Idiopathic chronic eosinophilic pneumonia: a clinical case report
The eosinophilic pneumonias are a heterogeneous group of pulmonary disorders, which may compromise only the air ways, the pulmonary parenchyma, or both, characterised by alveolar eosinophils and infiltration of pulmonary tissue, with or without peripheral blood eosinophilia. Idiopathic Chronic Eosinophilic Pneumonia (ICEP), detailed description was by Carrington in 1969, is a rare eosinophilic lung disease, of unknown aetiology, characterised by peripheral blood eosinophilia, chest radiograph infiltrates and prompt response to corticosteroid therapy. ICEP most commonly affects women of middle age and usual symptoms are cough, dyspnea, fever and weight loss. The Expanded mtDNA Phylogeny of the Franco-Cantabrian Region Upholds the Pre-Neolithic Genetic Substrate of Basques
The European genetic landscape has been shaped by several human migrations occurred since Paleolithic times. The accumulation of archaeological records and the concordance of different lines of genetic evidence during the last two decades have triggered an interesting debate concerning the role of ancient settlers from the Franco-Cantabrian region in the postglacial resettlement of Europe. Among the Franco-Cantabrian populations, Basques are regarded as one of the oldest and more intriguing human groups of Europe. Recent data on complete mitochondrial DNA genomes focused on macrohaplogroup R0 revealed that Basques harbor some autochthonous lineages, suggesting a genetic continuity since pre-Neolithic times. However, excluding haplogroup H, the most representative lineage of macrohaplogroup R0, the majority of maternal lineages of this area remains virtually unexplored, so that further refinement of the mtDNA phylogeny based on analyses at the highest level of resolution is crucial for a better understanding of the European prehistory. We thus explored the maternal ancestry of 548 autochthonous individuals from various Franco-Cantabrian populations and sequenced 76 mitogenomes of the most representative lineages. Interestingly, we identified three mtDNA haplogroups, U5b1f, J1c5c1 and V22, that proved to be representative of Franco-Cantabria, notably of the Basque population. The seclusion and diversity of these female genetic lineages support a local origin in the Franco-Cantabrian area during the Mesolithic of southwestern Europe, ∼10,000 years before present (YBP), with signals of expansions at ∼3,500 YBP. These findings provide robust evidence of a partial genetic continuity between contemporary autochthonous populations from the Franco-Cantabrian region, specifically the Basques, and Paleolithic/Mesolithic hunter-gatherer groups. Furthermore, our results raise the current proportion (≈15%) of the Franco-Cantabrian maternal gene pool with a putative pre-Neolithic origin to ≈35%, further supporting the notion of a predominant Paleolithic genetic substrate in extant European populations
Estudio Epidemiológico Transversal para Comprender el Abordaje del Tratamiento en la Atención Habitual de Pacientes con Hemofilia B en España y Portugal
Introduction: Treatment options for patients with hemophilia B (PWHB) have improved in the last decade, but epidemiological studies are required to optimize clinical management and define unmet needs. Objective: The objective of the study is to investigate current disease management and quality of life (QoL) in PWHB in Spain and Portugal. Methods: Epidemiological, cross-sectional, multicenter study with moderately severe and severe PWHB. Sociodemographic, epidemiological, clinical, treatment, and QoL data were collected retrospectively over a 24-month period. Results: Of the 75 patients included in the study, 78.7% received prophylactic treatment and 21.3% on demand; 65.3% were treated with SHL-FIX and 10.7% with rIX-FP. Bleeding occurred in 81.3%, mainly in the form of spontaneous bleeding. Prophylaxis resulted in a lower annualized bleeding rate, better joint health, and better QoL, compared to on-demand treatments. Patients treated with rIX-FP throughout
the study had better disease control and QoL than those treated with SHL-FIX. Conclusion: With the largest sample to date of PWHB from Spain and Portugal, our understanding of the management and impact of hemophilia B is broadened and, in accordance with previous evidence, it shows the benefits of prophylaxis and the advantages that the FIX extended half life products life FIX can offer.info:eu-repo/semantics/publishedVersio
Desigualdades no acesso e utilização dos serviços de saúde na Região Metropolitana de São Paulo Health care services utilization and access inequalities in the Sao Paulo Metropolitan Region
Este artigo relata achados dos estudos de casos realizados em cinco municípios da Região Metropolitana de São Paulo (RMSP): São Paulo, Guarulhos, Santo André, Osasco e Franco da Rocha. As desigualdades no acesso aos serviços e sua utilização foram descritas a partir de tabulações avançadas da Pesquisa de Condições de Vida-PCV de 1998 do Seade. Analisamos informações a partir das variáveis posse ou não de planos de saúde, quintil de renda e faixa etária. Os atributos estudados foram: utilização dos serviços, tipo de cobertura do plano, procura pelos serviços e tempo médio de espera no atendimento. A comparação com estudos de outras regiões metropolitanas do Brasil, a partir da PNAD 98 do IBGE, permite afirmar que existem disparidades intra-regionais somente detectadas em estudos de menor escala espacial - os municípios. Apesar da RMSP possuir a maior cobertura de planos de saúde no Brasil, nota-se grande heterogeneidade interna. As desigualdades na posse de planos, no acesso, tempo de espera, tipo de cobertura se manifestam claramente na análise segundo quintil de renda e faixa etária. Conforme os resultados, sugerimos o aumento da capacidade de regulação do Estado, permitindo que os princípios da universalidade e eqüidade se estabeleçam no SUS, efetivando o direito à saúde.<br>This paper presents case study findings in five municipalities in the São Paulo Metropolitan Region. Inequalities in access to health care services and their utilization were described through advanced tabulation data from the 1998 SEADE Life Conditions Survey. The variables analyzed were: owning or not owning private health care insurance, income and age brackets. The health care service attributes studied were: health care services coverage by a health insurance plan, health services demands and average waiting time to receive health care. Compared with other studies, using the 1998 IBGE PNAD, the results allowed us to confirm interregional imbalances which can only be detected in shorter special scale studies: the municipalities. Despite showing the high private health insurances coverage the São Paulo Metropolitan Region has a great inner heterogeneity. The inequalities in private health care insurance, access, waiting time, and type of insurance coverage were observed through income quintiles and age classes analyses. Findings suggest that an expansion of the State's regulation capacity is necessary in order to empower the Brazilian Health Care System principles of universality and equity to be qualified to offer Brazilians the right to access health care services
Downstream green processes for recovery of bioactives from algae
Nowadays, macro- and microalgae are being increasingly used as promising raw materials for the food, cosmetic and pharmaceutical industries, thanks to their biodiversity and its variety on valuable bioactive compounds such as carbohydrates, polyunsaturated lipids, proteins and pigments, among others. Furthermore, more efficient and environmentally friendly processes for bioactives’ recovery are requested not only by the industry but also by the society. This chapter presents an overview on the use of downstream green processes, mainly based on compressed fluids extraction techniques, in order to recover bioactives from algae that can be lately used in several potential applications for the food, pharmaceutical and cosmetic industries, which is the pillar of algae-based biorefinery.Peer reviewe
