    Clinical trials field strategies with novel vaccines produced in Brazil

    OBJECTIVE: To report field strategies applied to clinical trials with vaccines developed by Instituto Butantan in Campinas, Brazil, in 2004 and 2006. METHODS: This report describes the planning and the operational issues of two clinical trials conducted to evaluate immunogenicity and safety of recombinant Hepatitis B vaccine combined with BCG vaccine (BCG/VrHB-IB) and quadrivalent diphteria-tetanus-Haemophilus influenzae type b-cellular pertussis vaccine with low lipopolysaccharide (DTPm/Hib). RESULTS: The main field strategies applied were: a) Partnership between the researchers and managers from Municipal Health Department and b) Research procedures at home or in Health Centers attended by participants. In the first study, BCG vaccine and VrHB-IB (combined or separated) were given to 552 newborns in the maternity, followed by two subsequent doses of VrHB-IB vaccine in households. The second study included 241 infants at Health Centers, which were vaccinated with DTPm/Hib vaccines concomitantly to the others recommended by the National Immunization Program. In both studies, blood samples were taken at home. No adverse events occurred during the experimental period. The field strategies used in those clinical trials allowed adherence by 90.2 and 93.8% of the participants of the first and second study, respectively. The vaccines were given according to the recommendation of National Immunization Program and blood samples were obtained according to the protocol schedules. CONCLUSIONS: The field strategies were important to guarantee enrollment and protocol compliance, causing little interference in families' daily routine, pediatrics appointments and children's vaccine.OBJETIVO: Relatar as estratégias de campo utilizadas em dois ensaios clínicos com vacinas desenvolvidas pelo Instituto Butantan, em 2004 e 2006. RESULTADOS: As principais estratégias de campo utilizadas foram: a) Parceria entre os pesquisadores e os gestores da Secretaria Municipal de Saúde e b) Realização dos procedimentos da pesquisa nos domicílios ou nos Centros de Saúde frequentados pelos participantes. No primeiro estudo, foram vacinados 552 recém-nascidos na maternidade com a BCG/VrHB-IB (combinadas ou separadas) e nos domicílios, com as duas doses subsequentes de VrHB-IB. O segundo estudo incluiu 241 lactentes em Centros de Saúde da rede municipal, vacinados com tetravalente bacteriana (com componente pertussis total ou modificado). Em ambos os estudos, amostras de sangue foram colhidas nas residências. Não houve relatos de eventos adversos. A adesão foi de 90,2% para o primeiro estudo e 93,8%, para o segundo. As vacinas foram administradas nas datas preconizadas pelo Programa Nacional de Imunizações e as coletas de sangue, de acordo com o cronograma dos estudos. CONCLUSÕES: As estratégias utilizadas facilitaram o recrutamento das crianças e garantiram cumprir o protocolo da pesquisa com alta adesão, sem interferir no vínculo da família com o Serviço de Saúde, no calendário vacinal ou no seguimento pediátrico dos participantes.OBJETIVO: Relatar las estrategias de campo utilizadas en dos ensayos clínicos con vacunas desarrolladas por el Instituto Butantan, en 2004 y 2006. Las vacunas fueron administradas en las fechas preconizadas por el Programa Nacional de Inmunizaciones y las muestras de sangre fueron recogidas conforme al cronograma de estudios. CONCLUSIONES: Las estrategias utilizadas facilitaron el reclutamiento de los niños y garantizaron cumplir con el protocolo de la investigación con alta adhesión, sin interferir en el vínculo de la familia con el Servicio de Salud, en el calendario de vacunas o en el seguimiento pediátrico de los participantes.20220

    Serum immunoglobulins in children perinatally exposed to human immunodeficiency virus

