Metachronous bladder metastases from renal cell carcinoma: a case report and review of the literature
INTRODUCTION: adrenal gland, parotid gland, pharynx, eye and bladder are rare localizations of metastases of renal cell carcinoma (RCC). We report a case of metachronous RCC metastases to the bladder in a patient with a medical history of transitional cell carcinoma (TCC) of the bladder. MATERIALS AND METHODS: a case study and review of the relevant literature are presented. RESULTS: during a follow-up cystoscopy examination following treatment of TCC, a single 5-mm lesion was detected and endoscopically resected. The histology of the resected sample was confirmed to be RCC, comparable to a primary kidney cancer and not recurrent TCC. CONCLUSION: the patient had a probability of metastases three years after nephrectomy of 62.9%. Survival rates following single metastasectomy are 60% and 38% at three and five years, respectively; metachronous diagnosis has a better prognosis than synchronous. During RCC follow-up, each lesion should be considered as a possible metastasis of RCC
pT3 N0 Laryngeal Squamous Cell Carcinoma: Oncologic Outcomes and Prognostic Factors of Surgically Treated Patients
Objectives/Hypothesis: To assess the disease control, survival rates, and prognostic factors of exclusive surgical treatment for patients with pT3 N0 laryngeal squamous cell carcinoma (LSCC). Study Design: Multicentric retrospective cohort study. Methods: Multicentric retrospective case series of previously untreated patients with pT3 R0N0 LSCC, who received exclusive surgery between 2011 and 2019. Tumor location; subsite involvement; grading; and lymphatic, vascular, and perineural invasion were reported. Overall survival (OS), disease-specific survival (DSS), and disease-free survival (DFS) were measured. Results: Fifty-four patients (mean age 67.1; male sex 83.3%; mean follow-up period 37 months) underwent total laryngectomy (48.1%) or partial laryngectomy (51.9%). Ipsilateral or bilateral neck dissection was performed in 46 (85.2%) cases. Perineural invasion was more frequent in case of supraglottic involvement than glottic involvement (85.7% vs. 14.3%, P =.03). Five (9.3%) patients experienced recurrence (3 local recurrences, 1 nodal recurrence, 1 distant recurrence). Rate of recurrence differed between glottic (0%), supraglottic (80%), and transglottic (20%) tumors (P =.01), with a lower risk yielded by glottic involvement (odds ratio [OR], 0.05, 95% confidence interval [95% CI], 0.01–0.56, P =.01). A higher risk was recorded in case of perineural invasion (OR, 66.0, 95% CI, 1.41–3085.3, P =.03). The OS, DSS, and DFS were 79.6%, 96.3%, and 90.7%, without differences regarding the type of surgery. The DFS was lower in case of supraglottic involvement when compared to purely glottic LSCC (83.9% vs. 100%, P = 0.02). Conclusions: Exclusive surgery is a safe option for patients with pT3 R0N0 LSCC. Adjuvant treatments or closer follow-up monitoring might be considered in case of supraglottic involvement or perineural invasion. Level of Evidence: 4 Laryngoscope, 131:2262–2268, 2021
Supramolecular Assemblies in Silver Complexes: Phase Transitions and the Role of the Halogen Bond
Weak interactions (hydrogen bonds, halogen bonds, CH···πand Ï€-πstacking) can play a significant role in the formation of supramolecular assemblies with desired structural features. In this contribution, we report a systematic investigation on how a halogen bond (XB) can modulate the structural arrangement of silver supramolecular complexes. The complexes are composed of X-phenyl(bispyrazolyl)methane (X = Br, I) and I-alkynophenyl(bispyrazolyl)methane ligands functionalized in meta (L3Br, L3I) and para (L4Br, L4I, L4CCI) positions on a phenyl ring with the purpose of providing different directionalities of the X function with respect to the N,N coordination system. The obtained [Ag(L)2]+ moieties show remarkable geometric similarities, and the L4Br, L4I, and L4CCI ligands exhibit the most conserved types of supramolecular arrangement that are sustained by XB. The increased σ-hole in L4CCI with respect to L4I leads to an occurrence of short (and strong) XB interactions with the anions. [Ag(L4I)2]PF6 and [Ag(L4I)2]CF3SO3 are characterized by the presence of three different phases, and the single-crystal evolution from phase-1 (a honeycomb structure with large 1D cavities) to phase-3 (solventless) occurs by a stepwise decrease in the crystallization solvent content, which promotes an increase in XB interactions in the lattice. Synthetic wastewaters treatment by electrocoagulation to remove silver nanoparticles produced by different routes
