    Demographic profile of 266 mother-infant dyads presenting to a multidisciplinary breast-feeding clinic: a descriptive study

    Worldwide, sustained breastfeeding rates are lower than optimal. Mothers and infants with suboptimal breastfeeding present to a wide variety of practitioners to assist their goal of total breastfeeding. To support these mother-infant dyads, a multi-disciplinary chiropractic and midwifery feeding clinic was opened alongside a chiropractic teaching clinic in England. To improve understanding of the presentation and clinical needs of these nursing dyads, a descriptive study was developed. This paper provides an overview of the cases that presented to this clinic over nine months, with a focus on feeding difficulties, birth and musculoskeletal problems in the infant. Mothers frequently reported more than one type of feeding problem, which most commonly included difficulty attaching, painful feeding, and a one sided feeding preference. Birth intervention was more common in this population than the national averages, particularly forceps deliveries. Musculoskeletal problems were prevalent in the babies presented to the feeding clinic, most commonly of the thoracic spine, cervical spine and sternocleidomastoid. Although causation cannot be established from this type of study, intervention at birth, feeding difficulties and musculoskeletal problems frequently coexisted in these infants. Therefore, including musculoskeletal care for infants as part of support for suboptimal breastfeeding may be appropriate

    A compact tunable single-mode fiber laser is developed by using a novel combination of high-gain erbium: ytterbium (Er:Yb) phosphate fiber and fiber Fabry-Perot (FFP) cavity configurations. Experiments demonstrate the shortest Er:Yb phosphate FFP laser ever reported, which has a 100µm cavity length with a continuous wavelength tuning range over 4.52nm, as limited by the sharp fiber gain peak. In addition, an alternative 3-mirror laser design has also demonstrated single-mode lasing operation

    ACCURACY ASSESSMENT OF A UAV-BASED LANDSLIDE MONITORING SYSTEM

    Landslides are hazardous events with often disastrous consequences. Monitoring landslides with observations of high spatio-temporal resolution can help mitigate such hazards. Mini unmanned aerial vehicles (UAVs) complemented by structure-from-motion (SfM) photogrammetry and modern per-pixel image matching algorithms can deliver a time-series of landslide elevation models in an automated and inexpensive way. This research investigates the potential of a mini UAV, equipped with a Panasonic Lumix DMC-LX5 compact camera, to provide surface deformations at acceptable levels of accuracy for landslide assessment. The study adopts a self-calibrating bundle adjustment-SfM pipeline using ground control points (GCPs). It evaluates misalignment biases and unresolved systematic errors that are transferred through the SfM process into the derived elevation models. To cross-validate the research outputs, results are compared to benchmark observations obtained by standard surveying techniques. The data is collected with 6 cm ground sample distance (GSD) and is shown to achieve planimetric and vertical accuracy of a few centimetres at independent check points (ICPs). The co-registration error of the generated elevation models is also examined in areas of stable terrain. Through this error assessment, the study estimates that the vertical sensitivity to real terrain change of the tested landslide is equal to 9 cm

    Expansion of Vortex Cores by Strong Electronic Correlation in La2x_{2-x}Srx_xCuO4_4 at Low Magnetic Induction

    The vortex core radius \rv, defined as the peak position of the supercurrent around the vortex, has been determined by muon spin rotation measurements in the mixed state of \lscox for x=0.13x=0.13, 0.15, and 0.19. At lower doping (x=0.13 and 0.15), \rv(T) increases with decreasing temperature T, which is opposite to the behavior predicted by the conventional theory. Moreover, \rv(T\to0) is significantly larger than the Ginsburg-Landau coherence length determined by the upper critical field, and shows a clear tendency to decrease with increasing the doping x. These features can be qualitatively reproduced in a microscopic model involving antiferromagnetic electronic correlations.Comment: 6 pages, 4 figures, to be published in Phys. Rev.

