Psychological interventions for coronary heart disease
A meta-analysis was conducted on 35 trials involving 10,703 individuals who had experienced a myocardial infarction and were randomised to an intervention involving some form of psychological therapy. Ten of these studies involved individuals with confirmed psychiatric diagnoses. Moderate quality evidence found no reduction of risk for total mortality or revascularisation procedures in comparison to usual care. Low quality evidence found no risk reduction for non-fatal MI although there was a 21% reduction in cardiac mortality. There was also some evidence of benefit on measures of psychological morbidity including anxiety, depression, and stress. It is concluded that psychological interventions may reduce cardiac mortality, although stronger evidence is required before this can be definitively concluded. Angstsymptome und Angststörungen sind ein hÀufiges PhÀnomen bei Patienten mit
koronarer Herzerkrankung (KHK), das in der Praxis jedoch in der Regel nicht
erkannt und behandelt wird. Dabei weist die bisherige Studienlage nicht nur
auf eine BeeintrÀchtigung der LebensqualitÀt im Zusammenhang mit Angst,
sondern auch auf deren negative Effekte hinsichtlich der kardialen Prognose
der Patienten. Ziel dieser Arbeit war es, den Wissensstand hinsichtlich der
Behandlung und zugrundeliegender Mechanismen zu erweitern. Konkret sollte die
EffektivitÀt eines psychotherapeutischen Gruppenprogramms hinsichtlich der
Reduktion der Angstsymptomatik bei Patienten mit KHK und erhöhten Angstwerten
evaluiert werden (Studie 1 und 2). Ziel der Studie 3 war es, den Zusammenhang
zwischen Angstsymptomatik und Cortisol-AusschĂŒttung, als ein mögliches
Bindeglied zwischen Angst und kardialer Prognose, bei KHK-Patienten zu
untersuchen. Methode: Studie 1 wurde als Pilotstudie im Sinne einer
Machbarkeitsstudie durchgefĂŒhrt. Bei einer kleinen Stichprobe (N=58) von KHK-
Patienten wurden Angst und DepressivitÀt im Verlauf von 18 Monaten mit Hilfe
der Hospital Anxiety und Depression Scale (HADS) erhoben. AuĂerdem wurde das
Interesse, an einer psychotherapeutischen Intervention teilzunehmen, erfragt.
Die DurchfĂŒhrbarkeit und EffektivitĂ€t des Behandlungsprogramms wurde anhand
einer Substichprobe (N=9) erhoben. Die zweite Studie wurde als randomisiert-
kontrollierte Studie durchgefĂŒhrt. Es wurden N=62 Patienten mit KHK und
erhöhten Angstwerten in der HADS-Angstskala (â„8) rekrutiert. Die Teilnehmer
wurden randomisiert einer Interventionsgruppe oder einer Kontrollgruppe
zugewiesen. Die Interventionsgruppe nahm an einem 6monatigen
Behandlungsprogramm teil, die Kontrollgruppe erhielt keinerlei Intervention.
Im Rahmen der dritten Studie wurden bei einer Substichprobe von Studie 2
(N=47) anhand von 4 Speichelproben morgendliche Cortisolwerte zu den
Zeitpunkte 0, +30, +45 und +60 Minuten nach dem Aufwachen erhoben. Die Werte
von Patienten mit hohen (â„11) und moderat erhöhten (8-10) Werten in der HADS-
Angst-Skala wurden verglichen. Ergebnisse: In Studie 1 konnte gezeigt werden,
dass ein hoher Prozentsatz der befragten KHK-Patienten an der Teilnahme an
einer psychotherapeutischen Behandlung interessiert waren. Die psychische
BeeintrÀchtigung der Patienten wies ohne Behandlung eine hohe StabilitÀt auf.
