Strategy to avoid vascular injuries in revision total hip arthroplasty with intrapelvic implants
Aims: Our objective was describing an algorithm to identify and prevent vascular injury in patients with intrapelvic components. Methods: Patients were defined as at risk to vascular injuries when components or cement migrated 5 mm or more beyond the ilioischial line in any of the pelvic incidences (anteroposterior and Judet view). In those patients, a serial investigation was initiated by a CT angiography, followed by a vascular surgeon evaluation. The investigation proceeded if necessary. The main goal was to assure a safe tissue plane between the hardware and the vessels. Results: In ten at-risk patients undergoing revision hip arthroplasty and submitted to our algorithm, six were recognized as being high risk to vascular injury during surgery. In those six high-risk patients, a preventive preoperative stent was implanted before the orthopaedic procedure. Four patients needed a second reinforcing stent to protect and to maintain the vessel anatomy deformed by the intrapelvic implants. Conclusion: The evaluation algorithm was useful to avoid blood vessels injury during revision total hip arthroplasty in high-risk patients
Prevalence of obesity in asthma and its relations with asthma severity and control
OBJECTIVE: To determine the prevalence of obesity in asthmatic patients attending at an outpatient clinic, and to investigate its relationships with asthma severity and level of asthma control. METHODS: In a cross-sectional study we recruited patients aged 11 years and older with confirmed asthma diagnosis from the outpatient asthma clinic of Hospital de Clínicas de Porto Alegre, Brazil. They underwent an evaluation by a general questionnaire, an asthma control questionnaire and by pulmonary function tests. Nutritional status was classified by body mass index (BMI). RESULTS: 272 patients were included in the study. Mean age was 51.1 ± 16.5 years and there were 206 (74.9%) female patients. Mean BMI was 27.5 ± 5.3 kg/m², and 96 (35.3%) patients were classified as normal weight, 97 (35.7%) as overweight and 79 (29%) as obesity. There was a significant higher proportion of female than male patients (34.3% vs. 13.2%, p = 0.002) in the obesity group. There were no significant differences with respect to asthma control (p = 0.741) and severity classification (p = 0.506). The FEV1% predicted was significantly higher in the obese than in the non-obese group (73.7% vs. 67.2%, p = 0.037). Logistic regression analysis identified sex (OR = 3.84, p = 0.002) as an independent factor associated with obesity. CONCLUSIONS: This study showed a high prevalence of obesity in asthmatic patients. Obese and non-obese subjects were similar in regard to asthma severity and level of asthma control. Female sex was associated with obesity in this asthma population
Analysis of postoperative monitoring of patients undergoing shoulder arthroscopy for anterior instability
ABSTRACT OBJECTIVE: Analyze the postoperative follow-up of patients undergoing shoulder arthroscopy for treatment of anterior instability and correlate with the prevalence of recurrence. METHODS: A six-question survey was applied by phone and mail to 65 patients, seeking information on the current result of the surgical procedure. All patients were treated arthroscopically for anterior shoulder instability, with at least 12 months of postoperative time. Patients with associated posterior labial lesions and revision surgeries were not included. RESULTS: At the time of the survey the patients had a median of 56 (IQR: 34.5-110.5) postoperative months. The mean sample age was 24.6 years (maximum = 47, minimum = 12; SD = 7.3). Complaint of pain in the shoulder was observed in 20 patients (30.7%). Dislocation recurrence was observed in 10 patients (15.3%). Forty-four patients (67.6%) considered their shoulder normal, which was more frequent in non-recurrence patients (p< 0.001). Forty-three patients (66.1%) returned to their previous level of sport and there was no difference between recurrence and non-recurrence patients (p= 0.456). It was found that the prevalence of recurrence was 5.6 (95% CI: 1.30-24.46) times higher in individuals who abandoned monitoring before six months postoperatively (p= 0.012). CONCLUSION: The abandonment of postoperative monitoring in the early stages, when the patients receive orientation for muscle strengthening, proprioceptive education, and dangerous movements to avoid, can increase the rates of recurrent shoulder dislocation in patients treated for anterior instability by arthroscopy

Métodos: Estudo transversal, envolvendo pacientes, com idade igual ou superior a 11 anos e diagnóstico de asma confirmado, do ambulatório clínico do Hospital de Clínicas de Porto Alegre, Brasil. Os pacientes foram submetidos à avaliac¸ão através de um questionário geral, questionário de controle da asma e teste de func¸ão pulmonar. O estado nutricional foi classificado conforme o índice de massa corporal (IMC). Resultados: Foram incluídos no estudo 272 pacientes, sendo 206 (74,9%) pacientes do sexo feminino. A média de idade foi 51.1 ± 16.5 anos. O IMC médio foi 27.5 ± 5.3 kg/m2, sendo 96 (35.3%) pacientes classificados como eutróficos, 97 (35,7%) como sobrepeso e 79 (29%) como obesidade. Observou-se significativamente maior proporc¸ão de pacientes do sexo feminino no grupo de obesidade quando comparados aos pacientes do sexo masculino (34.3% vs. 13.2%, p = 0.002). Não houve diferenc¸a significativaemrelac¸ão ao controle da asma (p = 0.741) e classificac¸ão de gravidade (p = 0.506). O FEV1% predito foi significativamente maior nos pacientes obesos quando comparados aos não obesos (73.7% vs. 67.2%, p = 0.037). A análise de regressão logística identificou gênero como fator independente associado com a obesidade. (OR = 3.84, p = 0.002). Conclusão: O presente estudo observou alta prevalência de obesidade em pacientes asmáticos. Indivíduos obesos e não obesos tiveramsimilaridade para gravidade e controle da asma. O gênero feminino foi associado com obesidade nesta populac¸ão asmática