    OBJECTIVE: Hypergammaglobulinemia is an early manifestation of perinatal HIV infection. Our objective was to analyze the differences in serum immunoglobulin levels between infected and seroreverting children and their association with clinical outcome. METHODS: We carried out a historical prospective study with 107 infected and 90 seroreverting children. We compared the IgA, IgG, and IgM levels between infected and seroreverting patients within the first 18 months of life; IgA, IgG, and IgM as surrogate markers of infection; and IgA, IgG, and IgM levels within the first 5 years in infected children, according to clinical outcome. The Mann-Whitney test was used for comparison between groups. Surrogate markers were assessed according to sensitivity, specificity, positive and negative predictive values, and Youden's index. RESULTS: Infected children, when compared to seroreverters, showed significantly higher levels of IgM between the 1st and 5th trimesters; IgA and IgG between the 2nd and 6th trimesters (P less than or equal to 0.05). Levels of IgA greater than or equal to 90 mg/dl in the 2nd trimester and IgG greater than or equal to 1,700 mg/dl or 1,200 mg/dl in the 2nd and 3rd trimesters were associated with HIV infection, with Youden's indexes of 0.97, 0.92, and 0.93, respectively. Infected children in the B and C categories, compared to those in the N and A, showed higher levels of IgM between the 2nd and 4th years, and IgA between the 3rd and 5th year (P > 0.05). CONCLUSIONS: The temporal progression of IgA, IgG, and IgM levels showed an early and intense stimulation to the synthesis of immunoglobulin in infected children. Clinical and epidemiological indicators showed that such levels may be surrogate markers of infection. Higher IgM and IgA levels between the 2nd and 5th years in more severely infected children suggest a dysfunction in immune regulation secondary to persistent antigenic stimulation.OBJETIVO: a hipergamaglobulinemia é uma manifestação precoce da infecção perinatal por HIV. O objetivo foi analisar as diferenças nos níveis séricos de imunoglobulinas entre crianças infectadas e sororreversoras, e sua associação com a evolução clínica. MÉTODOS: Em um estudo prospectivo histórico, avaliaram-se 107 crianças infectadas e 90 sororreversoras. Compararam-se: IgA, IgG e IgM entre infectados e sororreversores nos primeiros 18 meses de vida; IgA, IgG e IgM como marcadores indiretos de infecção; IgA, IgG e IgM nos 5 primeiros anos em infectados, de acordo com a evolução clínica. Utilizou-se o teste de Mann-Whitney para a comparação entre grupos. Na avaliação de marcadores indiretos, analisaram-se Sensibilidade, Especificidade, Valores Preditivos Positivo e Negativo, e índice J. RESULTADOS: Infectados, em relação a sororreversores, apresentaram níveis significativamente superiores de IgM, do 1o ao 5o trimestre; IgA e IgG, do 2o ao 6o trimestre (P 90 mg/dl no 2o trimestre e IgG > 1.700 mg/dl ou 1.200 mg/dl no 2o e 3o trimestres associaram-se à infecção por HIV com índices J de 0,97, 0,92 e 0,93, respectivamente. Crianças infectadas nas categorias B e C, comparadas àquelas nas categorias N e A, apresentaram níveis superiores de IgM, do 2o ao 4o ano e IgA, do 3o ao 5o ano (P £.0,05). CONCLUSÕES: A evolução temporal dos níveis de IgA, IgG e IgM demonstra um estímulo intenso e precoce à síntese de imunoglobulinas em infectados. Indicadores clínico-epidemiológicos demonstram que tais níveis podem ser marcadores indiretos de infecção. Níveis superiores de IgM e IgA do 2o ao 5o ano em crianças infectadas com maior gravidade sugerem disfunção na regulação imune secundária ao estímulo antigênico persistente.20921

    The impact of maternal HIV infection on cord blood lymphocyte subsets and cytokine profile in exposed non-infected newborns

    <p>Abstract</p> <p>Background</p> <p>Children born to HIV+ mothers are exposed intra-utero to several drugs and cytokines that can modify the developing immune system, and influence the newborn's immune response to infections and vaccines. We analyzed the relation between the distribution of cord blood lymphocyte subsets and cytokine profile in term newborns of HIV+ mothers using HAART during pregnancy and compared them to normal newborns.</p> <p>Methods</p> <p>In a prospective, controlled study, 36 mother-child pairs from HIV+ mothers and 15 HIV-uninfected mothers were studied. Hematological features and cytokine profiles of mothers at 35 weeks of pregnancy were examined. Maternal and cord lymphocyte subsets as well as B-cell maturation in cord blood were analyzed by flow cytometry. The non-stimulated, as well as BCG- and PHA-stimulated production of IL2, IL4, IL7, IL10, IL12, IFN-γ and TNF-alpha in mononuclear cell cultures from mothers and infants were quantified using ELISA.</p> <p>Results</p> <p>After one year follow-up none of the exposed infants became seropositive for HIV. An increase in B lymphocytes, especially the CD19/CD5+ ones, was observed in cord blood of HIV-exposed newborns. Children of HIV+ hard drug using mothers had also an increase of immature B-cells. Cord blood mononuclear cells of HIV-exposed newborns produced less IL-4 and IL-7 and more IL-10 and IFN-γ in culture than those of uninfected mothers. Cytokine values in supernatants were similar in infants and their mothers except for IFN-γ and TNF-alpha that were higher in HIV+ mothers, especially in drug abusing ones. Cord blood CD19/CD5+ lymphocytes showed a positive correlation with cord IL-7 and IL-10. A higher maternal age and smoking was associated with a decrease of cord blood CD4+ cells.</p> <p>Conclusions</p> <p>in uninfected infants born to HIV+ women, several immunological abnormalities were found, related to the residual maternal immune changes induced by the HIV infection and those associated with antiretroviral treatment. Maternal smoking was associated to changes in cord CD3/CD4 lymphocytes and maternal hard drug abuse was associated with more pronounced changes in the cord B cell line.</p

