    3 years of liraglutide versus placebo for type 2 diabetes risk reduction and weight management in individuals with prediabetes: a randomised, double-blind trial

    Background: Liraglutide 3·0 mg was shown to reduce bodyweight and improve glucose metabolism after the 56-week period of this trial, one of four trials in the SCALE programme. In the 3-year assessment of the SCALE Obesity and Prediabetes trial we aimed to evaluate the proportion of individuals with prediabetes who were diagnosed with type 2 diabetes. Methods: In this randomised, double-blind, placebo-controlled trial, adults with prediabetes and a body-mass index of at least 30 kg/m2, or at least 27 kg/m2 with comorbidities, were randomised 2:1, using a telephone or web-based system, to once-daily subcutaneous liraglutide 3·0 mg or matched placebo, as an adjunct to a reduced-calorie diet and increased physical activity. Time to diabetes onset by 160 weeks was the primary outcome, evaluated in all randomised treated individuals with at least one post-baseline assessment. The trial was conducted at 191 clinical research sites in 27 countries and is registered with ClinicalTrials.gov, number NCT01272219. Findings: The study ran between June 1, 2011, and March 2, 2015. We randomly assigned 2254 patients to receive liraglutide (n=1505) or placebo (n=749). 1128 (50%) participants completed the study up to week 160, after withdrawal of 714 (47%) participants in the liraglutide group and 412 (55%) participants in the placebo group. By week 160, 26 (2%) of 1472 individuals in the liraglutide group versus 46 (6%) of 738 in the placebo group were diagnosed with diabetes while on treatment. The mean time from randomisation to diagnosis was 99 (SD 47) weeks for the 26 individuals in the liraglutide group versus 87 (47) weeks for the 46 individuals in the placebo group. Taking the different diagnosis frequencies between the treatment groups into account, the time to onset of diabetes over 160 weeks among all randomised individuals was 2·7 times longer with liraglutide than with placebo (95% CI 1·9 to 3·9, p<0·0001), corresponding with a hazard ratio of 0·21 (95% CI 0·13–0·34). Liraglutide induced greater weight loss than placebo at week 160 (–6·1 [SD 7·3] vs −1·9% [6·3]; estimated treatment difference −4·3%, 95% CI −4·9 to −3·7, p<0·0001). Serious adverse events were reported by 227 (15%) of 1501 randomised treated individuals in the liraglutide group versus 96 (13%) of 747 individuals in the placebo group. Interpretation: In this trial, we provide results for 3 years of treatment, with the limitation that withdrawn individuals were not followed up after discontinuation. Liraglutide 3·0 mg might provide health benefits in terms of reduced risk of diabetes in individuals with obesity and prediabetes. Funding: Novo Nordisk, Denmark

    Multi-messenger observations of a binary neutron star merger

    On 2017 August 17 a binary neutron star coalescence candidate (later designated GW170817) with merger time 12:41:04 UTC was observed through gravitational waves by the Advanced LIGO and Advanced Virgo detectors. The Fermi Gamma-ray Burst Monitor independently detected a gamma-ray burst (GRB 170817A) with a time delay of ~1.7 s with respect to the merger time. From the gravitational-wave signal, the source was initially localized to a sky region of 31 deg2 at a luminosity distance of 40+8-8 Mpc and with component masses consistent with neutron stars. The component masses were later measured to be in the range 0.86 to 2.26 Mo. An extensive observing campaign was launched across the electromagnetic spectrum leading to the discovery of a bright optical transient (SSS17a, now with the IAU identification of AT 2017gfo) in NGC 4993 (at ~40 Mpc) less than 11 hours after the merger by the One- Meter, Two Hemisphere (1M2H) team using the 1 m Swope Telescope. The optical transient was independently detected by multiple teams within an hour. Subsequent observations targeted the object and its environment. Early ultraviolet observations revealed a blue transient that faded within 48 hours. Optical and infrared observations showed a redward evolution over ~10 days. Following early non-detections, X-ray and radio emission were discovered at the transient’s position ~9 and ~16 days, respectively, after the merger. Both the X-ray and radio emission likely arise from a physical process that is distinct from the one that generates the UV/optical/near-infrared emission. No ultra-high-energy gamma-rays and no neutrino candidates consistent with the source were found in follow-up searches. These observations support the hypothesis that GW170817 was produced by the merger of two neutron stars in NGC4993 followed by a short gamma-ray burst (GRB 170817A) and a kilonova/macronova powered by the radioactive decay of r-process nuclei synthesized in the ejecta

