Excess Mortality for Abdominal Aortic Aneurysms and the Potential of Strict Implementation of Cardiovascular Risk Management: A Multifaceted Study Integrating Meta-Analysis, National Registry, and PHAST and TEDY Trial Data
Objective: Previous studies imply a profound residual mortality risk following successful abdominal aorta aneurysm (AAA) repair. This excess mortality is generally attributed to increased cardiovascular risk. The aim of this study was (1) to quantify the excess residual mortality for patients with AAA, (2) to evaluate the cross sectional level of cardiovascular risk management, and (3) to estimate the potential of optimised cardiovascular risk management to reduce the excess mortality in these patients.
Methods: Excess mortality was estimated through a systematic review and meta-analysis, and through data from the Swedish National Health Registry. Cardiovascular risk profiles were individually assessed during eligibility screening of patients with AAA for two multicentre pharmaceutical AAA stabilisation trials. The potential of full implementation of cardiovascular risk management was estimated through the validated Second Manifestations of ARTerial disease (SMART) risk scores algorithm.
Results: The meta-analysis showed a similarly impaired survival for patients who received early repair (small AAA) or regular repair (≥ 55 mm), and a further impaired survival for patients under surveillance for a small AAA. Excess mortality was further quantified using Swedish population data. The data revealed a more than quadrupled and doubled five year mortality rate for women and men who had their AAA repaired, respectively. Evaluation of the level of risk management of 358 patients under surveillance in 16 Dutch hospitals showed that the majority of patients with AAA did not meet therapeutic targets set for risk management in high risk populations, and indicated a more pronounced prevention gap in women. Application of the SMART risk score algorithm predicted that optimal implementation of risk management guidelines would reduce the 10 year risk of major adverse cardiovascular events from 43% to 14%.
Conclusion: Independent of the rupture risk, AAA is associated with a worryingly compromised life expectancy with a particularly poor prognosis for women. Optimal implementation of cardiovascular risk prevention guidelines is predicted to profoundly reduce cardiovascular risk
Activator protein-1 (AP-1) signalling in human atherosclerosis: results of a systematic evaluation and intervention study
Animal studies implicate the AP-1 (activator protein-1) pro-inflammatory pathway as a promising target in the treatment of atherosclerotic disease. It is, however, unclear whether these observations apply to human atherosclerosis. Therefore we evaluated the profile of AP-1 activation through histological analysis and tested the potential benefit of AP-1 inhibition in a clinical trial. AP-1 activation was quantified by phospho-c-Jun nuclear translocation (immunohistochemistry) on a biobank of aortic wall samples from organ donors. The effect of AP-1 inhibition on vascular parameters was tested through a double blind placebo-controlled cross-over study of 28 days doxycycline or placebo in patients with symptomatic peripheral artery disease. Vascular function was assessed by brachial dilation as well as by plasma samples analysed for hs-CRP (high-sensitivity C-reactive protein), IL-6 (interleukin-6), IL-8, ICAM-1 (intercellular adhesion molecule-1), vWF (von Willebrand factor), MCP-1 (monocyte chemoattractant protein-1), PAI-1 (plasminogen activator inhibitor-1) and fibrinogen. Histological evaluation of human atherosclerosis showed minimal AP-1 activation in non-diseased arterial wall (i.e. vessel wall without any signs of atherosclerotic disease). A gradual increase of AP-1 activation was found in non-progressive and progressive phases of atherosclerosis respectively (P<0.044). No significant difference was found between progressive and vulnerable lesions. The expression of phospho-c-Jun diminished as the lesion stabilized (P<0.016) and does not significantly differ from the normal aortic wall (P<0.33). Evaluation of the doxycycline intervention only revealed a borderline-significant reduction of circulating hs-CRP levels (−0.51 μg/ml, P=0.05) and did not affect any of the other markers of systemic inflammation and vascular function. Our studies do not characterize AP-1 as a therapeutic target for progressive human atherosclerotic disease
Search for Large extra spatial dimensions in the dielectron and diphoton channels in collisions at 1.96 TeV
We report on a search for large extra spatial dimensions in the dielectron
and diphoton channels using a data sample of 1.05 \invfb of \ppb collisions at
a center-of-mass energy of 1.96 TeV collected by the D0 detector at the
Fermilab Tevatron Collider. The invariant mass spectrum of the data agrees well
with the prediction of the standard model. We find 95% C.L. lower limits on the
effective Planck scale between 2.1 and 1.3 TeV for 2 to 7 extra dimensions.Comment: 7 pages, 3 figures, 3 tables, submitted to Phys. Rev. Let
Search for charged massive long-lived particles with the D0 detector
We search for charged massive long-lived particles using 1.1 fb of
data collected by the D0 detector at the Fermilab Tevatron Collider.
Time-of-flight information is used to search for pair produced long-lived tau
sleptons, gaugino-like charginos, and higgsino-like charginos. We find no
evidence of a signal and set 95% C.L. cross section upper limits for staus,
which vary from 0.31pb to 0.04pb for stau masses between 60 GeV and 300 GeV. We
also set lower mass limits of 206 GeV (171 GeV) for pair produced charged

