    Effects of smoking on interdigestive gastrointestinal motility

    The effect of smoking on interdigestive gastrointestinal motility is little studied but may play a role in gastrointestinal morbidity. We studied gastroduodenal motility in 10 volunteers (five smokers and five nonsmokers) using a water-perfused pressure catheter. A pH probe was placed in the duodenal bulb. Baseline motility was recorded until phase III of the migrating motor complex had occurred in the stomach three times in order to record two complete cycles of MMC activity. Subjects then began smoking until phase III activity occurred again (mean duration of smoking 117 min). During the control period, all subjects had normal MMC cycles and there were no differences between smokers and nonsmokers. While smoking, no gastric phase III was observed in any subject and gastric motility was markedly reduced. In seven of 10 subjects, smoking did not prevent the occurrence of normal duodenal phase III activity. Three subjects had no duodenal phase III activity during smoking. The duodenal pH profile did not change during smoking and motilin levels continued to fluctuate in conjunction with phase III activity. In conclusion, smoking abolished phase III activity in the stomach without affecting the plasma motilin cyclic fluctuations or duodenal bulb pH.     Hemophilus influenzae type B cellulitis in adults

    Cellulitis due to Hemophilus influenzae type B is a rare but treatable event in adults. Herein is described a 67-year-old woman with anterior neck cellulitis caused by H. influenzae type B, documented by positive blood culture results. Six additional cases reported in the literature are reviewed. The following clinical syndrome emerges: the patient is usually older than 50 years of age, and pharyngitis develops first, followed by the onset of high fever and rapidly progressive anterior neck swelling, tenderness, and erythema associated with dysphagia. Because the causative organism may be resistant to ampicillin, the early use of chloramphenicol is recommended along with a beta-lactamase-resistant penicillin or cephalosporin (to cover other potential pathogens), or an appropriate third-generation cephalosporin that would also adequately cover all possible pathogens.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/26033/1/0000106.pd

    A randomized, controlled trial to assess a novel colorectal cancer screening strategy: the conversion strategy

    Our study was a randomized, controlled trial to assess a novel strategy that provides comprehensive colorectal cancer screening in a single visit versus traditional sigmoidoscopy and, where appropriate, colonoscopy on a subsequent day. METHODS : Consecutive patients referred for screening were randomized to control or so-called “conversion” groups. Patients in the control group were prepared for sigmoidoscopy with oral phospho-soda. Those with an abnormal sigmoidoscopy were scheduled for colonoscopy on a future day after oral polyethylene glycol preparation. In the conversion group, patients were prepared with oral phospho-soda. Patients with a polyp >5 mm or multiple diminutive polyps were converted from sigmoidoscopy to colonoscopy, allowing comprehensive screening in a single visit. Clinical outcomes were assessed by postprocedure physician and patient questionnaires. RESULTS : Two hundred thirty-five patients were randomized (control = 121, conversion = 114). In the control group, 28% had an indication for colonoscopy. Three of 33 (9%) with an abnormal sigmoidoscopy did not return for colonoscopy. At colonoscopy, 27% had a proximal adenoma. In the conversion group, 28% had an abnormal sigmoidoscopy and underwent conversion to colonoscopy. Forty-one percent undergoing colonoscopy in the conversion group had a proximal adenoma. Physicians reported no differences in preparation or procedure difficulty, whereas patients reported no differences in the level of comfort or overall satisfaction between groups. When queried regarding preferences for future screening, 96% chose the conversion strategy. CONCLUSIONS : The conversion strategy led to similar outcomes compared to traditional screening while improving compliance with colonoscopy in patients with an abnormal sigmoidoscopy.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/73097/1/j.1572-0241.2000.02231.x.pd

    Expression and regulation of cytochrome P-450I genes (CYP1A1 and CYP1A2) in the rat alimentary tract

    Cytochrome P-450 (P450) enzymes in the mucosa of the alimentary tract may be involved in the activation and/or inactivation of ingested xenobiotics and procarcinogens. Since the multiple P450 enzymes have overlapping substrate specificities and, in some cases, similar antigenic determinants, definitive identification of P450 genes that are expressed in various tissues requires molecular analysis. In this study, a sensitive and specific polymerase chain reaction assay along with hybridization analysis was used to examine the expression of the CYP1A gene family in the rat alimentary tract. CYP1A1 mRNA was expressed throughout the alimentary tract in untreated rats, as determined by the polymerase chain reaction. However, Northern blot analysis detected [beta]-naphthoflavone, CYP1A1 mRNA and enzymatic activity was markedly induced in each alimentary tissue including esophagus, fore-stomach, glandular stomach, small intestine and colon. A single dose of inducer resulted in a rapid rise in CYP1A1 mRNA which was shown by nuclear run-on assays to be primarily due to an increase in transcription of the CYP1A1 gene. CYP1A2 mRNA was detected in significant amounts only in glandular stomach following induction although the polymerase chain reaction assay identified low levels of CYP1A2 mRNA in several other tissues. The definitive identification of the CYP1A genes that are expressed in alimentary tissue will allow the design of experiments to investigate the importance of these enzymes in the metabolism of carcinogens, and ultimately carcinogenesis, in the gastrointestinal tract.     3 - 14 Micron Spectroscopy of Comets C/2002 O4 (Honig), C/2002 V1 (NEAT), C/2002 X5 (Kudo-Fujikawa), C/2002 Y1 (Juels-Holvorcem), 69P/Taylor, and the Relationships among Grain Temperature, Silicate Band Strength and Structure among Comet Families

