    Prevalence of smoking and incidence of initiation in the Latin American adult population: the PLATINO study

    Background: the PLATINO project was launched in 2002 in order to study the prevalence of chronic obstructive pulmonary disease (COPD) in Latin America. Because smoking is the main risk factor for COPD, detailed data on it were obtained. the aim of this paper was to evaluate the prevalence of smoking and incidence of initiation among middle-aged and older adults (40 years or older). Special emphasis was given to the association between smoking and schooling.Methods: PLATINO is a multicenter study comprising five cross-sectional population-based surveys of approximately 1,000 individuals per site in São Paulo (Brazil), Santiago (Chile), Mexico City (Mexico), Montevideo (Uruguay) and Caracas (Venezuela). the outcome variable was smoking status (never, former or current). Current smokers were those who reported to smoke within the previous 30 days. Former smokers were those who reported to quit smoking more than 30 days before the survey. Using information on year of birth and age of smoking onset and quitting, a retrospective cohort analysis was carried out. Smoking prevalence at each period was defined as the number of subjects who started to smoke during the period plus those who were already smokers at the beginning of the period, divided by the total number of subjects. Incidence of smoking initiation was calculated as the number of subjects who started to smoke during the period divided by the number of non-smokers at its beginning. the independent variables included were sex, age and schooling.Results: Non-response rates ranged from 11.1% to 26.8%. the prevalence of smoking ranged from 23.9% (95% CI 21.3; 26.6) in São Paulo to 38.5% (95% CI 35.7; 41.2) in Santiago. Males and middle-aged adults were more likely to smoke in all sites. After adjustment for age, schooling was not associated with smoking. Using retrospective cohort analysis, it was possible to detect that the highest prevalence of smoking is found between 20-29 years, while the highest incidence is found between 10-19 years.     Effect of quantum entanglement on Aharonov-Bohm oscillations, spin-polarized transport and current magnification effect

    We present a simple model of transmission across a metallic mesoscopic ring. In one of its arm an electron interacts with a single magnetic impurity via an exchange coupling. We show that entanglement between electron and spin impurity states leads to reduction of Aharonov-Bohm oscillations in the transmission coefficient. The spin-conductance is asymmetric in the flux reversal as opposed to the two probe electrical conductance which is symmetric. In the same model in contradiction to the naive expectation of a current magnification effect, we observe enhancement as well as the suppression of this effect depending on the system parameters.     Connectivity in the Deep: Phylogeography of the Velvet Belly Lanternshark

    PublishedThe velvet belly lanternshark, Etmopterus spinax, is a deep-sea bioluminescent squaloid shark, found predominantly in the northeast Atlantic and Mediterranean Sea. It has been exposed to relatively high levels of mortality associated with by-catch in some regions. Its late maturity and low fecundity potentially renders it vulnerable to over-exploitation, although little remains known about processes of connectivity between key habitats/regions. This study utilised DNA sequencing of partial regions of the mitochondrial control region and nuclear ribosomal internal transcribed spacer 2 to investigate population structure and phylogeography of this species across the northeast Atlantic and Mediterranean Basin. Despite the inclusion of samples from the range edges or remote locations, no evidence of significant population structure was detected. An important exception was identified using the control region sequence, with much greater (and statistically significant) levels of genetic differentiation between the Mediterranean and Atlantic. This suggests that the Strait of Gibraltar may represent an important bathymetric barrier, separating regions with very low levels of female dispersal. Bayesian estimation of divergence time also places the separation between the Mediterranean and Atlantic lineages within the last 100,000 years, presumably connected with perturbations during the last Glacial Period. These results demonstrate population subdivision at a much smaller geographic distance than has generally been identified in previous work on deep-sea sharks. This highlights a very significant role for shallow bathymetry in promoting genetic differentiation in deepwater taxa. It acts as an important exception to a general paradigm of marine species being connected by high levels of gene-flow, representing single stocks over large scales.     Non-small-cell lung cancer resectability: diagnostic value of PET/MR.

    Purpose To assess the diagnostic performance of PET/MR in patients with non-small-cell lung cancer. Methods Fifty consecutive consenting patients who underwent routine 18F-FDG PET/CT for potentially radically treatable lung cancer following a staging CT scan were recruited for PET/MR imaging on the same day. Two experienced readers, unaware of the results with the other modalities, interpreted the PET/MR images independently. Discordances were resolved in consensus. PET/MR TNM staging was compared to surgical staging from thoracotomy as the reference standard in 33 patients. In the remaining 17 nonsurgical patients, TNM was determined based on histology from biopsy, imaging results (CT and PET/CT) and follow-up. ROC curve analysis was used to assess accuracy, sensitivity and specificity of the PET/MR in assessing the surgical resectability of primary tumour. The kappa statistic was used to assess interobserver agreement in the PET/MR TNM staging. Two different readers, without knowledge of the PET/MR findings, subsequently separately reviewed the PET/CT images for TNM staging. The generalized kappa statistic was used to determine intermodality agreement between PET/CT and PET/MR for TNM staging. Results ROC curve analysis showed that PET/MR had a specificity of 92.3 % and a sensitivity of 97.3 % in the determination of resectability with an AUC of 0.95. Interobserver agreement in PET/MR reading ranged from substantial to perfect between the two readers (Cohen’s kappa 0.646 – 1) for T stage, N stage and M stage. Intermodality agreement between PET/CT and PET/MR ranged from substantial to almost perfect for T stage, N stage and M stage (Cohen’s kappa 0.627 – 0.823). Conclusion In lung cancer patients PET/MR appears to be a robust technique for preoperative staging
