Prävention und Gesundheitswesen : digitale Epidemiologie - eine neue Zeit ist angebrochen!
Fachärztinnen und Fachärzte für «Prävention und Gesundheitswesen» oder schlichtweg für «Public Health» befassen sich traditionell mit der Epidemiologie von Erkrankungen, deren Determinanten und Verbreitung in Bevölkerungen. Gesundheitsförderung, Prävention oder Gesundheitsmonitoring und Systementwicklung, all diese Prozesse brauchen Daten, die erhoben, analysiert und interpretiert werden müssen, um eine nachhaltige und ganzheitliche Bevölkerungsgesundheit zu gewährleisten. Im digitalen Zeitalter stehen auf einmal sehr viele Gesundheitsdaten elektronisch zur Verfügung, die Auskunft geben über den Gesundheitszustand der Bevölkerung. Die Fachärzteschaft «Prävention und Gesundheitswesen» hat sich anlässlich einer gemeinsamen Fortbildung mit der Vereinigung der Kantonsärztinnen und -ärzte der Schweiz am 17. November 2016 mit Experten dazu ausgetauscht (Antoine Flahault, Institute of Global Health; Séverine Rion Logean, Swiss Re; Marcel Salathé, EPFL; Dr. Olivia Woolley, EPFZ). Die Anzahl an Veranstaltungen zum Thema, die in den darauffolgenden Monaten allein in der Schweiz stattfanden, weisen ebenso wie die steigende Anzahl an Publikationen von 84 im Jahr 2000 auf fast 500 im Jahr 2016 darauf hin: digitale Epidemiologie ist ein «hot topic». Doch was bedeutet digitale Epidemiologie? Und welche Konsequenzen hat sie für «Public Health»
Incidence of drug-induced liver injury in medical inpatients
Objectives: Drug-induced liver injury (DILI) is a common concern. However, data on DILI epidemiology in inpatients are sparse. Methods: To investigate the incidence of DILI, we screened all patients in the pharmacoepidemiological inpatient database according to the CIOMS (Council for International Organisation of Medical Science) criteria, which consist of the evaluation of some clinical chemistry laboratory liver parameters (CIOMS laboratory criteria) and the exclusion of any disease-related causes for the liver injury. Thus, only cases with probable or certain causality according to the World Health Organization criteria were included. Results: Among a total of 6383 patients, liver parameters were determined in 4610, and 489 among them fulfilled the CIOMS laboratory criteria. However, 401 patients had to be excluded because of disease-related liver injury and, thus, the study cohort consisted of 4209 patients at risk for DILI. Among a total of 88 DILI cases, 31 had no documented normal baseline liver parameters and, thus, represented prevalent cases. The remaining 57 represented incident DILI cases. Thus, the incidence of DILI was 1.4% (95% CI 1.0, 1.7). The drug classes most frequently causing DILI were heparins, antibacterials, tuberculostatics and antineoplastic agents. Among those, antineoplastic agents and tuberculostatics showed the highest incidence. Liver injury was not mentioned among the diagnoses or in the physician's discharge letter in about 52-68% of all cases. Conclusion: Approximately 1 in 100 patients develops DILI during hospitalisation in a department of medicine. Incidences of DILI were highest for antineoplastic agents and tuberculostatics. DILI is frequently missed and, therefore, DILI detection by diagnoses will result in misleadingly low incidence rate
13C, 15N Resonance Assignment of Parts of the HET-s Prion Protein in its Amyloid Form
The partial 15N and 13C solid-state NMR resonance assignment of the HET-s prion protein fragment 218-289 in its amyloid form is presented. It is based on experiments measured at MAS frequencies in the range of 20-40kHz using exclusively adiabatic polarization-transfer schemes. The resonance assignment within each residue is based on two-dimensional 13C--13C correlation spectra utilizing the DREAM mixing scheme. The sequential linking of the assigned residues used a set of two- and three-dimensional 15N--13C correlation experiments. Almost all cross peaks visible in the spectra are assigned, but only resonances from 43 of the 78 amino-acid residues could be detected. The missing residues are thought to be highly disordered and/or highly dynamic giving rise to broad resonance lines that escaped detection in the experiments applied. The line widths of the observed resonances are narrow and comparable to line widths observed in micro-crystalline samples. The 43 assigned residues are located in two fragments of about 20 residue
Beta-escin has potent anti-allergic efficacy and reduces allergic airway inflammation
<p>Abstract</p> <p>Background</p> <p>Type I hypersensitivity is characterized by the overreaction of the immune system against otherwise innocuous substances. It manifests as allergic rhinitis, allergic conjunctivitis, allergic asthma or atopic dermatitis if mast cells are activated in the respective organs. In case of systemic mast cell activation, life-threatening anaphylaxis may occur. Currently, type I hypersensitivities are treated either with glucocorticoids, anti-histamines, or mast cell stabilizers. Although these drugs exert a strong anti-allergic effect, their long-term use may be problematic due to their side-effects.</p> <p>Results</p> <p>In the course of a routine <it>in vitro </it>screening process, we identified beta-escin as a potentially anti-allergic compound. Here we tested beta-escin in two mouse models to confirm this anti-allergic effect <it>in vivo</it>. In a model of the early phase of allergic reactions, the murine passive cutaneous anaphylaxis model, beta-escin inhibited the effects of mast cell activation and degranulation in the skin and dose-dependently prevented the extravasation of fluids into the tissue. Beta-escin also significantly inhibited the late response after antigen challenge in a lung allergy model with ovalbumin-sensitized mice. Allergic airway inflammation was suppressed, which was exemplified by the reduction of leucocytes, eosinophils, IL-5 and IL-13 in the bronchoalveolar lavage fluid. Histopathological examinations further confirmed the reduced inflammation of the lung tissue. In both models, the inhibitory effect of beta-escin was comparable to the benchmark dexamethasone.</p> <p>Conclusions</p> <p>We demonstrated in two independent murine models of type I hypersensitivity that beta-escin has potent anti-allergic properties. These results and the excellent safety profile of beta-escin suggest a therapeutic potential of this compound for a novel treatment of allergic diseases.</p