International audienceNanoscience is a field that has stood out in recent years. The accurate long-term health and environmental risks associated with these emerging materials are unknown. Therefore, this work investigated how to eliminate silver nanoparticles (AgNPs) from synthetic effluents by electrocoagulation (EC) due to the widespread use of this type of nanoparticle (NP) in industry and its potential inhibition power over microorganisms responsible for biological treatment in effluent treatment plants. Synthesized AgNPs were studied via four different routes by chemical reduction in aqueous solutions to simulate the chemical variations of a hypothetical industrial effluent, and efficiency conditions of the EC treatment were determined. All routes used silver nitrate as the source of silver ions, and two synthesis routes were studied with sodium citrate as a stabilizer. In route I, sodium citrate functioned simultaneously as the reducing agent and stabilizing agent, whereas route II used sodium borohydride as a reducing agent. Route III used d-glucose as the reducing agent and sodium pyrophosphate as the stabilizer; route IV used sodium pyrophosphate as the stabilizing agent and sodium borohydride as the reducing agent. The efficiency of the EC process of the different synthesized solutions was studied. For route I, after 85 min of treatment, a significant decrease in the plasmon resonance peak of the sample was observed, which reflects the efficiency in the mass reduction of AgNPs in the solution by 98.6%. In route II, after 12 min of EC, the absorbance results reached the detection limit of the measurement instrument, which indicates a minimum reduction of 99.9% of AgNPs in the solution. During the 4 min of treatment in route III, the absorbance intensities again reached the detection limit, which indicates a minimum reduction of 99.8%. In route IV, after 10 min of treatment, a minimum AgNP reduction of 99.9% was observed. Based on these results, it was possible to verify that the solutions containing citrate considerably increased the necessary times required to eliminate AgNPs from the synthesized effluent, whereas solutions free of this reagent showed better results on floc formation and, therefore, are best for the treatment. The elimination of AgNPs from effluents by EC proved effective for the studied routes
Impressive long-term response with chemo-endocrine therapy in a premenopausal patient with metastatic breast cancer: A case report
RATIONALE: Patients with, or who develop, metastatic breast cancer have a 5-year relative survival of about 25%. Endocrine therapy clearly improves outcomes in patients with estrogen receptor-positive breast cancer. In the metastatic setting, the primary goal of treatment is to maintain long-term disease control with good quality of life. Rarely, exceptional responders achieve durable disease control, and potential cures cannot be ruled out. PATIENT CONCERNS: We report the case of a 39-year-old woman with primary breast cancer and associated synchronous bone metastases, who experienced a disease response of 12 years with hormonal therapy as maintenance after first line chemotherapy, with a good toxicity profile. DIAGNOSIS: The patient was diagnosed with estrogen receptor + human epidermal growth factor receptor 2 (HER2)- metastatic breast cancer with synchronous bone metastases. INTERVENTIONS: This patient was treated with chemotherapy for 6 cycles as a first-line therapy following by endocrine treatment given as a maintenance therapy. OUTCOMES: Our patient experienced a progression-free survival >12 years with an exceptionally good quality of life. LESSONS: Our anecdotal experience highlights the existence of exceptional responders among patients with hormone receptor-positive metastatic breast cancer, who achieve clinical remission and durable disease control with endocrine therapy. Point-of-care pancreatic stone protein measurement in critically ill COVID-19 patients
Introduction Pancreatic stone protein (PSP) is a novel biomarker that is reported to be increased in pneumonia and acute conditions. The primary aim of this study was to prospectively study plasma levels of PSP in a COVID-19 intensive care unit (ICU) population to determine how well PSP performed as a marker of mortality in comparison to other plasma biomarkers, such as C reactive protein (CRP) and procalcitonin (PCT).Methods We collected clinical data and blood samples from COVID-19 ICU patients at the time of admission (T0), 72 h later ( T1), five days later (T2), and finally, seven days later. The PSP plasma level was measured with a point-of-care system; PCT and CRP levels were measured simultaneously with laboratory tests. The inclusion criteria were being a critical COVID-19 ICU patient requiring ventilatory mechanical assistance.Results We enrolled 21 patients and evaluated 80 blood samples; we found an increase in PSP plasma levels according to mixed model analysis over time (p < 0.001), with higher levels found in the nonsurvivor population (p < 0.001). Plasma PSP levels achieved a statistically significant result in terms of the AUROC, with a value higher than 0.7 at T0, T1, T2, and T3. The overall AUROC of PSP was 0.8271 (CI (0.73-0.93), p < 0.001). These results were not observed for CRP and PCT.Conclusion These first results suggest the potential advantages of monitoring PSP plasma levels through point-of-care technology, which could be useful in the absence of a specific COVID-19 biomarker. Additional data are needed to confirm these results

Molecular detection of canine bufaviruses in wild canids
Novel protoparvoviruses genetically related to human and non-human primate bufaviruses (BuVs) have been detected recently in respiratory and enteric specimens collected from dogs and cats. In this study, by molecular screening of archival collections of faecal samples from wolves and foxes, we detected BuVs with a rate of 17.1% (7/41) and 10.5% (9/86), respectively. Sequence analysis of a portion of the ORF2 gene region of nine positive samples showed that the viruses in these samples were closely related to BuVs (97.5–99.0% nucleotide sequence identity) found in domestic carnivores