    Treatment Patterns and Health Resource Utilization Among Patients Diagnosed With Early Stage Resected Non–Small Cell Lung Cancer at US Community Oncology Practices

    AbstractBackgroundPlatin-based adjuvant chemotherapy has extended survival in clinical trials in patients with completely resected non–small cell lung cancer (NSCLC). There are few data on the use of adjuvant therapy in community-based clinical practice in the United States.Materials and MethodsThis was a retrospective observational study using electronic medical record and billing data collected during routine care at US community oncology sites in the Vector Oncology Data Warehouse between January 2007 and January 2014. Patients aged ≥ 18 years with a primary diagnosis of stage IB to IIIA NSCLC were eligible if they had undergone surgical resection. Treatment patterns, health care resource use, and cost were recorded, stratified by stage at diagnosis.ResultsThe study included 609 patients (mean age, 64.8 years, 52.9% male), of whom 215 had stage IB disease, 130 stage IIA/II, 110 stage IIB, and 154 stage IIIA. Adjuvant systemic therapy after resection was provided to 345 (56.7%) of 609 patients, with lower use in patients with stage IB disease (39.1%) than stage II to IIIA disease (64.9-68.2%) (P < .0001). The most common adjuvant regimen at all stages was the combination of carboplatin and paclitaxel. There were no statistically significant differences in office visits or incidence of hospitalization by disease stage. During adjuvant treatment, the total monthly median cost per patient was 17,389.75(interquartilerange,17,389.75 (interquartile range, 8,815.61 to $23,360.85).ConclusionAdjuvant systemic therapy was used in some patients with stage IB NSCLC and in the majority of patients with stage IIA to IIIA disease.     Effect of mutual coupling on torque production in switched reluctance motors

    In many cases, the normal operation of switched reluctance machines requires excitation of two or more phases simultaneously. When multiple phases are conducting simultaneously, the flux paths from each phase will overlap, which may lead to localized saturation. In such cases, the flux linkage must be considered a function not just of the current in the test winding but of all excited windings. The degree of mutual coupling between phases influences the per-phase magnetization curves and torque characteristics. In machines with even phase numbers, the degree of mutual coupling between phases varies due to discontinuities in the phase polarity arrangement. From nonlinear finite element simulations, it is possible to compare the i - loop diagrams under single-phase and multiphase excitations, and hence the torque produced. The mutual flux linkage from each phase can be calculated separately for each rotor position using the frozen permeability method, to further analyze the mutual coupling effects. For a given excitation current profile, the torque can be maximized by careful arrangement of the phase polarities

    With their relatively fast variability time-scales, Galactic X-ray binaries provide an excellent laboratory to explore the physics of accretion and related phenomena, most notably outflows, over different regimes.     Effect of oxygen supply on metabolism of immobilized and suspended Escherichia coli

    The effect of reduced oxygen supply on the production of a recombinant protein (plasmid-encoded β-galactosidase) was investigated in Escherichia coli. A novel modified bubble tank reactor was used to provide a direct comparison between immobilized and suspended cells in identical environments except for the immobilization matrix. Decreased oxygen supply led to increased β-galactosidase synthesis by both immobilized and suspended cells. Immobilized cells produced similar amounts of β-galactosidase as the suspended cells. Lactose consumption and acetate production, on a per cell basis, were significantly higher in immobilized cells, suggesting that immobilized cells utilized fermentative metabolism.     Nonlocal Effects and Shrinkage of the Vortex Core Radius in YNi2B2C Probed by muSR

    The magnetic field distribution in the vortex state of YNi2B2C has been probed by muon spin rotation (muSR). The analysis based on the London model with nonlocal corrections shows that the vortex lattice has changed from hexagonal to square with increasing magnetic field H. At low fields the vortex core radius, rho_v(H), decreases with increasing H much steeper than what is expected from the sqrt(H) behavior of the Sommerfeld constant gamma(H), strongly suggesting that the anomaly in gamma(H) primarily arises from the quasiparticle excitations outside the vortex cores.Comment: 4 pages, 4 figures, submitted to Phys.