    Os M. pretenderam traçar um quadro da História da Alimentação, não como um novo ramo epistemológico da disciplina, mas como um campo em desenvolvimento de práticas e atividades especializadas, incluindo pesquisa, formação, publicações, associações, encontros acadêmicos, etc. Um breve relato das condições em que tal campo se assentou faz-se preceder de um panorama dos estudos de alimentação e temas correia tos, em geral, segundo cinco abardagens Ia biológica, a econômica, a social, a cultural e a filosófica!, assim como da identificação das contribuições mais relevantes da Antropologia, Arqueologia, Sociologia e Geografia. A fim de comentar a multiforme e volumosa bibliografia histórica, foi ela organizada segundo critérios morfológicos. A seguir, alguns tópicos importantes mereceram tratamento à parte: a fome, o alimento e o domínio religioso, as descobertas européias e a difusão mundial de alimentos, gosto e gastronomia.     Índice híbrido de eficácia e eficiência para lojas de varejo Effectiveness and efficiency hibrid index for retail stores

    As análises de desempenho atualmente utilizadas no varejo se atêm à avaliação dos diversos índices de desempenho de forma isolada. A metodologia de Análise Envoltória de Dados - DEA oferece a possibilidade de geração de um índice de eficiência único e mais completo, tendo como base a razão ponderada entre os produtos gerados e os insumos necessários para tal. Diferentemente de outras técnicas estatísticas, que utilizam medidas de tendência central para avaliação e comparação de unidades produtivas entre si, a metodologia DEA as compara às melhores, proporcionando assim oportunidades de benchmark, mostrando-se com isso, mais rica no apoio à decisão. Considerar no modelo a opinião dos especialistas, principalmente no que diz respeito à eficácia, se torna um ponto imperativo na atualidade, para conferir credibilidade aos resultados e fazer isso de forma mais simples é um diferencial. A criação e a simulação de unidades produtivas artificiais, ou não observadas, mostram-se como uma alternativa viável e vantajosa para incorporação do conceito de eficácia ao modelo DEA, frente aos problemas com que se pode defrontar ao optar por restrições aos pesos. Esse artigo apresenta a aplicação de todos os pontos até então comentados a um caso real do varejo, propondo um índice único de avaliação de lojas com base na metodologia DEA, incorporando por meio de unidades produtivas artificiais a opinião dos especialistas referentes à eficácia.<br>Nowadays, various methodologies are utilized to analyze and measure performance in retail industries. Data Envelopment Analysis (DEA) offers analysts and retailers a unique, thorough, and more meaningful way of measuring performance. The DEA based index uses the ratio between the products generated and resources used. Unlike other statistical techniques that utilize measures of central tendency for evaluation and comparison of productive stores among themselves, DEA methodology compares these stores to the best ones in the industry. This allows for the opportunity for benchmarking and makes DEA a more meaningful and helpful tool for decision support. The opinions and judgements of an expert in the analysis, concerning mainly effectiveness, have become more and more an undoubted necessity in gaining credibility. The simplicity and effectiveness of the DEA methodology helps to achieve this and turns it into a unique alternative for completing analysis. This paper presents the application of the all techniques mentioned above to obtain a single evaluation index based on the DEA method for a real case in the retail industry incorporating the expert opinions on effectiveness through artificial production units