Measurements of differential cross sections of Z/gamma*+jets+X events in proton anti-proton collisions at sqrt{s}=1.96 TeV
We present cross section measurements for Z/gamma*+jets+X production,
differential in the transverse momenta of the three leading jets. The data
sample was collected with the D0 detector at the Fermilab Tevatron proton
anti-proton collider at a center-of-mass energy of 1.96 TeV and corresponds to
an integrated luminosity of 1 fb-1. Leading and next-to-leading order
perturbative QCD predictions are compared with the measurements, and agreement
is found within the theoretical and experimental uncertainties. We also make
comparisons with the predictions of four event generators. Two
parton-shower-based generators show significant shape and normalization
differences with respect to the data. In contrast, two generators combining
tree-level matrix elements with a parton shower give a reasonable description
of the the shapes observed in data, but the predicted normalizations show
significant differences with respect to the data, reflecting large scale
uncertainties. For specific choices of scales, the normalizations for either

Search for resonant diphoton production with the D0 detector
We present a search for a narrow resonance in the inclusive diphoton final
state using 2.7 fb-1 of data collected with the D0 detector at the Fermilab
Tevatron ppbar Collider. We observe good agreement between the data and the
background prediction, and set the first 95% C.L. upper limits on the
production cross section times the branching ratio for decay into a pair of
photons for resonance masses between 100 and 150 GeV. This search is also
interpreted in the context of several models of electroweak symmetry breaking
with a Higgs boson decaying into two photons.Comment: 7 pages, 3 figures
Measurement of the t-channel single top quark production cross section
The D0 collaboration reports direct evidence for electroweak production of
single top quarks through the t-channel exchange of a virtual W boson. This is
the first analysis to isolate an individual single top quark production
channel. We select events containing an isolated electron or muon, missing
transverse energy, and two, three or four jets from 2.3 fb^-1 of ppbar
collisions at the Fermilab Tevatron Collider. One or two of the jets are
identified as containing a b hadron. We combine three multivariate techniques
optimized for the t-channel process to measure the t- and s-channel cross
sections simultaneously. We measure cross sections of 3.14 +0.94 -0.80 pb for
the t-channel and 1.05 +-0.81 pb for the s-channel. The measured t-channel
result is found to have a significance of 4.8 standard deviations and is
consistent with the standard model prediction.Comment: 7 pages, 6 figure
Measurement of trilinear gauge boson couplings from WW + WZ to lnu jj events in pp-bar collisions at sqrt{s}=1.96 TeV
We present a direct measurement of trilinear gauge boson couplings at gammaWW
and ZWW vertices in WW and WZ events produced in pp-bar collisions at
sqrt{s}=1.96 TeV. We consider events with one electron or muon, missing
transverse energy, and at least two jets. The data were collected using the D0
detector and correspond to 1.1/fb of integrated luminosity. Considering two
different relations between the couplings at the gammaWW and ZWW vertices, we
measure these couplings at 68% C.L. to be kappa_{gamma}=1.07^{+0.26}_{-0.29},
lambda =0.00^{+0.06}_{-0.06} and g_{1}^{Z}=1.04^{+0.09}_{-0.09} in a scenario
respecting SU(2)_L x U(1)_Y gauge symmetry and kappa =1.04^{+0.11}_{-0.11} and

Measurement of the W boson mass
We present a measurement of the W boson mass in W -> ev decays using 1 fb^-1
of data collected with the D0 detector during Run II of the Fermilab Tevatron
collider. With a sample of 499830 W -> ev candidate events, we measure M_W =
80.401 +- 0.043 GeV. This is the most precise measurement from a single

Double parton interactions in photon+3 jet events in ppbar collisions sqrt{s}=1.96 TeV
We have used a sample of photon+3 jets events collected by the D0 experiment
with an integrated luminosity of about 1 fb^-1 to determine the fraction of
events with double parton scattering (f_DP) in a single ppbar collision at
sqrt{s}=1.96 TeV. The DP fraction and effective cross section (sigma_eff), a
process-independent scale parameter related to the parton density inside the
nucleon, are measured in three intervals of the second (ordered in pT) jet
transverse momentum pT_jet2 within the range 15 < pT_jet2 < 30 GeV. In this
range, f_DP varies between 0.23 < f_DP < 0.47, while sigma_eff has the average
value sigma_eff_ave = 16.4 +- 0.3(stat) +- 2.3(syst) mb.Comment: 15 pages, 13 figure
