Using Case's method for solving the oneâspeed transport equation with isotropic scattering, the Milne problem solution, the solution for a constant source in one halfâspace, and the Green's function solution are obtained for two adjacent halfâspaces. These problems have been solved previously by other methods. Opportunities for system level improvement in antibiotic use across the surgical pathway
Optimizing antibiotic prescribing across the surgical pathway (before, during, and after surgery) is a key aspect of tackling important drivers of antimicrobial resistance and simultaneously decreasing the burden of infection at the global level. In the UK alone, 10 million patients undergo surgery every year, which is equivalent to 60% of the annual hospital admissions having a surgical intervention. The overwhelming majority of surgical procedures require effectively limited delivery of antibiotic prophylaxis to prevent infections. Evidence from around the world indicates that antibiotics for surgical prophylaxis are administered ineffectively, or are extended for an inappropriate duration of time postoperatively. Ineffective antibiotic prophylaxis can contribute to the development of surgical site infections (SSIs), which represent a significant global burden of disease. The World Health Organization estimates SSI rates of up to 50% in postoperative surgical patients (depending on the type of surgery), with a particular problem in low- and middle-income countries, where SSIs are the most frequently reported healthcare-associated infections. Across European hospitals, SSIs alone comprise 19.6% of all healthcare-acquired infections. Much of the scientific research in infection management in surgery is related to infection prevention and control in the operating room, surgical prophylaxis, and the management of SSIs, with many studies focusing on infection within the 30-day postoperative period. However it is important to note that SSIs represent only one of the many types of infection that can occur postoperatively. This article provides an overview of the surgical pathway and considers infection management and antibiotic prescribing at each step of the pathway. Stability, Structure and Scale: Improvements in Multi-modal Vessel Extraction for SEEG Trajectory Planning
Purpose Brain vessels are among the most critical landmarks that need to be assessed for mitigating surgical risks in stereo-electroencephalography (SEEG) implantation. Intracranial haemorrhage is the most common complication associated with implantation, carrying signi cant associated morbidity. SEEG planning is done pre-operatively to identify avascular trajectories for the electrodes. In current practice, neurosurgeons have no assistance in the planning of electrode trajectories. There is great interest in developing computer assisted planning systems that can optimise the safety pro le of electrode trajectories, maximising the distance to critical structures. This paper presents a method that integrates the concepts of scale, neighbourhood structure and feature stability with the aim of improving robustness and accuracy of vessel extraction within a SEEG planning system. Methods The developed method accounts for scale and vicinity of a voxel by formulating the problem within a multi-scale tensor voting framework. Feature stability is achieved through a similarity measure that evaluates the multi-modal consistency in vesselness responses. The proposed measurement allows the combination of multiple images modalities into a single image that is used within the planning system to visualise critical vessels. Results Twelve paired datasets from two image modalities available within the planning system were used for evaluation. The mean Dice similarity coe cient was 0.89 ± 0.04, representing a statistically signi cantly improvement when compared to a semi-automated single human rater, single-modality segmentation protocol used in clinical practice (0.80 ±0.03). Conclusions Multi-modal vessel extraction is superior to semi-automated single-modality segmentation, indicating the possibility of safer SEEG planning, with reduced patient morbidity
Convex recovery of a structured signal from independent random linear measurements
This chapter develops a theoretical analysis of the convex programming method
for recovering a structured signal from independent random linear measurements.
This technique delivers bounds for the sampling complexity that are similar
with recent results for standard Gaussian measurements, but the argument
applies to a much wider class of measurement ensembles. To demonstrate the
power of this approach, the paper presents a short analysis of phase retrieval
by trace-norm minimization. The key technical tool is a framework, due to

