    Profiteers of environmental change in the Swiss Alps: increase of thermophilous and generalist plants in wetland ecosystems within the last 10years

    It has been predicted that Europe will experience a rise in temperature of 2.2-5.3°C within this century. This increase in temperature may lead to vegetation change along altitudinal gradients. To test whether vegetation composition has already changed in the recent decade due to current warming (and other concomitant environmental changes), we recorded plant species composition in 1995 and 2005/2006 in Swiss pre-alpine fen meadows (800-1,400ma.s.l.). Despite no obvious changes in the management of these fens, overall, plant species richness (cumulative number of plant species at five plots per site) significantly increased over this period. This was mainly due to an increase in the number of thermophilous, rich-soil-indicator and shade-indicator species, which corresponded to increased community productivity and shading within the vegetation layer. In contrast, fen specialists significantly declined in species numbers. The strongest species shifts occurred at the lowest sites, which overall had a higher colonization rate by new species than did sites at higher altitudes. Vegetation change along the altitudinal gradient was also affected by different types of land management: early-flowering species and species with low habitat specificity had high colonization rates in grazed fens, especially at low altitude

    Breast Cancer, Sickness Absence, Income and Marital Status. A Study on Life Situation 1 Year Prior Diagnosis Compared to 3 and 5 Years after Diagnosis

    Background: Improved cancer survival poses important questions about future life conditions of the survivor. We examined the possible influence of a breast cancer diagnosis on subsequent working and marital status, sickness absence and income. Materials: We conducted a matched cohort study including 4,761 women 40–59 years of age and registered with primary breast cancer in a Swedish population-based clinical register during 1993–2003, and 2,3805 women without breast cancer. Information on socioeconomic standing was obtained from a social database 1 year prior and 3 and 5 years following the diagnosis. In Conditional Poisson Regression models, risk ratios (RRs) and 95 % confidence intervals (CIs) were estimated to assess the impact of a breast cancer diagnosis. Findings: Three years after diagnosis, women who had had breast cancer more often had received sickness benefits (RR = 1.49, 95 % CI 1.40–1.58) or disability pension (RR = 1.47, 95 % CI 1.37–1.58) than had women without breast cancer.     Serpentine polymorphism: A quantitative insight from first-principles calculations

    Single-walled chrysotile nanotubes [...] of increasing size (up to 5004 atoms per unit cell, corresponding to a radius of 205 Å) have been modelled at the Density Functional level of theory. For the first time, it is demonstrated that the (n, -n) and (n, n) series present a minimum energy structure at a specific radius (88.7 and 89.6 Å, respectively, referring to the neutral surface), corresponding to a rolling vector of (60, -60) and (105, 105), respectively. The minima are nearly overlapped and are lower in energy than the corresponding slab of lizardite (the flat-layered polymorph of chrysotile) by about 3.5 kJ mol-1 per formula unit. In both cases, the energy profile presents a shallow minimum, where radii in the range of 63 to 139 Å differ in energy by less than 0.5 kJ mol-1 per formula unit. The energy of larger nanotubes has a trend that slowly converges to the limit of the flat lizardite slab. Structural quantities such as bond distances and angles of nanotubes with increasing size asymptotically converge to the flat slab limit, with no discontinuities in the surrounding of the minimum energy structures. However, analysis of the elongation of a rectangular pseudo-unit cell along the nanotube circumference indicates that the main factor that leads lizardite to curl in tubes is the elastic strain caused by the mismatch between the lattice parameters of the two adjacent tetrahedral and octahedral sheets. It is also shown in this study that the curvature of the layers in one of the lately proposed models of antigorite, the "wavy-layered" polymorph of chrysotile, falls within the range of radii of minimum energy for the nanotubes. These findings provide quantitative insights into the peculiar polymorphism of these three phyllosilicates. They show also that chrysotile belongs to those families of inorganic nanotubes that present a minimum in their strain energy profile at a specific range of radii, which is lower in energy with respect to their flat equivalent

    Effect of training-sample size and classification difficulty on the accuracy of genomic predictors

    Introduction: As part of the MicroArray Quality Control (MAQC)-II project, this analysis examines how the choice of univariate feature-selection methods and classification algorithms may influence the performance of genomic predictors under varying degrees of prediction difficulty represented by three clinically relevant endpoints. Methods: We used gene-expression data from 230 breast cancers (grouped into training and independent validation sets), and we examined 40 predictors (five univariate feature-selection methods combined with eight different classifiers) for each of the three endpoints. Their classification performance was estimated on the training set by using two different resampling methods and compared with the accuracy observed in the independent validation set. Results: A ranking of the three classification problems was obtained, and the performance of 120 models was estimated and assessed on an independent validation set. The bootstrapping estimates were closer to the validation performance than were the cross-validation estimates. The required sample size for each endpoint was estimated, and both gene-level and pathway-level analyses were performed on the obtained models. Conclusions: We showed that genomic predictor accuracy is determined largely by an interplay between sample size and classification difficulty. Variations on univariate feature-selection methods and choice of classification algorithm have only a modest impact on predictor performance, and several statistically equally good predictors can be developed for any given classification problem

