    Higher-order modulations in the skyrmion-lattice phase of Cu2_2OSeO3_3

    Using small angle neutron scattering, we have investigated higher-order peaks in the skyrmion-lattice phase of Cu2_2OSeO3_3, in which two different skyrmion lattices, SkX1 and SkX2, are known to form. For each skyrmion-lattice phase, we observed two sets of symmetrically inequivalent peaks at the higher-order-reflection positions with the indices (110)(110) and (200)(200). Under the condition where the SkX1 and SkX2 coexist, we confirmed the absence of the scattering at Q\mathbf{Q} positions combining reflections from the two phases, indicating a significantly weak double-scattering component. Detailed analysis of the peak profile, as well as the temperature and magnetic-field dependence of the peak intensity, also supports the intrinsic higher-order modulation rather than the parasitic double scattering. The two higher-order modulations show contrasting magnetic-field dependence; the former (110)(110) increases as the field is increased, whereas the latter (200)(200) decreases. This indicates that, in Cu2_2OSeO3_3, skyrmions are weakly distorted, and the distortion is field-dependent in a way that the dominant higher-order modulation switches from (110)(110) to (200)(200) under field. Monte Carlo simulations under sweeping external magnetic field qualitatively reproduce the observed magnetic-field dependence, and suggests that the higher-order modulations correspond to the superlattices of weak swirlings appearing in the middle of the original triangular-latticed skyrmions.Comment: 13 pages, 14 figure

    Impact of half-day clinical training in outpatient psychiatry on perception of mental illness by postgraduate interns

    Aim: Lack of contact with patients with mental illness may contribute to mental health stigma. We conducted a half-day training program in the outpatient psychiatry clinic of a rural general hospital for postgraduate interns in Nagasaki University Hospital. Our study investigated the effectiveness of this program in reducing stigma toward mental illness. It also examined the association between an intern’s perception of mental illness and their consideration of psychiatry as a career.Methods: Participants were 12 interns at Nagasaki University Hospital who competed a pre- and post- training questionnaire. The questionnaire assessed perceptions of mental illness using a semantic differential scale and measured consideration of psychiatry as a career on a 7 point Likert scale. Paired t-tests were used to compare mean pre- and post-training scores on the semantic differential scale. Pearson’s correlation was used to examine associations between semantic differential scores and consideration of psychiatry as a career.Results: Post-training scores were higher than pre-training scores on the items “warm” (P = 0.003), “clean” (P = 0.009), “bright” (P = 0.001), and “calm” (P = 0.003) as associated with mental illness. Consideration of psychiatry as a career significantly correlated with post-training score on “warm” (r = 0.587, P = 0.045).Conclusion: Interns can develop positive perceptions of mental illness after a half-day training program in a psychiatry outpatient clinic.     Observations of hard X-rays of auroral origin with Polar Patrol Balloons No. 8 and 10

    In the Polar Patrol Balloon (PPB) project, two balloons named PPB-8 and -10 were launched in rapid succession to form a cluster of balloons during their flight on January 13, 2003, from Syowa Station, Antarctica. In order to make the two-dimensional images for auroral X-rays and to obtain the energy spectra of auroras with energy range from 30 keV to 778 keV, the same instruments for hard X-rays were installed on PPB-8 and -10, respectively. These detection systems observed several auroral X-ray events during the flight. In particularly on January 25, 2003, strong auroral events were detected at about 0919 UT by PPB-10 and at 0927 UT by PPB-8. The aurora observed by PPB-10 was observed after about 8 min by PPB-8 located a 650 km west of PPB-10. The energy spectra of the bright aurora at 0919 UT and 0927 UT for PPB-10 and -8 is obtained as E0 = (78+-5) keV and (70+-5) keV, respectively

    Subjective achievement from psychiatry rotation in the Japanese postgraduate residency system: a longitudinal questionnaire study

    Background: Psychiatry rotation has been mandatory in the Japanese postgraduate residency system since 2020. Some psychiatry-related competency items are stipulated as mandatory for residents. The current study aimed to clarify whether psychiatry rotation affected residents’ subjective achievement of these competency items.Methods: This longitudinal study was conducted among postgraduate residents who completed a rotation in the psychiatry department at Nagasaki University Hospital across two academic years (2020–2021). The survey was administered at the start and at the end of the psychiatry rotation. Residents evaluated their subjective understanding and confidence regarding initiating treatment for these competency items using a six-point Likert scale. The average scores for each item were compared between pre-rotation and post-rotation.Results: In total, 99 residents (91.7%) responded to this survey. Residents had significantly higher scores at postrotation compared with pre-rotation in all psychiatry-related competency items in both subjective understanding and confidence in initiating treatment. Additionally, strong effect sizes were found for many items.Conclusion: Residents improved learning about psychiatry-related competency items through psychiatry rotation. This finding suggests that it is reasonable for psychiatry rotation to be mandatory in the current Japanese postgraduate residency system. The importance of psychiatry is likely to increase in both undergraduate and postgraduate medical education in the future. It is necessary to continuously update educational strategies to meet changing social needs over time. As this study was conducted at a single institution, a multi-center study is needed to expand the current findings