    We report 3 - 13 micron spectroscopy of 4 comets observed between August 2002 and February 2003: C/2002 O4 (Honig) on August 1, 2002, C/2002 V1 (NEAT) on Jan. 9 and 10, 2003, C/2002 X5 (Kudo-Fujikawa) on Jan. 9 and 10, 2003, and C/2002 Y1 (Juels-Holvorcem) on Feb. 20, 2003. In addition, we include data obtained much earlier on 69P/Taylor (February 9, 1998) but not previously published. For Comets Taylor, Honig, NEAT, and Kudo-Fujikawa, the silicate emission band was detected, being approximately 23%, 12%, 15%, and 10%, respectively, above the continuum. The data for Comet Juels-Holvorcem were of insufficient quality to detect the presence of a silicate band of comparable strength to the other three objects, and we place an upper limit of 24% on this feature. The silicate features in both NEAT and Kudo-Fujikawa contained structure indicating the presence of crystalline material. Combining these data with those of other comets, we confirm the correlation between silicate band strength and grain temperature of Gehrz & Ney (1992) and Williams et al. (1997) for dynamically new and long period comets, but the majority of Jupiter family objects may deviate from this relation. The limited data available on Jupiter family objects suggest that they may have silicate bands that are slightly different from the former objects. Finally, when compared to the silicate emission bands observed in pre-main sequence stars, the dynamically new and long period comets most closely resemble the more evolved stellar systems, while the limited data (in quantity and quality) on Jupiter family objects seem to suggest that these have spectra more like the less-evolved stars.Comment: 45 pages, 12 figure

    Mid-Infrared Spectrophotometric Observations of Fragments B and C of Comet 73P/Schwassmann-Wachmann 3

    We present mid-infrared spectra and images from the GEMINI-N (+Michelle) observations of fragments SW3-[B] and SW3-[C] of the ecliptic (Jupiter Family) comet 73P/Schwassmann-Wachmann 3 pre-perihelion. We observed fragment B soon after an outburst event (between 2006 April 16 - 26 UT) and detected crystalline silicates. The mineralogy of both fragments was dominated by amorphous carbon and amorphous pyroxene. The grain size distribution (assuming a Hanner modified power-law) for fragment SW3-[B] has a peak grain radius of a_p ~ 0.5 micron, and for fragment SW3-[C], a_p ~ 0.3 micron; both values larger than the peak grain radius of the size distribution for the dust ejected from ecliptic comet 9P/Tempel 1 during the Deep Impact event (a_p = 0.2 micron. The silicate-to-carbon ratio and the silicate crystalline mass fraction for the submicron to micron-size portion of the grain size distribution on the nucleus of fragment SW3-[B] was 1.341 +0.250 -0.253 and 0.335 +0.089 -0.112, respectively, while on the nucleus of fragment SW3-[C] was 0.671 +0.076 -0.076 and 0.257 +0.039 -0.043, respectively. The similarity in mineralogy and grain properties between the two fragments implies that 73P/Schwassmann-Wachmann 3 is homogeneous in composition. The slight differences in grain size distribution and silicate-to-carbon ratio between the two fragments likely arises because SW3-[B] was actively fragmenting throughout its passage while the activity in SW3-[C] was primarily driven by jets. The lack of diverse mineralogy in the fragments SW3-[B] and SW3-[C] of 73P/Schwassmann-Wachmann 3 along with the relatively larger peak in the coma grain size distribution suggests the parent body of this comet may have formed in a region of the solar nebula with different environmental properties than the natal sites where comet C/1995 O1 (Hale-Bopp) and 9P/Tempel 1 nuclei aggregated.Comment: 31 pages, 5 figure, accepted for publication in A

    Longitudinal Study of Falls among HIV-Infected and Uninfected Women: The Role of Cognition

    BackgroundAlthough fracture rates are higher in HIV+ than HIV- women, whether HIV infection increases risk of falls is unclear. We determined the longitudinal occurrence and risk factors for falls in the Women's Interagency HIV Study (WIHS), and explored associations with cognitive complaints.MethodsRecent (prior 6 months) self-reported falls were collected in 1,816 (1,250 HIV+; 566 HIV-) women over 24 months. Generalized estimating equation models using stepwise selection determined odds of any fall (versus none).ResultsHIV+ women were older than HIV- women (median 49 versus 47 years; P=0.0004), more likely to report neuropathy (20% versus 16%; P=0.023), and had greater central nervous system (CNS) medication use. At least one fall was reported in 41% HIV+ versus 42% HIV- women, including ≄2 falls in 25% HIV+ and 24% HIV- (overall P=0.30). Cognitive complaints were associated with falls among HIV+ (odds ratio [OR] 2.38; 95% CI 1.83, 3.09) and HIV- women (OR 3.43; 95% CI 2.37, 4.97); in adjusted models, cognitive complaints remained significant only in HIV- women (adjusted [aOR] 2.26; 95% CI 1.46, 3.48). Factors associated with any fall in adjusted analyses included: depressive symptoms and neuropathy (both HIV+ and HIV-); age, marijuana use, multiple CNS medications, and HCV infection (HIV+ only); and cognitive complaints, quality of life, hypertension and obesity (HIV- only).ConclusionsMiddle-aged HIV+ and HIV- women had similar fall rates. Among HIV+ women, factors affecting cognition such as age, depressive symptoms, marijuana use and multiple CNS medications were important predictors of falls, however, cognitive complaints were not
