    Retos en la incorporación al mercado laboral: análisis de la brecha en competencias [Challenges in entering the labour market: a competence gap analysis]

    Este artículo analiza el nivel de adquisición de competencias básicas y transversales a través de la autoevaluación realizada por los alumnos y la valoración realizada por las empresas en las que los alumnos han realizado prácticas durante las etapas finales de sus estudios. Para ello preparamos y distribuimos dos cuestionarios, uno para los estudiantes que desarrollaron las prácticas y otro para los tutores de estudiantes de las empresas. Para analizar los resultados nos hemos centrado en los procedimientos asociados con el análisis de varianza, ANOVA. Más concretamente hemos aplicado el modelo SERVQUAL. Este modelo considera la presencia de diferencias en términos de expectativas y percepciones entre las dos partes implicadas en una prestación de servicios. Identificar las principales brechas en el desarrollo y la evaluación de las competencias de los estudiantes se convierte en un tema clave, ya que es crucial a la hora de determinar no sólo la transición al mercado de trabajo, sino cómo alcanzar las habilidades que los estudiantes deben tener al inicio de su carrera profesional y, por lo tanto, sus posibilidades de éxito o fracaso, las probabilidades de un buen desempeño en la empresa o su capacidad de aprendizaje. [This article studies the acquisition level of basic and transversal skills through a self assessment made by the students and the assessment made by the companies where students have carried out internships during the final stages of their studies. To do this we prepared and distributed two questionnaires, one for the students who completed internships in a firm and another one for the firms’ students’ tutors. To analyse the results we have focused on procedures associated with the analysis of variance, ANOVA. More specifically we have applied the SERVQUAL model. This model considers the presence of differences in terms of expectations and perceptions between the two parties involved in a provision of services. Identifying the main gaps in the development and assessment of the students’ skills becomes a key issue, since it is crucial when determining not only their transition to the labour market, but how to achieve the skills level students must have when they start working and, therefore, their chances of success or failure, the odds of a good performance in the company, or their learning ability.

    Prosthetic shoulder joint infection by Cutibacterium acnes: does rifampin improve prognosis? a retrospective, multicenter, observational study.

    This retrospective, multicenter observational study aimed to describe the outcomes of surgical and medical treatment of C. acnes-related prosthetic joint infection (PJI) and the potential benefit of rifampin-based therapies. Patients with C. acnes-related PJI who were diagnosed and treated between January 2003 and December 2016 were included. We analyzed 44 patients with C. acnes-related PJI (median age, 67.5 years (IQR, 57.3-75.8)); 75% were men. The majority (61.4%) had late chronic infection according to the Tsukayama classification. All patients received surgical treatment, and most antibiotic regimens (43.2%) included β-lactam. Thirty-four patients (87.17%) were cured; five showed relapse. The final outcome (cure vs. relapse) showed a nonsignificant trend toward higher failure frequency among patients with previous prosthesis (OR: 6.89; 95% CI: 0.80-58.90) or prior surgery and infection (OR: 10.67; 95% IC: 1.08-105.28) in the same joint. Patients treated with clindamycin alone had a higher recurrence rate (40.0% vs. 8.8%). Rifampin treatment did not decrease recurrence in patients treated with β-lactams. Prior prosthesis, surgery, or infection in the same joint might be related to recurrence, and rifampin-based combinations do not seem to improve prognosis. Debridement and implant retention appear a safe option for surgical treatment of early PJI

    Chondroma is a benign tumour of mesenchymal origin that is composed of cartilage and rarely located in soft tissues, being described so far only in four cases, as located in the bladder, according to our knowledge. We describe the fifth case of a 67-year-old woman who consulted for microscopic haematuria, with an endoscopic finding of submucosal nodular image in the anterior wall of the bladder, which after resection and the histologic study shows cartilage and fibroconnective tissue, in part hyalinised, and positive immunohistochemical staining of cells with vimentin and S-100; this fact can support the diagnosis of bladder chondroma

    Implant removal in the management of prosthetic joint infection by Staphylococcus aureus: outcome and predictors of failure in a large retrospective multicenter study

    Objectives: To compare the characteristics and outcomes of cases with acute prosthetic joint infection (PJI; early post-surgical or hematogenous) by Staphylococcus aureus managed with implant removal (IRm) or debridement and retention (DAIR). To analyze the outcomes of all cases managed with IRm (initially or after DAIR failure). Methods: Retrospective, multicenter, cohort study of PJI by S. aureus (2003-2010). Overall failure included mortality within 60 days since surgery and local failure due to staphylococcal persistence/relapse. Results: 499 cases, 338 initially managed with DAIR, 161 with IRm. Mortality was higher in acute PJI managed initially with IRm compared to DAIR, but not associated with the surgical procedure, after propensity score matching. Underlying conditions, hemiarthroplasty, and methicillin-resistant S. aureus were risk factors for mortality. Finally, 249 cases underwent IRm (88 after DAIR failure); overall failure was 15.6%. Local failure (9.3%) was slightly higher in cases with several comorbidities, but independent of previous DAIR, type of IRm, and rifampin treatment. Conclusions: In a large multicenter study of S. aureus PJI managed with IRm, failure was low, but mortality significant, especially in cases with acute PJI and underlying conditions, but not associated with the IRm itself. Rifampin efficacy was limited in this setting

    The Not-So-Good Prognosis of Streptococcal Periprosthetic Joint Infection Managed by Implant Retention:The Results of a Large Multicenter Study

    Background.: Streptococci are not an infrequent cause of periprosthetic joint infection (PJI). Management by debridement, antibiotics, and implant retention (DAIR) is thought to produce a good prognosis, but little is known about the real likelihood of success. Methods.: A retrospective, observational, multicenter, international study was performed during 2003-2012. Eligible patients had a streptococcal PJI that was managed with DAIR. The primary endpoint was failure, defined as death related to infection, relapse/persistence of infection, or the need for salvage therapy. Results.: Overall, 462 cases were included (median age 72 years, 50% men). The most frequent species was Streptococcus agalactiae (34%), and 52% of all cases were hematogenous. Antibiotic treatment was primarily using β-lactams, and 37% of patients received rifampin. Outcomes were evaluable in 444 patients: failure occurred in 187 (42.1%; 95% confidence interval, 37.5%-46.7%) after a median of 62 days from debridement; patients without failure were followed up for a median of 802 days. Independent predictors (hazard ratios) of failure were rheumatoid arthritis (2.36), late post-surgical infection (2.20), and bacteremia (1.69). Independent predictors of success were exchange of removable components (0.60), early use of rifampin (0.98 per day of treatment within the first 30 days), and long treatments (≥21 days) with β-lactams, either as monotherapy (0.48) or in combination with rifampin (0.34). Conclusions.: This is the largest series to our knowledge of streptococcal PJI managed by DAIR, showing a worse prognosis than previously reported. The beneficial effects of exchanging the removable components and of β-lactams are confirmed and maybe also a potential benefit from adding rifampin
