lights on the International standards ISO 21001 published in 2018 by the International Organization for Standardization.  In this article, the author highlighted on the implementation of 11 principles of quality education stipulated in this Standard. In this context, he touched upon the social responsibility principle and the related 7 core subjects (building a governance system, the rights of learners, practicing in a fair operating way, students issues, the development of community, protecting the environment,  administrative and academic staff practices) (ISO 26000: Guidance on social responsibility, 2010) that distinguish the ISO 21001 from other international standards and made it a tool that could be applied by educational institutions to create their shared value strategies. In this context, the authors heard the voices of instructors and analyze their views concerning the implementation of the twenty-three requirements mentioned in the ISO 21001, through diffusing surveys to instructors working in educational organizations in Lebanon. The results indicated that 19 of the requirements are being applied effectively. Acupressure for Chemotherapy Induced Nausea and Vomiting in Breast-Cancer Patients: A Randomized, Placebo-Controlled Clinical Trial

    Get PDF
Introduction: Chemotherapy is an important treatment in cancer (CA) care and is associated with numerous side effects. Early studies reported that patients cited nausea and vomiting as the most distressing symptoms when receiving chemotherapy. Despite continuing improvements in antiemetic therapies, nausea and vomiting following chemotherapy treatment for CA remains a significant clinical problem for many patients and there is correlation between the intensity of anticipatory nausea in the clinic prior to their treatment infusion and subsequent post treatment of nausea and vomiting. Since pharmacological treatments have failed to completely manage nausea and vomiting, exploring the complementary, non-pharmacological, approaches that can be used in addition to pharmacological approaches becomes paramount. Acupressure at the P6 point is a value-added technique in addition to pharmaceutics; management for women undergoing treatment for breast cancer to reduce the incidence and intensity of delayed chemotherapy induced nausea and vomiting (CINV), since up to 60% of patients had been reported nausea despite the use of antiemetics Aims: The first aim  is to examine the efficacy of P6-acupressure in preventing chemotherapy induced nausea and emesis associated with highly emetogenic chemotherapy (i.e. doxorubicin as adjuncts to standard 5-HT3 antiemetics (granisetron) and dexamethasone antiemetic given as part of routine care in reducing acute nausea (during the day of treatment) and delayed nausea (2-5 days) following the day of chemotherapy. The second aim is to examine the efficacy of the acupressure bands in reducing vomiting and in maintaining Quality of Life (QOL). Patients and methods: A randomised, double-blind, placebo controlled trial. One group received acupressure with bilateral stimulation of P6 (n=42), a second group received bilateral placebo stimulation, (n=42) and a third group received no acupressure wrist band and served as a control group, (n=42). Acupressure was applied using a Sea-Band (Sea- Band UK Ltd., Leicestershire, England) which had to wear for the 5 days following the chemotherapy administration. Assessments of acute and delayed nausea and emesis, OOL, patients' satisfaction, recommendation of treatment and requirement of rescue antiemetic were obtained. Results: No significant differences were found in the incidence of acute nausea or emesis 24- h following chemotherapy by treatment groups. Significant difference was found in the severity of early nausea (0-6 scale) in the acupressure group M (SD) 1.62 (2.04) as compared to placebo group 2.17 (2.09), p=, 0006. The acupressure group had a statistically significant reduction in the incidence of delayed nausea 40% (17/42) as compared to the control group 62% (26/42) (p= ,0495). Further analyses indicated that significant difference existed in the intensity of delayed nausea by acupressure group mean (SD) 1.45 (1.73), p=, 0002 as compared to control 2.03 (1.91). Significant difference also existed in the intensity of delayed nausea by placebo group 1.33 (1.66), p=, 0010 as compared to control 2.03 (1.91). Here we noted a placebo effect. The percentage of the patients who had