    Novel non-alcoholic steatohepatitis model with histopathological and insulin-resistant features

    Although several non-alcoholic steatohepatitis (NASH) models have been reported to date, few of these models fully reflect the histopathology and pathophysiology of human NASH. The aim of this study was to establish a novel NASH model by feeding a high-fat (HF) diet and administering both carbon tetrachloride (CCl4) and the Liver X receptor agonist T0901317. Male C57BL/6J mice were divided into four groups (each n = 5): HF, HF + CCl4, HF + T0901317, and the novel NASH model (HF + CCl4 + T0901317). CCl4 (0.1 mL/kg) and T0901317 (2.5 mg/kg) were intraperitoneally administered four times and five times, respectively. The livers of the novel NASH model group presented a whitish colour. The serum levels of TNF-α and IL-6 were significantly increased in the novel NASH model group, and mice in this group exhibited histopathological features and insulin resistance reflective of NASH, i.e., macrovesicular hepatic steatosis, ballooning hepatocytes, Mallory-Denk bodies, lobular inflammation and fibrosis. The novel NASH model group presented significantly upregulated expression levels of mRNAs related to lipogenesis, oxidative stress, fibrosis and steatosis and significantly downregulated expression levels of mRNAs related to triglyceride export. We successfully established a novel experimental NASH model that exhibits similar histopathology and pathophysiology to human NASH

    The linkage between medical student readiness for interprofessional learning and interest in community medicine

    Objectives: The purpose of this study is to investigate the relationship between medical student readiness for interprofessional learning and interest in community medicine prior to incorporating community-oriented interprofessional education into the curriculum. Methods: A questionnaire was administered to students at Nagasaki University School of Medicine in Japan during each of three consecutive years (N=2244). The Readiness for Interprofessional Learning Scale (RIPLS) was administered in addition to a questionnaire to evaluate interest in community medicine. The Kruskal-Wallis and Steel-Dwass tests were used to determine differencesbetween school years. Correlation between the RIPLS score and interest in community medicine was evaluated with Spearman\u27s rank correlation coefficient. Relationships between RIPLS score and demographic parameters, and interest in community medicine were evaluated with multiple linear regression analysis. Results: Eighty-four percent (1891/2244) of students responded. The RIPLS score was highest in school year 1, followed by year 6, year 5,year 3, and years 4 and 2. Interest in community medicine correlated with the RIPLS score (rs = 0.332, p < 0.001),but less in year 1 (rs = 0.125, p = 0.002) than in other years. RIPLS score was significantly associated with gender, age, school year, interest in community medicine, but not the year that the survey was conducted. Conclusions: Community-oriented interprofessional education has the potential to improve attitudes towards interprofessional learning. When introducing this promising education into the curriculum from year 1, attracting students\u27 interest in community medicine should be considered

    Evaluation of safety for hepatectomy in a novel mouse model with nonalcoholic-steatohepatitis

    AIMTo investigate whether the liver resection volume in a newly developed nonalcoholic steatohepatitis (NASH) model influences surgical outcome.METHODSFor establishment of a NASH model, mice were fed a high-fat diet for 4 wk, administered CCl4 for the last 2 wk, and administered T0901317 for the last 5 d. We divided these mice into two groups: A 30% partial hepatectomy (PH) of NASH liver group and a 70% PH of NASH liver group. In addition, a 70% PH of normal liver group served as the control. Each group was evaluated for survival rate, regeneration, apoptosis, necrosis and DNA expression after PH.RESULTSIn the 70% PH of NASH group, the survival rate was significantly decreased compared with that in the control and 30% PH of NASH groups (P < 0.01). 10 of 32 mice in the NASH 70% PH group died within 48 h after PH. Serum aspartate aminotransferase (AST) levels and total bilirubin (T-Bil) in the NASH 70% PH group were significantly higher than the levels in the other two groups (AST: P < 0.05, T-Bil: P < 0.01). In both PH of NASH groups, signaling proteins involved in regeneration were expressed at lower levels than those in the control group (P < 0.01). The 70% PH of NASH group also exhibited a lower number of Ki-67-positive cells and higher rates of apoptosis and necrosis than the NASH 30% PH group (P < 0.01). In addition, DNA microarray assays showed differences in gene expression associated with cell cycle arrest and apoptosis.CONCLUSIONThe function of the residual liver is impaired in fatty liver compared to normal liver. A larger residual volume is required to maintain liver functions in mice with NASH