Lymph node metastasis in feline cutaneous low-grade mast cell tumours
Objectives This retrospective study aimed to determine the incidence of nodal metastatic disease in cats affected by low-grade cutaneous mast cell tumours (MCTs) in our study population. Methods The clinical records of two centres were retrospectively searched for cats with cutaneous MCTs that had undergone lymphadenectomy of enlarged and non-enlarged lymph nodes. All primary tumours were histologically reviewed by two experienced pathologists and graded as high- or low-grade based on the grading system for feline cutaneous MCT. We graded the lymph nodes based on the grading scheme used for canine MCTs and considered HN2 and HN3 nodes to be metastatic. The number of patients with nodal metastasis was calculated. Results We identified 17 cats with cutaneous MCT resection and concurrent lymphadenectomy. All 21 MCTs were graded as low grade and 30 nodes were removed, with 12 being considered early or overtly metastatic (HN2 or HN3, respectively). Based on nodal status, 10/17 (59%) cats were affected by nodal metastasis in our population. Conclusions and relevance In contrast to previous reports, high percentage of cats with cutaneous MCTs in which lymphadenectomy was performed were presented with metastatic lymph nodes. The clinical relevance of this finding and a potential benefit of lymphadenectomy must be determined in future studies
Die psychosozialen Belastungen von jungen Kindern mit kinderpsychiatrischen Störungen
Es wird ein Überblick gegeben über zwei Patientenkollektive von Kindern im Kindergartenalter, die in Regensburg und Landshut teilstationär behandelt wurden. Expansive und Sozialverhaltensstörungen waren in beiden Stichproben am häufigsten, gefolgt von emotional-ängstlichen Störungen und Bindungsstörungen. Risikofaktoren im Sinne psychosozialer Belastungen lagen bei ca. 80% aller Kinder vor. Diese Auffälligkeiten waren in jenen Kategorien häufiger, die innerfamiliäre Faktoren wie abweichende Kommunikationsstrukturen, Unangemessenheit elterlicher Anforderungen, abweichende Familiensituation und psychische Erkrankung eines weiteren Familienmitglieds bezeichnen
<i>Mycoplasma pneumoniae</i> IgG positivity is associated with tic severity in chronic tic disorders
Infectious pathogens may represent an environmental risk factor for chronic tic disorders (CTD). This cross-sectional study aimed to determine whether Mycoplasma pneumoniae (M. pneumoniae) IgG positivity is associated with the presence or severity of tics. We compared M. pneumoniae IgG positivity across three groups: children and adolescents (3-16 years) with CTD (CTD group; n=302); siblings (3-10 years) of people with CTD who developed tics within a seven-year follow-up period (tic onset group; n=51); siblings (4-10 years) who did not develop tics within the study period and were ≥ 10-years-old at their last assessment (unaffected group; n=88). The relationship between M. pneumoniae IgG positivity and the presence and severity of tics was analysed using multilevel models controlling for site, family relatedness, sex, age, presence of comorbid obsessive-compulsive and/or attention-deficit/hyperactivity disorder and use of psychotropic medication. M. pneumoniae IgG positivity was not associated with the presence of CTD, or the first onset of tics as compared to siblings who remained unaffected. M. pneumoniae IgG positivity was associated with a higher tic severity score within the CTD group (β=2.64, s.e.=1.15, p=0.02). It is possible that M. pneumoniae infection influences tic severity in CTD or, that having more severe tics, increases the risk of infection. However, it is more likely that the association observed in this study reflects a propensity toward enhanced immune responses in people with CTD and that, rather than a causal relationship, infection and greater tic severity are indirectly linked via shared underlying immune mechanisms
SwissGenVar: A Platform for Clinical-Grade Interpretation of Genetic Variants to Foster Personalized Healthcare in Switzerland.
Large-scale next-generation sequencing (NGS) germline testing is technically feasible today, but variant interpretation represents a major bottleneck in analysis workflows. This includes extensive variant prioritization, annotation, and time-consuming evidence curation. The scale of the interpretation problem is massive, and variants of uncertain significance (VUSs) are a challenge to personalized medicine. This challenge is further compounded by the complexity and heterogeneity of the standards used to describe genetic variants and the associated phenotypes when searching for relevant information to support clinical decision making. To address this, all five Swiss academic institutions for Medical Genetics joined forces with the Swiss Institute of Bioinformatics (SIB) to create SwissGenVar as a user-friendly nationwide repository and sharing platform for genetic variant data generated during routine diagnostic procedures and research sequencing projects. Its aim is to provide a protected environment for expert evidence sharing about individual variants to harmonize and upscale their significance interpretation at the clinical grade according to international standards. To corroborate the clinical assessment, the variant-related data will be combined with consented high-quality clinical information. Broader visibility will be achieved by interfacing with international databases, thus supporting global initiatives in personalized healthcare