    Clinical aspects and complete blood counts in children exposed to HIV-1: comparison between infected patients and seroreverters

    OBJECTIVE: to analyze the evolution of clinical and hematological aspects of children exposed to the vertical transmission of HIV-1, comparing infected patients with uninfected ones or seroreverters. METHODS: prospective, descriptive, longitudinal study. We analyzed 79 children born from HIV-1 infected mothers, under clinical follow up from March, 1996 until November, 1997, at the Immunodeficiency division of the Hospital de Clínicas da Unicamp (State University Hospital of Campinas). RESULTS: failure to thrive was observed in both groups, but was greater among seroreverters. Among the infected children, 23 mothers did not use AZT during pregnancy, 16 of them (61.5%) had been breastfed, four were classified into clinical category N, seven into A and fifteen into B. Clinical manifestations in patients younger than one year were seen in 18 infected children (69.2%). Anemia was observed in 73.1% of the infected group and in 41.5% of the seroreverters (P < 0.008). The comparison between the groups showed that the most common hematologic alterations in the infected children was microcytosis and hypochromia (P < 0.05), lymphopenia between 15 and 18 months (P < 0.05), monocytosis between 9 and 12 months (P < 0.05) and a tendency towards high ferritin levels, with no statistical significance. CONCLUSIONS: microcytic and hypochromic anemia were observed in both groups: iron deficiency in the uninfected children, and chronic disease anemia in the infected ones. The infected children presented with monocytosis and lymphopenia at an earlier stage.OBJETIVO: analisar evolutivamente aspectos clínicos e hematológicos de crianças expostas à transmissão vertical do HIV-1 e comparar as que se infectaram com aquelas que não se infectaram, ou soro-reversoras. MÉTODOS: trata-se de estudo prospectivo, descritivo, longitudinal. Foram analisadas 79 crianças, filhas de mães infectadas pelo HIV-1, sob seguimento entre março de 1996 a novembro de 1997, no Ambulatório de Imunodeficiência do Hospital de Clínicas da Unicamp. RESULTADOS: houve comprometimento pôndero-estatural em ambos os grupos, sendo maior nos soro-reversores. No grupo das crianças infectadas, 23 mães não fizeram terapia com AZT na gestação, 16 pacientes (61,5%) foram amamentados ao seio, quatro foram classificadas na categoria clínica N, sete na A, e 15 na B. Manifestações clínicas antes de um ano de idade ocorreram em 18 lactentes (69,2%). Anemia se manifestou em 73,1% das crianças infectadas, e em 41,5% das com soro-reversão (P < 0,008). A comparação entre os grupos mostrou que as alterações mais freqüentes nos infectados foram microcitose e hipocromia (P < 0,05), linfopenia entre 15 e 18 meses (P < 0,05), monocitose entre os 9 e 12 meses (P < 0,05) e uma tendência de altos níveis séricos de ferritina, mas sem significância estatística. CONCLUSÕES: anemia microcítica e hipocrômica ocorreu em ambos os grupos, sendo ferropriva entre os soro-reversores, e anemia de doença crônica, entre os infectados. Nesse referido grupo, monocitose e linfopenia foram manifestações precoces.50351

    Comparison of biochemical and immunological profile of pediatric patients with acute myeloid leukemia in relation to healthy individuals

    ABSTRACT OBJECTIVE: To compare the biochemical and immunological profiles of pediatric patients with acute myeloid leukemia (AML) with healthy children and adolescents. METHODS: This was a cross-sectional study in which 21 therapy-naïve patients with AML were compared with a group of 24 healthy individuals. The following data were analyzed: serum proteins, leucocytes and subgroups, erythrocytes, hematocrit, hemoglobin, platelets, cytokines in peripheral blood mononuclear cells cultures under spontaneous and BCG- or PHA-stimulated conditions, immunoglobulin A, and erythrocytic glutathione. Statistical analysis was performed using SPSS software, considering as significant p-values < 0.05. RESULTS: Serum albumin levels were higher (p < 0.0001) in the control group, as well as all the parameters related to red blood cells (p < 0.0001). For leucocytes and subgroups, no statistical difference was found between the AML and the control groups. For cytokines, the concentrations were significantly higher under spontaneous and BCG-stimulated conditions for TNF-a, IL-6, IL-10, and IFN-? in the control group. Under PHA-stimulated conditions, the concentration was higher (p = 0.002) only for IL-6. No difference was found between the two groups for the other cytokines and for IgA in the saliva. Erythrocytic glutathione was higher (p < 0.0001) in AML patients. CONCLUSIONS: It was possible to characterize the biochemical and immunological profile of pediatric patients with AML, as well as highlight some significant differences in these parameters when comparing with healthy children and adolescents
