    SUBSTANTIATION OF PSYCHOTHERAPEUTIC CORRECTION OF CONSEQUENCES OF SEVERE OBSTETRIC COMPLICATIONS IN THE PATIENTS, WHO NEARLY DIED (“NEAR MISS”)

    Purpose: To substantiate the necessity of personality-centered psychocorrective program of rehabilitation for the patients, who underwent critical obstetric conditions that nearly ended in fatal outcome (“near miss”).Materials and methods: 85 women, who underwent critical obstetric conditions and had psychotraumatic experience of the first labor (PTEL) with different variants of reproductive losses, were examined. Determination of socio-psychological profile, monitoring of gestation course.Results. The course and outcome of pregnancy in the group of patients with the loss of fetus, but with the preservation of a reproductive function, showed high clinical efficiency of the performed psychocorrective program of rehabilitation together with obstetric monitoring based on the strategy of perinatal risk.Summary. The determination of socio-psychological profile of the patients, who underwent critical obstetric conditions, specifies the orientation of the personality-centered psychotherapeutic program, which reduces negative consequences of psychotraumatic experience of the first labor. Optimization of the relation of psychogestational dominant types in the group of patients, who preserved the reproductive function, forms favourable attitude to realize it and assists in successful termination of wanted pregnancy

    A new application of Internet of Things and Cloud Services in Analytical Chemistry: Determination of bicarbonate in water

    [EN] In a constantly evolving world, new technologies such as Internet of Things (IoT) and cloud-based services offer great opportunities in many fields. In this paper we propose a new approach to the development of smart sensors using IoT and cloud computing, which open new interesting possibilities in analytical chemistry. According to IoT philosophy, these new sensors are able to integrate the generated data on the existing IoT platforms, so that information may be used whenever needed. Furthermore, the utilization of these technologies permits one to obtain sensors with significantly enhanced features using the information available in the cloud. To validate our new approach, a bicarbonate IoT-based smart sensor has been developed. A classical CO2 ion selective electrode (ISE) utilizes the pH information retrieved from the cloud and then provides an indirect measurement of bicarbonate concentration, which is offered to the cloud. The experimental data obtained are compared to those yielded by three other classical ISEs, with satisfactory results being achieved in most instances. Additionally, this methodology leads to lower-consumption, low-cost bicarbonate sensors capable of being employed within an IoT application, for instance in the continuous monitoring of HCO3- in rivers.     Development and Evaluation of Three Chatbots for Postpartum Mood and Anxiety Disorders

    In collaboration with Postpartum Support International (PSI), a non-profit organization dedicated to supporting caregivers with postpartum mood and anxiety disorders, we developed three chatbots to provide context-specific empathetic support to postpartum caregivers, leveraging both rule-based and generative models. We present and evaluate the performance of our chatbots using both machine-based metrics and human-based questionnaires. Overall, our rule-based model achieves the best performance, with outputs that are close to ground truth reference and contain the highest levels of empathy. Human users prefer the rule-based chatbot over the generative chatbot for its context-specific and human-like replies. Our generative chatbot also produced empathetic responses and was described by human users as engaging. However, limitations in the training dataset often result in confusing or nonsensical responses. We conclude by discussing practical benefits of rule-based vs. generative models for supporting individuals with mental health challenges. In light of the recent surge of ChatGPT and BARD, we also discuss the possibilities and pitfalls of large language models for digital mental healthcare

    CLINICOMORPHOLOGICAL SUBSTANITIATION OF REHABILITATION THERAPY AFTER ECTOPIC PREGNANCY

    Purpose: To demonstrate the need of rehabilitation actions for patients, nearly died from aborted tubal pregnancy.Material and Methods: Comparative clinical analysis of two groups of patients (1st and 2nd) after tubectomy due to abortedtubal pregnancy and different blood loss. The blood loss in 1st group did not exceed 500 ml, and in 2nd group (nearly diedpatients - «near miss») - more than 1500 ml. The 3 group was contained of died patients.Results: It was identified a significant causal dependence of pathomorphological changes of anterior pituitary and adrenal glandfrom the blood loss volume, characterizing compensatory mechanism in acute adrenocortical insufficiency, attributable to anyshock, including posthemorrhagic. The data, obtained in the group of maternal deaths, allow to extrapolate the identified changesand suppose the development of these changes for patients, nearly died from major bleeding due to aborted tubal pregnancy.Summary:The presented data of autopsy of characteristic morphological changes of anterior pituitary and adrenal gland,depending on the blood loss volume, determine the direction of rehabilitation for «near of miss» patients