The Clostridium difficile problem: A South African tertiary institution's prospective perspective
BACKGROUND AND OBJECTIVES: The aim of this study is to report the incidence of Clostridium difficile-associated disease (CDAD) in a tertiary-care hospital in South Africa and to identify risk factors, assess patient outcomes and determine the impact of the hypervirulent strain of the organism referred to as North American pulsed-field type 1 (NAP1). METHODS: Adults who presented with diarrhoea over a period of 15 months were prospectively evaluated for CDAD using stool toxin enzyme immunoassay (EIA). Positive specimens were evaluated by PCR. Patient demographics, laboratory parameters and outcomes were analysed. RESULTS: CDAD was diagnosed in 59 (9.2%) of 643 patients (median age 39 years, IQR 30 - 55). Thirty-four (58%) were female. Recent antibiotic exposure was reported in 39 (66%), 27 (46%) had been hospitalised within 3 months, and 14 (24%) had concomitant inflammatory bowel disease (IBD). Nineteen (32%) had community-acquired CDAD (CA-CDAD). The annual incidence of hospital-acquired CDAD (HA-CDAD) was 8.7 cases/10 000 hospitalisations. Two cases of the hypervirulent strain NAP1 were identified. Seven (12%) patients underwent colectomy (OR 6.83; 95% CI 2.41 - 19.3). On logistic regression, only antibiotic exposure independently predicted for CDAD (OR 2.9; 95% CI 1.6 - 5.1). Three (16%) cases of CA-CDAD reported antibiotic exposure (v. 90% of HA-CDAD, p<0.0001). Twelve (86%) patients had concomitant IBD (p<0.0001 v. HA-CDAD). CA-CDAD was significantly associated with antibiotic exposure (OR 0.04, 95% CI 0.01 - 0.24) and IBD (OR 9.6, 95% CI 1.15 - 79.8). CONCLUSION: The incidence of HA-CDAD in the South African setting is far lower than that reported in the West. While antibiotic use was a major risk factor for HA-CDAD, CA-CDAD was not associated with antibiotic therapy. Concurrent IBD was a predictor of CA-CDAD
Passing the baton to pharmacists and nurses: New models of antibiotic stewardship for South Africa?
The optimisation of antibiotic use to maximise patient care and safety through antibiotic stewardship forms one of the cornerstones of the global response to antibiotic resistance. Stewardship efforts in low- and middle-income countries are challenged by lack of healthcare professionals trained in infection. Therefore, in resource-poor settings, the traditional model of infection specialist-led stewardship may be impractical, requiring new models to be developed. A recent South African study across 47 Netcare hospitals nationally highlighted the role of pharmacists in this regard, proving that non-specialised pharmacists can drive a prospective audit, feedback collaborative strategy and a range of improvement science principles to reduce antibiotic consumption by the same levels as that documented in high-resource, infection specialist-led stewardship programmes. This editorial discusses the hidden opportunities to engage non-infection specialists in programmes to combat antibiotic resistance, expanding the cadres of healthcare professionals to lead stewardship programmes
The Viscous Nonlinear Dynamics of Twist and Writhe
Exploiting the "natural" frame of space curves, we formulate an intrinsic
dynamics of twisted elastic filaments in viscous fluids. A pair of coupled
nonlinear equations describing the temporal evolution of the filament's complex
curvature and twist density embodies the dynamic interplay of twist and writhe.
These are used to illustrate a novel nonlinear phenomenon: ``geometric
untwisting" of open filaments, whereby twisting strains relax through a
transient writhing instability without performing axial rotation. This may
explain certain experimentally observed motions of fibers of the bacterium B.

Primary composition and pigments of 11 red seaweed species from the Center of Portugal

Freitas (UI/BD/150957/2021).Macroalgae have been regarded as a natural food source since ancient times, their nutritional value being not only proven by recent studies, but also triggering further in-depth research efforts on the matter. The present study aims to provide an insight into the nutritional potential of selected red seaweed species collected in central Portugal by specifically comparing the moist yield and ash content, crude protein, total lipids, carbohydrates and pigment content between species and, ultimately, finding out if there are differences between taxa. The results obtained highlighted the most nutritionally appealing species, namely, Plocamium cartilagineum with respect to protein content (23.18% dw) and Sphaerocococcus coronopifolius with respect to carbohydrate content (40.23% dw), while none of the species studied showed a lipid content higher than 1.80% dw. Regarding pigment content, the highest concentrations of phycoerythrin, carotenoid and chlorophyll a were obtained, respectively, from P. cartilagineum (0.09 mg.mLâ1), Porphyra umbilicalis (1.88 ”g.gâ1 fw) and Jania rubens (38.41 ”g.mLâ1). We concluded that there are significant differences between the species studied regarding their nutritional profile, with a marked difference between Corallinales and all other species not belonging to this order; regarding pigment content, this variation between orders was not observed. Unstable Attractors: Existence and Robustness in Networks of Oscillators With Delayed Pulse Coupling
We consider unstable attractors; Milnor attractors such that, for some
neighbourhood of , almost all initial conditions leave . Previous
research strongly suggests that unstable attractors exist and even occur
robustly (i.e. for open sets of parameter values) in a system modelling
biological phenomena, namely in globally coupled oscillators with delayed pulse
interactions.
In the first part of this paper we give a rigorous definition of unstable
attractors for general dynamical systems. We classify unstable attractors into
two types, depending on whether or not there is a neighbourhood of the
attractor that intersects the basin in a set of positive measure. We give
examples of both types of unstable attractor; these examples have
non-invertible dynamics that collapse certain open sets onto stable manifolds
of saddle orbits.
In the second part we give the first rigorous demonstration of existence and
robust occurrence of unstable attractors in a network of oscillators with
delayed pulse coupling. Although such systems are technically hybrid systems of
delay differential equations with discontinuous `firing' events, we show that
their dynamics reduces to a finite dimensional hybrid system system after a
finite time and hence we can discuss Milnor attractors for this reduced finite
dimensional system. We prove that for an open set of phase resetting functions
