    Association between vitamin d deficiencies in sarcoidosis with disease activity, course of disease and stages of lung involvements

    Background: Despite negative association between 25-hydroxy vitamin D and incidence of many chronic respiratory diseases, this feature was not well studied in sarcoidosis. Current study investigated the association between 25-hydroxy vitamin D deficiency with sarcoidosis chronicity, disease activity, extra-pulmonary skin manifestations, urine calcium level and pulmonary function status in Iranian sarcoidosis patients. Results of this study along with future studies, will supply more effective programs for sarcoidosis treatment. Methods: Eighty sarcoidosis patients in two groups of insufficient serum level and sufficient serum level of 25-hydroxy vitamin D were studied. Course of sarcoidosis was defined as acute and chronic sarcoidosis. Pulmonary function test (PFT) was assessed by spirometry. Skin involvements were defined as biopsy proven skin sarcoidosis. 24-hour urine calcium level was used to specify the disease activity. Stages of lung involvements were obtained by CT-scan and chest X-ray. The statistical analyses were evaluated using Statistical Package for the Social Sciences. Results: A significant negative correlation was obtained between vitamin D deficiency in sarcoidosis patients and disease chronic course and stages two to four of lung involvements. Considering other parameters of the disease and vitamin D deficiency, no significant correlation was detected. Conclusions: In conclusion, results of the current study implies in the role of vitamin 25(OH)D deficiencies in predicting the course of chronic sarcoidosis. Furthermore, it was concluded that vitamin 25(OH)D deficiency can direct pulmonary sarcoidosis toward stage 2–4 of lung involvements

    Decreased serum levels of angiotensin converting enzyme (ACE)2 and enhanced cytokine levels with severity of COVID-19: normalisation upon disease recovery

    Background:SARS-CoV-2 causes coronavirus disease 2019 (COVID-19). Circulating soluble angiotensin-converting enzyme (sACE2), the main receptor for SARS-CoV-2, together with components of the renin-angiotensin system promote infection and disease severity. Objective: In this pilot study we followed the time-course of sACE2 levels in relation to systemic cytokines in severe and moderate COVID-19 patients treated with remdesivir/dexamethasone in combination. Methods: Peripheral blood was obtained upon admission from 30 patients (12 with moderate disease and 18 with severe disease) and 14 patients with PCR-confirmed mild COVID-19. Severe and moderate patients were treated with remdesivir (200mg/first day and 100mg/day for the remaining days ) and dexamethasone (100mg/day ). 6 healthy control subjects (HC) were also enrolled. Serum interleukin (IL)-6 and IL-8 and sACE2 levels were measured by ELISA at baseline and during treatment in severe and moderate patients and at baseline in mild and HCs. Results: Baseline sACE2 levels were lower in severe (p=0.0005) and moderate (p=0.0022) patients than in patients with mild COVID-19 and in HC (p=0.0023 and p=0.0012 respectively). Serum sACE2 levels increased in patients with severe disease recovered over time compared with moderate (p=0.0021) and severe (p=0.0411) COVID-19 subjects at baseline. Systemic IL-6 and IL-8 levels were higher in all patient groups compared with HC and were not significantly affected over time or by remdesivir/dexamethasone treatment for 5 days. Conclusion: Serum sACE2 levels increase in severe COVID-19 patients as they recover over time whilst circulating cytokines are unaffected. Future studies should link these results to clinical outcomes.     In vitro comparison of rotary instruments and K-files in root canal preparation of primary molars

    "nBackground and Aim: The role of applying rotary instruments in reducing preparation time and proper root canal preparation have been approved in permanent teeth. The aim of this study was to compare the rotary instruments (Hero642) and K-files in root canal preparation of primary molars. Time and canal transportation were compared. "nMaterials and Methods: In this experimental, invitro study, 30 human extracted primary molars with 8 mm length were selected and divided into two equal groups. After preparing access cavity, group 1was prepared by K-files and group 2 by Hero642 systems. Before canal preparation, with # 15 K-file and after canal preparation, with master apical file, canal curvature was evaluated by Weine technique using Radiovisiography. Apical transportation was calculated. Time taken for preparation was recorded by chronometer. The results were analyzed with T-test and Mann-Whitney. P<0.05 was considered as the level of significance. "nResults: The average canal transportation angle was 3.42 for Hero642 group and 7.33 for K-file group (p<0.04). The mean instrumentation time was 4.28 for Hero642 group and 8.76 minutes for K-file group (p<0.001). There was a significant difference between two groups. "nConclusion: Based on the findings of this study, it is suggested to apply Hero642 rotary instrument for primary molar canal preparation due to less preparation time and less canal transposition

    Prevalence of allergic bronchopulmonary aspergillosis in cystic fibrosis patients using two different diagnostic criteria

    Objective. There are different diagnostic criteria for the diagnosis of Allergic bron-chopulmonary aspergillosis (ABPA) in CF patients. In this present study we evalu-ated the prevalence of ABPA in Iranian CF patients by two more usual diagnostic criteria as ISHAM working criteria (A) and CF Foundation Consensus Conference criteria (B). Methods. Eighty-six CF patients were included in the study. All CF patients underwent for Aspergillus skin prick test (AST), Aspergillus-specific IgE (sIgEAf) and Aspergillus-specific IgG (sIgGAf), total IgE. The ABPA prevalence was estimated by two diagnostic criteria, (A) and (B) and compared. Results. The frequency of positive AST, total IgE, sIgEAf and sIgGAf were 47 (54.6), 9 (10.5), 42 (48.8) and 67 (77.9), respectively. The obtained rate of ABPA prevalence (10.5) was identical in two diagnostic criteria A and B (kappa value of 1.000). Conclusions. The applied diagnostic criteria had no significant effect on the reported rate of ABPA prevalence. © 2020 Associazione Allergologi Immunologi Italiani Territoriali e Ospedalieri-AAIITO

    Diagnosis of allergic bronchopulmonary aspergillosis in patients with persistent allergic asthma using three different diagnostic algorithms

    Background: Allergic bronchopulmonary aspergillosis (ABPA) has been reported in various degrees among patients with persistent allergic asthma (PAA). Currently, there is no gold standard approach for diagnosis of ABPA. Objectives: In the current study, we aimed the evaluation of three different mainly used algorithms as Rosenberg & Patterson (A), ISHAM Working Group (B) and Greenberger (C) for diagnosis of ABPA in 200 patients with underlying PAA. Methods: All patients were evaluated using Aspergillus skin prick test (SPTAf), Aspergillus-specific IgE (sIgEAf) and IgG (sIgGAf), total IgE (tIgE), pulmonary function tests, radiological findings and peripheral blood eosinophil count. The prevalence rate of ABPA in PAA patients was estimated by three diagnostic criteria. We used Latent Class Analysis for the evaluation of different diagnostic parameters in different applied ABPA diagnostic algorithms. Results: Aspergillus sensitisation was observed in 30 (15.0) patients. According to algorithms A, B and C, nine (4.5), six (3.0) and 11 (5.5) of patients were diagnosed with ABPA, respectively. The sensitivity and specificity of criteria B and C were (55.6 and 99.5) and (100.0 and 98.9) respectively. sIgEAf and sIgGAf showed the high significant sensitivity. The performance of algorithm A, in terms of sensitivity and specificity, was somewhat better than algorithm B. Conclusion: Our study demonstrated that the sensitivity of different diagnostic algorithms could change the prevalence rate of ABPA. We also found that all of three criteria resulted an adequate specificity for ABPA diagnosis. A consensus patterns combining elements of all three criteria may warrant a better diagnostic algorithm. © 2020 Wiley-VCH Gmb