    Моніторинг епідеміологічних чинників в системі управління безпечністю та якістю продуктів забою великої рогатої худоби

    Food security is one of the priorities of the state. The livestock sector is an important source of food resources. The purpose of our research was to monitor the elements of the safety and quality of the management system for the slaughter of cattle for the effects of the epidemiological (veterinary) factors. The material of our research was the reporting on veterinary medicine in the Tulchynsky District of Vinnytsya Oblast between the years 2013–2017, which we statistically processes. The analysis revealed over the last five years, outbreaks of rabies, leukemia of the cattle and pasteurellosis have been recorded in the area, and the number of leukemia foci of bovine animals decreases year by year, and pasteurelosis is gaining more and more epizootic significance. It was based on the reports provided prophylactic antiepizootic measures are constantly being carried out in the region. Thus, during the years 2013–2017, clinical, laboratory and serological studies of the cattle population were conducted in the district the stock was vaccinated, treatment and preventive treatments and veterinary and sanitary works were carried out. Analysis of the data on the volume of these measures demonstrate in most of cases they are carried out only in the agricultural enterprises, not covering the number of individual farmsteads of the inhabitants of the district. In private farms, mostly, only compulsory measures are funded from state budget. Such as tuberculinisation, serological studies for leukemia and brucellosis, vaccination against anthrax. After analyzing the number of the slaughtered cattles in the area, we found that 93.42% of the slaughter took place in the meat processing enterprises. During the research period, according to findings results of pre-slaughter clinical examination of animals, 185 were diagnosted, and during the post-mortem veterinary examination 8282 cases of diseases were detected. The study of the etiology of the revealed diseases, found all diseases revealed as a result of clinical examination before the slaughter were non-contagious, and from diseases revealed by the results of veterinary-sanitary examination after slaughter 83.17% were non-contagious, and the rest (16.83%) – invasive. Regarding the structure of detected invasive diseases, echinococcosis and fasciolysis were recorded. The correlation between the number of cases of detection during the post-mortem veterinary examination of invasive illnesses and the treatmentes-prophylactic treatments established a direct pattern between them: the number of treatment and prevention treatments for livestock from year to year increases, cases of detection of invasive diseases decrease. In general, the monitoring of epidemiological factors in the Tulchinsky district of the Vinnytsia region indicates that the system for managing the safety and quality products of slaughter of cattle is effective enough. But the facts that antiepizootic veterinary measures do not cover the entire livestock population of the district and the proportion of slaughter of cattle occurs in the yard, causing concern, and significantly reduce the effectiveness of the management system. Therefore, in our opinion, it can not be argued that the safety and quality control system for cattle slaughter products in the area fully guarantees the safety of the consumer.Продовольча безпека є одним із пріоритетних завдань держави, а галузь скотарства є важливим джерелом формування продовольчих ресурсів. Метою наших досліджень був моніторинг елементів системи управління безпечністю та якістю продуктів забою великої рогатої худоби за впливом епідеміологічних (ветеринарних) чинників. Матеріалом наших досліджень була звітність з питань ветеринарної медицини Тульчинського району Вінницької області за 2013–2017 роки, яку ми обробляли статистично.Проведений аналіз встановив, що за останні п’ять років у районі було зареєстровано спалахи сказу, лейкозу ВРХ і пастерельозу, причому кількість вогнищ лейкозу великої рогатої худоби із року в рік зменшується, а пастерельоз набуває все більшого епізоотичного значення. Встановлено, що в районі постійно проводяться профілактичні протиепізоотичні заходи. Так, протягом 2013–2017 років в районі проводились клінічні, лабораторні та серологічні дослідження поголів’я великої рогатої худоби, поголів’я вакцинували, були проведені лікувально-профілактичні обробки та ветеринарно-санітарні роботи. Але аналіз даних щодо обсягів зазначених заходів свідчить, що в більшості вони здійснюються лише у сільськогосподарських підприємствах, не охоплюючи поголів’я індивідуальних присадибних господарств мешканців району. В цих господарствах здебільшого проводяться лише обов’язкові заходи, що фінансуються за бюджетні кошти, зокрема, туберкулінізація, серологічні дослідження на лейкоз та бруцельоз, профілактична вакцинація проти сибірки. Проаналізувавши об’єми забою великої рогатої худоби в районі ми встановили, що 93,42% забою відбувалось в умовах м’ясопереробних підприємств. За дослідний період на м’ясопереробних підприємствах за результатами передзабійного клінічного огляду тварин було виявлено 185, а під час післязабійної ветсанекспертизи – 8282 випадки хвороб. Вивчення етіології виявлених хвороб встановило, що всі хвороби виявленні передзабійно були незаразними, а з хвороб виявлених післязабійно 83,17% склали незаразні, а решта (16,83%) – інвазійні. Щодо структури виявлених інвазійних хвороб, то реєстрували ехінококоз і фасціольоз. Водночас за подвірного забою не було виявлено жодного випадку хвороб. Аналіз співвідношення між кількістю випадків виявлення під час післязабійної ветсанекспертизи інвазійних хвороб і проведених лікувально-профілактичних обробок встановив пряму закономірність між ними – кількість лікувально-профілактичних обробок поголів’я великої рогатої худоби із року в рік збільшується, випадки виявлення інвазійних хвороб зменшуються. Загалом моніторинг епідеміологічних чинників у Тульчинському районі Вінницької області вказує на те, що система управління безпечністю та якістю продуктів забою великої рогатої худоби діє достатньо ефективно. Але ті факти, що протиепізоотичні ветеринарні заходи не охоплюють усе поголів’я великої рогатої худоби району і проводиться подвірний забій великої кількості великої рогатої худоби, викликають занепокоєння і значно знижують ефективність системи управління. Тому, на нашу думку, стверджувати, що система управління безпечністю та якістю продуктів забою великої рогатої худоби в районі повністю гарантує безпеку споживача, не можна