Ăngstlichkeit und DepressivitĂ€t konnten mit Hilfe des Behandlungsprogramms
signifikant reduziert werden. Die Ergebnisse von Studie 2 zeigen, dass sich
die Angstwerte der Teilnehmer beider Gruppen signifikant im Verlauf von 6
Monaten verringerten. Es konnten keine signifikanten Effekte der Intervention
nachgewiesen werden. In Studie 3 konnten signifikante Unterschiede in der
morgendlichen Cortisol-AusschĂŒttung zwischen Patienten mit hohen und moderat
erhöhten Angstwerten festgestellt werden. Diskussion und Schlussfolgerung: Die
Ergebnisse bestÀtigen, dass es grundsÀtzlich einen Behandlungsbedarf und
-bereitschaft hinsichtlich psychotherapeutischer Behandlungen bei Patienten
mit KHK gibt. Das untersuchte Programm erwies sich in der randomisiert-
kontrollierten Studie jedoch als nicht effektiv. Neben möglicherweise
erforderlichen Modifikationen der Inhalte oder des Formats des Programms lÀsst
sich aus diesem Ergebnis die Empfehlung fĂŒr zukĂŒnftige Studien ableiten,
Programme spezifischer zu gestalten und differentielle Behandlungseffekte bei
verschiedenen Subgruppen von Patienten zu analysieren. Der Befund einer
signifikant erhöhten morgendlichen Cortisol-AusschĂŒttung und der Tendenz eines
stÀrkeren Anstiegs des Cortisols bei KHK-Patienten mit hohen Angstwerten weist
schlieĂlich auf eine mögliche Beteiligung einer HPA-Achsen-Dysregulation an
dem Zusammenhang zwischen Angst und KHK.Symptoms of anxiety are frequent among patients with CHD and increased
prevalence rates of anxiety disorders were found in groups of cardiologic
patients. Elevated levels of anxiety are associated with an impaired health-
related quality of life. Furthermore, anxiety seems to be of prognostic value
for the development and the course of CHD. The main purpose of this study was
to investigate whether a psychotherapy intervention would reduce the level of
anxiety in anxious patients with a medically stable CHD (study 1 and study 2).
The additional aim of study 3 was to assess the association between anxiety
and cortisol as a potential link between anxiety and a worse cardiac
prognosis. Methods: In study 1 we assessed anxiety- and depression-scores in
the Hospital Anxiety and Depression Scale (HADS) over the course of 18 month
in a small sample (N=58) of CHD-patients. Additionally we investigated the
interest of coronary patients to take part in a psychotherapy intervention.
The feasibility and effects of the intervention were tested in a subsample of
N=9 patients. The second study was a randomized-controlled study. N=62
patients with CHD and elevated anxiety-scores (HADS-A â„8) were included.
Participants were randomized into an intervention- or a control-condition.
Patients in the intervention-group took part in a 6 month psychotherapy
intervention. The control-group did not receive any intervention. In the third
study 4 samples of salivary cortisol were obtained from a subsample (N=47) of
study 2. Cortisol was collected at 0, +30, +45 und +60 minutes after
awakening. Cortisol output of patients with high (â„11) and moderately elevated
(8-10) anxiety-scores in the HADS were compared. Results: In study 1 we found
a high percentage of coronary patients to be interested in a ppsychotherapy
intervention. Without psychotherapeutic treatment psychological distress
remained stable over the time of investigation. Intervention-group patients
achieved a significant reduction of depression and anxiety scores. The results
of study 2 show significant reductions in anxiety scores between baseline and
follow up for both groups. There were no significant effects of the
intervention on anxiety. In study 3 we found significantly higher cortisol
outputs in the first hour after awakening in patients with high anxiety scores
compared to patients with only moderately elevated scores. Discussion and
conclusion: The results of our studies show a general need for psychotherapy
interventions in patients with CHD. However, our intervention was not effectiv
in reducing the patientsâ anxiety scores in the randomized-controlled study.
Future trials should try to analyze effects for subgroups of patients and to
differentiate between subtypes of anxiety in order to identify which patients
are responsive to different kinds of treatments. Finally, our finding of
significantly higher cortisol outputs in coronary patients with high anxiety
scores provides further indications for a dysregulation of the HPA axis as a
potential link between anxiety and cardiac prognosis