    The postmastectomy pain syndrome: an epidemiological study on the prevalence of chronic pain after surgery for breast cancer

    The prevalence of the postmastectomy pain syndrome (PMPS) and its clinical characteristics was assessed in a group of patients who had undergone surgery for breast cancer at the Department of Surgery, Odense University Hospital, within the period of 1 May 2003 to 30 April 2004. The study included 258 patients and a reference group of 774 women. A questionnaire was mailed to the patients 1½ year after surgery and to the women in the reference group. The PMPS was defined as pain located in the area of the surgery or ipsilateral arm, present at least 4 days per week and with an average intensity of at least 3 on a numeric rating scale from 0 to 10. The prevalence of PMPS was found to be 23.9%. The odds ratio of developing PMPS was 2.88 (95% confidence interval 1.84–4.51). Significant risk factors were as follows: having undergone breast surgery earlier (OR 8.12), tumour located in the upper lateral quarter (OR 6.48) and young age (OR 1.04). This study shows that, although recent advances in the diagnostic and surgical procedures have reduced the frequency of the more invasive surgical procedures, there still is a considerable risk of developing PMPS after treatment of breast cancer

    A Longitudinal Comparison of Arm Morbidity in Stage I–II Breast Cancer Patients Treated with Sentinel Lymph Node Biopsy, Sentinel Lymph Node Biopsy Followed by Completion Lymph Node Dissection, or Axillary Lymph Node Dissection

    Background:\ud Long-term shoulder and arm function following sentinel lymph node biopsy (SLNB) may surpass that following complete axillary lymph node dissection (CLND) or axillary lymph node dissection (ALND). We objectively examined the morbidity and compared outcomes after SLNB, SLNB + CLND, and ALND in stage I/II breast cancer patients.\ud \ud Materials and Methods:\ud Breast cancer patients who had SLNB (n = 51), SLNB + CLND (n = 55), and ALND (n = 65) were physically examined 1 day before surgery (T0), and after 6 (T1), 26 (T2), 52 (T3), and 104 (T4) weeks. Differences in 8 parameters between the affected and unaffected arms were calculated. General linear models were computed to examine time, group, and interaction effects.\ud \ud Results:\ud All outcomes changed significantly, mostly nonlinearly, over time (T0–T4). Between T1 and T4, limitations decreased in abduction (all groups); anteflexion, abduction-exorotation, abduction strength (SLNB + CLND, ALND); flexion strength (SLNB + CLND); and arm volume (SLNB, SLNB + CLND). At T4, limitations in anteflexion (SLNB, ALND), abduction (SLNB + CLND, ALND), exorotation (ALND), abduction-exorotation (all groups), and volume (SLNB + CLND, ALND) increased significantly compared with T0. The SLNB group showed an advantage in anteflexion, abduction, abduction-exorotation, and volume. Groups changed significantly but differently over time in anteflexion, abduction, abduction/exorotation, abduction strength, flexion strength, and volume. Effect sizes varied from 0.19 to 0.00.\ud \ud Conclusion:\ud Initial declines in range of motion and strength were followed by recovery, although not always to presurgery levels. Range of motion and volume outcomes were better for SLNB than ALND, but not strength. SLNB surpassed SLNB + CLND in 2 of the range of motion variables. The clinical relevance of these results is negligible

    Quality of Life After Sentinel Lymph Node Biopsy or Axillary Lymph Node Dissection in Stage I/II Breast Cancer Patients: A Prospective Longitudinal Study

    Background:\ud Breast cancer patients’ quality of life (QoL) after surgery has been reported to improve significantly over time. Little is known about QoL recovery after sentinel lymph node biopsy (SLNB) in comparison to axillary lymph node dissection (ALND).\ud \ud Methods:\ud 175 of 195 stage I/II breast cancer patients completed the EORTC QLQ-C30: one day before surgery (T0) and after 6 (T1), 26 (T2), 52 (T3) and 104 (T4) weeks. Of these, 54 patients underwent SLNB, 56 SLNB+ALND and 65 ALND. General linear models and paired T-tests between T0–T4 and T1–T4 were computed. Complications, radiotherapy and systemic therapy were added to the model.\ud \ud Results:\ud Significant time effects were found on physical, role and emotional functioning. Physical and role functioning decreased between T0 and T1. At T4, SLNB patients’ functioning had increased to their T0 level; ALND (+/– SLNB) patients’ functioning had increased, but had not improved to T0 level. Emotional functioning increased linearly between T0 and T4. At T4, emotional functioning was significantly higher in all groups as compared with T0. No significant group or interaction (time × group) effects were found. Complications and chemotherapy had a significant negative effect on role, emotional and cognitive functioning. Complications had a significant effect on social functioning also. Effect sizes varied between 0.00 and 0.06.\ud \ud Conclusion:\ud Two years post surgery, breast cancer patients’ QoL is comparable to that shortly before surgery. Women rated their emotional functioning as even better. SLNB is not associated with a better QoL than ALND. However, undergoing systemic therapy and/or experiencing complications affects QoL negatively