delayed moderate to very severe nausea day 2-5 (≥3 on 0-6 scale) in the acupressure group is 55% (23/42 (p= 0206), in the placebo group 52% (22/42) (p= 0116), a statistically significant reduction existed as compared to control 79% (33/42). Here we noted a placebo effect. The incidence of delayed vomiting episodes day 2-5 was 48% (20/42), 64% (27/42), and 57% (24/42) in the acupressure, placebo and control group respectively. No significant differences were found between the groups. The mean of number of delayed emetic episodes day 2-5 was significantly less in the acupressure group 2.7 (1.87) as compared to placebo 3.3 (1.91), p=,0022 and control groups 2.07(1.20), P= ,0005. Requirement of rescue antiemetic was significantly lower in P6-acupressure (55%, 23/42), as compared to control group (76%, 32/42) (p= 0389). 81% (35/42)) of the patients in acupressure group were significantly satisfied with P6-acupressure as compared to placebo group 64% (27/42), p= 0.0471. 79% (34/42)) of the patients in acupressure group would recommend P6-acupressure to another patients as compared to placebo group 62% (26/42), p= 0,0533 . No statistically significant differences between groups were observed for the overall items response rate of the FACT-Scale which were 74/108, 67/108, 69/108 in the acupressure, placebo and control group respectively. Conclusion: P-6 Acupressure is efficacious for control of delayed chemotherapy related nausea and emesis and is a value-added method in addition to pharmaceutical management for women undergoing treatment for breast cancer. Placebo effect of acupressure decreased severity of delayed nausea day 2-5 but the mean of number of delayed emetic episodes and need of rescue antiemetics were reduced only by acupressure with the correct P6 point stimulation. Thesis (S.M.)--Massachusetts Institute of Technology, Dept. of Mechanical Engineering, 2013.Cataloged from PDF version of thesis.Includes bibliographical references (p. 175-181).Scaling or crystallization fouling of unwanted salts is one of the most challenging and expensive problems encountered in different applications such as heat exchangers and thermal water treatment technologies. Formation of dihydrated calcium sulfate scale, also known as gypsum, on a heated copper plate is studied in lab. The copper plate, held at a given temperature, is immersed in a supersaturated solution of calcium sulfate prepared at a given concentration. The flow conditions are governed by natural convection. A parametric study, in which surface temperature and the degree of supersaturation are varied, is set up and a scale inception time curve is plotted. No scale is observed at a supersaturation index smaller or equal to 1.4. Both higher temperatures and higher concentrations result in faster scale induction; however, the effect of temperature is more significant at lower degrees of supersaturation. SEM images of scale samples show needle-like crystals, the thinnest of which formed at a supersaturation index of 2.0. The classical nucleation theory of Mullin provides an excellent fit for the results. Interfacial energies calculated out of this model are in the reported ranges.by Mohamad H. Mirhi.S.M

Pemakaian Sistem Torrens dalam perundangan tanah Malaysia dan kesannya terhadap Iḥyā’ al-Mawāt: The application of Torrens System in Malaysian land law and its effect to Iḥyā’ al-Mawāt

    Get PDF
Iḥyā’ al-mawāt is one of the methods to own certain land in Islam. Anyone who clears land and works on it has a right of ownership on the land. The method to own land by iḥyā’ al-mawāt is actually was practiced in Tanah Melayu before the British colonialisation. However, during the colonial period, they introduced a land system which is known as the Torrens system. The Torrens system is a system of land administration in which registration is very important to recognised land ownership and it is in contradiction to iḥyā’ al-mawāt. This principle was applied through the enforcement of the National Land Code. The question arises whether ihya’ al-mawāt can still be applied and recognised under the Malaysian land law. Therefore, the objectives of this paper are to discuss iḥyā’ al-mawāt and also to determine its place in the Malaysian land law. This paper which employs the content analysis method of court case and statute finds that iḥyā’ al-mawāt is not recognised under the Malaysian land law and further, it is an offence under section 425 of the National Land Code