    The paper treats contemporary rock music (especially genres of heavy metal and black metal) in the countries with Muslim majority. The ambivalence of the attitude to singing and music in Islam is reflected in the paper and the main theological positions on the issue are presented. Methodologically basing on K. Kan-Harris’s concept of heavy metal as a transgressive practice, rock music popularity with modern Islamic youth is seen as a reflection of Islamic regions social transgression. The paper briefly presents the history of rock music in Islamic countries, discusses the work of particular groups in its connection to religion, demonstrates that the widespread popularity of rock music in Muslim regions is tightly linked to Muslim youth current problems: through music they try to overcome the conservatism of traditional society, national and religious disunity, to draw attention to gender inequality, to achieve freedom of expression, to find a new identity in the changing world.Данная работа посвящена анализу современной рок-музыки (особенно, жанров heavy metal и black metal) стран с мусульманским большинством. В статье отражена амбивалентность отношения к пению и музыке в исламе, представлены основные богословские позиции по этому поводу. Основываясь на концепции heavy metal как трансгрессивной практики,предложенной К.Кан-Харрисом, увлечение современной исламской молодежи рок-музыкой как отражение социальной трансгрессии исламских регионов. В статье кратко история истории рок-музыки в исламских странах, рассмотренное творчество группы в его взаимосвязи с религией, выявлено, что широкое распространение рок-музыки обусловлено ее тесной связью с актуальными проблемами, волнующими современную мусульманскую молодежь. Так, через музыку осуществляется попытка преодолеть консерватизм традиционного общества, национальную и религиозную разобщенность, привлечь внимание к вопросам гендерного неравенства, добиться свободы самовыражения, обрести новую идентичность в условиях меняющегося мир

    Study of vaginal microbiota in postmenopausal women with surgical correction of genital prolapse

    The study of the vaginal microflora of women with genital prolapse (GP), who are ref erred to surgical treatment, is of particular importance. Infectious and inflammatory complications after the surgical correction of GP significantly increase the recovery time, the length of the patient’s hospital stay and the cost of treatment. It is important to be able to change the qualitative composition of microflora, thereby exerting a beneficial effect on the course of the postoperative period, affecting the outcome of surgical treatment.
